[Evaluation of cardiovascular responses to handgrip tests in patients with ischemic heart disease by radionuclide method].

Left ventricular responses to handgrip exercise test in patients with ischemic heart disease were assessed by RI methods. We subdivided into three groups (A: delta EF more than 5% increased, B: delta EF -5%-+5% no change, C: delta EF less than 5% reduced), left ventricular contractility (LVEF/LVET) and Tl defect score changed significantly in global EF reduced group, moreover the usefulness of handgrip test was assured. This instrument is low cost, safety, small size, and available for wide ranged ages.

Gastroduodenal lesions after transcatheter arterial chemo-embolization in patients with hepatocellular carcinoma.

We investigated the incidence and endoscopic features of gastroduodenal lesions which appeared after transcatheter arterial chemo-embolization (TACE), performed 29 times in 25 patients with inoperative hepatocellular carcinoma. The new development or exacerbation of the gastroduodenal lesions after TACE was evident in 13 of the 29 (45%). The side of the lesions ranged from the gastric body to the second portion of the duodenum, and the lesions that developed were multiple ulcers in four, and multiple erosions with white coat in nine. The development of these gastroduodenal lesions may be due to mucosal ischemia caused by embolic materials, the toxic effect of antineoplastic drugs infuse, or to stress. In light of these events, upper gastrointestinal endoscopy should be added to the usual examinations done for patients undergoing TACE.

Endoscopic diagnosis of lymphangioma of the small intestine.

In three patients with lymphangioma of the small intestine, the preoperative diagnosis of lymphangioma was made by endoscopy with biopsy and radiology. A typical endoscopic finding was an elevated polypoid tumor, yellowish-white to tan. The surface was smooth, often with white specks, and could be impressed by touching lightly with biopsy forceps. Endoscopic examination revealed satellite lesions not detected radiologically. Lymphangioma has heretofore been successfully diagnosed preoperatively by endoscopy in the duodenum. This report of accurate preoperative diagnoses of lymphangioma in the small intestine, other than the duodenum, using endoscopy, may be the first to be documented.

Hydrogen breath test assessment of orocecal transit time: comparison with barium meal study.

Orocecal transit time was measured simultaneously by the hydrogen breath test and a barium meal study in 12 hospitalized patients, the objective being to determine whether the former test accurately represents the orocecal transit time, and to establish an adequate criterion for the transit time, based on the former test. Two definitions of orocecal transit time by the hydrogen breath test were evaluated: the time from lactulose ingestion to a sustained increase of over 5 ppm above fasting levels in the end-expiratory hydrogen concentration (definition A) and the interval to that of over 10 ppm (definition B). The orocecal transit time measured by the radiologic method was 63 +/- 9 min (mean +/- SEM), whereas that using definition A of the hydrogen breath test was 74 +/- 9 min, and that using definition B was 87 +/- 10 min. Transit times determined by both definitions closely correlated with that obtained by the radiologic method (A, r = 0.86, p less than 0.01; B, r = 0.81, p less than 0.01). Therefore, elevation of end-expiratory hydrogen concentrations seemed to coincide with cecal appearance of the head of the lactulose load. When the mean transit times were compared with findings in case of the radiologic method, definition A rather than B appeared to be more appropriate to determine orocecal transit time.

Membrane stabilization of vitamin E; interactions of alpha-tocopherol with phospholipids in bilayer liposomes.

13C Spin-lattice relaxation times (T1) of 13C-labeled alpha-tocopherol in three kinds of liposomes varying in their contents of arachidoyl residues have been measured by 13C-NMR spectroscopy. On the basis of T1 values, it is proved that the segmental motion of isoprenoid side chain of alpha-tocopherol tends to increase with an increase in the distance from the chromanol moiety, and that three methyl groups attached on the aromatic ring, have some affinity to unsaturated fatty acid residues rather than those of the isoprenoid side chain. These results are incompatible with the hypothesis of Diplock et al. (1) which 4'a- and 8'a-methyl groups of isoprenoid side chain are fitted in the Z-pockets of arachidoyl chain of polyunsaturated lipids in membrane.

Early carcinoma of the gastric cardia. A clinicopathologic study of 21 cases.

A clinicopathologic study was done on 21 cases of early carcinoma of the gastric cardia. The disease was preponderant in men, with a male-female ratio of 17:4. Five patients had no symptoms, while the other 16 had epigastric pain, epigastric discomfort, or other symptoms. Melena or hematemesis occurred in five patients. The first detection was made by barium study in 16 and by endoscopy in five. Detailed examinations using double-contrast radiography in both the prone right anterior oblique and supine left lateral projections were useful to detect the early carcinoma of the gastric cardia. All depressed-type carcinomas were incidentally detected during endoscopic examinations of benign peptic ulcers, therefore, whenever endoscopic examinations are being done for other entities, all the stomach including the cardia should be examined, carefully and continuously. Compared with early carcinomas of the other areas of the stomach, grossly elevated and histologically well-differentiated early carcinomas were more common in the gastric cardia. In the mucosa adjacent to the carcinoma, chronic gastritis was apparent in almost all cases, although intestinal metaplasia was found less frequently than in other areas of the stomach.

Acquired pseudoobstruction of the colon due to segmental hypoganglionosis.

A case of acquired pseudoobstruction of the colon due to segmental hypoganglionosis is reported. The patient became constipated during pregnancy and obstructive symptoms followed. Radiologically, the diseased segment was spastic and the proximal colon was extremely dilated. Microscopically, the involved segment showed marked diminution in the number of intramural ganglion cells and those present showed degenerative changes.

Anatomical involvement and clinical features in 91 Japanese patients with Crohn's disease.

