Ideomotor apraxia in patients with Alzheimer disease: why do they use their body parts as objects?

OBJECTIVE: Difficulty in pantomiming the use of tools is observed in patients with Alzheimer disease. Patients with Alzheimer disease tend to use their body parts as objects when they try to pantomime the use of tools. The body-parts-as-objects phenomenon was thought to consist of appropriate gross movement of patients' arms despite inappropriate patterns of handling tools with their hands. However, the mechanism has not been investigated fully. METHODS: The authors examined 11 healthy individuals and 24 patients with Alzheimer disease for ability to recognize tools, to pantomime the use of tools, to use actual tools, and to select an appropriate photograph of a handling pattern of the tool. RESULTS: In comparison with the patients with Alzheimer disease, the healthy individuals had higher scores for all tasks. The authors classified the patients into group A (patients with higher pantomime scores), group B (patients with middle level scores), and group C (patients with lower scores). Frequency of use of body parts as objects for patients in group B was higher than that of patients in group A and in group C. The mean Mini-Mental State Examination score for patients in group A was higher than that of patients in group B and in group C. In comparison with patients in group A, patients in group B showed lower scores on the hand-posture discrimination task without a tool, even after controlling the Mini-Mental State Examination score. CONCLUSIONS: The authors conclude that body-parts-as-objects responses in patients with Alzheimer disease occur because general cognition is lowered or because retrieval ability of the appropriate holding posture for tools is impaired.

Developmental morphology of the thalloid Hydrobryum japonicum (Podostemaceae).

We describe the unique development and branching of lobed thalli in Hydrobryum japonicum. Lobe formation begins with meristem initiation at random sites near the thallus margin fringed by protective tissues. As the protective tissues are successively peeled off particularly in the growing new lobes, the lobes become naked and then become fringed again by new protective tissues that develop from the marginal part of the new meristems. Subsequently the meristems become less active and are differentiated into parenchymatous ground tissue at maturity. The random pattern of meristem formation during the sporadic development gives rise to a nonorderly branching pattern of the thalli. Some other lobes (∼10%) are regenerated from injured parts of the thalli. The vegetative shoots arise endogenously near the thallus margin and are enclosed by the nonvascular strand nets. The rudimentary shoot apices remain embedded in the thalli. The thalli, though remarkably different from typical roots of other angiosperms, might be extremely transformed roots.

[A point mutation at Arg169 (CGG-->TGG) in hereditary protein C deficiency].

We investigated a 56-year-old Japanese man with protein C deficiency, who was referred to our hospital because of venous sinus thrombosis and pulmonary thromboembolism. Protein C (PC) activity and the corresponding antigen level in plasma were 66% and 106% of the normal values, respectively. Both the activity and antigen levels of protein C were reduced by approximately 50% in plasma from the patient's mother. All exons and their flanking intron regions were amplified by PCR from genomic DNA. Sequencing analysis of the PCR fragments revealed that the patient was heterozygous for a C to T substitution at nucleotide position 6218, resulting in a single amino acid substitution of arginine (CGG) by tryptophan (TGG) at codon 169 of the heavy chain. We analyzed the patient, his mother, and normal controls by a Sac II digestion study of exon 7 and found that the patient and his mother had the same C to T point mutation at base 6218. This mutation could have been responsible for the defective activation of the molecule and the resulting thrombotic disorder. The patient is now being treated with warfarin, and so far no further clinical thrombotic episode has occurred.

[The development of the endoscopic treatment].

With development and improvement of the endoscope equipment, the role of the digestive system endoscope is developing to the application from the pickup of mere lesion to precise diagnosis and endoscopic treatment. The range of the digestive system endoscopic treatment tends to also expand year by year. From the viewpoint of the medicine side, the update of the digestive system endoscope is reported this time. Digestive system endoscopic treatment can be divided roughly into the digestive tract region and the bile pancreas region. There are foreign body extraction, hemostasis methods(peptic ulcer, esophageal varix), excision for the tumor, dilatation methods for the stenosis, etc. as endoscopic treatment in the digestive tract region. In the bile pancreas region, there are endoscopic sphincterotomy, calculus removal of bile duct and ductus pancreaticus, drainage, dilatation methods for the stenosis, etc. The endoscopic treatment develops more and more on both regions with development of together various equipment and improvement in the technique. The invasion to the patient is less than the surgery for the endoscopic treatment, and it is possible to position with the therapy that it retained the function of the organ and was excellent concerning the QOL. It is anticipated with that the effort with the aim of further low invasion will advance in future too.

