[Treatment of mediastinal infection following cardiac surgery by continuous closed irrigation with povidone-iodine in early infancy].

Two patients aged 2 months and 9 days with postoperative mediastinal infection were treated by continuous closed irrigation with 0.5% povidone-iodine solution. Satisfactory eradication was achieved in both two patients. Although serum iodine level was elevated as high as 7,610 micrograms/dl in the neonatal patient, no disturbances of the thyroid function and the acid-base balance were seen. The usefulness of continuous closed irrigation in the neonatal or infantile period was suggested.

[Application of NMR in the neurology--in its future].

The development of NMR technique is surprisingly progressive also at present. In this article, firstly we want to report our clinical experiments about mainly congenital and development anomalies, vascular disorders, dysmyelinating or demyelinating disorders, and neurodegenerative disorders. Secondly we report about the new techniques of MR imaging and MR spectroscopy those are applicable in clinical studies at present and in the near future. Fast scan imaging technique using gradient echo acquisition enables us to perform CSF cine-mode study, MR angiography, and three-dimensional imaging. Detectability of intracranial calcification is higher in the fast scan images than in the conventional spin-echo images. MR imaging and spectroscopy using other than proton are not performed screening at present. However, in vivo MR imaging of sodium, fluorine, and phosphorus can be used, and their clinical utilities are now evaluated. In vivo MR spectroscopy of proton is also used in some institutes. Its clinical utility is already reported and this application in clinical studies is strongly expected.

[NMR spectroscopy of tumors].

Nuclear Magnetic Resonance Spectroscopy provides a non-invasive method for monitoring tumor bioenergetics and response to therapy. The 31 P spectrum appears to be an indicator of tumor bioenergetics, reflecting its extent of perfusion and degree of oxygenation. During growth, the level of high energy phosphates generally decreased while the level of low energy phosphates generally increased. This has been attributed to the development of vascular insufficiency. Tumor responses to chemotherapy, radiation therapy and hyperthermia appear to be accompanied by significant 31 P spectral changes. Specific metabolic pathways have been examined by 13 C NMR studies employing isotropically labeled metabolites. Detection of 1H compounds holds considerable promise for application to experimental, clinical oncology.

[Role of MRI in the pediatric central nervous system disorders].

Recent rapid development of the MRI system has enabled us to diagnose precisely the disorders of the central nervous system (CNS) also in neonates and young children. Because of a long studying time, the use of oral chloral hydrate or other alternative drugs for sedation, such as secobarbital and meperidine, is necessary for young children under 6 years of age. The advantages of MRI are the optional plane imaging, a high contrast resolution, and the artifact-free imaging from the surrounding bones and air. MRI can detect myelination disorders and the lesions in the posterior fossa, the middle fossa, and the spinal canal. These abnormalities are difficult to depict with conventional X-ray CT scanning. MRI is useful also for the survey of various congenital anomalies of the brain and the spine. Furthermore, it is sensitive enough to detect the CNS blood flow and the cerebrospinal fluid (CSF) flow. Arteriovenous malformation, moyamoya disease, and sinus thrombosis are diagnosed by MRI without using contrast media, CSF flow void phenomena in the aqueduct and the Monro's foramina are indexes of the CSF pathway obstruction and of normal pressure hydrocephalus.

[Staged repair of type A interrupted aortic arch using the Blalock-Park method in infancy: development of the reconstructed aortic arch].

Three infants with type A interrupted aortic arch, VSD and PDA underwent staged operation. Aortic arch reconstruction with Blalock-Park's method, PDA division, and PA banding were performed as the first operation. Subsequently all patients were underwent the second operation, consisting of VSD closure and PA debanding, and are doing well now. Pressure gradient at the anastomosis site, the diameters of the anastomosis site and the left subclavian artery, and the diameter ratios of the anastomosis site and the left subclavian artery to the ascending aorta were evaluated after the first and second operation. Two patients with good size of the left subclavian artery at the first operation showed excellent growth of the reconstructed aortic arch and trivial pressure gradient at the anastomosis site. The other patient, whose preoperative left subclavian artery was small (2.5 mm in diameter) resulted in residual stenosis at the anastomosis site. The development of the left subclavian artery was satisfactory in all patients. These data suggest that the aortic arch reconstructed with Blalock-Park's method can grow if the diameter of the anastomosis site has adequate size at the first operation.

