Pathological studies on esophageal varices treated with injection sclerotherapy.

Injection sclerotherapy is one of the most effective treatments for esophageal varices. To investigate the fate of esophageal varices embolized with a sclerosant (ethanolamine oleate), we have examined at autopsy the esophageal wall of 14 patients with esophageal varices. When the sclerosant was injected into the varices, at first thrombi formed. After two weeks, granulation tissue appeared and the thrombi were gradually replaced. The granulation tissues were organized slowly after 3 months and in the organized granulation tissue, microscopic recanalization of vessels were seen in 8 out of 9 cases. There was no recurrence of the original varices and de novo varices occurred in one patient. These observations indicate that the varices embolized with ethanolamine oleate become organized in three months after the treatment, and accordingly the esophageal varices are cured. It became also clear that recurrence of the original varices did not occur after the organization.

Primary malignant lymphoma of the appendix.

A 70-year-old Japanese woman with primary malignant lymphoma in the appendix was treated. The diagnosis was established after surgery. Histologically, the tumor was malignant lymphoma, lymphocytic, well differentiated, according to the Rappaport's classification which is a good prognostic type of extranodal malignant lymphoma. The patient was treated by ileocecal resection and without radiochemotherapy. The 36-month follow-up revealed neither evidence of recurrence nor metastases.

Isolation and characterization of a blood group active glycopeptide from porcine aorta.

A fucose-rich glycopeptide was prepared from the pronase digest of porcine thoracic aorta by gel-filtration through Sephadex G-100, DEAE-Sephadex A-25 column chromatography and alpha-amylase digestion. This glycopeptide was electrophoretically homogeneous. The large molecular size and chemical composition suggested that this glycopeptide was derived from mucin-type glycoprotein. The results of the beta-elimination reaction indicated that this glycopeptide contained the O-glycosidic linkages between galactosamine and serine/threonine. This glycopeptide exhibited blood group A and H activities. The present study revealed that the porcine thoracic aorta contains a blood group antigen of mucin-type glycoprotein nature.

Characterization of blood group active glycopeptides derived from porcine kidneys.

Sialoglycopeptide fractions were prepared from the pronase digest of porcine kidneys by DEAE-Sephadex A-25 column chromatography and gel-filtration through Sephadex G-100. Their chemical compositions and large molecular size suggested that these glycopeptides were derived from mucin-type glycoprotein(s). The results of the beta-elimination reaction indicated that they have the O-glycosidic linkages between N-acetylgalactosamine and serine/threonine. The glycopeptides exhibited blood group A and H activities. The present study revealed that the porcine kidney contains the blood group antigens of glycoprotein nature.

Expression of hepatitis B virus surface antigen P31 gene in Escherichia coli.

A hepatitis B virus surface antigen (HBsAg) P31-coding DNA was constructed from a DNA fragment of the plasmid pHBr330 containing the entire hepatitis B virus (HBV) adr DNA and a chemically synthesized adaptor. The P31 gene was inserted into an expression vector, pTRP771, having an Escherichia coli tryptophan operon (trp) promoter to give a recombinant plasmid pTRP P31-R. The distance between the Shine-Dalgarno (SD) sequence and the initiation codon of P31 gene was adjusted to 9 bp. The expression level of HBsAg by E. coli 294[pTRP P31-R] was significantly elevated, in contrast to that of HBsAg by E. coli 294[pTRP SS-6]. Western blotting analysis has shown that E. coli[pTRP P31-R] synthesizes a specific polypeptide P31 of about 31 kDal, which reacts with anti-HBsAg antibody. The binding studies with polyalbumins from various species have also suggested that HBsAg P31 specifically binds to polymerized human serum albumin.

A promoter for Bacillus subtilis expression vector.

A 117-bp EcoRI-PstI fragment with strong promoter activity (P1 promoter) was cloned from Bacillus subtilis chromosomal DNA and sequenced. The P1 promoter was shown to contain a putative -35 region (TTTACT) and -10 region (TAGATT), and promotes expression of cloned human interleukin-2 (IL-2) and human interferon-gamma (IFN-gamma) genes in B. subtilis.

The mechanism of exaggerated glucagon response to arginine in diabetes mellitus.

