[Cutaneous manifestations of Kawasaki disease. Apropos of 30 cases].

A series of 30 cases of Kawasaki disease has been studied retrospectively over a period of 11 years. The aim was to reassess the diagnostic value of the dermatological manifestations. A modification of the extremities was observed in 28 patients (23 had early inflammatory lesions, 25 had late desquamation). Exanthema was constant, polymorphous and most often urticaria-like. Vesicles, pustules or purpura were noted during the course of the eruption in 7 patients. A perineal eruption was observed in 17 cases and was found of good diagnostic value even though not pathognomonic. Cheilitis was the most frequent of buccopharyngeal modifications (93 p. 100). Conjunctival hyperemia was noted in 26 patients. Eight children had cardiovascular complications. Among these cases, the modification of the extremities seemed to be more pronounced and stomatitis and arthritis were apparently more frequent. Most of all, the inflammatory syndrome was significantly more severe as concerns CRP and polymorphonuclear leukocytes counts. Dermatological examination often rules out other diagnoses, such as measles, scarlet fever and staphylococcal toxic shock syndrome. However, a complete etiological workup remains mandatory.

Improved detection of antibodies to hepatitis C virus in dialysis patients using a second-generation enzyme immunoassay.

We tested serum samples from 99 patients undergoing maintenance hemodialysis for hepatitis C virus (HCV) antibodies using a first-generation, licensed anti-HCV enzyme immunoassay (EIA) and a second-generation anti-HCV EIA that detect three gene products c100-3, NS3, and core. Specimens that were repeatedly reactive by either or both screening assays were further evaluated by testing with supplemental EIAs and a dot blot immunoassay. There was 87.9% agreement between the licensed HCV EIA and the HCV EIA second generation. HCV EIA Second Generation detected 10 more positive specimens than HCV EIA, for an increase in detection from 33.3% (33/99) to 43.4% (43/99). We conclude that HCV EIA Second Generation improves detection of HCV infection in hemodialysis patients.

Pituitary gigantism.

A case of pituitary gigantism resulting from a pituitary adenoma which secreted growth hormone is described. The patient was successfully treated by surgery, which led to the normalisation of endogenous growth hormone secretion. An acceptable final height was achieved with high dose intramuscular testosterone treatment.

Therapeutic response of chronic active hepatitis B (CAHB) to a new immunomodulator: AM3. Immunohematological effects.

AM3, a biological response modifier (BRM) of polysaccharide/protein nature, was given by the oral route to 13 patients with chronic active hepatitis B (CAHB). After 12 months of daily treatment, 8 patients cleared serum HBV-DNA and HBeAg together with ALT normalization. Immunohaematologic studies showed how time of inhibition of viral replication was related to significant decreases of CD4, CD8 and B cell blood lymphocytes. After serum viral elimination, however, a significant haematologic rebound of peripheral blood mononuclear cells (PMNC): CD3, CD4 and CD8 lymphocytes was seen. These data, suggest that the antiviral activities of AM3 may be due to its immunodulatory capacities. These promising results, together with the absence of any side effects, justify the entry to trials with a larger number of patients. Furthermore, treatment with AM3 may help to elucidate the pathophysiology of CAHB.

Successful surgical management of rupture of the left ventricle and interventricular septum due to blunt chest trauma. Case report.

In a 35-year-old man blunt chest trauma caused rupture of the free wall of the left ventricle and the interventricular septum. Emergency pulse Doppler-2D echocardiography confirmed the clinical suspicion and immediate surgical repair was successfully undertaken. This appears to be the first reported case of survival after surgery for such combined cardiac rupture.

Contribution of different yeasts isolated from musts of monastrell grapes to the aroma of wine.

Volatile substances of wines obtained by fermentation of musts from 'Monastrell' grapes (Alicante, Spain) was studied for yeast isolated from such musts. The results of the statistical treatment performed show the importance of yeasts of low fermentative power, particularly Kloeckera apiculata, in the production of volatile substances. Saccharomyces cerevisiae var. chevalieri was found to be the most important yeast of high fermentative power.

Effect of obesity on endogenous secretion of growth hormone in Turner's syndrome.

