Reduction of testicular human chorionic gonadotropin receptors by human chorionic gonadotropin in vivo and in vitro.

Changes in rat and human testicular human chorionic gonadotropin (hCG) binding sites induced by hCG were estimated in vivo and in vitro. After a single administration of hCG, the specific 125I-hCG bindings were significantly reduced for 7 and 5 days in rat and human testes, respectively. Thereafter, 125I-hCG bindings had recovered to pretreatment values by the 14th day after the administration. Occupied hCG bindings accounted for about half of the reduced bindings on the day after administration of hCG. After this time, however, the occupancy did not contribute so much to the reduction of the bindings. In experiments in vitro using the organ culture technique, an exposure to hCG for 24 h induced a dose-related significant loss of the specific 125I-hCG bindings for 7 and 5 days in rat and human testes, respectively. Thereafter, the loss was gradually recovered. These patterns of changes in 125I-hCG bindings in vitro were similar to those in vivo. These findings suggest that the reduction in hCG binding sites by hCG is due to not only occupancy but also downregulation of the binding sites and that the testicular organ culture method used in the present study is useful to study hormonal regulation of testicular function, especially in human testes.

Isolated gonadotropin deficiency secondary to glioma in septum pellucidum.

A 21-year-old man, who had had normal sexuality beforehand, noticed a decrease in libido and potency, as well as loss of ejaculation. Endocrine evaluation showed normal serum levels of gonadotropins but a low testosterone level. The response to clomiphene citrate was poor while those to luteinizing hormone-releasing hormone and human chorionic gonadotropin were within normal limits. A tumor found in the septum pellucidum through brain-computerized tomography was resected. Histologically it proved to be a mixed tumor composed of astrocytoma and oligodendroglioma. Three months after the operation the patient had recovered normal sexual functions and endocrine evaluations, including the responsiveness to clomiphene citrate, had been restored. This case suggests the existence of some stimulatory fiber for the secretion of luteinizing hormone in the septum pellucidum.

[Microbiological evaluation of topical application of the controlled release strips containing ofloxacin (PT-01) in the human periodontal pocket].

The recognition that destructive periodontal diseases may be caused by specific microorganisms has led to an increased interest and usage of antimicrobial agents in periodontal therapy. In this study, the effect of topical application of ofloxacin (OFLX), a synthetic antibiotic, was microbiologically evaluated. The new developed controlled release strips containing OFLX (PT-01), in which there were structurally immediate- and sustained-releasing portions, were applied to the periodontal pockets of 27 adult subjects with periodontitis. Three different sites with a deep probing pocket depth (greater than or equal to 5 mm) were randomly selected in each patient, and were divided into three groups, i.e., PT-01 applied site (T), placebo-applied site (P) and control site (C). Periodontal treatments consisted of oral hygiene instruction and supragingival scaling on day 0 and 7, and subgingival scaling and root planing on day 14. PT-01 was weekly applied on day 0 to 35, and the subgingival plaque samples from each site were collected on day 0, 14, 21 and 42. The dynamics of subgingival microflora was investigated by dark field microscopy for determination of the %s of spirochetes, motile rods and coccoid cells, and anaerobic and aerobic cultivations for the determinations of the total number of subgingival bacteria, black-pigmented Bacteroides (BPB), Fusobacterium species and Actinobacillus actinomycetemcomitans. The results showed that the significant reduction of %s of spirochetes and motile rods and significant increase of % coccoid cells were found in only PT-01 applied sites during first 14 days. In this period, the total number of cultivable bacteria, BPB and Fusobacterium species were also significantly reduced in T sites. While, after subgingival scaling and root planing were performed, significant changes in the proportions and numbers of the subgingival microflora were found in all sites. Especially, PT-01 applied site showed significant improvement in the %s of spirochetes and motile rods as well as the total number of the bacteria. Moreover, the further microbiological determinations of the each isolate revealed that no detectable amounts of A. actinomycetemcomitans could be found from any samples in this study. These results suggested that weekly application of PT-01 in the periodontal pocket could have significant effects on the qualitative and quantitative improvements in the subgingival microflora. It was also suggested that the application of PT-01 might have ameliorating effect as adjuncts of mechanical subgingival plaque control in the periodontal treatment.

