Two hundred and thirty-five females anxious to lose weight were asked to state their anticipated rate of weight loss each week or each month after the first few weeks of dieting. They were divided into four groups of 50 subjects, one group enrolling in a slimming club for the first time, one group already attending the same slimming organisation and two groups on first attendance at one of two hospital obesity clinics. The remaining group comprised 35 subjects re-enrolling in a slimming club. Only 27 per cent of those interviewed on their first attendance had a realistic concept of the anticipated rate of weight loss (AROWL) while 60 per cent of those re-enrolling and 90 per cent of those currently attending a slimming club were realistic in their expectations. The younger subjects were significantly more obese and the least realistic concepts of the AROWL were held by those subjects most in excess of their ideal weight. This study highlights the frequency with which unrealistic concepts of the AROWL are held and suggests the need to reinforce realistic concepts of weight loss in the obese.

Gene gun-based DNA immunization using vectors encoding HIV-1 gp120 or influenza virus nucleoprotein result in Th2-like immune responses following successive immunizations. The codelivery of vectors encoding IL-2, IL-7, or IL-12 blocked this effect by markedly enhancing gp120-specific interferon gamma production, and suppressing IL-4 and IgG1 responses. An unbiased augmentation of all immune responses was observed by increasing the resting period between immunizations. In this case, IFN-gamma production following in vitro stimulation increased by over 1000-fold, while IL-4, IgG1, and IgG2a responses were elevated as well. Interestingly, cytokine gene codelivery, in the context of the longer resting period, provided no additional stimulation of Th1-like responses such as IFN-gamma and IgG2a production, although there was still some suppression of IL-4 production. These data demonstrate that the quality and magnitude of responses elicited following epidermal administration of DNA vaccines can be manipulated by multiple means.
Juvenile dermatomyositis (JDM) is an uncommon disease that features muscle weakness, a characteristic rash, and vascular changes in skin, muscle, and other organs. We report a patient with JDM who developed multiple angiokeratomas, one of which was calcified. Electron microscopy of an angiokeratoma revealed altered connective tissue consistent with abnormal collagen. To our knowledge, angiokeratomas occurring in association with JDM have not been previously reported. Abnormal collagen distinguishes the angiokeratomas in our patient from those seen in other conditions. We propose that the angiokeratomas resulted from a progressive compensatory response to the obliterative angiopathy of JDM.
A 17-week-old boy with a giant congenital melanocytic nevus (GCMN) of the left lower extremity was noted to have a reduction in circumference of the left lower extremity relative to the contralateral side. The skin overlying the GCMN was persistently warm when compared with the surrounding and contralateral skin. Comparative plain radiography, ultrasonography, and magnetic resonance imaging showed fat hypoplasia of the left lower extremity, with bone and muscle appearing unaffected. The possible role of cytokines produced by the nevus in fat hypoplasia in GCMN is discussed.
Metastatic Crohn's disease (MCD) is a rare extraintestinal manifestation of Crohn's disease characterized by the histologic finding of granulomatous dermatitis at a site noncontiguous to the gastrointestinal tract. An adolescent had MCD of the face that was initially mistaken for severe, treatment-resistant acne. Histopathologic and microbiologic evaluation, combined with clinical examination and response to therapy, ultimately led to the correct diagnosis. Mycobacterium paratuberculosis is a suspected cause of Crohn's disease. Polymerase chain reaction was used to detect genomic DNA specific for the organism in biopsy specimens from the patient's cutaneous lesions. This study failed to demonstrate M. paratuberculosis in the specimens.
Expression of genes encoding prodrug-activating enzymes can increase the susceptibility of tumor cells to prodrugs, and may ultimately achieve a better therapeutic index than conventional chemotherapy. CB1954 is a weak, monofunctional alkylating agent which can be activated by Escherichia coli nitroreductase to a potent dysfunctional alkylating agent which crosslinks DNA. We have inserted the nitroreductase gene into an LNCX-based retroviral vector, to allow efficient gene transfer and expression in colorectal (LS174T) and pancreatic (SUIT2, BxPC3, and AsPC1) cancer cell lines. A clone of LS174T cells expressing nitroreductase showed > 50-fold increased sensitivity to CB1954, and nitroreductase-expressing clones of pancreatic tumor lines were up to approximately 500-fold (SUIT2) more sensitive than parental cells. Concentrations of CB1954 minimally toxic to nontransduced cells achieved 100% cell death in a 50:50 mix of parental cells with SUIT2 cells expressing nitroreductase; and marked "bystander" cell killing was seen with just 10% of cells expressing nitroreductase. Significant bystander cell killing was dependent on a high cell density. In conjunction with regional delivery of vectors and tumor selectivity of cell entry and/or gene expression, nitroreductase and CB1954 may be an attractive combination for prodrug-activating enzyme gene therapy of colorectal and pancreatic cancer.
