Dietary intakes and adipose tissue levels of linoleic acid in peptic ulcer disease.

Adipose tissue levels of linoleic acid were determined from biopsies of subcutaneous abdominal fat of normal healthy controls (n 40) and from two patient groups with endoscopically evaluated non-ulcer dyspepsia (n 40) or peptic ulcer disease (n 38). The level (g/100 g) of adipose tissue linoleic acid in the normal healthy controls (15.0 (SD 4.1] was significantly (P less than 0.05) greater than that in patients with non-ulcer dyspepsia (12.8 (SD 3.5] and in patients with peptic ulcer disease (11.7 (SD 2.7]. A dietary history revealed a lower intake of linoleic acid and a significantly (P less than 0.05) lower intake of dietary fibre (g/d) for both the non-ulcer dyspepsia (15.9 (SD 6.2] and peptic ulcer disease (15.2 (SD 7.8] patients compared with normal healthy controls (20.2 (SD 11.2]. Adipose tissue linoleic acid tended to increase with indices of increasing socioeconomic status, although the differences between patient and controls were not confounded by socioeconomic status. Patients with dyspepsia reported more foods causing symptoms (onion, fried foods, alcohol, citrus fruits and spices) and more foods giving relief (milk, bread) compared with control orthopaedic patients.

Children's knowledge of thematic and taxonomic relations at two years of age.

An interesting conflict exists in the developmental literature concerning the strength of thematic versus taxonomic associations. "Dog-doghouse" is a thematic match, while "dog-horse" is a taxonomic match. This study compared the ability of children to identify both types of associations at 2 points during the third year of life: 26 months (N = 15) and 34 months (N = 24). A reinforced match-to-sample technique was used in which half of the matches were thematic and the other half were taxonomic. Both age groups were able to identify thematic matches, although children near 3 years of age were able to recognize a wider range of thematic associations than children near 2 years of age. The 26-month-olds were unable to identify taxonomic matches that had no perceptual basis, but the 34-month-olds were successful at this task; the 34-month-olds appeared to have a more general understanding of categories that was less tied to perceptual features.

Mapping and regulation of the pifC promoter of the F plasmid.

The pif region of the F plasmid, which causes abortive infection of Escherichia coli by T7 bacteriophage, is autogenously controlled by the product of the pifC gene. Here we describe the identification of the pif operon promoter by S1-nuclease mapping, and show that it is autoregulated at the transcriptional level and that its activity is modulated by integration host factor.

Prognostic cardiac catheterization variables in survivors of acute myocardial infarction: a five year prospective study.

The prognostic variables from predischarge coronary angiography and left ventriculography in survivors of acute myocardial infarction during the years 1974 to 1978 were evaluated in 143 patients (less than or equal to 66 years of age) with documented myocardial infarction who were then followed up prospectively for 5 years. One half of the study population had triple vessel coronary disease (greater than or equal to 50% stenosis). However, only 7% of patients had severely depressed left ventricular function with an ejection fraction less than or equal to 29%. Evaluation of the contribution of many clinical and angiographic variables to a first cardiac event (death, nonfatal reinfarction or coronary artery bypass surgery) was considered with Kaplan-Meier actuarial curves and multivariate Cox's hazard function analysis. A risk segment was defined as an area of contracting myocardium supplied by a coronary artery with a greater than 50% stenosis. Multivariate analysis demonstrated that right plus left anterior descending coronary artery stenoses (p less than 0.01), ejection fraction (p less than 0.01) and the presence of risk segments (p less than 0.05) were significant predictors of outcome. Furthermore, on separate multivariate analyses, the angiographic variables added significantly to the clinical variables to predict cardiac events over 5 years of follow-up. Therefore, in survivors of acute myocardial infarction who undergo cardiac catheterization, additive prognostic information is obtained that can be used to stratify risk over 5 years.

The long-term outcome of hepatitis B infection in hemodialysis patients.