Reduction of testicular human chorionic gonadotropin receptors by human chorionic gonadotropin in infertile men.

We measured testicular human chorionic gonadotropin (hCG) receptors in 30 infertile men either before or 1, 3, 5, 7, or 14 days after a single administration of 5000 IU of hCG. For 5 days the administration of hCG significantly reduced the testicular binding of 125I-hCG compared with that of the testes before administration. From the 7th day the binding capacity began to increase and returned to the preadministration level 14 days after the treatment. Occupied hCG binding sites accounted for about half of the reduction in binding sites on the day after administration of hCG. After this time, however, the occupancy did not contribute so much to the reduction in binding sites. These findings suggest that the reduction in testicular hCG binding sites after a single administration of 5000 IU of hCG is due to not only occupancy but also down-regulation of the binding sites.

Testicular gonadotropin receptors in men with hypogonadotropic hypogonadism.

To investigate the reason(s) why human chorionic gonadotropin (hCG) and human menopausal gonadotropin (hMG) treatment is not always effective for men with hypogonadotropic hypogonadism (HH), we measured hCG and follicle-stimulating hormone (FSH) receptors in testicular tissues obtained by biopsy from 5 men with idiopathic HH and 2 men with secondary HH. The difference between these 7 men and age-matched controls in the numbers of testicular hCG and FSH receptors were not significant. Histologic examination of the testes also showed that Leydig and Sertoli cells had a normal appearance. Therefore, responsiveness to gonadotropins seems to be preserved in the testes of men with HH. Fro these results it could be concluded that combined hCG and hMG treatment may be used with good results for the treatment of HH.

Direct inhibitory effect of estrogen on the human testis in vitro.

The direct inhibitory effects of estrogens on human testicular functions were investigated with a testicular organ culture technique. 125I-labeled human chorionic gonadotropin (125I-hCG) bindings in testes cultured in media containing diethylstilbestrol diphosphate (DESDP) began to dose-relatedly decrease a day after the start of the culture, and this decrease remained relatively constant during the 5-day culture. On the other hand, testosterone produced by the cultured testes time-relatedly decreased during the 5-day culture. From the above results it may be concluded that the direct inhibitory effect of estrogens on human testicular androgen production consists of not only the loss of testicular hCG receptors but also of other mechanisms at a distal step from hCG receptor activation.

[A case of signet ring cell carcinoma of the urinary bladder].

A case of primary signet ring cell carcinoma of the urinary bladder is reported. A 52-year-old man was admitted with the complaint of gross hematuria. Cystoscopy showed non-papillary tumor on the right lateral wall. Abdominal CT, Ga scintigraphy, upper gastrointestinal series and barium enema, revealed no signs of a tumor other than the bladder carcinoma. The routine hematologic and chemistry profiles showed no abnormalities except for the serum level of carcinoembryonic antigen elevated to 210 ng/ml. Total cystectomy and right nephroureterectomy with left cutaneous ureterostomy was performed and the surgical specimen showed adenocarcinoma of the bladder. The patient died 5 months after the operation, and autopsy was performed. No tumors were found on the mucosa of the whole digestive tracts or pancreas. This case might be of primary adenocarcinoma originated from the bladder. The literature on the differential diagnosis of the cases reported as bladder adenocarcinoma are reviewed briefly.

[A case of polycystic kidney with bilateral nephrectomy].

