Applying Orem's theory to the long-term care of adolescent transplant recipients.

The nursing theory of Dorothea Orem is applied to the care of the adolescent transplant recipient in the long-term setting of outpatient services. The theory of self-care is used as a framework to organize and direct nursing services to this select population. The following article discusses the framework that significantly contributes to maximizing the potential and quality of life for each recipient and assures the quality and utility of professional nursing services.

Acute tubular necrosis: preventing complications.

Any patient who demonstrates a decrease in urinary output, compromise in hemodynamic parameters, or exposure to nephrotoxic agents will be thoroughly assessed by the critical care nurse for signs or symptoms of acute tubular necrosis (ATN). Prompt recognition of the patient who is at risk, and early nursing interventions, can prevent or improve the course of ATN, the most common form of acute renal failure (ARF).

Assessing congenital heart defects in the cocaine-exposed neonate.

An estimated 100,000 "crack babies" are born each year in the United States. Because in addition to a variety of other congenital anomalies, neonatal cardiac defects have recently been linked to maternal cocaine use, the critical care nurse needs to conduct a detailed cardiac status assessment on all cocaine-exposed infants.

The CarePlan Card: building collaboration in medical and nursing orders.

Critical care nurses need a care plan tool to help integrate nursing interventions along with medical orders in order to develop a collaborative practice which respects physicians and nurses' roles in patient care. These authors describe a CarePlan Card which blends the traditional Kardex and care plan to create a dynamic, indispensable tool which coordinates both medical and nursing care and is responsive to the rapid changes seen in a critical care setting.
