A dominant activating mutation in the effector region of RAS abolishes IRA2 sensitivity.

Previously described mutations in RAS genes that cause a dominant activated phenotype affect the intrinsic biochemical properties of RAS proteins, either decreasing the intrinsic GTPase or reducing the affinity for guanine nucleotides. In this report, we describe a novel activating mutation in the RAS2 gene of Saccharomyces cerevisiae that does not alter intrinsic biochemical properties of the mutant RAS2 protein. Rather, this mutation, RAS2-P41S (proline 41 to serine), which lies in the effector region of RAS, is shown to abolish the ability of the IRA2 protein to stimulate the GTPase activity of the mutant RAS protein. This mutation also modestly reduced the ability of the mutant protein to stimulate the target adenylate cyclase in an in vitro assay, although in vivo the phenotypes it induced suggest that it retains potency in stimulation of adenylate cyclase. Our results demonstrate that although the effector region of RAS appears to be important for interaction with both target effector and negative regulators of RAS, it is possible to eliminate negative regulator responsiveness and retain potency in effector stimulation.

The interferon system in carcinoma of the cervix. Effect of radiation and chemotherapy.

Thirteen patients with advanced carcinoma of the cervix were studied for parameters of the interferon system compared with 40 age-matched and sex-matched controls. All patients had measurable serum interferon levels; controls did not. All patients had non-antibody-type interferon-inhibitory activity, and controls had none. Interferon-synthesizing potential was higher in controls than in patients. After successful radiation and chemotherapy, these parameters normalized in the patients. No change was seen in one patient who did not respond to therapy.

Cryptosporidiosis in children in Gaza.

A prospective survey of children admitted with gastro-enteritis to the Nasser Children's Hospital, Gaza revealed that 19% were excreting cryptosporidium, a significantly (p less than 0.001) greater percentage than that (7%) observed in children admitted for other reasons. Detection of cryptosporidium decreased when the change from hot dry to colder wetter weather occurred. Although Salmonella spp were isolated more frequently than cryptosporidium in children with diarrhoea (20% of cases), there was no statistically significant association between excretion of Salmonella spp and gastro-enteritis. A follow-up study of a cohort of the children with cryptosporidiosis indicated that over three-quarters were dehydrated and all were below their expected weight-for-age. There was a statistically significant association between cryptosporidium gastro-enteritis and evidence of respiratory tract infection.

Association of parents' education and fathers' occupation with prevalence of diarrhoea among children less than five years of age in Saudi Arabia.

A nationwide survey was undertaken in April 1987 in the Kingdom of Saudi Arabia and a questionnaire was administered to 1,656 mothers having children less than 5-years of age. The number of mothers with 1-3 children less than 5-years old was 1,535 and with more than 3 children was 129. The literacy rate of fathers was 70% and of mothers was 26.3%. Incidence of diarrhoea was significantly associated with fathers' occupation (p = 0.0011) but no significant association was found between the incidence of diarrhoea and parents' education.

Limb salvage in soft tissue sarcomas with selective combination of modalities.

One hundred and seventy-one consecutive patients with soft tissue sarcomas were treated in the period 1977-1986. Of 144 patients with extremity sarcomas, only eight (6%) were managed with amputation. The overall estimated 5-year survival rate is 64%, and that for patients with extremity tumors is 71%. The 5-year local recurrence rate in extremity sarcomas was 6% for patients with minimum surgical margins 2 cm or greater and no further local therapy, and 6% for those with narrower surgical margins and adjuvant postoperative radiation; 80 patients (56%) were in the former group and 64 (44%) in the latter. With a selective combination of modalities, limb salvage can now be practiced in 94% of the patients with acceptable local control and survival rates.

A pattern recognition study of acyclic ureide anticonvulsants.

A pattern recognition structure-activity study employing topological, geometric and physicochemical descriptors was performed on 27 acyclic ureide anticonvulsants. Twelve numerical descriptors were used as variables in a discriminant function analysis which categorized the ureide analogues with respect to their bioactivity. The results from the discriminant function analysis were interpreted to support a model for antiepileptic activity.

Segmental H-reflex studies in upper and lower limbs of patients with radiculopathy.

