1993 progress report from the International Bone Marrow Transplant Registry. Advisory Committee of the International Bone Marrow Transplant Registry.

The International Bone Marrow Transplant Registry is an organization devoted to scientific research in BMT. More than 230 transplant teams worldwide contribute detailed information about recipients of allogeneic and syngeneic BMT for study. Results of analyses are published in medical journals and presented at national and international scientific meetings (more than 60 publications and more than 500 presentations in the past 4 years). This collaborative research program has grown rapidly with more than 2000 cases reported annually. This report summarizes results of several recent investigations and reviews the state of BMT in leukemia and aplastic anemia.

Comparative observation of the changes in serum lipid peroxides influenced by the supplementation of vitamin E in burn patients and healthy controls.

A microfluorescence analytical method was used to determine the contents of serum lipid peroxides and vitamin E in 35 severely burned patients, among whom 18 had been given vitamin E, and in 113 healthy blood donors. A comparative survey showed that the serum vitamin E decreased most significantly from days 6 to 8 postburn in both groups of burned patients, while the concomitant serum lipid peroxides increased significantly and reached a peak value. In control patients, the serum vitamin E levels were lower and lipid peroxides higher than those of healthy people (P less than 0.05) from days 20 to 22 postburn. At this time in the vitamin E-treated patients, the serum vitamin E levels increased and lipid peroxides decreased to the levels of healthy people (P greater than 0.05). Therefore, appropriate supplementation with vitamin E in burn patients is necessary.

Preparation and characterization of the monoclonal antibodies against Japanese encephalitis virus.

Six mouse monoclonal antibodies (MoAbs) against Japanese encephalitis virus (JEV) were prepared and analyzed with indirect immunofluorescence assay (IFA), enzyme linked immunosorbent assay (ELISA), haemagglutination inhibition test (HI), neutralization test (NT), antibody dependent cell mediated cytotoxicity (ADCC) assay, antigenic site specific analysis and relative affinity measurement. These MoAbs could be divided into three classes by indirect immunofluorescence cross reactivity among four flaviviruses, 2H4, 2F2, and nG2 were type specific; 2D2 and mC3 were subgroup specific; and mG9 was family specific. 2H4 and 2F2 had higher neutralization activity, 2D2 and mC3 had the function of inducing ADCC effect, mG9 had higher titer in HI. The six MoAbs recognized five antigenic sites on JEV envelope glycoprotein, 2H4 and 2F2 recognized the same or very similar antigenic site and their relative affinity was ranked as: nG2 > 2H4 > 2D2 > mG9 > 2F2 > mC3.

Report from the International Bone Marrow Transplant Registry.

The number of allogeneic BMTs performed worldwide continues to increase. In addition, the characteristics of patients, donors, and selected treatments are changing. In BMT for leukemia during the 1980s there was a marked increase in transplants for early disease, the use of unrelated donors, and utilization of preparative regimens without radiation. Although treatment-related mortality declined during this period, there was only a modest decrease in relapse rates, indicating the need for more effective antileukemia strategies. The IBMTR collects data from many centers, and, as a consequence, it is uniquely suited to examine clinical situations in which patient accrual at a single institution would be insufficient for a study to be performed. In such analyses, BMT was shown to be an effective treatment for Ph1-positive ALL. Patients tended to have earlier relapses and lower probabilities of LFS than Ph1-negative ALL but the differences were not statistically significant. BMT was also shown to be effective in patients with acute leukemia failing to ever go into remission, most of whom would die within the first 6 months following diagnosis. The IBMTR is a premier example of international scientific collaboration. Its success is a consequence of the desire of investigators throughout the world to combine their clinical data for statistical analysis in order to accelerate and improve patient care.

[E coli-induced idiopathic portal hypertension in rabbits. An experimental study].

An experimental model of portal hypertension was induced in healthy rabbits by intraportal injection of E coli or a mixture of E coli and rabbit anti-E coli sera. In all established models, there were significant elevation of portal vein pressure, increase of spleen weight, decrease of whole blood cells count, infiltration of inflammatory cells and fibrosis in intrahepatic portal area, decrease of the number and discontinuation of intrahepatic portal vein branch on portography with gross normal appearance of the liver. Hence this model was similar to that seen in human idiopathic portal hypertension (IPH) in many aspects. We believe that bacterial toxin and the abnormal immune reaction due to repeated stimulation of bacterial antigen may play a role in the pathogenesis of human IPH.

Salter's osteotomy for the management of joint capsule.

Capsulotomy in primary Salter's osteotomy shaped by T or + and is difficult to tighten joint capsule. A new technique or L-shaped incision i.e., a L-shaped incision on the right hip and a reversed L-shaped incision on the left is designed to incise and sew the capsule. This technique can make the acetabulum fully exposed, and the capsule can be conveniently tightened with overlapping sutures. When it is combined with modified external fixation, redislocation after operation can be avoided.

Passive protection of mice, goats, and monkeys against Japanese encephalitis with monoclonal antibodies.

Six monoclonal antibodies (McAbs) against Japanese encephalitis virus (JEV) were tested for passive protection in JEV-infected mice, goats, and rhesus monkeys. mG9 and nG2 had no protective effect; mG3 and 2D2 had some protective effect, but not sufficient to be of therapeutic significance; and 2H4 and 2F2 had excellent protective efficacy in mice even 120 hr after infection when most of the mice in the virus control group were sick. The mixture of 2H4, 2F2, mC3 (M-McAb), and their F(ab')2 fragments showed excellent protection in mice, goats, and monkeys and was safe. The protective effects of McAbs correlated with their neutralization titers, but cytotoxicity-mediated activities also played a role in protection.

[Anteversion angle of the femoral neck in congenital dislocation of the hip].

The anteversion angle of femoral neck in 40 children, aged 18 months to 14 years (4.6 years on average), with congenital dislocation of hip was measured pre-operatively on X-ray films according to the way of Dr. Hu's method, and from the proximal femur exposed during operation. The mean value of them was 64.6 degrees and 44 degrees respectively, being statistically significant (P less than 0.01). The method of direct measurement of the anteversion angle during operation was described. The factors affecting the diversity of the measured angle value with different methods were discussed. It seems that the calculation formula used in Hu's method should somehow be corrected and a revision is put forward.
