Chondroma of the cervical spine. Case report.

The authors report a case of cervical chondroma presenting with a syndrome of spinal cord compression in a 76-year-old woman. Total surgical removal of the lesion was followed by partial neurological recovery. Chondromas of the vertebral column are rarely reported in the literature.

[Treatment of foraminal disk hernia by conservative hemi-transverso-arthro-pediculotomy].

The authors report a new approach of the intervertebral foramen using a conservative hemi-transverso-arthro-pediculotomy. It allows to remove one half of the posterior neural arch and to follow the nervous bundle from its origin to the extra-foraminal space. Removal of the foraminal disc herniation is easy and safe. The monobloc is replaced to prevent any instability.

[Indications for surgical treatment in dorsal disk pathology. 34 cases].

Thirty-four patients were operated for dorsal disk pathology by a postero-lateral approach. The first cases have been followed-up for over 4 years and have shown this method to be reliable. When associated with the "harpoon" marker technique, the surgical indications now include not only patients with progressive radiculo-medullary symptoms, but also cases of sudden decompensation and those limited to radicular symptoms.

[Total ethmoidectomy by frontal and transmaxillary approach].

Four patients with cancer of ethmoid with extension to base of skull were treated by total ethmoidectomy through a subfrontal and transmaxillary approach, the complementary paralateronasal approach being necessary in only one case. Apart from the approach routes, particular features of the technique used were essentially the size of the frontal flap extending to orbital roof, and mainly the confection of a pericranial flap formed of epicranial aponeurosis lined with frontoparietal periosteum and pedunculated at the orbital border. Exposure of the tumor by the neurosurgical and sublabial transmaxillary route allows correct excision of cancers extending to cribriform plate. The pericranial plate completes the traditional base of skull plastic operation and appears to reduce risk of cerebromeningeal complications. The technique is proposed for cancer infiltrating meninges: it is valid whatever the age of patients.

[Atlanto-axial arthrodesis by anterior retropharyngeal intermaxillo-hyoidal approach].

The authors describe the surgical technique of atlanto-axial fixation using a transcervical anterior approach. The cartilage of the articular processes of C1 and C2 are excised. A screw fixation of the articular processes is performed with two spongious screws. These are perpendicular to the articular surfaces. The trend is to obtain a bilateral articular fusion. The indications are discussed.

[Spinal epidural dumbbell-shaped cavernous angioma].

A 23 year-old man, with a two month-history of back pain, presented with spastic paraplegia and hypesthesia of both lower limbs. A myelo C.T. revealed a spinal epidural dumb bell-shaped mass extending in the intra and extra vertebral spaces through the enlarged intervertebral foramen. Total recovery was observed three months after surgery. Pathological examination of the tumor showed it to be a cavernous angioma. Primary epidural angioma is rare: in 2 reported dumb bell-shaped spinal epidural cavernous angiomas, radiological findings were also suggesting a spinal neurinoma.
