Initial evaluation of CYFRA 21-1 diagnostic performances as a urinary marker in bladder transitional cell carcinoma.

PURPOSE: CYFRA 21-1, an immunoradiometric assay developed for the detection of a soluble cytokeratin 19 fragment, is evaluated for its diagnostic performance in urine of patients with transitional cell carcinoma. MATERIALS AND METHODS: CYFRA 21-1 was investigated in serum and urine of 128 patients, including 48 with bladder transitional cell carcinoma (group 1), 44 with other urological pathological conditions (group 2) and 36 free of urothelial disease (group 3). Urinary cytopathology was also performed. RESULTS: Mean urinary CYFRA was 123.5 +/- 53, 11.9 +/- 4.8 and 2.3 +/- 0.2 ng./ml. for groups 1 to 3, respectively, and was significantly different. From the receiver operating characteristics curve, the optimal combination of 96% sensitivity and 74% specificity was determined for a threshold value of 4 ng./ml. while overall cytopathology sensitivity was 43%. CONCLUSIONS: Urinary CYFRA 21-1 may be a useful marker for diagnosing transitional cell carcinoma.

[A study of biological and histological criteria of thyroid autoimmunity in diffuse goiters with hyperthyroidism].

Hyperthyroidism is usually classified as follows: with diffuse or with toxic plurinodular goiter, respectively considered as autoimmune and non-autoimmune thyroid disorders. This classification seems partially inadequate as signs of thyroid immunity may be found in some plurinodular toxic goiter and alternatively may by lacking in some cases of Graves' disease. These observations led us to study the intensity of intrathyroidal autoimmune process (IAP) and the levels of TBIAb, TPO- and Thyroglobulin-antibodies in 105 cases with diffuse goiter, hyperthyroidism and elevated RAIU (92 women and 13 men, age 34 +/- 11, mean +/- SD). The intensity of intrathyroidal autoimmune process (IAP) was determined by one pathologist (HT) by semi quantitative method applicable to routine clinical use. Subtotal thyroidectomy was performed because of a large goiter (n = 29), a concomitant cold nodule (n = 20), a recurrent disease (n = 18), intolerance to antithyroid drugs (n = 5) or because patients chose surgical treatment (n = 33). All cases were rendered euthyroid at the time of surgery using antithyroid drugs or iodine. The results show a lack of IAP and undetectable levels of TBIAb, TPO- and Thyroglobulin-antibodies in 10%, 11%, 25% and 47% respectively. The intensity of IAP was not different in case of first episode or recurrence of hyperthyroidism and was not related to type or duration of medical treatment. Comparison of patients with or without IAP show higher levels of TPO- and thyroglobulin-antibodies but not of TBIAb in the former group (P < 0.005). TBIAb were higher when ophthalmopathy and/or dermopathy were present vs absent (p < 0.05) and were correlated with FT4 levels (p < 0.05). The negative predictive value of TBIAb, TPO- and thyroglobulin-antibodies to predict the lack of significant IAP was 42%, 65% and 64%. The total absence of clinical, biological and histological signs of thyroid autoimmunity was found in only one case (female aged 35 with first episode of hyperthyroidism and no family history of thyroid disease) (0.9%). These results suggest that routine available criteria of thyroid immunity (including IAP) have a low specificity. It follows that they are probably inadequate to screen cases of hereditary toxic familial hyperplasia, a rare entity of still unknown prevalence.

[Hormonal suppressive therapy of thyroid nodules].

We studied 426 patients in order to analyse the effectiveness of levothyroxine in treating patients with cold thyroid nodules. Nodule volume ranged initially from 0.1 to 10 mL and was measured over a 3 to 18 month-period of treatment using ultrasonography. Nodule variation was studied in each case according to levels of TSH and others factors (age, sex, familial thyroid history, initial volume of nodule and of thyroid gland, date of diagnosis, ultrasonic findings, duration of treatment). Nodule volume increased in 32.6% and significantly decreased (> 50%) in 35.8%. In 2.8% the nodule disappeared. No correlation was found between variation of thyroid nodules and TSH levels under treatment. Similarly no relationship was found between volume variation and other criteria. These results suggest that TSH suppressing doses of levothyroxine are of no advantage in reducing the volume of cold thyroid nodules compared to lower doses.

The PAQUID survey and correlates of subclinical hypothyroidism in elderly community residents in the southwest of France.