[Cardiac response during various activities in patients with ischemic heart disease evaluated by an ambulatory ventricular monitor (VEST)].

To assess the cardiac response to various exercise in ischemic heart disease (IHD), left ventricular function was continuously measured with an ambulatory ventricular monitor (VEST) in 3 normal subjects and 15 IHD patients. Treadmill exercise (early stage (E1), end stage (E2) and recovery standing state (RE], walking (WK) and climbing stairs (CS), was used for exercise. 15 IHD patients were divided into 4 groups by the LVEF response to treadmill exercise. In group 1 (G1), LVEF increased gradually at stage 1 and became plateau from stage 2 to end stage like normal group. In group 2 (G2), LVEF increased at stage 1 and became plateau from stage 2 but decreased from maximal EF more than 5% at end stage. In group 3 (G3), LVEF increased only at stage 1 and decreased immediately after stage 1 to end stage. The decrease of LVEF at end stage from standing was more than 5%. In group 4 (G4), LVEF decreased at stage 1 and became minimum at end stage. This grouping was well related to thallium redistribution (Tl RD) score and coronary arteriography (CAG) score. We could predict the severity of coronary artery disease from this grouping because all group 3 or 4 patients had severe coronary artery disease. VEST was useful for the evaluation for the tolerance to daily activities. Group 1, 2 and 3 tolerated daily activities while group 4 could not always. The degree of the increase in cardiac output during ischemia evaluated by VEST may be one of the useful index of the tolerance to exercise. In conclusion VEST was very useful for evaluating cardiac capacity of ischemic heart disease patients during various exercise.

[Arterial switch operation for simple transposition of the great arteries in infancy].

Five infants (22-42 days of life) underwent arterial switch operation for simple transposition of the great arteries under deep hypothermic cardio-pulmonary bypass. Three babies required prostaglandin E1 infusion to keep ductus arteriosus opened widely before surgery. Balloon atrioseptostomy was necessary in 0-10 days of life in all babies because of poor condition. Left to right ventricular peak pressure ratio ranged from 0.75 to 0.86, preoperatively. Four of the five survived the operation, and one died of coronary insufficiency because of kinking of the implanted coronary artery. Hypothermic circulatory arrest was used in three (38-41 minutes). Aortic cross clamp time was 70-100 minutes, and cold crystalloid cardioplegia was given only one time just after aortic clamp in 3 babies. Single dose of cardioplegia protected left ventricular muscle well in babies with transposition of the great arteries as same as multiple dose method used in those with ventricular septal defect.

[Thin-section computed tomography of the bronchi: right middle lobe, left lingular division, and lower lobes].

Thin (2 mm) section contiguous computed tomographic (CT) scans were obtained through the bronchi of the right middle lobe, the left lingular division, and the lower lobes in 24 patients. All segmental bronchi were identified except subsuperior and medial basilar segmental bronchi. Subsegmental bronchi of the left inferior lingula were seen in only 17%, but other subsegmental bronchi in greater than 75%. Variations in branching patterns were discussed.

[N-isopropyl I-123 p-iodoamphetamine (IMP) brain SPECT in Alzheimer's disease].

Eighteen patients with Alzheimer's disease (AD), 5 patients with Pick disease (PD), 6 patients with other types of degenerative dementia (O) and 12 age-matched normal control subjects (N) were studied using N-isopropyl p-[I-123]iodoamphetamine (I-123 IMP) with SPECT. Regional to cerebellar activity (R/CE) ratio and frontal to parietal (F/P) activity ratio were evaluated in each case. I-123 IMP-SPECT revealed focal abnormality in all cases in AD, PD, O group, while XCT and/or MRI were normal or showed cerebral atrophy without focal abnormal density or intensity. In AD group, R/CE ratio in all the regions except for bilateral Rolandic area and left primary visual cortex were significantly lower (p less than 0.05 or p less than 0.01) than that in N group, and F/P ratio were significantly higher (p less than 0.01) than that in P and O group. In conclusion, I-123 IMP-SPECT is useful to detect focal perfusion abnormality in dementia and may be of value in differentiating Alzheimer's disease from dementia of non-Alzheimer type.

