Aneurysm of the gastroduodenal artery.

We report a case of gastroduodenal arterial aneurysm incidentally discovered by abdominal ultrasonogram, in which three-dimensional computed tomography (3D-CT) was useful for diagnosis. An 85-year-old man was hospitalized because of fever and liver dysfunction. Carcinoma of the papilla of Vater was diagnosed by endoscopic retrograde cholangio-pancreatography, while abdominal ultrasonogram and CT scan revealed a round mass measuring 6.2x4.1 cm on the ventral side of the pancreatic head. Three-dimensional CT demonstrated that the mass arose from a branch of the common hepatic artery and projected downward, indicating a gastroduodenal arterial aneurysm. During pancreatoduodenectomy and aneurysmectomy, it was confirmed that this aneurysm arose from the gastroduodenal artery.

Craniosacral rhythm: reliability and relationships with cardiac and respiratory rates.

Craniosacral rhythm (CSR) has long been the subject of debate, both over its existence and its use as a therapeutic tool in evaluation and treatment. Origins of this rhythm are unknown, and palpatory findings lack scientific support. The purpose of this study was to determine the intra- and inter-examiner reliabilities of the palpation of the rate of the CSR and the relationship between the rate of the CSR and the heart or respiratory rates of subjects and examiners. The rates of the CSR of 40 healthy adults were palpated twice by each of two examiners. The heart and respiratory rates of the examiners and the subjects were recorded while the rates of the subjects' CSR were palpated by the examiners. Intraclass correlation coefficients were calculated to determine the intra- and inter-examiner reliabilities of the palpation. Two multiple regression analyses, one for each examiner, were conducted to analyze the relationships between the rate of the CSR and the heart and respiratory rates of the subjects and the examiners. The intraexaminer reliability coefficients were 0.78 for examiner A and 0.83 for examiner B, and the interexaminer reliability coefficient was 0.22. The result of the multiple regression analysis for examiner A was R = 0.46 and adjusted R2 = 0.12 (p = 0.078) and for examiner B was R = 0.63 and adjusted R2 = 0.32 (p = 0.001). The highest bivariate correlation was found between the CSR and the subject's heart rate (r = 0.30) for examiner A and between the CSR and the examiner's heart rate (r = 0.42) for examiner B. The results indicated that a single examiner may be able to palpate the rate of the CSR consistently, if that is what we truly measured. It is possible that the perception of CSR is illusory. The rate of the CSR palpated by two examiners is not consistent. The results of the regression analysis of one examiner offered no validation to those of the other. It appears that a subject's CSR is not related to the heart or respiratory rates of the subject or the examiner.

[Neuroimaging of motor disturbances].

High-field MRI has given us a great impact in the clinical diagnosis of various motor disturbances, because the structural abnormalities of the pyramidal tract, basal ganglia and cerebellum are easily observed with this technique. In case of lower motor neuron disease without clinical upper motor neuron sign, proton density-weighted image of the internal capsule and cerebral peduncle is useful for detecting the latent pyramidal tract degeneration. Differential diagnosis of parkinsonism is one of the most successful examples of the clinical use of high-field MRI. Postero-lateral putaminal T2-hypointensity which has been reported to be diagnostic for multiple system atrophy (MSA) might be seen in the aged normal subjects, whereas the proton density weighted image of the normals revealed no abnormal change in signal intensity. In MSA, not only T2-weighted but also proton density image show marked hypointensity in posterior part of putamen. Proton density image of pons, moreover, reveals "four of dice sign" representing the degeneration of the pontine transverse fibers. Another pathological condition which could be differentiated from PD with MRI is PSP. Characteristic "humming-bird sign" of the mid brain on the mid-sagittal section is thought to represent the atrophy of the rostro-dorsal mid-brain tegmentum, where the vertical gaze centers are located. MRI is also useful for differentiating the pathological processes of cerebellar atrophies. Cortical cerebellar atrophy with loss of Purkinje cells shows the disappearance of T2-hypointensity of dentate nucleus, while cerebellipetal atrophy like olivopontocerebellar atrophy causes T2-hyperintensity of cerebellar white matter making dentate hypointensity more conspicuous. Degeneration of the dentate neucleus is revealed by the T2-hyperintensity of dentate contour associated with atrophy of superior cerebellar peduncle at the level of rostral pons.

[A case of dural arteriovenous fistula accompanied by sinus occlusion. A serial study with CT scan].

