A mathematical model describing the glycemic response of diabetic patients to meal and i.v. infusion of insulin.

Nine type 1 diabetic patients were studied for 24 hours. During this period they were given three calibrated meals. The glycemia was feedback-controlled by means of an artificial pancreas. The blood concentration of glucose and the infusion speed of the insulin were measured every minute. The experimental data referring to each of the three meals were used to estimate the parameters of a mathematical model suitable for describing the glycemic response of diabetic patients at meals and at the i.v. infusion of exogenous insulin. From the estimate a marked dispersion of the parameters was found, both interindividual and intraindividual. Nevertheless the models thus obtained seem to be usable for the synthesis of a feedback controller, especially in view of creating a portable artificial pancreas that now seems possible owing to the realization (so far experimental) of sufficiently reliable glucose concentration sensors.

LDLR, GYPA, HBGG, D7S8 and GC allele and genotype frequencies in the northwest Italian population.

Allele and genotype frequencies for five PCR-based DNA markers (LDLR, GYPA, HBGG, D7S8 and GC) were determined in 100 unrelated individuals from Piedmont (Northwest Italy). All five Ioci met Hardy-Weinberg expectations in the sampled population. The combined PD and CE were, respectively, 0.995 and 0.697. Frequencies obtained were compared with other previously published data on Caucasian populations with no significant differences. The genetic data from this study, in addition to those already collected by other groups, contribute to the expansion of the Italian DNA database suitable for forensic casework and paternity testing.

[Esthetic problems in surgery of the male external genitalia: medico-legal aspects].

The author explains medical meaning of aesthetical function, personal aesthetic compound and a legal meaning of permanent impairment and damage compensation. Scar is the most well-known expression of aesthetical permanent impairment. Italian law privileges face scar, but increased medical liability trials raised the problems of connecting evaluation about skin scars caused by surgical fault. This research work, made in Turin University, introduces fixing method that draw up addition numbers to settle 5 chosen scar parameters: colour, relief, extension, seat, direction (named CRESO in abbreviation).

[Home delivery in Italy and the Netherlands. Experience and comparison].

The study of the emerging "phenomenon" of home birth calls, to a greater extent than for other forms of assistance provided by health workers, for a sophisticated analysis capable of integrating a knowledge of legal norms, economic and sociocultural aspects, technical assistance procedures, professional and corporative aspects, and in particular medicolegal aspects. The first part of this study outlines the legislative evolution of assistance in a hospital setting and the professional training of midwives and other operators involved in the birth process in Italy. These dispositions reveal the distinct characteristics and responsibility of the various types of health operators in this specific area of the health service, as well as the formulation and development of legislation on home birth, with special reference to Piedmont Region. This is followed by an analysis of home birth in Holland, giving a description of the context, the professional figures involved and the organisational and procedural set-up (based on economic considerations and on a territorial health planning through a series of very detailed networks that differ considerably from the form of obstetric and gynecological assistance in Italy related to pregnancy and birth). In the discussion the authors examine the evolution and topicality of informed consent, which is then referred to the specific theme of assistance for pregnant women opting for a home birth; they comment on the current state of affairs and highlight the lacunae in the proposed regional legislation that correctly offers a choice between hospital and home birth, but does not present the instruments for the prior verification of the midwife's capacity to perform the specific method or even the indications for providing an adequate level of information and collaboration from the pregnant woman. The authors conclude by presenting a series of suggestions and hypotheses regarding both training aspects for those assisting home birth in obstetric schools and for the institution of control mechanisms regarding this particular freelance profession within the context of the appropriate guild.

Orthopantomography and INAIL assessment of dental injury. Istituto Nazionale per l'Assicurazione contro gli Infortuni sul Lavoro.

The protection of workers' health is defended by the Italian Constitution, and sees its most extensive application in the work of the INAIL (Istituto Nazionale per l'Assicurazione contro gli Infortuni sul Lavoro-National Institute for Assurance against Work-related Accidents). In the T.U. (Testo Unico-Complete Text of Legislation) 1124/1965, dental injury is compensated with fixed percentages for permanent impairment, depending on whether the prosthesis applied is effectual or otherwise. The INAIL's primary task of assigning the necessary treatment and recovering the claimant's occupational aptitude requires that it meets in full the cost of his dental rehabilitation. In Turin in 1981 a systematic procedure was set up for the provision and monitoring of the dental treatment received by the injured person. Legislation relating to prostheses has further widened the scope of dental treatment. To deal correctly with the legal medicine aspects of dental rehabilitation, the authors illustrate a working procedure which is based on the reading of radiological evidence in order to determine the prior situation, extent of injury, results of the treatment and evaluation of residual permanent injury.