Comparative effect of theophylline and aminophylline on theophylline blood concentrations and peripheral blood eosinophils in patients with asthma.

The comparative effects of a new theophylline preparation (Theodrip) and aminophylline on blood concentrations of theophylline were examined in 74 patients with asthma. Subjects were intravenously administered 200 mg of Theodrip or 250 mg of aminophylline for 1 h. The mean increases in blood theophylline concentration after Theodrip or aminophylline administration were 8.80 +/- 1.80 mg/l and 8.81 +/- 2.15 mg/l, respectively. In addition, these patients were divided into four groups based on baseline theophylline concentrations before infusion of Theodrip or aminophylline: i) naïve patients (not administered theophylline); ii) those with a baseline theophylline concentration of 0-5 mg/l; iii) those with a baseline theophylline concentration of 5-10 mg/l; iv) those with a baseline theophylline concentration of 10-15 mg/l. Mean increases in blood theophylline concentration after administration of Theodrip in each group were similar to those after aminophylline administration. We found no significant differences between Theodrip and aminophylline. However, when the comparative effects of Theodrip and aminophylline on peripheral blood eosinophil counts were examined, Theodrip, but not aminophylline, reduced blood eosinophil counts. With acute exacerbations of bronchial asthma, it is expected that Theodrip, but not aminophylline, may have an antiinflammatory effect. In conclusion, it is suggested that Theodrip is a more useful drug than aminophylline in patients with acute exacerbations of bronchial asthma.

[Reproductive and developmental toxicity study of S-1090, cefmatilen hydrochloride hydrate (3)--A study on oral administration during the period of organogenesis in rabbits].

Cefmatilen hydrochloride hydrate (S-1090) at dosage levels of 6.25, 12.5 and 25 mg potency/kg/day was administered orally by gavage to groups of 13-16 pregnant rabbits daily during the period of organogenesis, and the effects of S-1090 on dams and fetuses were examined. Control animals were treated with a 0.5 w/v% aqueous solution of methylcellulose. Abortion was noted in 1 of the 16 females of the 12.5 mg potency/kg group and in 1 of the 14 females of the 25 mg potency/kg group, and death was noted in 1 of the 14 females of the 25 mg potency/kg group. Regarding the dams, decreased food consumption was noted in the 12.5 mg potency/kg group in the beginning of the dosing period. Suppressed body weight gain and decreased food consumption were noted in the 25 mg potency/kg group during the pregnancy period. At necropsy, thickening of the gastric mucosa, hemorrhage in the cecum, and higher values of cecum weight were also noted in this group. On the other hand, no effects of S-1090 were noted in general signs, body weight changes, food consumption, necropsy findings, or organ weights in the 6.25 mg potency/kg group. No effects of S-1090 were noted in the number of corpora lutea, number of implantations, implantation rate, death or resorption rate, number of live fetuses, sex ratio of live fetuses, fetal body weight of either sex, incidence of external anomalies, incidence of skeletal anomalies or variations, degree of ossification, or incidence of visceral anomalies. On the basis of the above results, the no observed adverse effect levels of S-1090 are estimated at 6.25 mg potency/kg/day for general toxicity and reproductive functions in dams, and at 25 mg potency/kg/day for development in fetuses under the conditions of the present study.

Developmental morphology of ovules and seeds of Nymphaeales.

Ovule and seed development in six species of Nymphaeales was examined. In the Cabombaceae the two species studied resemble some extant basal angiosperms by having a hood-shaped outer integument. A micropyle-hilum complex results when the outer integument and derived testa are lacking between the micropyle and the funiculus, thus the hood-shaped appearance. In the Nymphaeaceae the outer integument is annular at an early stage and then cup-shaped though it is semiannular at initiation in Nupar japonicum and Nymphaea alba. The micropyle and hilum are separated by an intervening testa. Developmental data on the formation of the outer integument, from semiannular to hood-shaped vs. from annular to cup-shaped, are useful for inferring the morphology of the outer integument from the relative position of the micropyle to the hilum in seed fossils. The oldest (early Cretaceous) probable nymphaealean seeds had the micropyle-hilum complex, suggesting that the hood-shaped outer integument may be primitive in the Nymphaeales. This needs to be tested by examination of this feature in other groups of basal angiosperms.

[Two-step interferon rebound therapy for refractory chronic hepatitis C].