The H-reflex was recorded from the flexor carpi radialis, vastus medialis, and soleus muscles in patients with radiculopathy at C7, L4, and S1 roots. Reflex parameters were compared to normal standards and correlated with other electrophysiologic (eg, electromyography) and clinical evaluations, as well as magnetic resonance imaging scan reports. The H-reflex had significantly smaller peak-to-peak amplitude and longer latency. The stimulus threshold for eliciting the reflexes was substantially higher than normal standards, and the reflex was polyphasic in most patients. A strong correlation was recorded between the pathologic changes in reflex parameters and electrophysiologic and clinical findings of different segmental lesions. Magnetic resonance imaging reports showed moderate correlation with the H-reflex changes. A test of specificity showed that soleus and vastus medialis H-reflexes were 100% specific for lumbosacral segments; flexor carpi radialis was 90% specific for cervical spinal segments. These results indicate that flexor carpi radialis, vastus medialis, and soleus H-reflexes are useful and valid methods for testing C7, L4, and S1 radiculopathy.

Gastric epithelioid leiomyoma presenting with spontaneous haemoperitoneum.

Gastric epithelioid leiomyomatous (leiomyoblastomas) tumours primarily affect middle-aged men and usually present with upper gastro-intestinal bleeding, manifesting as haematemesia and/or melaena. This report documents a case of gastric epitheliod leiomyoma presenting with spontaneous haemoperitoneum. Its possible relationship with haemangiopericytoma and glomus tumour is discussed.

Segmental H-reflex studies in upper and lower limbs of healthy subjects.

The H-reflex has been recorded from the flexor carpi radialis muscle in the upper limb and vastus medialis (quadriceps) muscle in the lower limb in relaxed healthy subjects. The characteristics of these reflexes have been compared with those of the soleus H-reflexes, recorded with the same technique, and similar behavior has been reported. The techniques and pitfalls of recording these segmental reflexes and their clinical and neurophysiologic significance are discussed in light of the relevant literature. These segmental reflexes will be useful in studying conduction and integrity of C7, L4, and S1 spinal segments.

Hydroxyurea plus pelvic irradiation versus placebo plus pelvic irradiation in nonsurgically staged stage IIIB cervical cancer.

Twenty-five patients with FIGO stage IIIB carcinoma of the cervix were entered into a prospective, double-blinded randomized study to evaluate the possible radiation potentiating properties (i.e., improved survival) of the S phase cell cycle specific inhibitor of DNA synthesis hydroxyurea (H). In contrast to our previous randomized trial of patients with FIGO stage IIIB carcinoma of the uterine cervix who were documented to be without paraaortic lymph node metastasis by pretherapy staging paraaortic lymphadenectomy [Piver et al.: J Surg Oncol 35: 129-134, 1987], these patients either refused surgical staging or, because of medical conditions, could not undergo pretherapy paraaortic lymphadenectomy. Patients were to receive 6,000 cGy/6 weeks to the pelvis by megavoltage radiation therapy followed by 2,500 cGy to point A by intracavitary and vaginal radium/cesium. The median pelvic cGy for the Hu patients was 5,967 cGy and for the placebo patients 6,013 cGy. Leukopenia (WBC less than 2,500/mm3) significantly increased in patients given hydroxyurea as compared with those given placebo (P less than 0.0001). There was no statistically significant difference relative to anemia, thrombocytopenia, radiation skin reactions, diarrhea, or radiation induced complications requiring surgical correction. The estimated 5 year disease-free interval was 54% for the hydroxyurea patients (median not reached) and 18% for the placebo patients with a median of 11 months. Because of these results, those of our previous reports, and those of the Gynecologic Oncology Group, we believe that hydroxyruea plus pelvic radiation should be the standard against which new potential radiation sensitizers are tested.

African histoplasmosis masquerading as carcinoma of the colon. Report of a case and review of literature.

A case of African histoplasmosis primarily affecting the large bowel--a very unusual manifestation--is reported. In the absence of classical clinical features, pathognomonic of large bowel histoplasmosis, the macroscopic appearance at surgery was confused with large-bowel cancer. The diagnosis was, therefore, retrospective and based on demonstration of large yeast cells of Histoplasma capsulatum var. duboisii in tissue sections of the lesions.

Rhinosporidiosis in a Nigerian.

A case of rhinosporidiosis in a Hausa man from northern Nigeria is reported. This apparently constitutes the first case recorded from Nigeria.

Chemical and nutritional assessment of sweetpotato proteins.

