[A case of multiple anomalies of cerebral vessels--fenestration of the middle cerebral artery aneurysm of the anterior communicating artery and arteriovenous malformation on the frontopolar region (author's transl)].

A case associated with multiple cerebral vascular anomalies, which consisted of fenestration of the middle cerebral artery, arteriovenous malformation and aneurysm of the anterior communicating artery, was reported. A 48 year-old male has been suffering from the left paralysis and mental disorder after the initial attack of subarachnoid hemorrhage, and the second attack resulted in the deterioration of the symptoms. He was admitted to our clinic on October 28, 1974. On neurological examination, mental disorders, such as disorientation, emotional incontinence, amnesia and acalculia, hemiplegia on the left and meningeal irritation signs were observed in admission period. Physical examination was negative. Cerebral angiographic findings were as follows: 1) Moderate vasospasm of the right internal carotid artery at the terminal segment, mild bowing of the anterior cerebral artery and stretching of the frontparietal opercular branches of the middle cerebral artery were observed. 2) Right frontpolar arteriovenous malformation fed by the frontobasal artery and the frontopolar artery, and drained via the aberrant cortical vein into the superior sagittal sinus. 3) Aneurysm of the anterior communicating artery was opacified by left carotid angiography. 4) An abnormal vessel derived from the terminal segment of the right internal carotid artery and terminated at the portion of the sphenoidal segment of the middle cerebral artery. Complete loop was formed between genuine middle cerebral artery and this abnormal artery. He was operated with dissecting microscope on November 11, 1974. The arteriovenous malformation at right frontopolar region was totally removed and aneurysm of the anterior communicating artery was clipped. According to the operative findings, the arachnoid membrane over the right frontopolar region was turbid and adhered to the adjacent tissues. On the contrary, no abnormal findings suggestive of previous subarachnoid hemorrhage were observed around the region of the anterior communicating artery aneurysm. These findings showed that subarachnoidal bleeding was caused by rupture of the arteriovenous malformation of right frontopolar region, but not by the aneurysm on the anterior communicating artery. The postoperative course was uneventful and during the hospitalization the patient starts on rehabilitation therapy. The authors discussed the genesis of fenestration of the middle cerebral artery and relation among these combined vascular anomalies. We inferred that fenestration of the middle cerebral artery arose from the in complete fusion of procursor vascular network in embryonic stage. Additionally, we emphasized that it was necessary to make a distinction between these two terms "fenestration" and "duplication".

[Isocount scanning in the diagnosis of the brain abscess (author's transl)].

The purpose of this study is to investigate salient diagnostic features of the brain abscess in RI brain scanning. In 27 patients with brain abscess, 37 scannings were performed in the various stages of their clinical courses. The results were positive in all cases. Among 27 cases, ten cases were investigated by means of the ISOCOUNT SCANNING SYSTEM, the technical detail of which was reported previously. It was proved that the isocount scanning method combined with the multilevel analysis was more sensitive diagnostic aid than the conventional RI scintigraphy. Using the newly developed method, it was not difficult to diagnose the brain abscess in its early stage, even in the stage of presuppurative cerebritis. Doughnut sign was seemed to be more frequently visualized by this method. Surgical indication of the brain abscess was discussed from nucleomedical view point, and it was emphasized that so-called doughnut sign in brain scanning would indicate the appropriate stage of the brain abscess to be operated on.

Some enzymatic properties of 3beta-hydroxysteroid oxidase produced by Streptomyces violascens.