As far as exaggerated arginine-induced glucagon secretion in diabetics is concerned, the authors have shown that both the restoration of blood glucose excursions and physiological insulinemia in response to arginine, obtained from an artificial endocrine pancreas (AEP) could normalize the glucagon secretory responses in diabetes mellitus. To clarify whether or not physiological glycemic excursions and/or plasma insulin profiles contribute to the normalization of the exaggerated glucagon response in diabetes mellitus, the following 4 investigations were conducted on each of 7 non-obese, non-insulin-dependent diabetic (NIDDM), and 8 insulin-dependent diabetic (IDDM) subjects, with the aid of AEP. Arginine was i.v. infused into both diabetic groups (1) in a hyperglycemic state without insulin infusion, (2) in perfect glycemic control with insulin infusion by AEP, (3) in glycemic control with AEP, but with lower plasma insulin profiles (parameters of the insulin infusion algorithm were made smaller than those of (2], (4) in a state where blood glucose levels were clamped at the same levels as obtained in (1) with the aid of glucose infusion controlled by AEP, and where physiological plasma insulin profiles were mimicked by infusing insulin at the same rates used in (2) with a pre-programmable insulin infusion system. The changes in the plasma glucagon (IRG) response in each experiment were compared with those seen in healthy subjects. For both diabetic groups it was found that: in (2) perfect normalization of glucagon response was achieved.(ABSTRACT TRUNCATED AT 250 WORDS)

Cutaneous manifestations of adult T cell leukemia/lymphoma. Report of three different forms.

The clinical and pathologic features of cutaneous lesions observed in three adult T cell leukemia/lymphoma (ATL) patients identified in Taiwan are described. They represent one classical case of ATL and two "smoldering" variants. The classical ATL patient when first seen had numerous erythematous or purpuric papules, nodules, and plaques with or without ulceration. The two "smoldering" cases developed encrusted purpuric plaques with subcutaneous erythematous nodules in one patient and pompholyx-like vesicular eruptions with tumor masses in another patient. The pompholyx-like eruptions have not been described before. Pruritus was the major complaint in two patients. Histopathologic studies revealed pleomorphic infiltration in the classical case and monomorphic infiltration with medium-sized cells in the two "smoldering" variants. Therefore, the cutaneous lesions of ATL are diverse and not pathognomonic. The diagnosis requires a high index of suspicion, detection of circulating characteristic multilobated lymphoid cells with T helper/inducer cell marker, and demonstration of serum antibody against the adult T cell leukemia virus-associated antigen.

Coexisting primary hyperparathyroidism and Albright's hereditary osteodystrophy--an unusual association.

Primary hyperparathyroidism associated with Albright's hereditary osteodystrophy was diagnosed in a 22 year old Japanese woman, the second such case to be reported. Albright's hereditary osteodystrophy (AHO) appears to be associated with a larger number of disorders than the well recognized pseudohypoparathyroidism. AHO and pseudo-pseudohypoparathyroidism are essentially identical.

Involvement of histamine in vascular permeability increase of the rat pleurisy induced by phorbol myristate acetate.

Rat pleurisy was induced by an intrapleural injection of 1 nmol (0.1 ml) of phorbol myristate acetate (PMA). Accumulation of pleural fluid peaked at a volume of 1 ml at 1 hr, and exudation rate peaked at 30 min when measured by the amount of dye leaked into the pleural cavity during 20 min. Histamine contents decreased in the pleural cell pellets markedly at 30 min and increased in the supernatant fluid simultaneously, reflecting histamine release that occurred in 30 min after PMA. Pretreatment with mepyramine maleate or methysergide 30 min prior to the PMA-injection suppressed the fluid volume significantly. The result indicates that there are involvements of 5-hydroxytryptamine and the H1-receptor of histamine in the vascular permeability increase at the early stage of PMA-pleurisy.

Ultrastructure of well-differentiated adenocarcinomas of the lung with special reference to bronchioloalveolar carcinoma.

The cytologic phenotypes of 20 well-differentiated pulmonary adenocarcinomas were determined by electron microscopy. On examination of more than 100 cells in each case, the tumors were classified according to the predominant cell types. Nine cases (45%) were of mucous cell type, further divided into 7 cases of bronchial surface epithelial cell type, 1 case of bronchial gland cell type, and 1 case of metaplastic bronchiolar goblet cell type. The remainder included 5 cases (25%) of Clara cell type, 2 cases (10%) of type II cell type, and 4 cases (20%) of mixed cell type. The predominant histologic pattern by light microscopy was "typically" bronchioloalveolar (Manning et al.'s type 1) in the metaplastic goblet cell tumor and papillary in most Clara cell-type tumors, while it was glandular in bronchial surface and bronchial gland cell types, although variable in type II cell or mixed cell type. Therefore, bronchioloalveolar carcinomas, when histologically defined inclusive of papillary tumors, present cytologic phenotypes also related to the bronchioloalveolar epithelium, i.e., metaplastic goblet or Clara or type II cell subtypes, which is in accordance with some previous reports. These tumors could be distinguished from the other (glandular) adenocarcinomas that show primarily bronchial mucous cell differentiation.