The solubility of sweetpotato nitrogen increased outside the pH range 3-6 for the pulp, and 2-5 for the peel of the two varieties namely, "Abees" and "Giza 69". The minimum nitrogen extractability occurred between the pH range 3-4, and 4-5 for the peel and pulp, respectively. The sweetpotato protein isolates were prepared separately from both peel and pulp of the two varieties. The former had the highest values of carbohydrates and ash while the latter had the highest values of protein and fat. The in-vitro digestibility of sweetpotato proteins and casein by pepsin-pancreatin was studied. Casein was more easily digested than the proteins of sweetpotatoes, whose digestibilities were higher in the pulp than in the peel of the two varieties. No trypsin inhibitor activity was detected in peel and pulp of both varieties under test. The effect of the variety of sweetpotatoes on the protein patterns was studied using the polyacrylamide gel electrophoresis and varietal specific patterns were obtained.

Selective combination of modalities in soft tissue sarcomas: limb salvage and survival.

One hundred fifty-two consecutive patients with soft tissue sarcomas were operated in the period 1977 through 1985. Eighty-seven patients with minimum resection margin of 2 cm or greater had no further local therapy, whereas 65 patients with minimum margin less than 2 cm had adjuvant postoperative radiation. Of 121 patients with extremity sarcomas, only 5 (4%) were managed with amputation. The overall 5-year survival rate is 58%, and for patients with extremity tumors, 67%. The 5-year local recurrence rate in extremity sarcomas was 10% for patients with minimum surgical margins 2 cm or greater and no further local therapy, and 6% for those with lesser surgical margins and adjuvant postoperative radiation. With selective combination of modalities limb salvage can now be practiced in 96% of the patients with acceptable local control and survival rates.

[Atresia of the aortic isthmus from the radiological point of view].

The most important angiographic features of atresia of the aortic isthmus are illustrated by three patients. The first patient to have reached an advanced age despite atresia of the aortic isthmus is reported and the diagnosis was confirmed at autopsy. The haemodynamic effects, the age at operation and the incidence of other associated cardiovascular malformations are discussed with reference to the literature.

The pharmacokinetics and maternal and neonatal effects of epidural lidocaine in preeclampsia.

The pharmacokinetics and maternal and neonatal effects of epidural lidocaine were compared in ten preeclamptic and five normotensive women undergoing cesarean section at 36-40 weeks of gestation. Lumbar epidural anesthesia was achieved using 15-20 ml of 2% lidocaine without epinephrine. Serial venous samples for lidocaine levels were drawn from all the mothers during the procedure and up to 6 hr after the initial injection. Umbilical venous and arterial samples were drawn at delivery for measurement of neonatal acid-base status and lidocaine levels. There were no significant differences between normotensive and preeclamptic patients in the total dose of lidocaine, peak maternal plasma concentration, volume of distribution, maternal elimination half-life and umbilical vein/maternal vein ratios. The calculated area under the concentration time curve in preeclamptic patients (18.5 +/- 4.7 micrograms X hr X ml-1) was significantly greater than in normotensive mothers (14.1 +/- 1.3 micrograms X hr X ml-1) (P less than 0.02). Total maternal body clearance in preeclamptic patients (24.5 +/- 7.1 L/hr) was significantly lower than in normotensives (31.1 +/- 4.4 L/hr) (P less than 0.05). Neonatal outcome as evaluated by Apgar scores, umbilical arterial and venous blood gas tensions, umbilical vein/maternal vein ratios, and early neonatal neurobehavior scores at 4 hr and 24 hr after birth were similar in the two groups. The results indicate that the total maternal body clearance of lidocaine is prolonged in preeclampsia, and repeated administration of lidocaine can result in higher blood levels than in normotensive parturients.

Some functional properties of oilseed proteins.

Oilseeds have potential food uses because of their high protein content. Besides, these proteins when added to a type of foods, supply desirable functional properties, such as whipping capacity and viscosity, emulsification and water and oil holding capacities. Rapeseed and soybean protein isolates were found to possess whipping capacity followed by those of sunflower, peanut, sesame, cottonseed and safflower. The addition of sugar improved the whipping properties of oilseed proteins. The whipping capacity of oilseed proteins decreased due to heating at 100 degrees C for time of 15 to 60 min. Soybean protein had the highest emulsifying capacity compared with the other oilseed proteins. The heated oilseed proteins had emulsification properties similar to or better than the control. Glandless cottonseed protein had high water and oil holding capacities. The water holding capacity of oilseed proteins decreased gradually as the duration of heating at 100 degrees C was increased. On the other hand the heated oilseed proteins had oil holding capacities similar to or better than unheated proteins.