Little information on the long-term outcome of hepatitis B virus (HBV) infection in hemodialysis patients is available. We studied 49 hemodialysis patients, seen at three centers between 1969 and 1985, who developed HBV infection. Patients were studied retrospectively and followed for up to 10 years (mean 52 +/- 5 months). Only 20% (n = 10) of patients converted to hepatitis B surface antigen (HBsAg) negative, the majority of whom did so within 6 months of becoming HBsAg positive. Twenty-nine percent (n = 14) of patients developed chronic elevation of liver enzymes which remitted in one patient. Only one patient died from liver disease. We conclude that HBV infection in hemodialysis patients more often results in persistent antigenemia and chronic elevation of liver enzymes than is the case in patients without kidney disease. However, the risk of death from liver disease is low.

Distal esophageal ballooning following Heller myotomy.

Eccentric ballooning of the distal esophagus was observed radiographically in 11 of 23 patients (48%) who underwent Heller myotomy for achalasia. While, to the authors' knowledge, ballooning at the site of myotomy incision has not been described previously in the radiologic literature, it should be recognized as a normal and frequent postoperative finding. Radiologists should be aware of this finding so that it is not mistaken for postoperative abnormalities following esophagomyotomy or an epiphrenic diverticulum.

Role of perceptual and conceptual similarity in category matching at age two years.

Perceptual and categorical similarity were varied independently in a concept-matching task administered to 26-month-old children (N = 25). 44 test sets were assembled. Each set included a 3-dimensional standard and an array of 4 pictures, one of which was a member of the same basic or superordinate category as the standard. On each trial, placing the standard on the matching picture resulted in activation of 1 of 12 mechanical displays. The test sets varied in the perceptual and categorical resemblance of the standard to the match and to the nonmatches, as determined by adults' ratings. Perceptual similarity proved to be the primary determinant of difficulty level. When perceptual resemblance of the standard and the match were equated, superordinate and basic matches were equally difficult. When perceptual resemblance was minimal, most children were unable to recognize matches at either the basic or superordinate level.

A novel role for interleukin 2 in antibody responses: down-regulation of T helper cell activation.

This report provides new insights into the role of the T cell growth factor interleukin 2 (IL 2) for the regulation of antibody responses. Evidence is presented that IL 2 down-regulates T helper cell (Thc) activation but not Thc effector function, i.e., Thc-B cooperation. Thus, reagents which block the IL 2 pathway, e.g., cyclosporin A (CsA) or IL 2 receptor monoclonal antibodies (IL 2R Mab) enhanced Thc activation although T cell proliferation was blocked. In contrast, CsA or IL 2R Mab blocked Thc-B cooperation, suggesting that IL 2 is required for this step. The regulatory role of IL 2 was reconfirmed by the addition of exogenous IL 2 into the cultures which reversed the enhancing or blocking effect of CsA.

Behavioral trait associated with conditioned immunity.

Inbred strains of mice were tested for their activity in an open field. Animals selected for high or low activity ("tails" of the normal distribution) were further inbred through nine generations (brother x sister) with further selection at each generation. Reciprocal skin grafts between the two groups were performed to ensure that little/no genetic drift occurred. Using a cyclophosphamide:saccharin conditioning paradigm (R. Ader & N. Cohen (1975) Psychosom. Med. 37, 333-342) we show that mice preselected for high activity in an open field were those in which it was most easy to demonstrate conditioned immunosuppression. There was no difference in the conditionability of the two groups as assessed by taste aversion. By use of a cross-fostering design we conclude that immunological conditioning (in adult mice) can be affected by a characteristic of the nursing mother which is associated with activity in an open-field trial.

Hepatitis B disease in dialysis and transplant patients. Further epidemiologic and serologic studies.

