Trends for coronary heart disease and stroke and their risk factors in Japan.

Disease surveillance and population surveys of risk characteristics in a northeast rural community of Japan (1965 census population, 7,030) are combined in an attempt to relate morbidity and risk factor trends for coronary heart disease and stroke during the last 2 decades. Between 1964 and 1983, the incidence of coronary heart disease (i.e., combined myocardial infarction, angina pectoris, and sudden death) did not change significantly among men and women ages 40-69, and was lower than that for stroke. The incidence of all stroke declined about 60% for both men and women, ages 40-69, with a significant decrease in cerebral hemorrhage for both sexes and in cerebral infarction for men. Between 1963-1966 and 1980-1983, significant upward shifts occurred in the means and distributions of serum total cholesterol and serum total protein in every age and sex group, primarily during the 1st decade. Age-adjusted mean cholesterol levels rose 22 mg/dl to the 1980-1983 mean of 179 mg/dl in men ages 40-69. In women ages 40-69, the mean rose 29 mg/dl to 192 mg/dl. Among nutrients, animal fat intake doubled in men ages 40-59 from 4.5% of daily calories in 1969 to 9.6% in 1980-1983. Animal protein intake also increased, from 5.8% to 7.1%. Most of this increase occurred between 1969 and 1972-1975 and may be attributable to an increased intake of meat, eggs and dairy products. From 1963-1966 to 1980-1983, mean relative weight index rose significantly for all age-sex groups except men ages 50-69. Mean systolic and diastolic blood pressure levels declined for every age-sex group, with a 15-mm Hg age-adjusted decrease in systolic, 4-mm Hg decrease in diastolic pressure among men ages 40-69, and a 11-mm Hg systolic and 4-mm Hg diastolic decrease for women. Two cohorts of men and women ages 40-69 at baseline were followed for disease incidence: an early cohort (2,257 persons) followed from 1963-1966 to 1973 and a later cohort (2,711 persons) followed from 1972-1975 to 1983. In these cohorts, significant risk prediction for cerebral hemorrhage and infarction was obtained with blood pressure level and end organ effects in the electrocardiogram and fundus photographs. Serum cholesterol was inversely associated with cerebral hemorrhage in the early cohort but not in the later.(ABSTRACT TRUNCATED AT 400 WORDS)

Injection and sampling methods for drug residue study in calf muscle.

Eight calves, weighing 50-150 kg, were given intramuscularly 5 ml of ampicillin (ABPC) aqueous suspensions (200 mg potency/ml) in their right and left gluteal and femoral regions. All calves were sacrificed one hour later to confirm the location of injected drug. The drug was found in a muscle layer when injected with a needle 15 mm long to the following positions, 1. the midpoint between the central position of the gluteal region (CG) and the tuber coxae (M-CTc), 2. the midpoint between CG and the tuber ossis ischii (M-CTo), 3. the central position of M. semimembranaceus in the femoral region (CF). Seven calves, weighing 130-150 kg, were given intramuscularly 5 ml of ABPC suspensions at M-CTo and CF and sacrificed one hour (4 calves) and 3 days (3 calves) later. ABPC diffused along the long axis of the muscle fibers but not to the radial direction. ABPC was detected only in the injected muscle layer even after 3 days indicating that the drug did not diffuse to the neighboring muscles. In the injected muscle layer, concentration of ABPC was remarkably different from part to part. From these results, sampling of the injected muscle for the drug residue study was proposed as follows: 1. isolate about 100 g of muscle just under the stick point marked on the skin considering the direction of drug diffusion, and 2. isolate separately about 200 g of the surrounding muscle to confirm if the sampling is appropriate.

[Cytokine production in inflamed human gingival tissues--interleukin-6].

Interleukin-6 (IL-6) is involved in the final differentiation of B cells into antibody-producing cells. Recent studies reveal that IL-6 plays an important role in inflammation. Histopathological studies showed that a large number of plasma cells in periodontitis is usually seen in the apical parts of cellular infiltrates beneath the periodontal connective tissues. This evidence suggests that IL-6 may play a critical role in the development of periodontitis. Therefore, we examined IL-6 production in the gingival tissues. Twelve periodontitis patients and five gingivitis patients were included in this study. Nine individuals with healthy periodontium acted as control subjects. Biopsy specimens were dissected into fragments 3 mm in diameter and plated onto 24-well culture plates with RPMI 1640 medium. IL-6 activity in the culture supernatants was measured by IgM production assay using the cell line SKW6-CL4. IL-6 activity was detected at significantly higher levels (P less than 0.001) in culture supernatants from the gingival tissues in periodontitis (23.2 +/- 14.4 units/ml) and gingivititis (12.5 +/- 3.4 units/ml) than in control tissues (2.3 +/- 1.2 units/ml). Subsequently, the relationship between IL-6 activity and clinical stages was examined. The IL-6 levels before initial preparation (23.2 +/- 14.4 units/ml) were significantly higher (P less than 0.001) than those after initial preparation (1.4 +/- 1.8 units/ml), but were not associated with either periodontal pocket depth or the extent of alveolar bone resorption in periodontitis.(ABSTRACT TRUNCATED AT 250 WORDS)

[Study of cytokine production in inflamed human gingival tissues in periodontitis. Interleukin-1 (IL-1 alpha, beta) and tumor necrosis factor (TNF alpha)].