Nocturnal growth hormone secretion over a 12 hour period was assessed at 20 minute intervals in 25 prepubertal subjects with Turner's syndrome and 11 normal prepubertal girls of short stature to try and elucidate the relationship between body weight and endogenous secretion of growth hormone in Turner's syndrome. There were no differences in mean growth hormone concentration, age, height, or growth velocity between the two groups. There was an age related decline in mean growth hormone concentration in patient's with Turner's syndrome in contrast to the age related increase in controls. Mean percentage of ideal body weight was significantly higher in the Turner's syndrome group than among controls and it increased with age. There was a strong inverse relationship between mean growth hormone concentration and percentage of ideal body weight in those with Turner's syndrome. Covariate analysis of the multiple linear regression of mean growth hormone concentration on age and percentage of ideal body weight in Turner's syndrome indicated that percentage of ideal body weight had a significant effect on endogenous secretion of growth hormone when age was held constant, but not the other way round. We conclude that the age related decline in endogenous secretion of growth hormone in Turner's syndrome is partly the result of increasing body weight with age. The significant influences of biological variables such as age and body weight in the interpretation of measurements of endogenous secretion of growth hormone in Turner's syndrome should be emphasised.

Insulin-like growth factor and growth hormone secretion in juvenile chronic arthritis.

Insulin-like growth factor (IGF-1) concentrations were determined in a series of 23 children with juvenile chronic arthritis in conjunction with anthropometric assessment. When standardised z scores were used significant decreases in height and weight were shown in comparison with the normal age/sex matched means. Severe growth disturbance was seen, particularly in those with prolonged disease duration, which was independent of corticosteroid treatment, indicating disease activity itself is a major factor in the growth retardation. Eight children had low IGF-1 z scores--that is, less than -2.00 from age/sex matched mean. Low IGF-1 z scores were associated with low weight z scores but not with low height z scores. Overnight growth hormone secretory profiles were determined in 10 patients, including seven with low IGF-1, and showed generally normal secretion in all but one patient, who subsequently attained normal concentrations coincident with catch up growth. Increased pulse frequency of overnight secretion was commonly seen. Low IGF-1 concentrations probably result from varying factors, particularly nutritional, but do not reflect marked endocrinological abnormalities in most patients.

[Complementary transthoracic and transesophageal echo-doppler in the diagnosis of rupture of the sinus of Valsalva in right heart. Apropos of 4 cases].

Aneurysms of the sinus of Valsalva are rare and usually present with rupture communicating with the right heart cavities. Echocardiography is a powerful diagnostic tool in ruptured sinus of Valsalva aneurysms, especially the Color Doppler mode which shows abnormal, continuous flow as a mosaic of colour extending from the aortic root to the right heart cavities. The four case reports describe rupture of the right sinus of Valsalva aneurysm into the right atrium (2 cases) and the right ventricle (2 cases). Transesophageal echocardiography was performed in two cases and provided valuable information by confirming the site of rupture and the length of the diverticulum. The results illustrate the accuracy of Doppler echocardiographic techniques by the transthoracic and transoesophageal approaches in the diagnosis of ruptured sinus of Valsalva aneurysms.

[Pulmonary orifice stenosis: the medical solution. Apropos of 62 cases].

Sixty two patients with moderate or severe stenosis of the pulmonary orifice (SPO) underwent balloon catheter valvuloplasty. Thirty one were aged under 5 years. The mean right ventricle-pulmonary artery trunk (RV-PAT) gradient fell from 75 (+/- 26) to 23 (+/- 10) mmHg (p less than 0.001). All dilatations except one were effective with only a few incidents and no mortality. Six (+/- 2) months later, hemodynamic evaluation of 45 patients (70%) showed stability of the RV-PAT gradient at 26 (+/- 18) as compared with 23 (+/- 11) mmHg. Ten patients required a second dilatation because of a residual gradient of greater than 35 mmHg. The result was favourable in all ten cases, the RV-PAT gradient falling from 53 (+/- 17) to 13 (+/- 8) mmHg (p less than 0.001). This series confirmed the efficacy, reproducibility and safety of this technique which led to a lowering of ventriculo-pulmonary gradient, persistent at six months, to an infra-surgical level, at the price of pulmonary incompetence (27%) which was always well tolerated in the mid-term. This valvuloplasty may be suggested for all cases of SPO with a gradient of greater than 35 mmHg.

A comparison of short-term psychoeducational and support groups for relatives coping with chronic schizophrenia.

The differences between two short-term, multifamily group intervention programs for relatives of hospitalized chronic schizophrenics were examined in a nonequivalent comparison group design. The psychoeducational intervention consisted of interactive instructional activities. The support group intervention consisted of nonstructured discussions. Analysis of covariance on adjusted posttest means indicated a differential treatment effect for depression and satisfaction for the psychoeducational group. The findings suggest that the process of a support group may not be compatible with a short time frame.

Hepatitis C infection in two urban hemodialysis units.

