Microwave tissue coagulation applied clinically in endoscopic surgery.

On the basis of the ample basic knowledge acquired by repeated experiments, we applied our microwave tissue coagulator in endoscopic surgery, in a total of 59 patients with benign and malignant lesions encountered during a period of two years beginning in July, 1981. Hemostasis was achieved in 96.5% of all the cases with bleeding lesions in the digestive tract. Stenosis was alleviated in 86% of the cases with esophageal or rectal stenosis. Furthermore, the technique was successfully used for hemostasis and tumor reduction in inoperable early cancer cases. Our device is unique in that the electrode is thrust into tissue, which assures us of a satisfactory result. In this point, it must be clearly distinguished from an electrocoagulator or a laser coagulator, which is associated with a risk of injuring intact tissue. The range of coagulation is adjustable by changing the length of the monopolar antenna and electric output, and by employing a coaxial cable of appropriate thickness. In conclusion, our microwave tissue coagulator can be used easily and safely in clinical endoscopic surgery.

[Analysis of intestinal motility by enteric sound recording system--clinical evaluation of aclatonium napadisilate and prostaglandine F2 alpha on intestinal movement].

In patients with so-called postoperative enteroplegia after laparotomy, effects of aclatonium napadisilate (abbreviated as TM-723 hereinafter) and prostaglandine F2 alpha (abbreviated as PGF2 alpha) on intestinal motility were determined using an enteric sound recording system (phonointestinography) and simultaneously the time required for breaking wind after operation was examined. TM-723 was administered at a dose of 50 mg from the intestinal fistula after dissolved in 5 ml of physiological saline, while PGF2 alpha was administered as an i.v. drip infusion at a dose of 2 mg dissolved in 500 ml of 5% glucose solution. Intestinal movement was determined at McBurney point using the enteric sound recording system developed at our department. The time required for breaking wind after operation was determined in three groups received TM-723, PGF2 alpha and placebo, respectively. No significant intergroup difference was observed. As to effect on intestinal motility, TM-723 increased intestinal motility when it was reduced in so-called postoperative enteroplegia phase. Its efficacy became higher with a decrease in motility. On the contrary, TM-723 rather tended to depress intestinal motility when it was increased. On the other hand, PGF2 alpha tended to increase intestinal motility regardless of motility in enteroplegia phase.

[Study on the function of the sphincter of Oddi in patients with gallstone disease by dynamic hepatobiliary scintigraphy].

By using hepatobiliary scintigraphy, function of the sphincter of Oddi in 88 patients undergoing cholecystectomy with or without common bile duct exploration and T-tube drainage for gallstone disease was studied. These patients consisted of three groups; 57 patients with gallbladder stones (Group I), 25 with common duct stones migrated from the gallbladder (Group II) and 6 with primary common duct stones (Group III). Time-activity curves (T-A curve) obtained from the region of terminal end of the common bile duct were classified into 4 types which were different from each other. On the T-A curve, the rate of decrease (ROD, %) was defined as the difference between the count rate in the maximum and that at the 20 min after the maximum. The ROD in Group I was significantly greater than that in Group II or III (p less than 0.01). The ROD in Group II was also significantly greater than that in Group III (p less than 0.05). The increase of ROD in Group III after cholecystokinin-pancreozymin administration was less remarkable as compared with that in Group II. Thus, we conclude that the function of the sphincter of Oddi in these three groups may be different from each other.

Palliative surgery for unresectable carcinoma of the head of the pancreas, ampulla, and distal end of the common bile duct in Japan. A statistical review.

To elucidate the approximate present status of palliative surgery for unresectable carcinoma of the head of the pancreas, ampulla, and distal end of the common bile duct in Japan, surgical methods in 151 institutions were collected and analyzed. Biliary drainage alone was performed in 144 institutions (95 percent) and in 80 of them, cholecystectomy and choledochojejunostomy with a Roux-Y procedure was adopted. Gastric bypass with biliary drainage was routinely performed in 13 institutions (8.7 percent), whereas in 62 institution (41.3 percent), combined biliary and gastric drainage operations were performed as the primary procedures in selected cases. In over half the institutions, neither gastrectomy nor vagotomy was provided. Some means to alleviate the pain caused by tumor invasion were performed in about a third of the institutions (49), and about 80 percent of them added some manipulations of the celiac plexus.

[Clinical investigation of intratumoral OK-432 administration].

The clinical effect of intratumoral administration of OK-432 was evaluated. The patients with nonresectable advanced cancer were divided into three groups according to administration method of OK-432 at laparotomy: Group I; Intratumoral administration, Group II; Scattering to the abdominal cavity, and Group III; No OK-432, administration. Improvement in suppression of tumor progression was more frequently noticed in the Group I compared to the Group II and III, but there was no significant difference in terms of survival time and rate among the three groups.
