Reversal of severe fatty hepatic infiltration after intestinal bypass for morbid obesity by calorie-free amino acid infusion.

Hepatic fatty infiltration complicating jejunoileal bypass can be massive and may require restoration of gastrointestinal continuity. This fatty infiltration appears to be caused by protein depletion associated with adequate or high carbohydrate intake. The present study has shown that calorie-free amino acid alimentation can reverse these changes. In three of thirteen patients who underwent 12 inch to 6 inch jejunoileal bypass procedures, symptomatic hepatomegaly developed with near total replacement of hepatocytes by massive fatty infiltration. After undergoing liver scan, liver biopsy, and liver function tests, the patients were started on a peripheral infusion of 2L per day of a 4.25 per cent crystalline amino acid solution, allowing for fat mobilization while preserving body protein stores. All oral intake was withheld except for water. At the end of a fourteen to twenty-one day infusion period, serum albumin levels increased by 1 gm in all patients. Decreases in liver volume of 83, 45, and 40 per cent occurred. During the infusion period ketonuria was 4 plus in all patients indicating active lipolysis. Weight loss was impressive (17, 19, and 40 pounds). All patients showed marked symptomatic improvement, and postinfusion liver biopsy specimens showed a return to near normal architecture. Maintenance of normal liver size by a high-protein, low-carbohydrate diet was observed in a five to seven month follow-up period. In contrast to previous studies using standard hyperalimentation solutions, the use of calorie-free amino acid solutions reverses the hepatic fatty infiltration seen after intestinal bypass by mobilization of fat. This fat mobilization does not occur as readily in the presence of large amounts of glucose.

Leucocyte migration studies in Crohn's disease using Crohn's colon homogenate and mitochondrial and microsomal fractions.

Leucocytes from 38 patients with Crohn's disease were tested for evidence of migration inhibition in the presence of preparations of colon from a patient with this disease. The occurrence of migration inhibition showed a positive correlation with clinically active disease and a negative correlation with immunosuppressive treatment. It was not seen with leucocytes from 12 healthy subjects.

Mixed-lymphocyte reaction as a measure of immunological competence of lymphocytes from patients with Crohn's disease.

Peripheral blood lymphocytes from 12 patients with Crohn's disease and from a group of normal volunteers have been tested for transformation under stimulation of allogeneic cells in a mixed lymphocyte culture and by phytohaemagglutinin (PHA) in vitro. There was a significant depression of transformation in the mixed lymphocyte culture where lymphocytes from the Crohn's disease patients constituted the responding population. With PHA stimulation, no difference in transformation was seen between the patients and the control group. This is interpreted as indicating a depression in cellular immunity in Crohn's disease.

An evaluation of the rosette inhibition test in Crohn's disease.

Serial rosette inhibition tests were performed on nine patients with Crohn's disease undergoing immunosuppressive therapy in an attempt to estimate the minimum amount of drug necessary to maintain immunosuppression. The rosette inhibition titre was found to be higher in immunosuppressed patients than in normal subjects. However, there was no consistent correlation of the rosette inhibition titre with the degree of activity of the disease. The test is therefore not sufficiently discriminative to be used as a basis on which to control drug dosage.

Leucocyte migration studies with spleen preparations in Crohn's disease.

Thirty-nine patients with Crohn's disease and 17 normal people were tested for hypersensitivity to sarcoid and normal spleen suspensions using the leucocyte migration test. In 26% of the Crohn's cases migration was inhibited, whereas inhibition was not significant in the control group. Migration inhibition did not appear to be related to duration or activity of the disease. Sixty-two percent of the Crohn's patients showed migration stimulation against normal spleen, a phenomenon that did not occur in the control group.

Cerebral atrophy in young cannabis smokers.

Evidence of cerebral atrophy was demonstrated by air encephalography in ten patients with histories of consistent cannabis smoking over a period of 3-11 years. The average age of the patients was 22 years; all were males. Amphetamines and lysergide (L.S.D.) had also been taken, but in much smaller amounts. Measurements of the lateral and third ventricles were significantly different from those in thirteen controls of a similar age-group.