It seems to be generally agreed that periodontal disease is a local manifestation of a systemic immune response. Interleukin-1 (IL-1), which has multiple biologic activities, is detected in the gingival sulcus fluid of periodontitis sites. Recent investigations have revealed that IL-1 and tumor necrosis factor (TNF) are analogous to osteoclast activating factor and promote bone resorption. These findings have suggested the possibility that IL-1 and TNF may play a significant role in the initiation and development of periodontal disease. However, it remains to be determined whether these cytokines influence periodontal tissue breakdown in periodontitis. To elucidate the mechanisms of tissue breakdown in periodontitis, we examined cytokine production by human periodontitis gingival tissue. Twelve periodontitis patients were included in this study. Control subjects with healthy periodontium consisted of nine individuals. Gingival samples were biopsied from inflamed or healthy gingival tissues. Biopsy specimens were dissected into fragments 3 mm in diameter and plated onto 24 well culture plates with RPMI 1640 medium. IL-1 activity was measured by a growth inhibition assay using melanoma cell line A 375. An enzyme-linked immunosorbent assay (ELIZA) was used for measuring levels of human IL-1 alpha, IL-1 beta. TNF alpha activity was measured by a growth inhibition assay using cell line LM2D6. IL-1 activity was detected in significantly (p less than 0.001) higher levels in culture supernatants from gingival tissues in periodontitis (48.0 +/- 23.3 units/ml) than in control tissues (2.3 +/- 0.6 units/ml), however, levels of IL-1 activity were not associated with periodontal pocket depth or extent of alveolar bone resorption in periodontitis.(ABSTRACT TRUNCATED AT 250 WORDS)

[Evaluation of left atrial function by 99mTc gated blood pool scan].

99mTc gated blood pool scans were studied to assess the left atrial function. Relationship between filling time and rapid emptying time was y = 0.695 x + 109 (r = 0.761, p less than 0.05, n = 14) in LAO and LPO projections. On the other hand, relationship between slow filling and rapid emptying time was good closely (y = 0.846 x + 16.9 (p less than 0.01, r = 0.975, n = 8] in standard and retrograde acquisition. To assess the left atrial function by standard acquisition in LAO projection should be available for clinical use.

[Advance of disability and prognosis in Duchenne muscular dystrophy--a comparison between institutionalized care and home care].

Rate of advance of disability and age at death were studied in patients with Duchenne muscular dystrophy (DMD). A comparison was made between DMD patients under care in their own homes and those hospitalized in National Suzuka Hospital. In 45 patients with home care, rate of advance of disability was considerably high in their age of 6 to 12 years, the average being 0.83 stage per year after the N.Y. University-Ueda's rating stage of disability. In the age of 12 to 17 years, however, the disease progression was much slower, the average being 0.14 stage per year. Beyond 18 years old, the average stage of disability was constantly 7.7, meaning almost bedridden stage. On the contrary, in 56 patients with institutionalized care, the average rate of advance was lower, being 0.5 stage per year in the age of 7 to 15 years and 0.13 stage per year from 15 to 23 years of age. The result of statistical analysis indicated that the stage of disability was significantly higher in the patients under the home care than those with institutionalized care at the age of 12-16, 18 and 19 years. The age when the patients became unable to walk was 9.7 +/- 1.8 years (mean +/- SD) in those with home care and 10.9 +/- 2.3 years in those with institutionalized care. The difference was statistically significant (p less than 0.01). The age at death was 18.3 +/- 3.8 years in 19 DMD patients with home care and 20.4 +/- 3.6 years in 33 patients with institutionalized care. [A resected case of metastatic lung cancer after curative operation in appendiceal cancer].

A rare intense of a metastatic lung cancer after an operation for a primary appendiceal cancer is reported. A 51-year-old woman was admitted to Yamato Municipal Hospital due to a right lower abdominal pain. Subsequently, a curative right hemicolectomy was performed for a primary appendiceal cancer. Two and a half years later, a chest X-ray and a CT scan revealed a solitary coin lesion in the right lower lung, and she was readmitted to our hospital where a percutaneous needle aspiration of a portion of this lesion was carried out. A cytological examination of this specimen led to a diagnosis of a mucinous adenocarcinoma, with a suspected appendiceal cancer metastasis. Thus, a wedge resection of the right lower lobe was performed. A year and a half later, the patient is still alive and shows no signs of a recurrence. In addition to discussing this case, autopsied cases of appendiceal cancer in literature are also reviewed.

