Neurofibromatosis in pregnancy. Four case reports and review of the literature.

Neurofibromatosis (Recklinghausen's disease) is an autosomal dominant disorder that affects multiple organ systems. The disease usually manifests itself during puberty although it occasionally occurs during infancy and childhood. Pregnancy seems to exacerbate the condition. Four cases of neurofibromatosis during pregnancy are presented. Three of the patients had uneventful pregnancies and deliveries, and the fourth had severe thrombocytopenia. This complication has not previously been reported in association with neurofibromatosis in pregnancy. By close supervision of pregnant patients with neurofibromatosis, most complications can be avoided.

[Digestive symptoms of scleroderma (author's transl)].

The authors successively study the oral, peri-oral, esophageal, intestinal and anal manifestations of systemic scleroderma. They point out that dental, salivary, and esophageal involvement is of great interest for diagnosis and prognosis. They particularly describe the malabsorption syndrome including anatomical and ultrastructural lesions of the smooth muscle fibers. They present an original study of arrector pili in clinically and histologically non-involved skin of patients undergoing scleroderma. Finally they state that any suspected or diagnosed scleroderma needs a digestive check-up as follows: radiologic examination of the teeth, biopsy of the minor salivary glands of the lower lip, radiologic study of esophageal motility and radioactive C 14 glycocholate test.

[Lymphography and fine needle puncture biopsy in the evaluation of lymph node involvement. Apropos of 207 cases].

Percutaneous fine needle puncture biopsy was performed on lymph nodes taking up contrast during lymphography in 207 patients with various disorders: urogenital (174 cases), gynecologic (13 cases), hematologic (17), others (3). At the present time, in fact, and simultaneously with the development of physical methods of diagnostic exploration (ultrasonography, computed tomography) needle puncture biopsy has become of increasing importance to confirm a suspected diagnosis. Needle puncture biopsy under television control is a simple, painless and practically nontraumatic procedure. When performed under precise conditions and a strict methodology (routine sampling as a function of the primary glandular chains), it establishes the correct diagnosis in a large number of lymphographies, enabling a rapid and more precise evaluation of the degree of extension and thus a better therapeutic choice.

Carotid cross-over bypass. Cerebral revascularization after ligation of common carotid artery.

A rationale is presented for extra-anatomic carotid cross-over bypass to maintain or restore blood flow to the internal carotid artery distal to sites of disruption and ligation of a common carotid artery. Anatomic evidence indicates that the attachment of the carotid sheath to the hyoid bone is a barrier to spread of infection cephalad to that level. A patient with infected and disrupted right common carotid artery associated with an esophageal fistula was treated by double ligation of the artery. Contralateral hemiplegia 48 hours later forced consideration of cerebral revascularization. Left carotid angiography demonstrated patent cerebral vessels on the right, with retrograde filling of the right internal carotid artery to the bifurcation. These findings were interpreted as consistent with technical feasibility of external carotid to external carotid cross-over vein bypass in a suprahyoid location, avoiding reconstruction in an infected area and resulting in prompt recovery of function.

[Bone lesions in micromegakaryoblastic leukemia].

The authors report four cases of skeletal lesions associated with acute megakaryoblastic leukemia. In this particular form of the disease, the periostal appositions are more frequent and more severe than in other types of acute leukemia. A very marked osteolysis with true metaphyseal lacunar zones are also observed. These particular types of bone lesion are related to the especially widespread cellular proliferation in megakaryoblastic leukemia, and their occurrence implies a very bad prognosis.

Combination chemotherapy for advanced ovarian cancer: a prospective randomized trial comparing hexamethylmelamine and cyclophosphamide to doxorubicin and cyclophosphamide.

Forty-three patients with advanced stage III and IV epithelial cancers of the ovary were entered in a prospective randomized study comparing doxorubicin and cyclophosphamide to hexamethylmelamine and cyclophosphamide chemotherapy. No statistical difference in the overall response rates or survival times was demonstrated. However, the combination of doxorubicin and cyclophosphamide was more effective in inducing a response in histologically poorly differentiated cancers than in well-differentiated and moderately differentiated cancers, and the therapeutic implications of this observation are discussed.

Traumatic disruption of a right aortic arch.

This is the first reported case of a traumatic disruption of the thoracic aorta occurring in a patient with a right aortic arch. This unique case emphasizes the need to be aware of the possibility of a traumatic aneurysm even under unusual anatomical circumstances, and the necessity for early aortography. It further emphasizes that aortic fixation by the ligamentum arteriosum is a major factor in the genesis of traumatic injuries of the thoracic aorta, and establishes the benefit of a right lateral thoracotomy in providing excellent surgical exposure.

An analysis of weak mutagens in the Ames assay.

Nitrosopiperidine, sodium nitrite and 1,2 epoxybutane were tested in the Ames agar incorporation assay in an attempt to establish exact criteria for detecting the activity of these weak mutagens. As regards minimum concentrations it was determined that at 500 microgram per plate a statistical analysis and several concentrations of the activating system are required in order to manifest an indication of the mutagenicity of 1,2 epoxybutane. The mutagenicity of sodium nitrite was detectable at 80 microgram/plate while nitrosopiperidine produced a 2.5-3.2 increase over background at 250 microgram/plate. It is concluded that since weak mutagenicity cannot be predicted it is advisable to utilize the maximum concentrations possible within the limits of solubility and bacterial toxicity. At the higher concentrations 1,2 epoxybutane, nitrosopiperidine and sodium nitrite produced revertant colony counts equal to 7, 17 and 62 times above background, respectively.

[Bone locations of systemic mastocytosis: radiological and scintigraphic study (author's transl)].

Bone radiological signs in six cases of systemic mastocytosis are described: lesions may be diffuse or circumscribed and of the condensation and/or rarefaction type. In most cases, however, the lesions are of the mixed type. Bone mastocytosis may occur without any cutaneous lesions and this emphasizes the value, from the diagnostic point of view, of radiological examinations of the skeleton. Diagnosis is defined and confirmed by three features: the association of condensing lesions and bone rarefactions in different regions of the skeleton; the presence of mastocytic proliferation in stained biopsy specimens from the posterior iliac bone which is not decalcified; fixation of 99m tc pyrophosphate in the axial skeleton on scintigraphy.

["Transparent circles" lymph glands: an unusual appearance on lymphography in certain proliferative lymphoid syndromes: prognostic value].

The authors describe 27 cases in which a very particular type of lymphographic image was observed, associating disseminated lesions in all the retroperitoneal lymph glands, and an appearance of "circled" glands of very weak density. This image may be rarely observed in Hodgkin's disease and non-Hodgkin type hematosarcomas. It is frequently observed, however, during the course of angio-immunoblastic lymphadenopathy. Whatever the etiology, this image is of particularly poor prognostic significance.
