Chromosomal anomalies in cryptorchidism.

Chromosome analysis was performed in 160 patients with cryptorchidism. Chromosomal anomalies were found in 7 patients (4.4%). The incidence of chromosomal abnormalities was not significantly different between patients with bilateral or unilateral cryptorchidism. Of 7 patients, 1 had sex chromosomal aberration, 2 had marker chromosome and 4 had autosomal anomalies. Additional congenital anomalies were observed in 1 with sex chromosomal aberration, 2 with marker chromosome and 2 with autosomal anomaly. These facts indicate that we had better perform chromosome analysis in all patients with bilateral or unilateral cryptorchidism.

Role of symptom scoring and uroflowmetry in patients with diabetic cystopathy.

Forty-two patients with diabetes mellitus were evaluated by uroflowmetry and the AUA Symptom Index. After urodynamic and physiological investigations 12 patients were diagnosed as having diabetic cystopathy, while 22 patients as being urodynamically normal. In patients with diabetic cystopathy, most of the parameters in uroflowmetry were significantly different from those of normal subjects. Subjective symptom assessment revealed increased total obstructive scoring (p < 0.05). However, irritative symptom indexes in patients with diabetic cystopathy were not different from those of the normal subjects. Although uroflowmetry and the AUA Symptom Index are not specific to identify detrusor impairment, these methods appeared to be useful to decide upon further urodynamic investigations.

Derivative Y chromosome resulting from a t(Y;15) (q12;q11.2) in a boy with Prader-Willi syndrome.

Chromosome analysis of peripheral lymphocytes from a boy with Prader-Willi syndrome showed the presence of 45 chromosomes, including der(Y) resulting from a t(Y;15) (q12;q11.2). In situ hybridization using DYZ3 and D15S11 showed a positive signal and negative signal in derivative Y chromosome, respectively. The deficiency of 15pter q11.2 may not influence the clinical manifestation of Prader-Willi syndrome.

Malacoplakia of the bladder: efficacy of enoxacine therapy.

Malacoplakia of the urinary bladder was seen in a 69-year-old woman with haematuria. We firstly treated her with distigmine bromide, ascorbic acid and trimethro-prim-sulfamethoxazole. Haematuria subsided but the lesion did not change after 6 weeks. Thereafter we tried enoxacine therapy. Eight weeks after the medication the tumorous lesion disappeared. Bladder biopsy performed 4 months after the initiation of the treatment revealed predominantly granulation tissue. We can find no previous case treated successfully with long-term enoxacine.

Comparison of the nephrotoxicity between ioversol and iohexol.

To evaluate the renal toxicity of intravenous administration of ioversol and iohexol, the urinary N-acetyl-beta-glucosaminidase (NAG) to creatinine ratio was measured before and after drip infusion pyelography. Thirty patients received intravenous ioversol and 29 patients received iohexol. The urinary NAG to creatinine ratio tended to increase after respective contrast media: 5% increase in the ioversol-treated, and 8% in the iohexol-treated patients. There were no significant differences in these values; the nephrotoxicity of ioversol appeared to be just as insignificant as that of iohexol.

Dihydroxyphenylglycol in pheochromocytoma: its diagnostic use for norepinephrine dominant tumor.

PURPOSE: To discern whether the 3,4-dihydroxyphenylglycol produced in adrenal medulla is associated with altered urinary excretion, we compared the tissue and urinary levels of this catecholamine metabolite, epinephrine and norepinephrine in 23 patients with pheochromocytoma and 26 subjects with renal cell carcinoma. MATERIALS AND METHODS: Tissue and urine concentrations of dihydroxyphenylglycol, epinephrine and norepinephrine were determined by catechol-O-methyl-transferase-based radioenzymatic method and high performance liquid chromatography with electrochemical detection. RESULTS: Contents of dihydroxyphenylglycol, epinephrine and norepinephrine in pheochromocytomas were higher than those in normal adrenal medullae. Removal of pheochromocytomas lowered urinary excretion of these catecholamines and dihydroxyphenylglycol. Norepinephrine content correlated closely with dihydroxyphenylglycol content in normal adrenal medullae and pheochromocytomas but not with epinephrine content in either tissue. Contents of norepinephrine and dihydroxyphenylglycol correlated significantly with urinary norepinephrine excretion and dihydroxyphenylglycol excretion in patients with pheochromocytoma but not in subjects with renal cell carcinoma. The norepinephrine-to-dihydroxyphenylglycol ratio in a 24-hour preoperative urine collection from patients with pheochromocytoma was higher than that from subjects with renal cell carcinoma, which was almost similar to that of epinephrine dominant pheochromocytomas. CONCLUSIONS: Our study suggests that dihydroxyphenylglycol in urine in subjects with renal cell carcinoma is predominantly of a neuronal origin and urinary dihydroxyphenylglycol in patients with pheochromocytoma is of tumor origin. A high level of norepinephrine-to-dihydroxyphenylglycol ratio in urine can be used to determine the diagnosis of the norepinephrine dominant type pheochromocytoma. Measurement of at least a few markers is preferable for precise biochemical diagnosis of pheochromocytoma.

