[Embryonal alcohol syndrome ].

The permanent alcohol abuse of a pregnant woman is of high risk for the embryo especially during the first weeks of pregnancy. It is prerequisite to the embryonic alcohol syndrome that the ethylalcohol passes the placenta. Subsequently hypotrophic infants with multiple malformations of minor and major quality are born. Part of the malformations are characteristic, part of them are unspecific for the disease. They altogether represent the entity of the embryonic alcohol syndrome. Occurrence and intensity of the disease depend on the daily quantity of pure alcohol, especially during the first month of pregnancy, on the level of non-metabolized ethylalcohol in the mother's blood, and on the stage of the maternal alcohol disease according to Jellinek. To prevent the embryonic alcohol syndrome means to prevent the maternal chronic alcohol abuse.

[Leukemic chromosome clone. I. Risk factor in the acute lymphatic leukemia of children].

As long as the aetiology of acute lymphatic leukaemia of children is not known its therapy is based on clinical experience. Among the values of experience those factors will play a part, the evidence of which during the ALL initial stage will be a risk for successful therapy and survival rate. This results in a choice of more aggressive variants of modern therapy schemes. In a cytogenetic study made in 35 children with ALL it was tested, whether even leukaemic chromosome clones will be a risk for the course of acute leukaemia. The duration of the first remission and survival rate were considered as criteria. The evidence of a leukaemic chromosome clone could be shown to be followed by a short survival rate, irrespective of the stage of the disease where the clone had been observed first. Thus, cytostatic therapy in those ALL patients who are affected with luekaemic chromosome aberration of stem line character should be aimed at the complete annihilation of the clone, irrespective of other remission criteria. The failure of blood and bone-marrow cultures as early as during the untreated initial stage indicated a primary cellular immuno-insufficiency. This combination of cell immuno-depression with high peripheral leukocytes connts and a primary mediastinal tumour or a generalizing lymphosarcoma respectively, was the highest risk up till now for the course of the disease. Judging from the duration of the first remission and the survival rates, the consecutive schemes of therapy did not differ in their effect on leukaemia with pathological stem lines. On the basis of the present study the impression could not be excluded that up till now long term survival rates could be attributed rather to individual manners of response than to the modern therapy scheme.

Fees: are they getting out of hand?

In the past, Medical Economics has conducted an annual survey of fees charged by office based physicians in general practice, family and internal medicine, obstetrics-gynecology and pediatrics, and general surgery. This year, the data-base was expanded to include six other specialties. The article provides a series of tables exhibiting the fees charged by physicians for first office visits, return visits, history and physical exams, and other general procedures specific to particular specialties. It is pointed out that the fee for return office visits has doubled in eight years. The increase in costs of routine services accounts for much of the 9.6% increase in physicians' fees during the last year. Primary care doctors have initiated the highest fee hikes for 1978. This is attributed to young physicians entering the field and charging more than their older colleagues. Surgical fees have doubled in cost in the last ten years. For the past three years, physicians' fees have increased at the average rate of 9.5%, usually exceeding the Consumer Price Index. Physician services only account for 20% of the entire health care bill, but due to their visibility, they are subject to criticism and careful review. Since the pressure is on for health care cost containment, the author advises physicians to think again before raising their fees in the near future.

[Cytogenetic studies following immune induction therapy].

This work deals with the question whether the immunisation with BCG-vaccine and leukemic cells leads to morphological changes of the chromosomes from children with acute leukemia. In respect of the small number of patients, and of the variant numbers in the single groups it is concluded, that immunological induction therapy is a possible gravitating factor in manifestation of isolate breaks and complex aberrations. It is not known whether one of them or both the components would be necessary, whether dosage and duration of vaccination have any influence, and how long after vaccination the gravitating influence may be seen.
