[Contribution of Doppler investigations of the deep venous axes to the prevention and diagnosis of thrombo-embolic disease (author's transl)].

The venous disease study by Doppler ultrasound technique provides a precious help to its diagnosis because the dubiousness and limits of clinical examination. The main indications of Doppler ultrasound deep veins examination are: --early diagnosis of thrombophlebitis, venous insufficiency and arterio-venous fistulae. --a non invasive monitoring of medical or surgical treatment of venous pathology and best appreciation of venous return in right heart cardiopathies, respiratory disease and abdominal surgery. --help in central venous catheterization. The sensibility of Doppler ultrasound in venous thrombosis is 86.5% and its specificity 91% in a study of 110 patients with comparison Doppler-venography. These results quite comparable with other studies, indicate a poor sensibility for the diagnosis of distal thrombi or partially occluding clots. In association with other non invasive examination Doppler ultrasound allows a better indication of phlebography in the detection of thrombophlebitis in high risk patients as well as a post operative survey with high sensitivity and good efficiency.

[Exploration of major veins by Doppler ultrasonography. Indications and limits (author's transl)].

Doppler ultrasonography is a first-choice, non-invasive, inexpensive and repeatable procedure for exploring major veins and for detecting venous thrombosis, venous insufficiency and arterio-venous fistulae. It also provides invaluable information on post-operative haemodynamics in cases of thrombectomy, venous shunt and vena cava clip, and helps in evaluating the effectiveness of thrombolytic agents. Its sensitivity is satisfactory: in 110 patients with suspected phlebitis, there was an 86,5% agreement between the results of doppler ultrasonography and those of phlebography. False negative results are encountered with distal thromboses and, unfortunately, floating clots (there were 3 cases of undetected iliac vein floating clots in this series). It seems rational to use Doppler ultrasonography in patients with high risk venous diseases. The method will not replace phlebography, but when applied on correct indications, it may substantially reduce the number of negative or unassessable results.

Comparative studies of adaptive responses of fatty acid synthetase activities in rat liver and adipose tissue.

The specific activities of the fatty acid synthetases in the cytosolic fraction of livers and epididymal fat pads from fed, fasted or refed rats were determined. Refed rats received diets which provided, as the primary energy sources, sucrose or starch (75%) or both (59%) with beef tallow or safflower oil (16%) or the monosaccharide components (75%). From these determinations of fatty acid synthetase activity, the relative contribution of each tissue to the rat's overall lipogenic capacity was estimated. In rats fed a cereal-based stock diet ad libitum the adipose tissue accounted for 58% of total activity. During a 2-day fast the hepatic activity decreased from 14 units/100 g body weight to 2.7 units whereas the adipose tissue activity fell from 19 to 13.4 units/100 g body weight. At this time, 82.8% of the activity was in the latter tissue. After refeeding the sucrose diet for 2 days, the hepatic activity had increased 45-fold to 122.3 units/100 g body weight while the adipose tissue activity increased only 2.2-fold to 29.1 units, 19% of the total activity. We estimate that these adaptive responses deteriorate slowly (12.3 days, adipose tissue: 15.4 days, liver) with continued refeeding to the levels present in rats fed the cereal-based stock diet. Activation of residual or constituative enzyme was the major factor in the adipose tissue response whereas activation and induction played roles in the hepatic response. Responses to the refeeding of the other diets were of lesser magnitude. Generally our estimates based on fatty acids synthetase activities are consistent with other estimates based on radiolabel incorporation, NADPH generation and rate-limiting enzyme activities.

The prevalence of peptic ulcer in an institution for the mentally retarded.

Peptic ulcer was studied at a California institution for the mentally retarded. Of the 1,700 residents at risk, ninety-one were to have X-ray or endoscopy evidence of peptic ulcers, a prevalence rate of 5.4 percent. Ulcer prevalence in the duodenum was more than twice that in the stomach and oesophagus. All oesophageal, gastric, and duodenal erosions or ulcer craters, acute or chronic, were considered. The more severely retarded patients showed a higher ulcer prevalence than those less retarded, and males demonstrate higher rates than females. The presence of medical and neurological disabilities, including limitations of vision and communication, was positively associated with ulcer development. The results indicate a need for further study on aetiology of peptic ulcers in the retarded.

Unique T cell Ia antigen expressed on a hybrid cell line producing antigen-specific augmenting T cell factor.

Hybrid cell lines were established by fusion between keyhole limpet hemocyanin(KLH) binding T cells of A/J mice and an AKR T cell tumor line, BW5147. Hybrids were selected for the presence of Ia antigen and KLH-specific augmenting activity of their extracts in the secondary antibody response. The detailed phenotypic and functional analysis of 1 of these clones, FL10, is reported here. The hybrid was positive for both Thy1.1 and Thy1.2 antigens and possessed the Lyt-1+,2-,3- phenotype. Both VH and Ia determinants were detected on their cell surface. The IA locus was mapped in the I-A subregion, but the Ia specificities were serologically distinct from those of B cell Ia antigen. This was demonstrated by the fact that anti-Ia antiserum preabsorbed with B cells could react with the hybrid cells, whereas none of the monoclonal anti-Ia specific for private and public determinations of Iak could. The extract from the cell line specifically augmented the in vitro secondary antibody response against dinitrophenylated KLH, and this activity was removed by absorption with antigen and conventional anti-Ia antisera. The results indicate that the cell line, FL10, carries Ia antigen unique to the T cell, which is associated with the antigen-specific augmenting molecule.

