Malignant lymphoma originating in the jejunum of a long-term follow-up case of protein-losing enteropathy.

A 50-year-old female patient, who had been followed for 15 years for protein-losing enteropathy, was hospitalized due to epigastric pain. Examination on admission revealed that the patient was in the sub-ileus state. On the 26th day after admission, she complained of severe abdominal pain and shortly after she went into shock. The emergency laparotomy documented intestinal perforation and a tumor. The perforated site was right at the middle of tumor. The histological and histochemical studies identified the tumor as malignant lymphoma of B lymphocyte lineage. As far as we know, this is the third case of malignant lymphoma occurring in the jejunum in a patient with protein-losing enteropathy in Japan. The possible relationship between lymphomas and protein-losing enteropathy is discussed.

Leptin receptor of Zucker fatty rat performs reduced signal transduction.

Zucker fatty (fa/fa) rats exhibit overt obesity, hypercholesterolemia, hyperlipidemia, and hyperglycemia as recessive traits. The fa mutation has been determined to be a missense mutation in the extracellular domain of the leptin receptor. We report herein the construction of CHO cells that stably express the fa-type leptin receptor and the characterization of this receptor using mRNA expression levels of the immediate early genes, c-fos, c-jun, and jun-B, which are induced by leptin as a criterion of signal transduction. The fa-type receptor not only exhibits a slightly reduced leptin-binding affinity, but also performs reduced signal transduction.

Regression of an aggressive abdominal desmoid tumor in a patient with familial adenomatous polyposis by hyperthermoradiotherapy.

A case of huge desmoid tumor successfully treated by hyperthermoradiotherapy is described. A 23-year-old man with familial adenomatous polyposis was operated upon for a desmoid tumor in the mesenterium involving the right kidney and small intestine in 1988. In 1990, the tumor recurred and could not be resected because of the involvement of the vena cava. The tumor grew larger and larger, and occupied two-thirds of the right lower quadrant. Several therapies using sulindac, tamoxifen, prednisolone, indomethacin, luteinizing hormone-releasing hormone analogue, and ascorbate were all ineffective. Finally, the combination of radiation and hyperthermia was used over a 6-month period. At the end of the hyperthermoradiotherapy, the tumor in the abdominal wall was markedly reduced in size, and the protruded abdominal wall became flat. To our best knowledge, this is the first report of the successful treatment of a huge desmoid tumor by hyperthermoradiotherapy.

[The differences of the cerebral and spinal vessels in sensitivity to PaCO2 and vasoconstrictors].

We investigated the differences in the response to arterial CO2 tension and vasoconstrictors between the cerebral and spinal vasculatures using cranial and spinal window technique which provided the direct observation of pial vessels. Pentobarbital anesthetized dogs (n = 18) (CO2 tension; n = 6 and vasoconstrictor; n = 12) were instrumented for measurement of pial vessel diameters by intravital microscopy in the cranial and spinal window preparation simultaneously. We achieved hypocarbia (20-25 mmHg) followed by adjusting CO2 levels for normocarbia (35-40 mmHg) and for hypercarbia (55-60 mmHg) using CO2 gas addition. After obtaining the desired PaCO2, the measurements were made. In the next experiment, we administered 2 different concentrations of epinephrine or phenylephrine solutions (1:2 x 10(6), 1:2 x 10(5)) through the window, and the measurement was made sequentially. The response of cerebral and spinal vasculature to change in PaCO2 was almost similar. Topical application of both drugs produced a significant constriction of spinal pial arterioles compared with the cerebral ones, while epinephrine but not phenylephrine constricted cerebral pial venules compared with spinal ones. These results suggest that the responses to vasoconstrictor of cerebral and spinal pial vessels are not similar, and high sensitivity of spinal arterioles to vasoconstrictors may possibly contribute to a risk of ischemic damage of the spinal cord.

[An experimental study of a newly developed silicone coated membrane oxygenator].