Ninety-three patients with Crohn's disease who had not undergone bowel resection at the time of diagnosis (during the years 1969-1983) were selected for the study, to elucidate whether initial anatomical involvement correlates with clinical manifestations, complications, laboratory findings, and prognosis. Forty-one patients had isolated small intestine disease (44%), 37 ileocolic (40%), 13 colonic disease alone (14%), and two rectal or appendix involvement (2%). Statistically significant symptoms were fever and rectal bleeding with colon involvement, and number of liquid or very soft stools in 1 week with ileocolic or colon involvement. Statistically significant complications included intestinal obstruction with ileocolic disease, perianal fistulas, and arthritis with colonic disease. Increases in ESR, C-reactive protein, and white blood cell in patients with colon involvement were among the statistically significant laboratory findings. Gammaglobulin was significantly higher in patients with ileocolic or colon involvement than in those with small intestine disease. Red cell count and hemoglobin were significantly lower in patients with ileocolitis than in patients with small intestine involvement. Crohn's disease activity index (CDAI) was significantly higher in patients with ileocolic or colon involvement than in those with small intestine involvement only. These findings confirm that colonic involvement renders the disease more severe than involvement of the small intestine. Surgery was required for 22 patients (24%) during a 2.8-year follow-up, but the difference among the three groups was not significant.

Chediak-Higashi syndrome with intestinal complication. Report of a case.

A 21-year-old woman with Chediak-Higashi syndrome developed intestinal lesions similar to those of Crohn's disease. Radiographic findings showed a stenotic lesion with multiple nodular changes in the ascending colon and cecum, and a longitudinal ulceration in the terminal ileum. In addition, multiple oval-shaped ulcerations were seen between the terminal ileum and descending colon. However, serial histological examinations revealed only an infiltration of lymphoid cells, which was a nonspecific finding. Though there are few reports of gastrointestinal involvement in Chediak-Higashi syndrome, such an intestinal complication may develop in patients long surviving with this disease.

Increased arachidonic acid composition of phospholipids in colonic mucosa from patients with active ulcerative colitis.

The long chain fatty acid composition of phospholipids in colonic mucosa was determined by high performance liquid chromatography in nine patients with active ulcerative colitis and eight healthy controls. The arachidonic acid composition was 12.5 +/- 1.4 mol % (mean +/- 2 SEM) in the inflamed colonic mucosa from the patients with active ulcerative colitis and 6.8 +/- 1.2 mol % in the intact mucosa from healthy controls (p less than 0.001). In the inflamed colonic mucosa, oleic acid and palmitoleic acid were concomitantly decreased (p less than 0.001 and p less than 0.02, respectively), while docosahexaenoic acid was increased (p less than 0.05). Histopathological examination showed that there was a three fold increase in the cell density of inflammatory infiltrate in the lamina propria of the inflamed colonic mucosa (p less than 0.001). The cell density of inflammatory infiltrate correlated with the arachidonic acid composition of phospholipids in colonic mucosa (r = 0.89, p less than 0.005). These findings indicate that inflammation alters the long chain fatty acid composition of phospholipids in colonic mucosa. The observed increase in the arachidonic acid composition of phospholipids in inflamed colonic mucosa may contribute to the enhanced arachidonic acid metabolism in patients with active ulcerative colitis.

The trend of coronary heart disease and its risk factors based on epidemiological investigations.

Changes in the incidence of coronary heart disease (CHD) in the past 20 years have been observed in workers in the city and residents in rural areas and the risk factors of CHD studied. Findings show that the incidence of CHD in the residents in rural areas has hardly changed, and remains still low. However, the incidence of CHD in workers in the city has recently shown a tendency to increase. Furthermore, a study of CHD risk factors shows that in the rural areas residents hypertension has a strong influence on the incidence of CHD, while the role of serum total cholesterol is weak. In the city workers a high serum total cholesterol has become a risk factor. Therefore, it appears that the incidence of CHD caused by hypertension and by hypercholesterolemia are mixed in Japan. At present it appears that the influence of hypertension is becoming weaker, while the influence of hypercholesterolemia is becoming stronger. The resulting balance between them may be expressed as a rise and fall of CHD in Japan at present.

[The evaluation of cardiac function and the effect of therapy in acute myocardial infarction--comparison by radio nuclide method in acute and chronic phase].

The cardiac function and the effect of therapy in patients with acute myocardial infarction (AMI) were assessed in over 100 patients by analysis of 201Tl-scintigraphy and 99mTc-gated pool study in our ICU. The cardiac function and 201Tl-defect ratio were compared with the results obtained in chronic phase. Sixteen of them were treated with intravenous urokinase (UK) within 6 hours from onset. The other 18 patients without UK treatment served as a control group. Significant correlation was recognized between 201Tl-defect ratio and peak-CPK levels, peak-GOT levels, peak-LDH levels. Significant correlation (r = -0.655, r = -0.713) were found between 201Tl-defect ratio and LVEF in acute and chronic phase. The UK group showed a significant increase of LVEF as compared with the control group in patients with antero-septal (A/S) AMI. In inferior (INF) AMI, no significant differences were observed UK and control group in LVEF, RVEF and 201Tl-defect ratio. In chronic phase, improvements of LVEF and 201Tl-defect ratio were observed in patients with A/S AMI. But no significant differences of LVEF, RVEF and 201Tl-defect ratio were observed in A/S AMI in acute and chronic phase. The UK group showed a significant increase of LVEF (50.1%) as compared with the control group of A/S AMI in chronic phase. We have demonstrated that a combination of 201Tl-scintigraphy and 99mTc-gated pool study are useful techniques in ICU, to evaluate the cardiac function and the effect of thrombolysis therapy and thus greatly contribute to the primary care of AMI cases.