A 56-year-old woman in chronic hemodialysis had been suffering from uncontrollable fever for the past 7 months. Her original disease was diagnosed as familial polycystic kidney and three of her five brothers were found to have the same disease. Her chromosome was 46,XX,21P+ and laboratory examination revealed severe anemia, malnutrition, liver dysfunction, pyuria and candidiasis of urine. Abdominal echogram and CT scan revealed polycystic kidneys and multiple liver cysts. She was admitted to our hospital and was diagnosed as having pyelonephritis of the right kidney. As her condition was not improved by conservative therapy right nephrectomy was performed. One month later, spiking fever and left tenderness reappeared. Those symptoms could not be controlled by conservative therapy and left nephrectomy was performed again. Pathological examination on nephrectomized kidneys showed interstitial nephritis, hyaline degeneration and proliferative change of glomeruli, microabscess, colloid of tubules and calcification of parts of Henle's loops. Nephrectomy has been performed in 1.6 to 10.0% of polycystic kidneys due to references since 1952. Eight of the 22 polycystic kidneys (36.3%) seen at our hospital during the past 10 years have been removed.

[Intra-arterial chemotherapy combined with mitomycin C and degradable starch microspheres in unresectable metastatic liver carcinoma].

DSM, an agent which occludes arterial blood flow temporarily, was experimentally proved to enhance the uptake of co-administered anti-cancer drugs in the target organ and, conversely, to reduce the systemic side effects. Intra-arterial chemotherapy combined with MMC and DSM was performed on eight cases of unresectable metastatic liver carcinoma. The primary site of all these cases (7 gastric, 1 rectal) was resected. A catheter was inserted into the proper hepatic artery via the gastroduodenal artery, and the other end was connected to an implantable drug delivery system which was placed at the abdominal wall. DSM (180 to 900 mg) mixed with MMC (4 to 20 mg) was injected through the system every one, two or three weeks. The response rate in evaluable 7 cases was 57% (4 partial response, 1 minor response, 2 no change) and prognoses in the frequently treated group (7 times or more) were better than in the group with less frequent treatment (3 times or less). Pain (25.0%), nausea and vomiting (20.6%), and high fever (11.8%) were side effects, but they could be treated by medication. Our results suggest that intra-arterial chemotherapy combined with MMC and DSM is one of the effective treatments for unresectable metastatic liver carcinoma.

[Lymph node metastasis of early gastric cancer and lymph node dissection].

Five hundred and four cases of early gastric cancer and their status of lymph node metastases were analyzed. The patients were classified into 4 groups by age; Group A: Younger than 50 years, Group B: 50-59, Group C: 60-69 and Group D: Over 70 years. The tumors were divided into 2 histologic groups, differentiated type and undifferentiated type. The results are as follows. 1. The incidence of lymph node metastasis was 2.4% in mucosal cancer and 17.1% in submucosal cancer. 2. The incidence of lymph node metastasis in Group D was remarkably lower compared to that in other younger groups. 3. No lymph node metastasis was found in mucosal cancer of differentiated type. 4. Positive node was usually found in the regional nodes near the tumor and in the nodes adjacent to the left gastric artery. 5. Extended lymph node dissection is not necessary for the patient over 70 years and for mucosal cancer of differentiated type.

[Extension and reduction of surgery of cancer occupying mainly the upper third part of the stomach--indications and limitations].

To improve the therapeutic result of patients with advanced carcinoma of the upper third of the stomach, it is necessary to extend further the range of resection and dissection especially in case of invading the lower esophagus with serosal involvement. The extended surgery consists of the following: abdominophrenicotomy or abdominothoractomy, lower esophagectomy and total gastrectomy, dissecting completely the second group of nodes combined with distal pancreatectomy and splenectomy, a block resection of adjacent organs in case of direct invasion, and even dissection of nodes in the lower mediastinum, within the hepatoduodenal ligament, behind the head of the pancreas and along the superior mesenteric vein. Operative procedures are, however, to be palliative in case of moderate or severe peritoneal or hepatic involvements or distant metastasis to the fourth group of nodes. Reduced procedures in case of no serosal invasion nor lymph node metastasis consist of 1/2 proximal gastrectomy and of dissection of the first group of nodes especially in superficial carcinoma. However, in cases free from serosal invasion but with metastasis to the first group of nodes or in cases with a slight serosal invasion, total gastrectomy with dissection up to the second group of nodes is recommended.