Local chemotherapy for rectal polyposis: intraluminal administration of 5-fluorouracil for postoperative control of adenomas in the retained rectum in familial polyposis.

We administered the anticancer drug 5-fluorouracil locally to treat rectal adenomas in four patients with familial adenomatous polyposis who had undergone total colectomy and ileorectal anastomosis. The drug was administered either in suppository form or in high concentration as a rectal enema. One patient was treated successfully with 5-fluorouracil suppositories without any side effects, but the suppository regimen had to be stopped in three other patients because of severe rectal urgency. However, one of these patients was treated effectively with rectal administration of a high-dose 5-fluorouracil solution. Adenomas near the anastomotic site or dentate line were difficult to treat. Although severe urgency was seen in most patients, we think that the intraluminal administration of anticancer agents may prove effective in treating polyposis of the rectum retained after colectomy in familial polyposis. It deserves further study.

[Pathophysiological analysis of rapidly progressive periodontitis].

Pathophysiological features were studied on 7 patients with rapidly progressive periodontitis but without any evidence of systemic disease, to analyse the clinical pathogenesis. The patients consisted of 5 females, 2 males, between the ages of 32 and 42 years. All patients had severe and rapid alveolar bone destruction on the basis of radiographic measurement. Abnormal serum levels of IgG and IgM were detected in some patients. Higher IgG level was found in 4 patients and higher IgM level was found in 2 patients. The proportion of lymphocyte subsets was calculated in mononuclear cells from peripheral blood of patients. Higher OKT4/OKT8 ratio was found in all patients. The percentage of OKT4 positive cells in 2 patients was higher than that in normal subjects while the percentage of OKT8 positive cells in 4 patients was lower than that in the healthy controls. Microorganisms from periodontal pockets were examined in 5 patients. Bacteriodes was isolated in all 5 patients and Haemophilus actinomycetemcomitans in 2 patients.

[Study on the genesis of posturally induced crackles from hemodynamic data--in patients with ischemic heart disease having normal respiratory function].

The presence of fine crackles is suggestive of heart failure in patients without pulmonary disease. We have been interested in the clinical observation that fine crackles are frequently detected when posture was changed from sitting to supine positions or in patients going from sitting position to supine position with passive legs elevation in patients without obvious evidence of heart failure. We named these crackles, "the posturally induced crackles (PIC)". We have already reported that PIC was frequently detected in patients with ischemic heart disease. The present study was performed to estimate the mechanism of the genesis of PIC and to clarify its significance. Seventy-three patients with ischemia heart disease were included in this study. Pulmonary sounds were auscultated in sitting and supine positions and during passive elevation of both legs in a supine position. Patients were divided into 3 groups according to the presence or absence of fine crackles, i.e., those in whom fine crackles were not detected in either position (PIC (-)), those in whom fine crackles were detected in a supine position or during passive elevation of both legs, but not in a sitting position (PIC (+)), and those in whom fine crackles were detected even in a sitting position (Persistent crackles). We measured various hemodynamic parameters (cardiac index, RA pressure, PA pressure and PAW) and parameters of pulmonary circulation (pulmonary blood volume, pulmonary "venous" compliance) in these 3 groups and comparisons were made between them.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of metastatic lung tumor mimicking left atrial myxoma by echocardiography].

We report a case of lung metastatic tumor originated from submandibular osteosarcoma, which shows rare advancement and findings. This lung metastatic tumor advanced from left lower lung to left atrium through the left lower pulmonary vein, which penetrated to the left ventricle in the diastolic phase mimicking left atrial myxoma. 67Ga-citrate images showed that hot accumulation were recognized behind the heart in planar image and clear advancement from the lung to the heart in the SPECT image. 67Ga-citrate SPECT image should be available for cardiac tumors.

[A case of lupus pneumonia treated by pulse therapies and plasma exchange therapy].

A case of interstitial pneumonia accompanying SLE (lupus pneumonia) was reported. Shortness of breath in this patient was aggravating very rapidly even during methyl prednisolone pulse therapy, but the patient was relieved from this symptom soon after double filtration plasma exchange was carried out. We concluded that some cases which are resistant to pulse therapy and whose prognosis is very poor might be saved by plasma exchange.

[Primary pulmonary cryptococcosis treated with transbronchial injection of amphotericin B].

