Some evidence for interaction of D-cycloserine with DNA.

D-Cycloserine showed a decreased lethality against paraquat-treated E. coli than against non-treated cells. E. coli K12 mutants which were deficient in DNA repair were more sensitive than the wild type strain to D-cycloserine. Furthermore, D-cycloserine caused PM2 DNA cleavage in the presence of Fe2+, and the cleavage was strongly inhibited by singlet oxygen scavengers. These results indicate that D-cycloserine interacts with DNA and an active oxygen species is involved in the interaction.

[2 cases of multicentric glioma].

We reported two cases of multicentric glioma. The first case was 60-year-old female. In CT scan findings, separate mass lesions were observed in the right parieto-occipital area, left parietal lobe and left thalamic region. The second case was 53-year-old male. In CT scan findings, separate mass lesions were observed in the right fronto-parieto-temporal area and left frontal lobe. These two cases were considered to be metastatic brain tumor and radiation therapy and chemoimmunotherapy were performed. But in the postmortem examinations, both cases were evident to be not metastatic brain tumor but glioblastoma multiforme. And tumor cells were not found in commissural or other nerve pathways and cerebrospinal fluid system histologically. So our cases were considered as multicentric glioma. The pathogenesis of multicentric glioma are not understood till now. Therefore, it is interesting that the different histological findings were observed in the each tumor lesion of the first case.

Non-Hodgkin's lymphomas. analysis of 109 Japanese cases with the use of LSGJ classification.

Recently the Lymphoma Study Group of Japan (LSGJ) proposed a new classification of non-Hodgkin's lymphomas (NHLs), diving NHLs into the follicular group consisting of three subsets and the diffuse group, 7. Each subset of the diffuse group is further divided into B or T cell types according to immunologic markers and/or morphologic prediction. In the review of 118 malignant lymphomas, the authors studied the 109 cases of NHLs, attempting to assess the clinicopathologic utility of this classification. Morphologic criteria, enzyme histochemistry, and immunoperoxidase technique were used to ensure the accuracy of the immunologic phenotyping. The results suggest that the LSGJ classification is easily reproducible and yields a more precise clinicopathologic correlation than traditional, morphologic classifications. Consistent with similar studies in Japan, this study demonstrated a low incidence of Hodgkin's disease (7.6% of all lymphomas) and follicular lymphomas (8.3% of all NHLs) and a high incidence of T cell lymphomas (34.9% of all NHLs). The incidence (45.9%) of extranodal presentation was high. These four features seem characteristic of lymphomas in Japan.

[Long term survival of holoprosencephaly with shunting procedure (author's transl)].

A long survived case of holoprosencephaly with hydrocephalus has been infrequently reported in the literature. One such case were presented on this report. This male baby by cesarean section because of his large head circumferenced 45 cm. He was admitted to our clinic diagnosed as congenital hydrocephalus at the third day from birth. On admission the circumference of his head was measured 49 cm, but other external malformations were not noticed. Right transaxillary carotid angiogram showed anterior cerebral artery appeared like azygos anterior cerebral artery. Absence of falx and fornix were suspected pneumoventriculography and transillumination of the head. Finally he was diagnosed as holoprosencephaly accompanied with hydrocephalus by computed tomography. Ventriculo-peritoneal shunt was performed at the 22nd day of his life. On follow-up after discharge, mental and motor development was not so poor. He was able to speak a few of simple words one year after discharge. At present 4 years and 6 months after discharge, he can do simple conversation and walk with assistance. Repeated computed tomography shows the frontal cerebral mantle is thickening in 36 mm at present. Good resulted case of holoprosencephaly as presented here could not be found in the literature. Electron microscopic studies of the endolymphatic sac in Ménière's disease.

Five specimens of the endolymphatic sac taken from patients with Ménière's disease during epidural drainage surgery were examined under the electron microscope. The clinical history and histopathological findings are presented. The most common and prominent finding was perisaccular fibrosis of the endolymphatic sac which appears to disturb the free absorption of endolymph in the endolymph in the endolymphatic sac resulting in endolymphatic hydrops. Well-preserved epithelial cells of the endolymphatic sac could only be found in one of the specimens.

Hereditary myeloperoxidase deficiency.

Subjects with neutrophil myeloperoxidase (MPO) deficiency have been rarely reported. In part this may be due to the lack of a simple screening technique that would detect them. With the routine use of a cytochemical leukocyte differential counter that employs MPO stains, over a 40-mo a period 8 unrelated probands with partial MPO-deficiency and one with complete deficiency were identified. Family studies have identified 23 additional, partially deficient subjects. The largest pedigrees demonstrate that the carrier state or partial MPO deficiency is inherited in an autosomal dominant pattern. Leukocytes from a subject with complete MPO deficiency and from some subjects with partial deficiency have impaired bactericidal activity against S. aureus. Superoxide generation was increased and chemiluminescence decreased in MPO-deficient leukocytes. Eleven subjects were prospectively followed for 18-30 mo. Only two partially deficient subjects have had serious infections consisting of recurrent streptococcal cellulitis and aseptic meningitis. These data suggest that leukocyte MPO deficiency is a common inherited defect that results in minimal clinical problems, supporting the concept of multiple leukocyte bacterial killing systems.

Sideroblastic anemia as a preleukemic event in patients treated for Hodgkin's disease.

Sideroblastic anemia after treatment for Hogdkin's disease was seen in two patients 3 years after completion of radiation therapy and chemotherapy. This was followed in both by the development of myelomonoblastic leukemia. No evidence of recurrent Hodgkin's disease was present in either patient. Our observation suggests that development of sideroblastic anemia in patients previously treated for Hodgkin's disease is probably secondary to the treatment and is a preleukemic event.

Acquired myeloperoxidase deficiency and recurrent infections in a patient with acute myelomonocytic leukemia.

Acquired myeloperoxidase deficiency has been reported in several hematological malignancies. The clinical course of a patient with acute myelomonocytic leukemia is described which was characterized by staphylococcal infections prior to therapy and again during a period of relapse. Neutropenia was not a feature of these two periods but in vitro studies revealed decreased bacterial killing capacity and decreased neutrophil myeloperoxidase activity. Infectious complications were not observed during drug-induced remission when bacterial killing capacity and myeloperoxidase activity were improved toward normal. These observations suggest that the myeloperoxidase deficient neutrophils were derived from leukemic progenitors.

Measurement of eye deviation under the nonfixative condition in patients with peripheral labyrinthine disorders, especially Meniere's disease.

Among 232 Japanese patients with peripheral disorders, 98 had a unilateral Meniere's disease. In these patients and 20 healthy adults, recordings of eye deviation were made utilizing DC-ENG under two conditions, i.e. eyes closed (EC) and eyes open in the absolute darkness (AD). (1) 90 % tolerance interval of AD. was significantly smaller than that of EC in controls. (2) Appearance of nystagmus in 5 groups with labyrinthine lesions was compared with the direction of deviation toward and against the affected side in both conditions. (3) Concerning unilateral Meniere's, the relationship among deviation, nystagmus and caloric response was analyzed statistically and the clinical application discussed.
