The pharmacokinetics and maternal and neonatal effects of epidural lidocaine in preeclampsia.

The pharmacokinetics and maternal and neonatal effects of epidural lidocaine were compared in ten preeclamptic and five normotensive women undergoing cesarean section at 36-40 weeks of gestation. Lumbar epidural anesthesia was achieved using 15-20 ml of 2% lidocaine without epinephrine. Serial venous samples for lidocaine levels were drawn from all the mothers during the procedure and up to 6 hr after the initial injection. Umbilical venous and arterial samples were drawn at delivery for measurement of neonatal acid-base status and lidocaine levels. There were no significant differences between normotensive and preeclamptic patients in the total dose of lidocaine, peak maternal plasma concentration, volume of distribution, maternal elimination half-life and umbilical vein/maternal vein ratios. The calculated area under the concentration time curve in preeclamptic patients (18.5 +/- 4.7 micrograms X hr X ml-1) was significantly greater than in normotensive mothers (14.1 +/- 1.3 micrograms X hr X ml-1) (P less than 0.02). Total maternal body clearance in preeclamptic patients (24.5 +/- 7.1 L/hr) was significantly lower than in normotensives (31.1 +/- 4.4 L/hr) (P less than 0.05). Neonatal outcome as evaluated by Apgar scores, umbilical arterial and venous blood gas tensions, umbilical vein/maternal vein ratios, and early neonatal neurobehavior scores at 4 hr and 24 hr after birth were similar in the two groups. The results indicate that the total maternal body clearance of lidocaine is prolonged in preeclampsia, and repeated administration of lidocaine can result in higher blood levels than in normotensive parturients.

Some functional properties of oilseed proteins.

Oilseeds have potential food uses because of their high protein content. Besides, these proteins when added to a type of foods, supply desirable functional properties, such as whipping capacity and viscosity, emulsification and water and oil holding capacities. Rapeseed and soybean protein isolates were found to possess whipping capacity followed by those of sunflower, peanut, sesame, cottonseed and safflower. The addition of sugar improved the whipping properties of oilseed proteins. The whipping capacity of oilseed proteins decreased due to heating at 100 degrees C for time of 15 to 60 min. Soybean protein had the highest emulsifying capacity compared with the other oilseed proteins. The heated oilseed proteins had emulsification properties similar to or better than the control. Glandless cottonseed protein had high water and oil holding capacities. The water holding capacity of oilseed proteins decreased gradually as the duration of heating at 100 degrees C was increased. On the other hand the heated oilseed proteins had oil holding capacities similar to or better than unheated proteins.

Tuberculosis of the orbit.

Two cases of orbital tuberculosis in white Canadians are reported. The two patients did not suffer from pulmonary tuberculosis, the orbital disease was associated with tuberculosis sinusitis in the first case and blood-borne from constrictive tuberculous pericarditis in the second case. Acid-fast bacilli are difficult to detect in the pathological specimens and the diagnosis is usually based on the following: (1) the positive tuberculin skin test; (2) the caseating granulomatous inflammatory lesion on histopathology, which is highly suggestive of active tuberculosis; (3) the positive culture for Mycobacterium tuberculosis if the specimens are obtained early in the course of the disease; and (4) the complete resolution of the disease with the specific antituberculous medications.

Posterior lenticonus.

Two patients with posterior lenticonus; one was unilateral and the other was bilateral. The second patient had bilateral cataract extractions and the lenses were examined histopathologically. Postoperatively she did not have any amblyopia. The purpose of this presentation is to illustrate two points from the clinicopathological findings: The pathogenesis of the anomaly is mainly due to inherited thinning and weakness of the central part of the posterior lens capsule. We suggest that the posterior lenticonus without complicated cataract is not a main cause of profound amblyopia. The amblyopia is dependent on the age at which the cataract develops.

Abnormal blood vessels on the optic disc.

Twenty-five patients with abnormal blood vessels on the optic nerve head were examined both clinically and with fluorescein angiography to determine the diagnostic significance of these abnormal vessels. Six patients had congenital abnormal vessels--prepapillary or optociliary arteries or veins, or vessels associated with congenital anomalies of the disc. Nineteen patients had acquired abnormal vessels that were associated with retinal vein occlusion, chronic glaucoma, chronic papilledema, meningioma or vasculitis. The clinical and fluorescein angiographic features that differentiated these groups are discussed in detail.