Synthesis of 11C-methylated inulin as a radiopharmaceutical for imaging brain edema and pulmonary edema.

11C-methylated inulin, supposedly useful for imaging of brain edema and pulmonary edema, was prepared using cyclotron produced 11CO2. The synthesis consists of the production of 11C-methyl iodide and its coupling with inulin alkoxide sodium in dimethylsulfoxide as solvent. 11C labeled inulin was purified by alcohol precipitation. The radiochemical yield of pure 11C-inulin was 34% of 11CO2 30 min after the end of bombardment. The blood clearance and body distribution of 11C was observed in rabbits after i.v. injection of 11C-inulin. The blood clearance curve was composed of a sum of three exponential functions. The gamma camera image showed that the 11C activity in blood moved quickly to kidneys and urine and a small dose of radioactivity remained persistently in edematous tissues, i.e. the edematous lung tissues produced by oleic acid treatment.

Measurement of human blood brain barrier integrity using 11C-inulin and positron emission tomography.

Positron emission tomography (PET) using 11C-inulin was demonstrated to be applicable to the clinical measurement of blood brain barrier permeability and cerebral interstitial fluid volume. Kinetic data were analyzed by application of a two compartment model, in which blood plasma and interstitial fluid spaces constitute the compartments. The blood activity contribution was subtracted from the PET count with the aid of the 11CO inhalation technique. The values we estimated in a human brain were in agreement with the reported values obtained for animal brains by the use of 14C-inulin.

ST-T isointegral analysis of exercise stress body surface mapping for identifying ischemic areas in patients with angina pectoris.

ST-T isointegral analysis of body surface mapping was used in an attempt to localize ischemic areas on exercise tests. In 28 patients with angina pectoris and 10 healthy subjects, body surface potential was recorded with 87 leads, and ST isopotential and ST-T isointegral maps were constructed. In all 10 healthy subjects, the basic pattern of the ST-T isointegral map showed no significant change after exercise. In 23 of 28 patients with angina pectoris (82%), alterations in the ST-T isointegral map after exercise were observed. They were divided into four types (anterior, inferoposterior, lateral, and global) according to the distribution of negative values, which were well correlated with the extent of ischemic area determined by thallium myocardial scintigraphy and coronary angiography. The postexercise ST-T isointegral map was normalized after administration of nitroglycerin in four of five patients. In five patients (18%) who did not show abnormalities on the postexercise ST-T isointegral map, the magnitude of maximal ST depression was significantly smaller than that observed in the other 23 patients with angina pectoris (0.14 vs 0.23 mV on the average, p less than 0.05). It was concluded that the exercise test with ST-T isointegral mapping is a new method for noninvasive detection of location and severity of ischemic regions.

Technetium-99m dimercaptosuccinic acid uptake in long-term catheterized kidney. Comparison with renal function.

We studied 23 long-term catheterized kidneys in 14 patients. The uptake of 99mtechnetium dimercaptosuccinic acid (99mTc-DMSA) was measured at one- and two-hour intervals after injection, and the uptake was corrected for variations in renal depth. These values were compared with inulin, creatinine, and para-amino hippurate (PAH) clearances which were measured in each kidney by collecting urine through long-term catheterization. Correlation coefficient was obtained between PAH clearance corrected for the body surface area and the two-hour uptake of 99mTc-DMSA. The correlation coefficients between the two-hour uptake of 99mTc-DMSA and the clearance values are not significantly different from those between the one-hour uptake and the clearance values. Corrections of the uptake for variations in renal depth did not improve the correlation coefficients. The results show that 99mTc-DMSA is an excellent method to estimate the renal plasma flow and the one-hour uptake without correction for renal depth is clinically sufficient to evaluate the split renal function.
