Avascular necrosis associated with anticardiolipin antibodies.

The association between antiphospholipid antibodies (aPL) and venous and arterial thromboses has been well described. However the association between aPL and avascular necrosis has not been widely reported. We report the association of avascular necrosis with the primary anticardiolipin antibody syndrome in a patient with a history of hemiplegic migraine and cerebrovascular infarcts. This phenomenon appears to be secondary to hemostatic abnormalities associated with the presence of aPL.

Is silicone associated with connective tissue disease?

Connective tissue diseases have been described in patients who have had silicone augmentation mammoplasty. Several possible mechanisms of pathogenesis are postulated. Although otherwise indistinguishable from other connective tissue disease, these patients may experience improvement or remission following implant removal. Current data suggest that the risk of disease after silicone breast augmentation is less than one percent. A large well-designed epidemiologic study will be needed to confirm this association.

Polyarthritis associated with cutaneous T cell lymphoma.

This is the first reported synovial biopsy of a patient with concurrent onset of cutaneous T cell lymphoma and polyarthritis. The biopsy revealed a nonspecific chronic inflammatory process. Immunoperoxidase technique using monoclonal antibodies showed a mixture of helper T cells, suppressor T cells and B cells in the synovial infiltrate. Tumor infiltration was not seen. Our findings indicate that the arthritis associated with cutaneous T cell lymphoma is most likely a reactive phenomenon.

Spontaneously reactivation of acetylcholinesterase inhibited by diisopropylfluorophosphate.

When electric eel acetylcholinesterase is inhibited by diisopropylfluorophosphate at 0 degree C most of the enzyme is irreversibly inactivated. However, 0.13--0.18% of the initial activity returns spontaneously after the unbound inhibitor is removed or when the inhibited enzyme is diluted into a large excess of competing substrate. A subsequent inhibition-reactivation cycle results in an essentially complete return of activity with minimal aging. The extent of aging increases substantially when inhibition and reactivation are performed at temperatures above 0 degree C. The free energy of activation for spontaneous reactivation was determined to be 20.1 kcal . mol-1. This large free energy of activation indicates that the reactivation process is a typical dephosphorylation reaction. The computer program used in determining the rate constants and the final extent of reactivation may be widely applicable in similar kinetic studies.

Respiratory allergies and skin test reactivity in high school students in Tenerife, Canary Islands, Spain.

The prevalence of skin test reactivity to 22 aeroallergens and of allergic respiratory diseases was determined in 501 high school students on the island of Tenerife, Spain. Two hundred seventy-seven students (55.2%) had at least one positive prick skin test (wheal > or = 2 mm). Two hundred sixteen students (43.1%) had symptoms of upper or lower respiratory tract allergies [24 (4.7%) had asthma with or without rhinitis and 192 (38.3%) seasonal or perennial rhinitis alone]. Two hundred eighty-five students (56.8%) were asymptomatic. One hundred per cent of the students with asthma, 87.5% of the students with rhinitis and 27.7% of the asymptomatic students had at least one positive skin test. The prevalence of positive skin tests was significantly higher in symptomatic than in asymptomatic students (p = 0.0001). One hundred seventy students (33.9%) had a family history of respiratory allergic diseases. The prevalence of positive skin tests among these students was significantly higher than in students without such history (p = 0.0001). Thus, there is a high prevalence of allergic respiratory diseases and skin test reactivity to aeroallergens among high school students in Tenerife. Significant correlations were found between family history, positive skin tests and respiratory allergic symptoms.