Orthopantomography and the determination of majority age.

The problem of determining a subject's age to ascertain whether he can be criminally charged brings to prominence all the methods of investigation which can be used to define a person's biological age. The methodologies used to determine dental age still represent today the most sensitive means of arriving at this end. Researchers in Turin in 1980 elaborated an equational formula to determine a subject's presumable age by means of orthopantomography, basing the examination on the mineralization times of 4.5, 4.7, 4.8. After thorough checks carried out on sample groups of urban inhabitants, it was noted that the phases of mineralization of the 3rd molar can provide an answer to the judicial authorities' query as to whether the subject in question has come of age. The phase of mineralization of the 3rd molar is evaluated according to the table we have proposed during a previous conference and in another paper, which establishes 12 phases of mineralization. Focusing our attention on the final 12th phase, corresponding to the complete mineralization of the tooth roots, it was noted that this was present in 93 subjects, of whom 43 male and 50 female. A check of the personal data of these subjects revealed that for the male group age was between 6875 and 9396 days (between 18 years 10 months and 25 years 9 months), whereas for the female group age was between 6711 and 9275 days (between 18 years 4 months and 25 years 5 months). In other words, none of the subjects was found to be under 18 years of age.

Water buffalo (Bubalus bubalis) hemoglobins: an electrophoretic and chromatographic study.

1. Hemoglobins from three phenotypes of Italian water buffalo (Bubalus bubalis), named AA, AB and BB, were selected by starch gel electrophoresis at alkaline pH and analyzed using polyacrylamide gel isoelectric focusing and subsequent analysis of titration curves to reveal differences between two types of hemoglobin identified as Hb fast and Hb slow. 2. Globins from Hb fast and Hb slow were purified by fast protein liquid chromatography (FPLC). Electrophoretic differences were found in the respective alpha-chains using polyacrylamide gel disc-electrophoresis at acid pH, polyacrylamide gel isoelectric focusing and by subsequently analyzing titration curves. 3. The results suggest that the alpha chains of Hb fast and Hb slow, called I alpha and II alpha, respectively, differ in at least two aminoacid residues. Subsequently, these amino acids were identified as lysine and cysteine.

[Aluminum lung as an occupational disease. Case reports].

The cases are examined of 13 males, mean age 53, granted disability pensions by the Turin I.N.A.I.L. in 1975-86 as suffering from lung disease caused by the inhalation of dust and exhalation of aluminium. The patients had worked in bauxite smelting for Al2O3 production and the preparation of synthetic abrasives (8 cases), in milling using synthetic abrasives (1 case) in the cold grinding of aluminium for paint production (3 cases) and in the electrolytic processing of aluminium (1 case). In line with the latest reports in the literature the present series confirms the possibility that interstitial fibrosis can be caused by exposure to aluminium in industries other than bauxite processing. The relatively benign character of modern aluminium lung is also confirmed.

[Talcosis as an occupational disease. Case histories].

The case are examined of 49 people (45 males, 4 females), mean age 60 exposed to the risk of talc inhalation in a talc processing and packing factory who were granted disability pensions by the Turin I.N.A.I.L. on the grounds of silicosis in 1975-86. Radiologically the cases presented small round patches in 28 subjects small irregular ones in 15 and large patches in 6. In functional terms, examination revealed ventilatory insufficiency that was primarily restrictive in 17 cases, obstructive in 11 and mixed in 21.

[The thickness of the soft tissue of the face for person identification].

A skull may give important information about race, sex, facial details and contribute, even without dental study, to personal identification in case of skeletal remains of unknown origin. It was shown by some authors that upon the cranial framework we may build bit by bit all soft tissues until details of physiognomy take shape and a reasonable reproduction of a living human head emerges. This work is based on the careful measurement of the tissue thickness of several cadavers' head and face in some chosen sites, to obtain mean values, which are the starting point for the restoration of physiognomy details. Collected data calculation gave four simple tables, with thickness mean values in millimetres. These tables successively determine groups according to age, sex and body condition.
