Anastomotic recurrence in the oesophagus complicating gastrectomy for adenocarcinoma of the stomach.

Gastric adenocarcinomas often spread to the distal oesophagus. Failure to control the disease in this area during total and proximal subtotal gastrectomy results in recurrence at the oesophageal anastomosis. The incidence of recurrence in a series of 351 such patients was 10 per cent and was influenced by the location of the main lesion, the stage of the disease, the presence of tumour at the margin of resection and the length of clearance of the oesophageal margin. Recurrence were prevented only with in vivo margins greater than 12 cm. Dysphagia combined with radiological signs of oesophageal obstruction was diagnostic of anastomotic recurrence regardless of the results obtained by oesophagoscopy and biopsy. Treatment was seldom effective in patients developing recurrence. Complete excision of the entire recurrent process offered the only hope for a long survival in these patients. Prevention by obtaining adequate oesophageal clearance at the time of gastrectomy is the only reasonable approach to this problem. The adequacy of resection cannot be judged accurately by intraoperative palpation of the oesophagus or by frozen section examination of the surgical margins.

Leiomyoma of the nipple.

Three patients with leiomyoma of the nipple are described. The tumors all appeared to have arisen from the muscularis mamillae and aerolae. One lesion recurred 9 years after incomplete excision. Two of the tumors were clinically apparent painful nodules while the third was an asymptomatic, incidental microscopic finding in a mastectomy performed for carcinoma. As in these cases and others described in the literature, patients with leiomyoma of the nipple or areola often present with a painful, ill-defined mass. Complete excision is indicated to prevent recurrence.

Mass spectrometric analysis of blotted proteins after gel electrophoretic separation by matrix-assisted laser desorption/ionization.

The molecular masses of electroblotted proteins were determined with a time-of-flight mass spectrometer by matrix-assisted laser desorption/ionization directly from blot membranes. Therefore standard proteins, separated by polyacrylamide gel electrophoresis, were electroblotted onto polyvinylidene difluoride or polyamide membranes by standard procedures. Pieces of membrane containing the protein of interest were soaked in matrix solution and analyzed in the mass spectrometer.

Nano-electrospray ionization mass spectrometry: addressing analytical problems beyond routine.

The advent of nano-electrospray ionization (nano-ESI) has considerably extended the usability of ESI in the analytical mass spectrometric laboratory. One of the remarkable features of nano-ESI is its extremely low sample consumption. Only a few microliters of analyte solution (10(-5)-10(-8) M) are sufficient for molecular weight determination and structural investigations by MS/MS. But nano-ESI is more than just a minimized-flow ESI; the low solvent flow rate also affects the mechanism of ion formation. As a consequence, the area of ESI-MS applications is significantly enhanced. Oligosaccharides, glycosides as well as glycoproteins can be analyzed more easily than with normal ion spray. The same holds for the analysis of non-covalent complexes sprayed directly from aqueous solutions.
