Analysis of Bcl-2, Bax and Survivin genes in uterine cancer.

To investigate the role of the apoptosis-related genes, Bcl-2, Bax and Survivin genes were analyzed. For the Bax gene, abnormality was detected in 1 of 7 cervical and 1 of 6 endometrial cancer cell lines, 1 of 25 cervical cancer tissues and none of 17 endometrial cancer tissues using PCR-SSCP. In 4 cervical and 2 endometrial cancer cell lines, the ratio of Bcl-2 to Bax expression was higher than the control ratio using Western blotting. Survivin mRNA was detectable in all cell lines and all cancer tissues. The data suggested that these apoptosis-related genes may play important roles in the pathway of carcinogenesis of human uterine cancer.

[A case of an 86-year-old patient treated with thoracoscopic pericardiectomy of recurrent pericardial effusion].

We performed thoracoscopic pericardiectomy to an 86-year-old patient with recurrent pericardial effusion of unknown origin. Etiology of pericardial effusion was established by this procedure. Postoperative course was uneventful. She was discharged within two weeks and has been well 8 months postoperatively. Thoracoscopic surgery is superior to thoracotomy in terms of less invasion and postoperative pain. Thoracoscopic surgery is replacing standard thoracotomy in the management of many thoracic diseases.

[Basic patterns of learning disability: follow-up study of very low birth weight children].

The purpose of this study was to characterize learning disability (LD) in low birth weight children (VLBW). Longitudinal mental development of VLBW without major handicap was investigated using several neuropsychological tests (ITPA, BGT, WISC-R) suitable for their ages. The tests at ages of 5-6 years demonstrated delays in visual perception and visual motor integration. In the school-age, impairments of language understanding and abstraction were suggested. The process of mental development in VLBW with LD was reported in detail on the cases requiring special education. Some cases with a pervasive developmental disorder (PDD) were also presented to show difference from LD.

[Clinical evaluation of PCR method for detection of cytomegalovirus DNA].

Polymerase chain reaction (PCR) to detect Cytomegalovirus (CMV)-DNA from the clinical specimens is useful to diagnose CMV infection. Eighty-one specimens of 31 patients including peripheral blood, bronchioalveolar lavage fluid, biopsy tissues, feces, urine, sputum and etc. and normal peripheral blood from 59 volunteers were used in this study. After DNA extraction each samples was amplified by the seminested PCR using primers recognizing sequences in the Immediate-early gene of CMV. This PCR method specifically detected more than 10 virus copies even in the presence of the genomic DNA. CMV-DNA was detected in only one of 59 normal peripheral bloods (1.7%). Six of 31 patients were clinically diagnosed as CMV infection by anti-CMV therapy. These 6 patients were positive in the peripheral blood by PCR for CMV, and 5 of them were positive in other samples. However, 3, 5 and 1 of 25 patients, who were clinically diagnosed as not having CMV infection, were also positive in peripheral blood, in the other samples and in both, respectively. The PCR method was able to examine any clinical samples. To examine both the peripheral blood and the samples from infected organs is helpful for the diagnosis of CMV infection.

[Five cases of systemic sclerosis with associated with intestinal pseudo-obstruction].

OBJECTIVES: We experienced five cases of systemic sclerosis (SSc) associated with pseudo-obstruction, and evaluated their clinical characteristics and outcome. METHODS: Five patients with SSc were found to have intestinal pseudo-obstruction. We studied the clinical characteristics, treatments and outcome of these five case from medical records. RESULTS: The intestinal symptoms were free air in three and pneumatosis cystoides intestinalis (PCI) in two of the five patients. Cisapride was administered to all patients and octreotide and dinoprost to three patients, but the effects were not adequate and intravenous hyperalimentation (IVH) had to be introduced in four patients. Drug treatment was effective in patients in the early stage of intestinal symptoms, but IVH had to be introduced in patients with a long course. Septicemia occurred in two of the four patients on IVH and an infection occurred at the catheter insertion site in one patient. These two patients died of septicemia. Of five patients we experienced with SSc associated with intestinal pseudo-obstruction, two had PCI and four required IVH at home. Two of the four on home IVH developed septicemia. CONCLUSIONS: Intestinal pseudo-obstruction is one of the poor prognostic factors for SSc.

Effect of intermittent liver ischemia on outcome in patients with hepatocellular carcinoma on liver cirrhosis.

The influence on postoperative liver function of intermittent normothermic hepatic ischemia in cirrhotic patients was studied retrospectively. The mean total ischemia time was 88 (range 30-140) minutes in the hemi-hepatic occlusion group, and 68 (range 10-187) minutes in the total occlusion group. There were no operative deaths due to hepatic failure. Postoperative liver function improved within 1 week of the operation. There was no significant difference in the incidence of postoperative complications between the groups. Thus normothermic hepatic ischemia is tolerated for up to 180 minutes in the cirrhotic liver when an intermittent technique (15 minutes clamped and 5 minutes unclamped) is used.

Host immune profiles and genotyping analysis of HIV-1 among Japanese hemophiliac patients.