Meningioma and the ophthalmologist. A review of 80 cases.

We reviewed the charts of all patients with pathologically proven meningiomas who were admitted to the Montreal General Hospital between 1960 and 1977. Of the 80 patients reviewed, we found approximately one third of them had ophthalmological symptoms of which visual loss, field defect, and diplopia were the most common. Most of these patients also had neurological symptoms, but they were often nonspecific. One half of the patients presented with chronic symptomatology such as headache, mental change, and visual loss. One third of the patients presented acutely with seizures, hemiplegia, or dysphasia. Of the investigations done, the angiogram and brain scan were most often diagnostic, while the skull x-ray and EEG were often normal. Meningioma of the sphenoidal ridge, parasellar area, and occiput most often produced visual deficits. In almost one half of these patients, the visual deficit was initially misdiagnosed.

Combined surgery and cryotherapy for diffuse malignant melanoma of the conjunctiva.

Two patients who had refused exenteration for widespread conjunctival melanoma were treated by a combination of cryotherapy and surgical excisions. Cryotherapy was delivered only to the extensive areas of flat intraepithelial melanocytic proliferation (precancerous melanosis), while the surgical excisions were performed on the focal nodules, representing localized invasive melanoma. In our two patients, the invasive nodules measured, respectively, 1.2 and 1.5 mm in greatest thickness, placing them in a low to borderline risk group for metastasis. Repeated cryoapplicatons were required to control the widespread flat intraepithelial disease. The conjunctiva tolerated these procedures well because the substantia propria is not sacrificed as it must be in surgical conjunctivectomy, allowing comparatively normal reepithelialization to occur from adjacent zones, after the treated epithelium containing the melanocytes sloughs. No evidence of invasive melanoma (cancerous melanosis) has developed in any of the cryotreated areas of intraepithelial disease (precancerous melanosis); sequential biopsies have established that the atypical melanocytes have disappeared from the epithelium.

Eccrine sweat gland carcinoma of the eyelid with orbital involvement.

A 70-year-old man had a sweat gland carcinoma in his right upper eyelid that was slowly growing, but it clinically remained confined to the lid tissues for a period of 25 years. The tumor ultimately extended deeper into the orbit and invaded the sclera, the optic nerve sheath, and the ethmoidal sinuses. Histopathologic examination disclosed malignant features with a variety of cellular patterns. The results of electron microscopic studies confirmed the origin of the tumor from the eccrine sweat glands.

Acquired retinal macroaneurysms.

Five patients with retinal macroaneurysms are described. The aneurysms may be an incidental finding or they may cause visual loss from retinal, preretinal and vitreous hemorrhages or from macular edema or exudation. The prognosis is good in those cases presenting with hemorrhage but is poorer in those with macular problems. Acquired aneurysms are found in older patients who are hypertensive or have vascular disease. We believe that retinal emboli play an important part in their development.

Differential diagnosis of early opacification of the portal vein and its tributaries during arteriography.

Twenty-two cases with communication of an artery and the portal vein or one of its tributaries are discussed. Four conditions in which relatively significant arterio-portal shunts may exist can be differentiated: (1) angiodysplasias or arteriovenous malformations, (2) cirrhosis of the liver and inflammatory lesions, (3) traumatic and postoperative lesions, and (4) benign and malignant tumors. The significance of the portal vein's early opacification during arteriographic examinations of the abdominal organs is discussed, and the findings are compared to those reported in the literature.

[Mural thrombus in the left ventricle. Frequency and radiological appearances (author's transl)].

Two hundred angiocardiograms were analysed. Twenty-two patients had mural intraventricular thrombi. The radiological appearances are described. Systemic emboli occurred in four patients. Elucidation of the source of emboli following the demonstration of peripheral embolic occlusions by means of an angiocardiogram of the left ventricle is not difficult and should be performed as an additional examination.

[The angiographic and morphologic features of angiomyolipoma of the kidney (author's transl)].

The angiographic and morphologic findings of angiomyolipomas of the kidney are described in three patients. These benign tumours contain three types of vessel in addition to fat and smooth muscle. One vessel type is thick-walled with fibrosis of its walls and no internal elastic lamina. The second type consists of sinuses with considerable variations in calibre. The third type resembles capillaries and is found in the muscular parts of the tumour. Because of the abnormal structure of their walls, these vessels are easily damaged and may cause massive bleeding.
