Forensic neuropathological and immunohistochemical studies on Alzheimer-type dementia (AD/SDAT).

Alzheimer-type dementia (ATD) was discussed not only from the aspects of forensic neuropathology by using various techniques for general stains and silver-impregnation, but also immunohistochemically by using antiserum against glial fibrillary acidic protein which is a specific marker for astrocytes. There were marked differences in microscopic findings of ATD group and control group of elderly subjects whose intellectual have been maintained. The main histological features of ATD were presence of numerous senile plaques and neurofibrillary changes, and both of them were thought to be helpful diagnosis markers. Double stains by glial fibrillary acidic protein (GFAP) stain using avidin-biotin peroxidase complex (ABC) method and Congo red stain were performed. By the former stain, typical senile plaques, encircled by reactive astrocytes and penetrated interior by their processes, were demonstrated. By the latter stain, amyloid core was well detected. Another method which is particularly sensitive to amyloid, such as amyloid core or amyloid angiopathy, is the thioflavin S stain. Immunohistochemical stains for glial fibrillary acidic protein demonstrated an intense participation within the plaques by processes of reactive astrocytes. Using the autopsy criteria for the diagnosis of Alzheimer's disease reported by Khachaturian, senile plaques were counted on microscopic magnification x200 field. Numerous plaques satisfying thoroughly the criteria were detected. The report by Blessed et al. that there is a highly significant correlation between mean plaque counts in sections of cerebral cortex of elderly subjects and degrees of dementia is very helpful for forensic pathologists and is highly evaluated.

Direct solid-phase enzymoimmunoassay of testosterone in saliva.

This competitive, solid-phase enzymoimmunoassay for testosterone in saliva is carried out on microtiter plates and involves no chromatographic or extraction steps. With an overnight incubation the detection limit of the assay is 230 fg per well (16.1 pmol/L). There was a good correlation (correlation coefficient 0.95) between testosterone concentrations measured with and without prior extraction of the saliva samples. Repeated assay of three control saliva samples containing a range of testosterone concentrations (200-1000 pmol/L) gave within- and between-assay coefficients of variation of 5.5-13.2%. The analytical procedure is simple and closely resembles already published procedures for the determination of progesterone and estrone (with extraction) in saliva. One person can assay 200 samples in 24 h and the assay is suitable for reproductive and sports medical applications, particularly for projects involving serial sampling and yielding large numbers of samples.

Aetiological agent of enterically transmitted non-A, non-B hepatitis.

Virus-like particles (VLPs) with a mean diameter of 32 nm were recovered from the stools of three acute phase cases of enterically transmitted non-A, non-B hepatitis (ET-NANBH) occurring in the Soviet Union, North Africa and North America. VLPs from two of these cases were studied in detail and were shown to react specifically with antibody in acute phase sera obtained from other cases of ET-NANBH in Asia, the Soviet Union, North Africa and North America. Partially purified VLPs were found to sediment at 183S in sucrose gradients and to cross-react with antibody in acute phase sera from geographically isolated cases of ET-NANBH. The latter virus preparations were also used to document the seroconversion of experimentally ET-NANBH-infected cynomolgus macaques to 32 nm VLPs. Our findings indicate that one virus or class of viruses is responsible for the majority of ET-NANBH.

The CAMDEX: a standardized instrument for the diagnosis of mental disorder in the elderly: a replication with a US sample.

The Cambridge Mental Disorders of the Elderly Examination (CAMDEX) was developed by Roth, et al (1986) to assist in the early diagnosis and measurement of dementia in the elderly. In this study the CAMDEX was administered to a mixed group of independently diagnosed elderly psychiatric patients and control subjects in the United States. The CAMDEX was found to have a high interrater reliability with a mixed group of clinicians of varying backgrounds. The diagnostic scales and the cognitive section of the CAMDEX demonstrated considerable promise in distinguishing between independently diagnosed populations of depressed, demented, and normal subjects. The results suggest comparability between samples of subjects in England and the US, and that the CAMDEX is a promising instrument for use in both research and clinical settings.

[Bacteriologic characteristics of 25 strains of Vibrio cholerae, biotype eltor isolated in Mali].

This work studies bacteriological characters of 25 strains of V. cholerae isolated in Mali during the outbreak of 1984-1987. These strains were precisely identified by biochemistry characters, toxin secretion, presence of a structure considered as attachment factor. Antibiotic susceptibility study confirmed the great sensibility of V. cholerae to this products, and allowed to classify them according to their Cl90. This contribution of the laboratory is very interesting for the control of the disease and the choice of standardized scheme of treatment.

Frequency of occult residual melanoma after excision of a clinically positive regional lymph node.

A retrospective study of the medical records of 102 patients with Stage II malignant melanoma was conducted to determine the frequency of occult residual melanoma after excision of a clinically positive regional lymph node. Twenty-one patients met the study criteria for evaluation. Fifteen of 22 dissections were positive for melanoma (68.1%). These results support definitive regional lymph node dissection if the results of excisional biopsy are abnormal. No conclusions can be drawn from these data regarding the survival advantage of therapeutic regional lymph node dissection.

Pharmacokinetic analysis of a case of isopropanol intoxication.

A comatose 46-year-old woman, admitted to the emergency room, had isopropanol and acetone concentrations of 2000 and 120 mg/L, respectively, in her serum. She had no known history of acute isopropanol intoxication and was otherwise physically healthy. Pharmacokinetic analysis showed that the elimination of both isopropanol and its major metabolite acetone obeyed apparent first-order kinetics with half-lives of 6.4 and 22.4 h, respectively. These data contrast with the commonly held view that isopropanol is slowly metabolized. Concentrations of these analytes in cerebrospinal fluid 6 h after admission were similar to those in serum. This is the first report of the pharmacokinetics of both agents in a nonalcoholic person, and it gives the first data on concentrations of these substances in cerebrospinal fluid.

Two-dimensional echocardiographic imaging of left atrial appendage thrombi.

The utility of two-dimensional echocardiography in the diagnosis of left atrial thrombi is well documented. One major limitation of this technique, however, has been the failure to successfully image left atrial appendage thrombi. This report discusses the presumptive diagnosis in three patients of pathologically confirmed left atrial appendage thrombi using a modified short-axis parasternal two-dimensional echocardiographic view.

Hepatitis B transmission in school contacts of retarded HBsAg carrier students.

A prospective study was performed to assess the rate of transmission of hepatitis B from mentally retarded carrier students to susceptible mentally retarded peer students or school staff. Eighteen hepatitis B carriers were integrated into a public school program for the developmentally disabled, and serum hepatitis B markers were monitored in staff and students over 14 months. During this time no susceptible staff acquired hepatitis B serologic markers. Of susceptible students, 1.8% demonstrated seroconversion, but the route of infection is likely to have been outside the classroom environment. The results indicate that the risk of hepatitis B transmission is low in this particular school program.
