Extracorporeal shockwave lithotripsy monotherapy for large renal calculi.

Extracorporeal shockwave lithotripsy (ESWL) monotherapy with a Dornier HM3 lithotripter was used to treat 199 large (greater than or equal to 3 cm) renal calculi. Calculi were classified as solitary (29), multiple (152) or staghorn (18) with stone-free rates of 55.2, 39.5 and 55.6% respectively. The stone-free rate was not statistically related to stone size, site or multiplicity. Complications occurred in 24 patients (12.8%) and were more common with solitary and staghorn calculi. ESWL is not recommended as primary treatment for most patients with large renal calculi.

Renal angiography in potential living related renal donors in Iraq.

Between July 1983 and March 1986, 160 potential living related renal donors underwent renal angiography. One hundred and twenty-eight have (Group 1) and 32 have not (Group 2) become renal donors. The addition of selective renal angiography did not alter the pre-operative arterial prediction made on flush aortography in 102 of 109 renal donors who had both investigations. At surgery, nine of the 13 unexpected arterial findings were in patients who had both forms of angiography. Complications of angiography in 16 patients (11%) included failed groin puncture (two), groin haematoma (eight), haematocoele (one) and loin pain (five). Three patients had renal artery damage detected at surgery, one of which resulted in graft loss. The arterial anatomy demonstrated was similar in 22 of the 23 patients in Group 2 who had flush and selective angiography. One patient (3%) developed a small groin haematoma. Forty-six patients (29%) had multiple renal arteries demonstrated angiographically. This paper highlights the incidence of renal artery damage following selective renal angiography and suggests that flush aortography provides adequate pre-operative information.

A morphological study of the effect of the Cavitron ultrasonic surgical aspirator system near human peripheral nerves.

The Cavitron ultrasonic surgical aspirator system was used to clear superficial tissue from the greater auricular nerve in four human subjects. Following stimulation, the nerves were removed together with an untreated section and processed for examination by light microscopy. Significant damage had been sustained by these nerves, which was not apparent in the control sections. Disruption of the perineurium, intraneural hemorrhage, endoneural edema, and degenerative changes of the myelinated nerve fibers were seen. Damage to neural tissue was evident 0.5 to 1.5 mm in advance of the point of contact with the cutting tip.

Lymphomas of salivary glands.

Primary lymphomas arising in salivary glands are very uncommon. The histologic classification of 40 cases of lymphomas in salivary gland tissue submitted to the British Salivary Gland Tumour Panel is reported, and, for 30 of the patients for whom adequate information was available, the clinical presentation, management, and outcome have been analyzed. Lymphomas in salivary glands represented 1.7% of all reported salivary neoplasms. The majority developed in the parotid glands of patients aged between 50 and 70 years. Only four cases gave a premorbid history compatible with sicca syndrome. In this series, non-Hodgkin's lymphomas predominated; 23 were Grade I, and 13 were Grade II. Treatment regimens were not uniform, but are outlined. Survival ranged from 5 to 111 months. Median survival for the group was 49 months. Prognosis was not influenced by the clinical stage of disease at presentation. Four cases of lymphoma arising in benign lymphoepithelial lesions are included. None had clinical symptoms of sicca complex. Prognosis for this group was found to be as favorable as the others.

Benign lymphoepithelial lesion: a less than benign disease.

The precise clinical correlate of the benign lymphoepithelial lesion is unclear. Thirty-six cases of benign lymphoepithelial lesions (BLL), reported to the British Salivary Gland Tumour Panel between 1971 and 1984, have been reviewed. Eighty per cent arose in the parotid gland and 20% were bilateral; 83% were in females and the mean age at presentation was 55.26 years. Only 50% presented with, or developed, symptoms of sicca complex, Sjogren's syndrome or related autoimmune disease. Two cases of BLL had, or went on to develop, extra salivary lymphomas and 5 cases of BLL had lymphomatous change in the initial biopsy. A further case had carcinoma within the benign lymphoepithelial lesion. None of those who developed lymphoma had sicca or Sjogren's syndrome but 3 of them had rheumatoid arthritis. The incidence of lymphomas (salivary or extra-salivary) in this series is very much higher than that reported in Sjogren's patients and amounted to 20% overall.

The stuffy nose: a comparative study of two common methods of treatment.

This study compares the efficacy of submucosal diathermy and cryotherapy in the treatment of nasal obstruction caused by engorged inferior turbinates. Subjectively they are equally effective but cryotherapy has the advantage that it can be performed under local anaesthetic and does not require hospitalization. Rhinomanometry suggests that cryotherapy causes more destruction of the submucosal vascular plexus than submucosal diathermy. Scanning electron microscopy 6 weeks postoperatively demonstrates that both treatment modalities cause widespread damage to the mucociliary epithelium.

Assessment of nasal airway patency: a comparison of four methods.

Two established methods (active posterior and passive anterior rhinomanometry) and 2 new methods (peak nasal inspiratory flow rate and apparent nasal volume) were used in 12 volunteers to assess the patency of the nasal airways under each of 4 conditions (baseline, post-exercise, nasal histamine and nasal cocaine). All methods showed the congestant effect of histamine but the peak nasal inspiratory flow and apparent nasal volume techniques were more sensitive to the 'decongesting' manoeuvres, (exercise and cocaine). Useful objective quantitative data on the patency of the nasal airways and its changes in response to stimuli can be obtained by simple, cheap and readily available techniques. Subjective sensation is a poor guide to the state of patency of the nasal airways.

Fine needle aspiration biopsy of neck lumps in a district hospital.

This paper examines the results of 117 fine needle aspiration biopsies performed on head and neck lumps over the first 3 years of its use in a district general hospital. The cytological diagnosis has corresponded exactly with the histological diagnosis in 51% of cases and the concurrence rate has been 84%. Fifty-seven (49%) of the 117 masses proved to be malignant and 79% of these were correctly diagnosed by needle biopsy. A further 12% of the aspirations were highly suspicious of malignancy. There were no false positive aspirations (specificity 100%) and 4 false negative results (sensitivity 93%). No complications were encountered.