Electrical pelvic floor stimulation: a possible alternative treatment for reflex urinary incontinence in patients with spinal cord injury.

The present study demonstrates the clinical experience of pelvic floor stimulation using percutaneous implantable electrodes and implantable electrical stimulator for the treatment of reflex urinary incontinence in patients with spinal cord injury. Pelvic floor stimulation was carried out on six paraplegic patients who had urinary incontinence from an overactive bladder. After the percutaneous implantation of a pair of electrodes, chronic stimulation was carried out by employing an implanted receiver or an external pulse regulator. Within 4 to 16 weeks of electrical stimulation urinary incontinence was improved in four of the six patients. In two of these six patients, incontinence was completely abolished subjectively. Urodynamic investigations demonstrated an increased volume at the first unstable contraction (P < 0.01) in all of the patients. Inhibition of detrusor overactivity was obtained from this procedure. The stimulation effect appeared to be constant during chronic stimulation. This new procedure probably provides a stable and reliable stimulation effect for long term treatment, and may be an alternative treatment for previous external electrical pelvic floor stimulation.

Pathological changes of traumatic dislocated testis.

We present a 17-year-old patient with a right testicular dislocation. In this case, dislocated testis had been considered as hematoma and therefore remained untreated for 4 months. The patient underwent right orchiopexy 4 months after the injury. Intraoperative testicular biopsy revealed impaired spermatogenesis and existence of many alternative Sertoli cells, but no atrophy of seminiferous tubules. Six months after the operation, semen analysis of the patient revealed the deformity of most sperms. Eight months after the operation, rebiopsy of the right testis disclosed slight improvement in the spermatogenesis of the testis. Damage of the testis appears to be not severe in dislocation and may be recovered by repositioning.

Spontaneous hemorrhage during pregnancy secondary to renal angiomyolipoma.

We report a case of spontaneous hemorrhage secondary to renal angiomyolipoma treated with percutaneous transcatheter arterial embolization. Fourteen and 18 months after the procedure, the patient showed repeated spontaneous hemorrhage during pregnancy. However, a healthy female infant was delivered at the 39th week of the pregnancy. We discuss the management of renal angiomyolipoma in young married women before pregnancy.

Correlations between parameters in uroflowmetry and histological compositions in patients with benign prostatic hyperplasia.

The objective of this study was to determine whether area densities of histological elements within prostatic adenoma affect objective and subjective parameters of patients with prostatic hyperplasia. Modified uroflowmetry parameters including Qmax/T100, TQmax/T100 and voiding efficiency were also measured. The area densities of histological elements did not affect uroflowmetry parameters. In addition, subjective symptom score was not correlated with morphometric findings. Our findings suggest that the degree of infravesical obstruction as measured by uroflowmetry parameters and symptom scores cannot be explained by the tissue compositions of adenomas alone.

Testotoxicosis proved by immunohistochemical analysis and successfully treated with cyproterone acetate.

We present a case of testotoxicosis diagnosed on the basis of immunohistochemical investigation of the testis. The patient was treated with the antiandrogen cyproterone acetate at a dose of 70-130 mg/m2/day. Clinical and biochemical features are consistent with the diagnosis of testotoxicosis. The long-term experience with cyproterone acetate had been generally satisfactory. There are, to our knowledge, no reports on cyproterone acetate treatment of testotoxicosis.

Age-related changes of histological composition in established benign prostatic hyperplasia.

To clarify the effect of aging on the histological composition of established benign prostatic hyperplasia (BPH), quantitative morphometric analyses were performed. Specimens were obtained from 23 patients who underwent transurethral resection of the prostate due to symptomatic BPH. Smooth muscle cells, fibrous tissue, glandular cells, glandular lumen and vascular tissue were quantified. Stromal and glandular components of established BPH did not correlate with age, whereas the proportion of vascular tissue was inversely correlated with age (r = -0.572, p = 0.0046). In the present series, the main characteristic of BPH tissue of elderly men appeared to be decreased vascularity.