The aim of this study was to assess the prevalence of increased thyroid-stimulating hormone (TSH) levels (subclinical or undiagnosed hypothyroidism) in a representative sample of a community-living elderly population and to examine the relationships with cognitive functions and depressive symptoms. An epidemiological study was made of a cohort of 2792 subjects over 65 years of age. A blood sample was obtained from 425 volunteers of this cohort and assayed for TSH. Every subject participating in the study was interviewed and given a battery of neuropsychological tests by a psychologist. Three hundred and eighty-one subjects (89.7%) had normal TSH levels; 18 subjects (4.2%) had TSH lower than 0.4 microU/ml, associated in two of them with hyperthyroxinaemia. Twenty-six subjects (6.1%) had increased TSH levels, associated in 18 of them with a normal free thyroxine level and in eight with a low free thyroxine level. Increased TSH levels were significantly linked with female sex and with the presence of symptoms of depression on the CES-D scale but not with impairment of cognitive function.

Thyroid function in brain-dead donors.

Twenty brain-dead potential organ donors were studied prospectively to establish thyroid function. Two or three consecutive blood samples were obtained during brain death. Seven times a sample was available before brain death occurred. Free triiodothyronine (FT3) fell in most patients (80%). Very low (less than 1.6 pmol/l) and subnormal levels (between 2 and 3 pmol/l) were found in 65% and 15% of the patients, respectively. Serum reverse total triiodothyronine (rT3) was inversely correlated with FT3. Free thyroxine (FT4) was less often decreased (mean 14.68 +/- 1.42 pmol/l) and 35% of the patients had normal levels. Mean thyroid stimulating hormone (TSH) remained normal (0.71 +/- 0.15 microU/ml). The study of consecutive samples during brain death did not show a constant, progressive decrease in hormonal levels. There is no statistical difference between values observed before and after brain death. No correlation was found between FT3 levels and hemodynamic data or immediate allograft function. The pattern of thyroid function in these patients was typical of the sick euthyroid syndrome with a low T3 or low T3 and low T4 serum levels. This syndrome usually does not need to be treated. However, many experiment findings and some clinical data argue in favor of T3 therapy in donors and possibly in recipients. The dosage regimen must be adjusted to be effective without causing harm to multiorgan donors before it can be widely used. It remains to be proved that low FT3 serum indicates low intracellular FT3 and worse metabolic function in clinical conditions.

Biocompatibility of polyacrylamide microcapsules implanted in peritoneal cavity or spleen of the rat. Effect on various inflammatory reactions in vitro.

The biocompatibility of polyacrylamide microcapsules was investigated by implanting microcapsules in the peritoneal cavity or the spleen of rats. The capsules were retrieved every four weeks for twenty weeks. They remained isolated and free in the peritoneal cavity, but led to a slight inflammatory reaction in the spleen. These results were confirmed, in vitro, by determinations of chemiluminescence and interleukin I levels, and by evaluation of attachment and growth of human fibroblasts. The microcapsules did not enhance these inflammatory reactions, and they were a poor support for cellular proliferation.

[Plasma thyroglobulin in treated hypothyroidism in children].

In order to appreciate the value of the dosage of thyroglobulin (Tg) in the reappraisal of the classification of hypothyroidism after the onset of substitutive treatment and the supervision of patients treated with L-thyroxin (LT), plasma Tg and FT4 levels were studied in 42 samples from 21 hypothyroid children (ages ranging from 18 months to 16 years) under LT treatment. These patients were divided into 2 groups according to the results of scanning: group I: a thyroid (n = 8) and group II: ectopic or hypoplastic thyroid gland (n = 13). A control group consisted of 60 apparently healthy children of the same ages. Tg was undetectable in 11 samples of 6 children but significant levels (6.8 to 17 ng/ml) were found in 5 samples of 2 children. In group II, Tg could be measured in 20 of 26 samples, the mean level (+/- SEM) not being different from that in the control group (14.32 +/- 2.25 and 18.12 +/- 1.28 ng/ml, respectively). However, in this group, Tg levels seemed to be lower (9.75 +/- 3.94 ng/ml) in samples from patients with LT excess than in euthyroid or hypothyroid patients whose values of Tg were 15.8 +/- 3.36 ng/ml and 16 +/- 3.91 ng/ml, respectively.

[Biological aspects of endemic goiter in the Pyrennes Atlantiques (author's transl)].

In an endemical goiterous area (Oloron Sainte-Marie 64, France) a thyroid investigation has been realised on a certain number of people having a goiter (G) and those without goiter (N.G.). Various explorations such as T4 D, R T3 U, TSH RIA, T3 RIA. Achilles' Reflex, cholesterol have shown no significant difference nor any abnormality inspite of a global deficit in iodine (35 ng/24 h). It can only be retained an absence in negative corelation T3 TSH (P less than 0.05).
