Epidermoid carcinoma of the lacrimal sac: a clinicopathological case report.

Epidermoid carcinomas of the lacrimal sac are rare. We report a case that illustrates the difficulties in their diagnosis. When the diagnosis was made in our patient the tumour had extended beyond the sac wall and treatment was unsuccessful. The prognosis of these tumours depends on early diagnosis and complete excision, which demands a high degree of suspicion in all cases of chronic tearing, chronic dacryocystitis or a nonreducible mass in the area of the lacrimal sac. Consultation with an otolaryngologist is esential in both diagnosis and management.

Electron microscopy in iris nevus syndrome.

In a patient with the clinical features of the iris nevus syndrome who was treated for glaucoma a cataract developed later. The sector of iris removed during cataract extraction was examined by electron microscopy and was found to be covered by ectopic corneal endothelium with an underlying atypical Descemet's membrane. Abnormalities in the iris included aggregates of melanophagic cells in the anterior stroma and fibrosis in relation to the ectopic endothelium. These findings suggest that many of the iris abnormalities seen in this disorder occur on a reactive basis.

Sebaceous gland carcinoma of the lid.

At the Montreal General Hospital over the past 15 years five sebaceous gland carcinomas of the lid (3.5% of all malignant tumours of the lid) were diagnosed and treated. The five cases illustrate well the different clinical presentations of these tumours, knowledge of which is essential for early diagnosis. Once the diagnosis is suspected, a full-thickness lid biopsy is required. Management should then consist of wide excision, with preparation of frozen sections during the operation to ensure tumour-free borders.

Cemento-ossifying fibroma of the orbit.

Cemento-ossifying fibroma presents with ophthalmic symptoms and signs rarely. We report the clinical and pathological findings in a case of cemento-ossifying fibroma of the right maxilla with extension into the orbital floor causing intermittent vertical diplopia, proptosis, and upward displacement of the globe. Compression of the nasolacrimal duct produced epiphora early in the course of the disease. Fibrous dysplasia has often been diagnosed in other cases of benign monostotic fibro-osseous conditions. Ossifying fibroma is easily confused with fibrous dysplasia. The histopathological difference between the two lesions is described.

Measles immunization in Saudi Arabia: the need for change.

This paper describes the measles immunization programme in Saudi Arabia and the change from the single-dose schedule with the Schwartz vaccine to the double-dose schedule with the Edmonston-Zagreb vaccine. The recent measles-mumps-rubella school campaign is also described.

Effect of gender on reporting of MMR adverse events in Saudi Arabia.

We evaluated the safety of a measles-mumps-rubella (MMR) immunization campaign for Saudi children (age range: 6-13 years) and gender differences in reporting post-MMR adverse events. After vaccinations were administered, we monitored 160 schools for 14 days and 19 hospitals in the 8 cities under study for 10 weeks. Incidence rates were: all MMR adverse events, 26.5/10,000 MMR vaccines (significantly higher in females than males); Urabe strain aseptic meningitis, 1.0/295,000; fever, 40.2/10,000 (females) and 0.9/10,000 (males); and parotitis, 5.4/10,000 (females) and 0.9/10,000 (males). Combined MMR vaccine containing the Urabe mumps strain was safe for children aged 6-13 years. Gender differences regarding reactogenicity were evident and should be considered when designing future studies.