A dural arteriovenous fistula (AVF) of the posterior fossa was found in a 58-year-old businessman suffering from a month's history of dementia. Bilateral transverse sinus thrombosis was also found and his symptom was attributed to venous congestion caused by the sinus occlusion. He had received partial resection of the right trigeminal neurinoma two years before, and the operation was thought to have caused the sinus occlusion. Dural AVF of the posterior fossa usually causes tinnitus or headache, whereas dementia seen in present case is relatively rare. In the present case, serial study of cranial CT scan demonstrated evolution of dural AVF after sinus occlusion. Venous congestion was thought to re-open the physiological micro-AVF to cause the dural AVF.

Pulmonary mycotoxicosis: a clinicopathologic study of three cases.

Pulmonary mycotoxicosis (PM), also termed organic dust toxic syndrome or silo unloader's syndrome, is an acute illness resulting from massive inhalation of microbial toxins in organic dusts. It has not been well described histologically. Three cases of PM are presented in this report. Open lung biopsies were examined in each case. All of the patients were farmers with no prior lung disease. One had burning in his eyes, throat, and chest after exposure to moldy silage; chills, fever, dry cough, malaise, and weakness developed within 24 hours. Two patients presented with fever, progressive dyspnea, cough, and fatigue within 24 hours of emptying a corncrib, cleaning a chicken coop, and baling hay. Bilateral alveolar and interstitial infiltrates on chest roentgenograms and leukocytosis with neutrophilia were observed in all of the three patients. Two patients became hypoxemic and required mechanical ventilation. Histologic examination showed acute and organizing diffuse alveolar damage in two biopsy specimens and an acute bronchopneumonia in the third. One specimen had 1- to 10-microm ovoid organisms demonstrable with methenamine silver stains; cultures grew Fusarium and Penicillium species. The other two biopsy specimens had negative tissue cultures and special stains for organisms, although Penicillium species were grown from a preoperative bronchoalveolar lavage in one case. The two patients on mechanical ventilation recovered completely with high-dose steroids. The third patient recovered without steroids. No patient had residual functional deficits or chest radiographic abnormalities. PM can be distinguished from allergic and infectious diseases common in individuals exposed to large amounts of organic dust by its clinicopathologic features.

[Miliary tuberculosis associated with subcutaneous tuberculous abscess].

A 62-year-old woman was admitted because of a tumor on her right thigh, fever, generalized lymphadenopathy, and diffuse nodular shadows on chest X-ray films. She was given a diagnosis of miliary tuberculosis based on the findings of a cervical lymph-node biopsy and a broncho-alveolar lavage. Acute respiratory failure and disseminated intravascular coagulation developed, but resolved after the start of anti-tuberculous therapy. The tumor on the right thigh was diagnosed as a subcutaneous tuberculous abscess because tuberculous bacilli were detected in tumor tissue samples obtained by aspiration. The patient's fever disappeared and the abnormal shadows on her chest X-ray films receded significantly after drainage of the subcutaneous abscess. These findings suggested that miliary tuberculosis was associated with the subcutaneous tuberculous abscess in this case.

[Interstitial pneumonia preceding systemic sclerosis].

We describe a 57-year-old man with interstitial pneumonia associated with systemic sclerosis. About 3 years prior to the appearance of distinctive signs of systemic sclerosis, he was admitted to our hospital with a chronic dry cough. A chest roentgenogram on admission revealed reticulonodular shadows in both lung fields. There were no abnormal laboratory findings. Open lung biopsy specimens revealed patterns indicative of usual interstitial pneumonia, and myxomatous connective tissue within the lumen of the airways. Skin biopsy specimens showed heightened levels of collagen in the dermis, a finding consistent with systemic sclerosis. The patient was given a diagnosis of lung involvement preceding systemic sclerosis despite the absence of concurrent skin symptoms.

Functional importance of heat shock protein 90 associated with insulin receptor on insulin-stimulated mitogenesis.

The role of stress proteins on the function of insulin receptor is not well understood. In the rat-1 fibroblasts overexpressing human insulin receptors, heat shock protein (Hsp) 90 was co-immunoprecipitated with insulin receptors and the association was not affected by insulin stimulation. A GST-fusion protein containing the intracellular insulin receptor beta subunit was associated with Hsp 90 in vitro, suggesting the direct interaction of this protein with insulin receptor beta-subunit. Furthermore, microinjection of anti-Hsp 90 antibody into these cells completely inhibited insulin-stimulated mitogenesis. However, neither epidermal growth factor-stimulated nor serum-stimulated mitogenic signal in the cells was affected by the antibody microinjection. These results suggest that Hsp 90 constitutively binds to insulin receptor beta-subunit, which may be necessary for insulin signaling in mitogenesis.