The 3beta-hydroxysteroid oxidase produced by Streptomyces violascens was purified from the culture broth by procedures including batch-wise treatment on DEAE-cellulose, ammonium sulfate fraction, gel filtration on Sephadex G-75, and column chromatography on DEAE-cellulose. The highly purified enzyme preparation exhibited no significant absorption maxima in the visible region other than a maximum at 280 nm. Optimum pH and temperature for the enzyme activity were approximately pH 7.5 and 50 degree, respectively. The Michaelis constant (Km) for cholesterol determined under two different experimental conditions were 4.5 and 6.7 X 10(-4) M. The enzymatic activity was remarkably inhibited by various metal salts such as FeC1(3), FeSO4, AgNO3, etc. On the other hand, neither EDTA nor Fe-chelating agents had any inhibitory effect on the enzymatic activity, while other metal-binding agents, KCN and NaN3, caused significant inhibition. The enzyme activity was inhibited almost completely by N-bromosuccinimide and iodine but not p-chloromercuribenzoate. The highly purified enzyme did not require any external electron acceptors other than oxygen. In addition, the activity was not influenced by the addition of external electron donors. The enzyme showed a high substrate specificity for 3beta-hydroxysteroids and the relative oxidation rates were 100 for cholesterol, 91 for 5alpha-cholestan-3beta-ol, 83 for pregn-5-en3beta-ol-one, 80 for androst-5-en-3beta-ol-17-one, 64 for 5alpha-androstan-3beta-ol-17-one, ect. The oxidation of cholesterol by the enzyme was remarkably inhibited by the addition of 5alpha-cholestan-3beta-ol, 5alpha-cholestan-3-one, 5beta-cholestan-3beta-ol, 5alpha-cholestane3beta, 5alpha-doil or 5alpha-lanosta-8, 24-den 3beta-ol. These findings indicate the present enzyme belongs to the class of 3beta-hydroxysteroid oxidase but some of its physical and enzymatic properties obviously differ from those of 3beta-hyroxysteroid oxidase of Brevibacterium.

[A case of the anterior choroidal artery aneurysm combined with the abnormal intracranial vascular network (author's transl)].

A case of the anterior choroidal artery aneurysm combined with the abnormal intracranial vascular net-work was reported. The patient was 43-year-old male who was attacked by subarachnoid hemorrhage in February 25th 1975. When he was admitted to our clinic 30 days after the ictus, his general status was good and the neurological examination showed no particular findings except only slightly accelerated deep tendon reflexes on the left side. Cerebrospinal fluid still remained xanthochromic although no nuchal rigidity was denoted. Based on the angiographic four vessel studies, the circulatory condition of the patient's brain was summarized as follows; the internal carotid arteries were stenosed or occluded between the C1 and C2 segment on both sides, and abundant collateral circulation was developed mainly around the circle of Willis making an angiographically peculiar vascular net-work in tha base of the brain. Another angiographic finding to be noticed was a berry aneurysm which originated from the distal part of the left anterior choirdal artery. No special treatment was performed on him. He was discharged without any neurological residuals. It would be difficult to find out any hemodynamic relationship between the occlusion of the internal carotid arteries and occurence of the aneurysm. But the abnormally dilated anterior choroidal artery might suggest that the vessel wall of this artery would be burdened in the abnormal distension stress due to the increased transaxial pressure in this artery. Unqder such a hemodynamically stressed state, it would be possible the aneurysm like outpouching of the vessel wall being developed in some fragile portion of the artery functioning as a prominent collateral circulation.

[Abscess of the brain (2nd report)--special reference to surgical indication of extracapsular excision of brain abscess (author's transl)].

Brain abscess persists as a serious diagnostic problem and critical therapeutic challenge since pre-antibiotic era. After antibiotic agents has been introduced, abscess of the brain seems to be a surgical curable intracranial suppurative disease, although surgical mortality and morbidity due to brain abscess are still distressingly high. Recently, the incidence of brain abscess are gradually increasing. In this present situation, it is necessary to reevaluate the previous method and surgical results of brain abscess. Even with new surgical techniques and antibiotics, the mortality rate and number of neurological deficits remain high, as previously methods, it is most important how to eliminate these problems. It was undertaken to determine the significant factors affecting the clinical management of patients with this serious problem. The general outlines of our neurosurgical treatment are given, with emphasis on our surgical schedule of brain abscess, especially brain abscess in congenital heart disease. In this paper, a review of the current status of the surgical methods, operative mortality and morbidity, diagnostic methods and the therapy of brain abscess has been discussed.
