[Pineal choriocarcinoma presenting massive ventricular hemorrhage--a case report].

A case of primary choriocarcinoma in the pineal region is reported. A six-year-old boy showing precocious puberty, suddenly developed coma by massive ventricular hemorrhage. He recovered gradually up to akinetic mutism, but whose clinical, hormonal and neuroradiological studies failed to demonstrate any pathological lesions. Four years later a small tumor was detected by CT in the pineal region. At the same time increased serum-HCG level was shown. Subtotal removal of the tumor was carried out by Stein's supracerebellar infratentorial approach. The pathology of the tumor showed the characteristics of choriocarcinoma, consisting of major necrotic area and of tumor cells, syncytiotrophoblast and cellular trophoblast. The former revealed strong activity of HCG by immunoperoxidase study. Postoperative chemotherapy with actinomycin-D and methotrexate was given for a month. In the course of these treatments, serum-HCG recovered within normal range. This patient shows no signs and symptoms of the recurrence 6 months after operation. Early diagnosis with aids of neuroradiology as well as hormonal studies and the early initiation of the treatment may prevent major complications like massive tumoral bleeding, also might achieve the favorable outcome of this tumor.

Ultrastructure of Crooke's cells with special reference to their secretory granules.

Twenty-seven postmortem pituitary glands were electron microscopically investigated. Eighteen of them revealed an accumulation of fine microfilaments in the cytoplasm ranging from 70 to 80 A in width, which is equivalent to a hyaline change in Crooke's cells seen with a light microscope. Since Crooke's changes were immunohistochemically evidenced to occur exclusively in ACTH cells, we examined their secretory granules quantitatively and concluded that the majority of them were ovoid, homogeneously electron dense and measured 200 to 450 nm in diameter averaging 332 nm. Occasionally, those irregularly shaped and larger than 600 nm were observed.

Antibiotic resistance of Erysipelothrix rhusiopathiae isolated from pigs with chronic swine erysipelas.

The susceptibility of 258 isolates of Erysipelothrix rhusiopathiae from slaughtered pigs affected with chronic erysipelas in Japan to antimicrobial agents was determined. A total of 111 (43.0%) strains showed resistance to erythromycin, oleandomycin, oxytetracycline, or dihydrostreptomycin. Plasmids were not detected. This is the first report of resistance of E. rhusiopathiae to these antibiotics.

[MCNU effectiveness on brain tumor. Part I: Antitumor activity in vitro on human glioma and neuroblastoma cell lines].

A new water-soluble nitrosourea ( MCNU ) was tested for its antitumor activity against fourteen human glioma cell lines and two neuroblastoma cell lines. Four experiments were performed to determine its antitumor activity: inhibition of cell growth, comparison with ACNU, morphological observation, and analysis of DNA histogram with flowcytometry . Seven out of 14 gliomas (50%) and one neuroblastoma cell lines showed more than 50% inhibition of cell growth in vitro, appearance of giant multinucleated cell morphologically, and DNA accumulation in G2+M and/or S phase of cell cycle in the medium of 10 micrograms/ml MCNU . Antitumor activity and spectrum of MCNU against human brain tumors were almost the same as with ACNU.

Ultrastructural changes in the exocrine pancreas of experimental pancreatolithiasis in dogs.

Histological changes in the pancreas at an early stage of pancreatolithiasis were observed using a canine model in which obstruction was induced by partial ligation of the major pancreatic duct. Light micrographs demonstrated pancreatic lesions similar to those in humans. Electron micrographs revealed dilated lumens of small ducts and degenerated ductal cells 3 months after the ligation. These changes became more severe and appeared more often when the period of ligation was prolonged up to 1 yr. Acinar cells demonstrated dilation of the rough endoplasmic reticulum and the Golgi apparatus, swelling of the mitochondria, and increase in the number of prezymogen granules. Microfilamentous substance appeared in markedly dilated rough endoplasmic reticulum and in the intercellular space as acinar cell lesions progressed and the basal membrane became disrupted. The substance might be involved in calculous formation, the incidence of which reached a plateau after 6 months of ligation, coinciding with the peak in appearance of the substance.

[Effect of interferon on malignant brain tumor].

The purpose of this investigation is to elucidate antitumor activity of human interferon-beta (Hu IFN-beta) against nervous system tumors. The study of growth inhibition in vitro with IFN against a variety of human glioma and neuroblastoma cell lines showed a significant response with sensitive cell lines at the concentration of 1 X 10(3) IU/ml in the medium. With resistant cell lines, however, tumor cells were proliferated as control even at the concentration of 10(4) IU/ml. The activation of lymphocyte-mediated cytotoxicity was also studied. The microcytotoxicity test with AJ glioma cells as target was used to evaluate it. In this system, percentage of cytotoxicity was 33 +/- 10.0% by lymphocytes obtained from six healthy adult men and four patients with malignant glioma, and it was increased to 71 and 72% seven days after intravenous administration of IFN (daily dose 3 X 10(6) IU) for the treatment of gliomas. The results indicate that human IFN-beta is considered effective in the treatment of malignant gliomas.

Vitamin B-6 deficiency in germfree rats.

Germfree and conventional rats were used to investigate the influence of gut microflora upon vitamin B-6 deficiency. The body weights of conventional rats fed a vitamin B-6-deficient diet for more than 5 weeks plateaued, but the rats remained alive until the end of week 9 of deficiency. The body weights of germfree rats fed the same deficient diet decreased, andmost of rats were moribund around the end of week 9 of deficiency; three died. Urinary xanthurenic acid excretion following tryptophan load continued to rise at each experimental week in deficient germfree rats, while such a progressive increase in the level of the excretion was not observed in deficient conventional rats. Autopsy revealed that some of deficient germfree rats suffered from partial or toal lunficient conventional rats. The lung atelectasis was presumed to be the result of the respiratory muscle insufficiency which was closely related to the changes of the peripheral nerves innervating these muscles. The results may suggest that the presence of gut microflora made some contribution to improvement of the vitamin B-6 deficiency.

Symptomatic Rathke's cleft cyst. Morphological study with light and electron microscopy and tissue culture.

A rare case is reported in which a symptomatic Rathke's cleft cyst was studied by light and electron microscopy and tissue culture. The findings are compared with those of a craniopharyngioma studied in the same way. The patient was a 26-year-old woman presenting with headache, chiasmatic syndrome, and hypopituitarism. A cyst containing a mural nodule was partially removed and an Ommaya reservoir placed in the operative site for further treatment. The cyst wall was composed of connective tissue and three kinds of epithelial cells: non-ciliated squamous, ciliated columnar, and mucous-secreting cells. The morphology of these cells in vitro was similar to prickle cells seen in craniopharyngioma and the epidermis. It is concluded that both Rathke's left cyst and craniopharyngioma originate in remnants of Rathke's pouch, but at times may show some histological differences.