We studied the immune profiles of 21 Japanese hemophiliac patients who had been infected with human immunodeficiency virus type 1 (HIV-1) by the blood preparations during the 1982-84 period, and carried out the genotyping of HIV-1 V3 region for uncultivated peripheral blood mononuclear cells (PBMC) from 8 patients. Ten years after infection, asymptomatic carrier (AC, 14 cases) yet outnumbered those in AIDS related complex (ARC, 4 cases) and AIDS (3 cases), and the CD4+ and CD8+ T-cell numbers were not correlated with clinical stages. Macrophage tropic sequences, as identified according to the known tropism determinants, appeared to be more frequent as the CD4+ T-cell numbers were higher. There was no correlation of the cell tropism with the disease stages, however. It might be due to HIV-1 of CD4+ T-cells being more productive. The mutation in the V3 region appears to differentially influence the escape from antibody attack and clinical stages, and the cell tropism may not be related to the cell immunity of the host.

A study comparing ultrasound images of tongue movements between open bite children and normal children in the early mixed dentition period.

The tongue surface movement of young patients with or without open bite was evaluated by ultrasound images taken on the posterior part of the tongue on the frontal plane to determine whether there is any relationship between the tongue movement and open bite malocclusion in the early mixed dentition period. The standardized reproducible ultrasound images were obtained using ultrasound diagnostic equipment that was fabricated in a cephalostat unit having a probe stabilizer. At the central portion of the tongue, no significant differences were detected in the duration or the momentum of tongue surface movement on a frontal plane was detected between the patients with and without open bite during swallowing. The farther the region was from the central portion, however, the longer the duration and the bigger the momentum of the tongue surface movement was found in the open bite group. Based on this evidence, it is suggested that an open-bite child swallows by using a tongue thrust motion that forms using a large depressed area in the tongue, whereas a normal child without tongue thrust forms a rather narrow groove in the central portion of the tongue when swallowing.

Coagulation factor X-binding protein from Deinagkistrodon acutus venom is a Gla domain-binding protein.

Factor IX/factor X-binding protein (IX/X-bp) is an anticoagulant isolated from the venom of Trimeresurus flavoviridis (habu snake) and binds predominantly to factor IX. In this study, we isolated IX/X-bp-like proteins from the venom of Deinagkistrodon acutus (hundred pace snake) with binding characteristics different from those of IX/X-bp. The complete amino acid sequence and binding characteristics of the main anticoagulant protein, named X-bp, were investigated. The concentrations of X-bp at half-maximal binding to solid-phase factors X and IX were 0.4 and 3 nM, respectively. The binding of X-bp to solid-phase factor X was inhibited by 50% by 6- and 9-fold excess concentrations of factor X and Gla domain (GD) peptide 1-44, respectively, but was not influenced by GD peptide 1-41 and Gla domainless factor X. X-bp bound two Ca2+ ions per molecule with Kd values of 16 +/- 0.7 (mean +/- SE, n = 6) and 103 +/- 10 microM. X-bp was a heterodimer of C-type lectin-like subunits. The 16 kDa chain (A chain) consisted of 129 amino acid residues and was 68% identical to the sequence of the A chain of IX/X-bp. The 15 kDa chain (B chain) consisted of 123 amino acid residues and was 87% identical to IX/X-bp. Three-dimensional model construction from the known fold of IX/X-bp showed that amino acid residues different from those of IX/X-bp are mostly on the molecular surface. Some of these are concentrated on a part of the concave surface which is considered to be the coagulation factor-binding site, presumably acting as a discriminator for ligand binding. These results indicated that X-bp isolated from D. acutus venom was a GD-binding protein, and the C-terminal region of GD peptide was critical for folding of the peptide.

The microglia/macrophage response in the neonatal rat facial nucleus following axotomy.

Microglia represent a population of brain macrophage precursor cells which are intrinsic to the CNS parenchyma. Transection of the facial nerve in the newborn rat causes death of the affected motor neurons which is accompanied by massive activation of local microglia. Many of these cells develop into macrophages as can be shown by immunocytochemistry for OX-42 and ED1. Using the new polyclonal microglial marker ionized calcium binding adapter molecule 1, iba1, in combination with immunocytochemical double-labeling for the proliferating cell nuclear antigen (PCNA), or [3H]thymidine autoradiography, and confocal microscopy, qualitative as well as quantitative differences can be demonstrated between the newborn and the adult axotomized rat facial nucleus. While microglial cells are the only cell population which responds to axotomy by cell division in the adult facial nucleus, GFAP positive reactive astrocytes can be shown to undergo mitosis following axotomy in the newborn rat. Furthermore, ED1 immunoreactivity, early expression of MHC class II molecules and morphological transformation of microglia into macrophages can only be observed under conditions of neuronal degeneration, i.e., in the neonatal rat facial nucleus. Thus, the combination of cellular markers described here should be useful for studies employing the neonatal rat facial nucleus as an in vivo assay system to test the efficacy of neurotrophic factors.

Estrogen attenuates endothelin-1 production by bovine endothelial cells via estrogen receptor.

To investigate the mechanism underlying the antiatherogenic effect of estrogen, we examined the role of estrogen in the regulation of endothelin-1 (ET-1) production by cultured bovine carotid arterial endothelial cells. Treatment of the cells with 17beta-estradiol dose dependently inhibited gene expression and peptide secretion of ET-1. This inhibitory effect of 17beta-estradiol was more marked when ET-1 production was stimulated by transforming growth factor-beta1. Simultaneous addition of ICI 182,780, a pure estrogen receptor antagonist, completely blocked the effect of 17beta-estradiol, indicating that 17beta-estradiol acted via estrogen receptor. These results suggest that reduced ET-1 production by endothelial cells may be involved in the vasoprotective effect of estrogen.