A 76-year-old man was admitted because of multiple nodular opacities in both lungs on chest X-ray. Cryptococcus was identified in the specimens obtained from bronchial brushing and bronchial washing. Primary pulmonary cryptococcosis was diagnosed as he had no underlying diseases and no foci in any other organs except the lung. Transbronchial injection (TBI) of amphotericin B was administered to the largest focus, in addition to the intravenous administration of miconazole. The chest tomogram 25 days after the first TBI showed marked decrease in the size of the largest focus, but no improvements were observed in other foci. TBI can be performed easily and safely, and perhaps more effectively than administration of amphotericin B with aerosol. This procedure may be of value in the treatment of pulmonary cryptococcosis.

[Dental radiographic survey of rapidly progressive periodontitis].

The purpose of this study was to determine the prevalence of rapidly progressive periodontitis (RPP) by radiographic evaluation in patient at the department of conservative dentistry, Ohu University hospital during two years from 1987 to 1988. The survey was conducted on 370 patients. The radiographic evaluation was done by the method of Schei. The results obtained were as follows. 1. The number of RPP patient was eight. The prevalence of RPP in all patients and periodontitis patients were 2.1%, 6.0%, respectively. RPP patients consisted of 4 females and 4 males. 2. The age of RPP patients ranged from 34 to 41 (mean age 37.5) years old. 3. The number of affected and missed teeth ranged from 4 to 19 (mean 12.1), 0 to 23 (mean 8.0), respectively in RPP patients. 4. Severe alveolar bone loss (more than 50%) was observed in the teeth except canine.

[Response of cardiac patients to dynamic exercise: analysis with "systolic" pressure-volume area].

In the present study, we constructed a left ventricular pressure-volume loop from catheterization data and RN-angiocardiography. By connecting the point of origin of the plane, not the point of Vd, with the end-systolic point of the loop, we calculated a "systolic" pressure-volume area (PVA), elastic potential energy (EPE), and stroke work (SW). A cardiac output (CO)-pulmonary artery wedge (PAW) pressure plot was constructed at rest and during exercise, using a bicycle ergometer, to evaluate cardiac pump function. End-systolic volume (ESV) was obtained from the left ventricular P-V loop, and the response of ESV to exercise was investigated. Thirty-five patients with various cardiac diseases were categorized in two groups according to the delta CO/delta PAW obtained from the shift of the CO-PAW plot during exercise; group I (n = 28) with the properly functioning hearts having the ratio greater than 0.12 l/min/mmHg and group II (n = 7) with the poorly functioning hearts having the ratio less than or equal to 0.12 l/min/mmHg. Group I was subdivided further into two subsets according to the changing pattern of ESV during exercise; group I-A with decreased or unchanged ESV (n = 20) and group I-B with increased ESV in which end-diastolic volume (EDV) was increased during exercise (n = 8). During exercise, PVA and SW were unchanged but SW/PVA increased in group I-A, suggesting improvement of external mechanical efficiency. SW/PVA was unchanged in group I-B, despite the increase in PVA and SW. This suggested that external mechanical work increased as a result of increased cardiac oxygen consumption. In group II, PVA increased, SW was unchanged and SW/PVA decreased, which could be explained by the mechanism that external mechanical work during exercise decreased as compared with that at rest. It was suggested that a different mechanism may have been responsible for the production of external mechanical work among patients in group I-A and in group I-B with properly functioning hearts judged from the CO-PAW plot. Therefore, it seems useful to calculate PVA, SW and SW/PVA during exercise using the left ventricular pressure-volume loop for evaluating cardiac function.

[Two cases of spontaneous pneumomediastinum in healthy young men].

We report two cases of spontaneous pneumomediastinum. Both were young males, and had no causal diseases or apparent precipitating episodes. Both recovered rapidly with bed rest only. Symptoms, signs, radiological diagnosis, and pathogenesis of pneumomediastinum are discussed.

Factors influencing the healing rate of gastric ulcer under treatment with cimetidine.

Factors influencing the healing rate were evaluated by multivariate analysis on 148 patients with gastric ulcer who were under treatment with cimetidine at a daily dose of 800 mg. Factors used were age, sex, therapeutic environment, duration of present ulcer pain, past history of gastric ulcer, the amount of tobacco, alcohol, and coffee consumed before and during the treatment, the period until symptoms disappeared after the commencement of the treatment, and the characteristics of the ulcers themselves. Of these factors, the five factors that had a significant delaying effect on the healing of gastric ulcers were: 1) having a past history of gastric ulcers, 2) symptoms not disappearing within 1 wk after the commencement of the treatment, 3) size over 20 mm, 4) ulcer located in an angle, and 5) round or oval shape of the ulcer. The patients with less than one of the unfavorable factors (n = 36) had the best healing rate (97.2%), compared with those with two (n = 61), three (n = 39), or four or more (n = 12) unfavorable factors. The healing rate of the latter three groups was 75.4%, 53.8%, and 33.3%, respectively (p less than 0.01). Thus, a prognostic score based on these five factors represents the severity of gastric ulcers with regard to the healing in patients prescribed cimetidine.