In almost all patients in whom interferon(IFN) treatment dose not result in persistent negative findings for HCV-RNA, HCV-RNA levels show a rebound after discontinuing the administration. In most patients, HCV-RNA levels after administration are increased compared to pretreatment values. When a rapid increase in HCV-RNA levels causes transient exacerbation of transaminase levels, HCV-RNA levels then rapidly decrease. In two-step interferon rebound therapy(TIRT), IFN is additionally administered when HCV-RNA levels are decreased. We previously reported that TIRT was useful for treating type 1b patients with an HCV-RNA level of 1 Meq/ml or more who did not respond to IFN treatment. In the year 2000, health insurance covered the additional administration of IFN. The use of TIRT for additional administration may further improve treatment response in patients who do not respond to IFN treatment.

[Two patients treated with intra-aortic balloon pump counterpulsation after subarachnoid hemorrhage].

Intra-aortic balloon pump counterpulsation (IABP) was used to treat two patients with symptomatic vasospasm after subarachnoid hemorrhage. One could not tolerate triple H therapy (hypertensive hypervolemic hemodilution) because of poor cardiac function and another suffered acute myocardial infarction after aneurysm surgery followed by cardiac failure. IABP increased cerebral blood flow and prevented cerebral infarction in the former case but this could not reverse cerebral ischemia in the latter. IABP may be one choice for patients with vasospasm after subarachnoid hemorrhage.

[Improvement in oxygenation by alpha stimulant].

In a 59-year-old female with cerebral aneurysm, oxygenation improved after the administration of phenylephrine. She had suffered subarachnoid hemorrhage and operated on 6 month before for ruptured aneurysm. Clipping of an un-ruptured cerebral aneurysm was planned. Anesthesia was induced and maintained with propofol, fentanyl and vecuronium. PGE1 0.01 microgram.kg-1.min-1, diltiazem 0.5 microgram.kg-1.min-1 and nicorandil 4 mg.h-1 were administered continuously. Arterial blood gas analysis revealed poor oxygenation (PaO2 100 mmHg at FIO2 0.5 with PEEP 5 cmH2O) and phenylephrine 0.1-0.2 microgram.kg-1.min-1 was added. This improved oxygenation (PaO2 150 mmHg) and PaO2 decreased again after discontinuation of phenylephrine when the closure began. The improvement of oxygenation by phenylephrine is thought to be enhancement of hypoxic pulmonary vasoconstriction.

Endothelial dysfunction is an independent factor responsible for vasospastic angina.

In order to evaluate peripheral endothelial function in patients with vasospastic angina (VSA), we measured flow-mediated dilation (FMD) of the brachial artery in patients with VSA and compared it with FMD in patients without VSA. Endothelial dysfunction is considered one of the mechanisms underlying VSA. However, its exact role remains to be clarified. The study included 30 patients with positive spasm-provocational test results without evidence of significant coronary stenosis (VSA group) and 30 patients with negative spasm-provocational test results without evidence of significant coronary stenosis (control group). In each patient, brachial artery diameter responses to hyperemic flow and glyceryl trinitrate spray were measured using high-resolution ultrasound. The carotid intima-media thickness was also measured as a marker of systemic atherosclerosis. FMD was lower in the VSA group (4.8+/-0.5%) compared with the control group (9.4+/-0.7%, P<0.0001). In the VSA group, FMD was not affected by coronary risk factors or the presence of atherosclerotic changes on coronary angiography. Glyceryl trinitrate-induced dilation did not differ between the two groups. The intima-media thickness was comparable between the VSA (0.85+/-0.04 mm) and control groups (0.81+/-0.05 mm). These findings indicated that peripheral endothelial function is impaired only in the VSA group, whereas the atherosclerotic changes were similar in the two groups. We conclude that endothelial dysfunction may be an independent factor responsible for the development of VSA.

[Comparison of management of advanced cancer in various organs].

The management of advanced cancer presents the greatest challenge to physicians involved in oncology. There will usually be a large burden of disease; cure is unlikely; and the needs of the patient in terms of pain control and palliation will also be important over and above the direct treatment of the disease. Different issues will arise depending on the site and pathological type of the cancer. Increasingly over the past few years, treatment protocols and guidelines have been developed for different cancers, but these can only be rough guides rather than definite treatment recommendations. Additionally in most cancers advanced disease offers the opportunity for evaluation of new treatments in Phase II studies and other trials. With the new generation of molecular targeted therapies, such as EGFR inhibitors, striking results are being seen in advanced disease that compare favourably with what has been seen previously. Other agents such as those which attack the tumour vasculature may also have promise in this setting. Palliation is also an important aspect of the management of advanced disease, and pain control in particular is an important component of patient management. In summary, the treatment of advanced disease provides a test bed for new agents, but this need to develop better cancer therapies must be balanced against patient needs for a pain-free and comfortable end to life.