As hepatitis B virus (HBV) infection in renal transplant recipients is associated with a high incidence of progressive liver disease it may be inadvisable to transplant hemodialysis patients with hepatitis B antigenemia. To determine the natural history of HBV disease in hemodialysis patients, all 49 patients on hemodialysis treatment for at least 1 year, at 3 centers, who developed circulating hepatitis B surface antigen (HBsAG), were studied. A subgroup of these patients (n = 31) aged less than or equal to 50 years, followed for 55 +/- 6 months after detection of HBsAg was compared with 22 previously studied HBsAg-positive transplant patients followed for 81 +/- 9 months. Significantly more transplant patients developed chronic hepatitis defined biochemically (P less than .001) and none of the transplant patients became HBsAg-negative compared with 19% of the hemodialysis group. Taking difference in follow-up into account, mortality was significantly higher in the transplant recipients (P less than .005) following development of HBsAg antigenemia, and the mortality difference was attributable to deaths from liver disease. A total of 36 serum samples from 14 of the 22 HBsAg-positive renal transplant recipients was analyzed for hepatitis B e antigen (HBeAg), antibody to hepatitis D virus (anti-HD), and hepatitis B virus deoxyribonucleic acid (HBVDNA) concentration. No serum sample was anti-HD-positive. Twelve of the 14 patients were HBeAg-positive. Five patients became HBeAg-negative, 3 of whom developed aggressive liver disease. One HBeAg-negative anti-HBe-positive patient had progression of liver disease from asymptomatic carrier status to chronic active hepatitis (CAH). Of 14 patients, 9 developed progressive CAH. HBVDNA concentration was not diagnostic of disease activity on liver biopsy. However only 1 sample of 10 measured in 5 patients with nonprogressive disease had a level greater than 100 pg/L, compared with 9 of 17 in the group who progressed to CAH. During the interval when the liver histology progressed from asymptomatic carriage or chronic persistent hepatitis (CPH) to CAH, the HBVDNA concentration increased by greater than 10 times baseline in 4 of 5 patients who had serial samples, whereas this did not occur in 4 patients with nonprogressive disease. We conclude that the long-term outcome of hepatitis B infection in transplant recipients is significantly worse than in hemodialysis patients. Therefore it may be inadvisable to transplant HBsAg-positive hemodialysis patients.(ABSTRACT TRUNCATED AT 400 WORDS)

A system of workload measurement for clinical dietitians.

Although the role of the clinical dietitian has undergone dramatic change in recent years, standards for staffing patterns that were developed in 1935 and often still in use. The need for workload measurement systems to quantify the daily activities performed by clinical dietitians led to the development of a new system at the University of Alberta Hospitals in 1984. The workload of clinical dietitians was divided into eight major categories and further divided into 60 activities. A one- or two-week study was conducted four times over an 18-month period. The results indicated that 44 to 54% of the dietitians' time was spent in activities involving direct patient care and team approach. The system has enabled this institution to quantify the workload of its clinical dietitians and may provide a vehicle to measure the impact of major changes on that workload. Since the system covers all possible activities of the clinical dietitian, it may be applicable for use in other institutions. In view of rising health care costs, hospitals must make greater efforts to improve the efficiency of their operations while maintaining the quality of health care services provided to patients. This issue, as it applies to clinical dietetics, was recently addressed by the Nutrition and Food Services Department at the University of Alberta Hospitals.

D protein of miniF plasmid acts as a repressor of transcription and as a site-specific resolvase.

Two activities of the D protein of the miniF plasmid have been found. Divergent promoters in ori-1 ("primary" replicative origin) of miniF are both repressed in cells which produce D protein. The mobilization of plasmids containing the ori-1 region by the F conjugation system is also repressed by D protein. In the former case D appears to act as a transcriptional repressor, whereas in the latter case D protein acts by resolving cointegrates of F and the mobilized plasmid. D protein resolves dimers whose monomer units contain the rfsF sequence needed for recA-independent, site-specific recombination of F. The nucleotide sequence of the D gene was determined. The D gene region contains two oppositely-oriented open reading frames which have the same reading phase and substantially overlap. Transposon insertion mutants were used to show that the gene for D protein occupies the top-strand (left-to-right) open reading frame.
