In a 52 years old man a backward epilepsy revealed a left fronto parietal tumor. The histopathological study shows that it is a meningial tumor composed around by meningocytic proliferation with a metastasis of lung cancer in the middle. This fact was proved only two months after neurosurgical intervention. Thirty three cases of cancer with metastasis in an intracranial primitive tumor have been collected in the literature. The authors study the clinical aspects and etiopathogenic hypothesis of this surprising anatomopathological fact.

Repeated Computerized axial Tomography (CAT) scanning of 4 patients with head injuries showed tardive development of 2 intracerebral hematomas and 2 extradural hematomas. The initial CAT scans were normal; the hematomas were late in developing. The value of repeated post-traumatic CAT scanning, and the pathogenesis of late developing hematomas are discussed.
Twenty nine cases of plasmocytic spinal cord tumors which were responsible for neurological troubles were collected over the 19 year period between 1962 and 1981. In 25 of the cases, spinal cord compression was the first sign of the tumor. In 4 of the cases, spinal cord compression was a secondary complication of an already known tumor. The various problems associated with this pathological process are discussed. The neurological problem is fairly straight forward: whether the compression is the first sign of the tumor, and whether it is a secondary complication, it must be surgically removed. Subsequent radiotherapy and chemotherapy are advisable. The oncogenous problem is more delicate. If the plasmocyte is a solitary tumor, and surgical removed is complete, the patient is, in theory, "heated", but should be regularly seen an outpatient basis. On the other hand, a multiple myeloma may first show it self as a spinal cord tumor, where as the patient is in reality suffering from a generalized disease.

The authors study the clinical dispositions of 46 arteriovenous malformations developed in deep cerebral area and detected in 41 cases by an haemorrhagic accident and after make a radiological classification. They class the M.A.V. of nucleus centralis with the localizations of the nucleus caudate's head, the insula and lentiformis nucleus and the capsulo thalamic area; the ventriculo-cisternalis M.A.V., the corpus callosum M.A.V. and complicated forms. With this classification they analyse the different operative technics that permit to obtain a complete extirpation in 26 cases about 39 operated patients. The results are excellent in 19 cases, good for 8 cases and mediocre in 4 cases. Eight patients are died during post operative time. The most important factors for the final result are the disturbance of consciousness before operative time and the malformations developed about several anatomical area. At last the authors insist on the fact that, if during next years, the embolism technics are not effective, the operation will be indicate in spite of the technical difficulties because the spontaneous evolution is always dangerous.
Three types of craniostenoses, plagiocephalies, trigonocephalies, and oxy-brachycephalies provoke modifications in the fronto-orbital region of the face, for which fronto-orbital remodelling osteostomies are proposed. Results obtained in 21 cases treated in this manner were very encouraging, as they were of good quality.
The author describes twenty cases of a type of external craniofacial fracture centred on the orbit, characterized by the frequency of ophthalmological complications, and mobilizing a segment of the frontosphenotemporal vault, and segments of the middle and anterior stages of the base of the skull. It is suggested that these be called frontosphenotemporal fractures.
A pseudo-tumoral cerebellar infarction developed in a 48-year-old man over the course of several days. External drainage improved the patients condition, and a subsequent operation showed a type I Arnold-Chiari malformation with cerebellar amygdalae descending to the level of C2. The infarcted postero-external part of the cerebellar lobe and the left amygdala were resected. The left postero-inferior cerebellar artery was thrombosed between the inferior border of the malformation and the posterior arch of C2. Almost total clinical recovery occurred. The Arnold-Chiari malformation had been clinically silent until the present infarction. A low dysraphia with a dermal sinus, sacral dehiscence and a low cord were present, but had caused no clinical disorders. The genetic origin of the dysraphism was confirmed by the presence of lumbosacral myelomeningocele with hydrocephalus in a relative of the patient. The relationship between postero-inferior cerebellar artery thrombosis and an Arnold-Chiari malformation is discussed: a causal relationship, already suggested in previous reports, is likely in the present case.
In those 12 observations, without associated lesion in the brain stem, diagnosis is done 3 times at autopsy, 8 times at surgical time, and once before it, on clinical and radiographic arguments. Diagnosis must be systematically evoked in front of vestibular symptoms, before severe signs, related to intracranial hypertension and brain stem compression, become evident. Iodoventriculography (7 times) and principally computerized tomography (2 times) bring conclusive arguments for diagnosis. Those infarcts are situated 9 times in the territory of the postero-inferior cerebellar artery and 3 times in the area supplied by the antero-superior cerebellar artery. Spontaneous evolution is always lethal, but prognosis after surgery is still severe (4 long term survivals in 9 cases) for the vascular system of those patients remains fragile.

The roof of the third ventricle is lying on the thalamus parts. When a tumor fills and expands the third ventricle it is always possible to lift up this velotrigonal roof like the cover of a box and to access to these lesions. The authors have approached 15 tumors of the third ventricle or pineal area by this route interthalamotrigonal about 78 cases of tumors operated in the Lille's neurosurgery department during 30 years. The technical possibilities are compared and the advantages of this route emphasized with usual access. The interthalamotrigonal access gives a great convenience during operation. It make easy the complete removal of removable tumors of the pineal area and the third ventricle. It is possible with minimum during operation and after effects for the patient.
This study, about 18 cases of AVA strictly in language areas (posterior end of the inferior frontal gyrus (Broca), posterior third of T1 and T2, supra-marginalis and angularis gyri), prepose to specify the possibilities and the legitimacy of surgical management as well as the technical parts of removal. Malformations may divided in to : 1 arterio-venous fistula, 3 medium AVAS, 8 important AVA, 6 cirsoïdum aneurysms. 15 patients presented one or several intracranial hemorrhage (ICH). 3 others were admitted before this complication : 2 epileptic seizures, 1 left hemicrania. 2 patients died because of important ICH before any surgical treatment. The 16 others were operated. Two died during post-operative course : one because of prior ICH with deep coma, the other third month after a good period. In one case upon 14, a disabling handicap concerning language is noted after an important ICH. Except an ethylic patient with post-operative acalculia, minor language handicaps were observed only in patients with hemorrhagic seizure, permissing always satisfactory life. The best results on vital and functional prognosis were obtained for patients with no prior important ICH. So we must indicate the radical removal of all known AVA or cirsoïd aneurysms. The opinion that these last malformations generally do not bleed, may produced mistakes with their weighty consequences.