We determined the prevalence of antibodies to the hepatitis C virus (anti-HCV) in 90 patients and 37 staff members of two hemodialysis units utilizing a recently developed anti-HCV recombinant based assay. Eleven patients (12%) were anti-HCV(+). Of these, eight (73%) had antibodies to the hepatitis B core antigen (anti-HBc) indicating prior hepatitis B infection; one patient was hepatitis B surface antigen (HBsAg)(+). All staff members were anti-HCV(-), although seven (19%) of them were anti-HBc(+). Alanine aminotransferase elevations were present at the time of the study in four anti-HCV(-) patients and in only one anti-HCV(+) patient. All anti-HCV(+) (mean 59 +/- 74; range 3 to 269 units) and 85% of anti-HCV(-) patients (mean 16 +/- 27; range 0 to 204 units) had received multiple blood transfusions (P = 0.348). Among 50 patients tested for human immunodeficiency virus (HIV), 43% of anti-HCV(+) as compared to only 7% anti-HCV(-) were positive (P = 0.003). There was a history of intravenous drug abuse (IVDA) in eight (72%) of the anti-HCV(+) patients and in only seven (9%) of the anti-HCV(-) group (P = 0.00001). The results of this serologic survey suggests that anti-HCV positivity is prevalent, although much less than anti-HBc, among our dialysis patients, whereas it was not detected among staff members. The prevalence rate of anti-HCV was statistically significantly higher among anti-HIV(+) and IVDA patients but not in multi-transfused patients.

Differentiation of Penicillium griseofulvum Dierckx isolates by enzyme assays and by patulin and griseofulvin analyses.

The production of patulin and griseofulvin by 49 different isolates of Penicillium griseofulvum Dierckx was analyzed by high-performance liquid chromatography. Eleven isolates were obtained from pistachio nuts, 37 were obtained from wheat seeds, and 1 was obtained from the American Type Culture Collection. Activities of 19 enzymes were also assayed by the API ZYM system. From these results it may be deduced that there are two different groups among the strains tested which cannot be distinguished by morphological and cultural characteristics. One group of isolates did not produce detectable amounts of patulin and griseofulvin when grown in sucrose-yeast extract and Wickerham media, while enzymatic activities were quantitatively distinct from the other group, which produced patulin and griseofulvin in variable proportions. Leucine arylamidase, phosphoamidase, and beta-D-glucosidase are the main enzymes with differing activities between the two groups. Differences in physiological characteristics among isolates of a single species reveal shortcomings in the classification of the penicillia based only on morphological criteria. Thus, determination of the ability to yield mycotoxins and antibiotics as well as determination of enzymatic activities appear to be very valuable tools in the taxonomy of these fungi and for food toxicology.

Cutis marmorata telangiectatica congenita and glaucoma.

A case of cutis marmorata telangiectatica congenita (CMTC) is described in association with congenital glaucoma. Approximately 68 cases of CMTC and only four cases associated with glaucoma have been reported. The type of glaucoma seems to be related to an abnormality of the filtration in the anterior angle, due to increased pressure in the episcleral vessels. The surgical treatment is trabeculectomy, and opening an alternative drainage pathway.

[Value of Naito's criteria in the surgical repair of tetralogy of Fallot].

Naito suggested criteria for the optimal reconstruction of the right ventricular outflow tract in total correction of tetralogy of Fallot based on the minimum acceptable diameter of the pulmonary artery. The value of these criteria was assessed in 78 operated patients by haemodynamic measurements at surgery and 6 months after operation. Using these criteria, a ratio of right ventricular/left ventricular systolic pressure less than 0.60 was obtained with the creation of pulmonary regurgitation which was usually moderate. Long-term assessment of these results is required.

[Doppler echocardiographic evaluation of right heart pressure in children with ventricular septal defect].

Thirty seven children with ventricular septal defects (VSD) alone or associated with other malformations were examined by Doppler echocardiography to evaluate systolic right ventricular pressure (SRVP) non-invasively. All patients underwent cardiac catheterisation within 48 hours of the Doppler estimation. The maximum interventricular pressure gradient could be assessed in 32 patients (86%) from the recording of the maximum velocity of the VSD jet using the simplified Bernoulli formula. The SRVP was calculated by subtracting the maximum interventricular pressure gradient from the systolic blood pressure measured by sphygmomanometry. The SRVP estimated by continuous wave Doppler ranged from 24 to 108 mmHg (mean: 60 mmHg) compared with 20 to 109 mmHg (mean: 64 mmHg) at catheterisation. Two types of correlation were sought: 1) between the maximum interventricular pressure gradient measured by Doppler echo and the peak-to-peak LV-RV pressure gradient (r = 0.95; SD = 6 mmHg; Y = 1.03 x X + 1.3); 2) and between the SRVP measured by Doppler and by catheterisation (r = 0.94; SD = 6 mmHg; Y = 1.06 x X + 7.7). These results show that the maximum velocity of the interventricular jet and thereby the SRVP may be accurately measured by Doppler echocardiography in the majority of patients with VSD.