[Venous Doppler ultrasonography. Methods, indications and limits].

The analysis of velocities by the Doppler effect has limited applications in venous pathology. There are, however, many potential advantages: the introduction of deep venous catheters, early diagnosis of venous thrombosis, studies of venous incompetence, right sided cardiopathies, follow-up of medical and surgical treatment (thrombolytics, venous thrombectomy, bypass surgery, vena cava clips). The results in a series of venous thrombosis are reported: the correlation between Doppler and phlebography was 86,5% in 110 investigations. Doppler sensitivity was 86% and specificity 91%. These results are compared to other reports. A 90% correlation was found with isotope venography in 32 cases in which the permeability of vena cava clips was studied. The sensitivity of doppler in the diagnosis of thrombosis of the clip was 75% and its specificity 100%. The main limitations were the lack of sensitivity in the diagnosis of distal thrombosis and incomplete occlusion (floating thrombi). As a complement to occlusive plethysmography and other non-invasive investigations, Doppler ultrasonography helps determine the indications for phlebography. It is useful in detecting high risk patients and is a sensitive, effective non-invasive method of surveillance in the postoperative period.

[Compared results of flail chests treatments : standard internal pneumatic stabilization, new technics of assisted ventilation, osteosynthesis (author's transl)].

The analysis of 170 cases of flail chests, divide into three groups according to the type of treatment, proves the efficiency of the new technics of assisted ventilation and of the osteosynthesis by Judet clasps. The most significant advantages of those two complementary methods are a lowered mortality rate and a reduced frequency of refractory hypoxy and infectious complications.

[Post-traumatic rupture of the pericardium with cardiac luxation associated with-myocardial infarction, and a rupture of the aortic isthmus (author's transl)].

A case of rupture of the pericardium with cardiac luxation is reported. Violent trauma is necessary to produce such lesions, and this explains the frequency of rapid death before the pericardium rupture is recognised. A definite diagnosis is only possible during operation in one out of two cases that survive. This lesion should be suspected in all cases of chest injury, and complementary examinations, particularly radiological and electrocardiographic, should be requested. Treatment is surgical.

[Arguments for and description of the Doppler venous examination (author's transl)].

Analysis of blood flow rates by Doppler effect on veins has many advantages: deep insertion of venous catheters, diagnosis of deep veins thrombosis, venous insufficiency, right heart cardiopathies, and supervision of medical and surgical treatment of venous disease. The agreement between Doppler examination and phlebography was, in venous thrombosis, 86.5 % for 110 examinations. Sensitivity of Doppler technique is 86%, specificity 91 %. If used to check permeability of vena cava interruption, concordance between Doppler ultrasound and isotopic phlebography was 90 % for 32 cases. The main limitation of the method is a poor sensibility in diagnosis of distal thrombosis and in not entirely occluding clots.

[Embryology, anomalies, dystrophy and pathology of the lower limbs arteries (author's transl)].

The development in the embryo of the lower limbs arteries is made by means of the retrogression of the ischiatic artery, which is initially preponderant, and of the persistence in the popliteal pit of an anastomosis between the original ischiatic axis and the final femoral axis. The tibial arteries originate from one or the other of these axes. The attention is turned to the areas rendered fragile by the complexity of their forming and likely to provoke some topographic anomaly or some dystrophic disease: persistence of the ischiatic artery, trapped popliteal artery, gelatinous dystrophy, Leriche-Fontaine aneurysmal dystrophy, anterior tibial hypoplasia. The repetition of such problems which are non-existent at the upper limbs and are localized to the same arterial segments, especially to the popliteal artery, puts stress upon an eventual dysplasic pathogenesis originating from the embryo.

[Doppler examination in temporal arteritis. Preliminary results (author's transl)].

Nine patients with histologically confirmed Horton's diseases were explored by Doppler ultrasound examination, before and after corticotherapy, of the cervical arteries, temporal arteries (N = 9), and branches of the ophthalmic artery: internal nasal (N = 9), central retinal, and supraorbital (N = 6). Excellent correlations existed between histological findings and results of Doppler examination, concerning the severity of the lesions (80% correlation). Circulatory slowing was evident in the ophthalmic vessels studied, with alterations in the shape of peak velocities in 6 of the 9 cases. Major abnormalities were present in 3 patients; two of them presenting transient or permanent amaurosis. Doppler curves improved after corticotherapy, their shape and amplitude becoming equivalent to those of a control population of the same age. Doppler examinations appear to be of diagnostic value in Horton's disease, as well as being able to predict the risk of ophthalmological complications; more effective surveillance of corticotherapy is possible.