Dicentric Y chromosome without evidence of mosaicism in an azoospermic male.

A phenotypically male of dicentric Y chromosome with no evidence of sex chromosomal mosaicism is reported. Staining of fluorescence in situ hybridization using Y chromosome centromere-specific DNA probe showed the presence of two bright fluorescent spots in Y chromosome. Testicular histology revealed maturation arrest at the stage of primary spermatocyte. It is suggested that structural anomaly of the Y chromosome is related to the disturbance of spermatogenesis.

Increased vascular collagen and noncollagenous protein synthesis contributes to sustain chronic phase of two-kidney, one-clip renovascular hypertension.

PURPOSE: Although the enhanced renin-angiotensin (R-A) system responsible for two-kidney, one-clip (2K-1C) hypertension is well known, there may be a shift with time so that this, hemodynamic factor plays a less important role and increased vascular resistance is predominant in sustaining hypertension. While increased vascular protein synthesis has been demonstrated in genetically hypertensive rats, we evaluated the possible relationship between vascular protein synthesis and 2K-1C hypertension with special reference to the R-A system. MATERIALS AND METHODS: Two-kidney one-clip rats were treated with splanchnicotomy, beta-aminopropionitrile (collagen inhibitor), or captopril (angiotensin converting enzyme inhibitor) in the acute or chronic hypertensive phase. 3H-proline was injected into rats, and incorporation rates of 3H-proline into vascular collagen, noncollagenous protein and elastin were counted. The plasma level of the R-A system was assayed. RESULTS: In the acute phase of 2K-1C hypertensive rats whose R-A system was enhanced, captopril treatment further enhanced plasma renin activity and plasma angiotensin I and suppressed plasma angiotensin II while reducing blood pressure. Synthesis of the vascular proteins was almost identical. In the chronic phase of 2K-1C hypertensive rats whose R-A system was within normal limits, increased incorporation rates of 3H-proline into noncollagenous protein or collagen of mesenteric arteries were decreased by splanchnicotomy or beta-aminopropionitrile and hypertension was lowered. Captopril failed to reduce protein synthesis. CONCLUSIONS: An enhanced R-A system participates in the pathogenesis of the acute phase of 2K-1C hypertension while increased noncollagenous protein and collagen syntheses of small arteries appear to play some role in the etiology of the chronic phase of hypertension.

Suppression of sympathetic nervous system attenuates the development of two-kidney, one-clip Goldblatt hypertension.

PURPOSE: This study tried to assess the possible contribution of the sympathetic nervous system to the onset of two-kidney, one-clip (2K-1C) Goldblatt hypertension. MATERIALS AND METHODS: The effect of chlorisondamine administration with or without subsequent splanchnicotomy on the development of hypertension was examined in 2K-1C rats with special reference to norepinephrine synthesis. RESULTS: The 2K-1C rats were treated either with chlorisondamine or chlorisondamine plus subsequent splanchnicotomy, so that the development of hypertension was effectively arrested for 4 weeks. An apparently high rate of release of norepinephrine in 2K-1C rats was reduced by treatment with chlorisondamine plus splanchnicotomy. A similar trend was also seen in plasma norepinephrine concentration and norepinephrine clearance, to a lesser extent. There were significant positive relationships between percent change of systolic blood pressure and apparent rate of release of norepinephrine, plasma norepinephrine concentration and norepinephrine clearance in 2K-1C rats and 2K-1C + chlorisondamine + splanchnicotomy rats. There were no significant relationships in these parameters in sham-treated rats. CONCLUSION: Increased sympathetic innervation appears to participate in the development of 2K-1C Goldblatt hypertension.

Serum concentration of iopamidol after retrograde urethrography in male patients with outlet obstructive symptoms.

Serum concentration of iopamidol was measured in 29 male patients with outlet obstructive symptoms after retrograde urethrography. Although no patient showed extravasation, serum iopamidol level was detectable in 4 (14%) patients. Since severe adverse effects are considerably reduced with iopamidol, we prefer to use this contrast medium for retrograde urethrography in male patients with outlet obstructive symptoms.

Successful treatment of vesicoureteric reflux by subureteric injection of silicone.

Although the technique of subureteric injection has been widely accepted as an alternative to reimplantation in the treatment of vesicoureteric reflux (VUR), the choice of the material to be used is controversial. We report our experience with the endoscopic treatment of VUR by a submucosal injection of silicone. A 24-year-old man who had VUR (Grade II) and renal insufficiency was treated by endoscopic injection of silicone. Cystography after the treatment showed absence of reflux.