The surface of polypropylene hollow fiber is successfully coated with very thin (0.2 micron) silicone layer. Using this fiber, membrane oxygenator was newly developed. To test its reliability biocompatibility and durability, experimental studies were performed using goats. Long-term (6 hours) normothermic cardiopulmonary bypass (CPB) model was applied in 10 animals. A conventional membrane oxygenator (Mera Excelung HPO-15H, MERA, Tokyo) was used in five goats as a control (C-NC group), and a new silicone coated membrane oxygenator, which is the exact same construction of C-NC group, in five (C-SC group). In the both groups, the same roller pump and circuit were used. The pump flow was 64.9 +/- 5.1 ml/kg in C-NC group, and 65.7 +/- 3.1 ml/kg in C-SC group. There were no significant differences between the two groups Hemodynamics changes during pumping in the both groups were similar, and all animals could successfully wean from CPB. The O2 transfer and CO2 removal functions of C-SC group showed the same ranges as compared with that of C-NC group, indicating clinically acceptable values. In the parameters of coagulation, fibrinolytic and complement activations, there were almost no differences between the two groups. As for hemolysis, however, plasma free hemoglobin of C-SC group (56.2 +/- 43.9 mg/dl) was lower than that of C-NC group (125.7 +/- 65.3 mg/dl). In the scanning electron microscopy, much less platelet clots and red blood cell adhesion were observed in C-SC group as compared with C-NC group. Chronic (96 hours) percutaneous cardiopulmonary support (PCPS) model was applied in 6 animals. In this model, a same conventional membrane oxygenator was used in three goats as a control (P-NC group), and a new silicone coated membrane oxygenator, in three (P-SC group). The O2 transfer and CO2 removal functions of the both groups showed the same ranges. Plasma free hemoglobin, the parameters of coagulation and fibrinolysis of P-SC group were lower than that of P-NC group, especially after 72 hours support. In conclusion, this new oxygenator was reliable, biocompatible and durable, and it is suitable not only for CPB, but also for long-term cardiac support.

[Retinoic acid-induced cell growth inhibition and differentiation in testicular carcinoma cells in culture].

Testicular germ tumor cells could be differentiated spontaneously or by some chemotherapeutic compounds. However, the mechanism by which the cells are differentiating from the stem cell remains unclear. The KU-MT cells, which were newly established from lung metastasis of testicular carcinoma, have been continuously producing alpha fetoprotein (AFP). Retinoic acids are well-known to induce cellular differentiation in culture and have already been applied for a clinical usage against leukemia. In the present study, all-trans-retionic acid (ATRA) elevated the level of AFP and inhibited the growth of KU-MT cells in vitro. ATRA also arrested the cell cycle in G1 and reduced the percentage of the S phase cell in terms of wild type p53, leading to apoptosis in part. Retinoids, especially retinoic acid receptor (RAR)-alpha specific agonists induced laminin production, a marker of endodermal differentiation; whereas arotinoid, a retinoid not bound to RAR-alpha, did not affect laminin expression. In summary, retinoic acids could mediate cell growth and differentiation of testicular tumor through RAR-alpha.

Function of FK506 binding protein (FKBP) in chick embryonic cardiac development.

FK506 binding protein (BP) 12, an immunophilin of FK506-binding proteins, is involved in intra-cellular signal transduction through the calcineurin-nuclear factor pathway. FKBP12 is reported to be associated with the ryanodine-receptor and IP3 Ca2+ channels, and to regulate cell proliferation via binding transforming growth factor (TGF)-beta receptor and cyclin dependent kinase (CDK). To elucidate the function of FKBP12 in cardiac development, we analyzed the temporal profile and regulation of FKBP12 expression in chick heart and in cultured cardiomyocytes. FKBP12 is expressed in embryos as early as day 4 and is predominantly associated with cardiomyocytes and osteo-chondrocytes. Tissue FKBP level in the heart increases with development. Immunohistochemically, the distribution and levels of FKBP12 appear to be related to sarco-endoplasmic reticulum Ca-ATPase 2 (SERCA2) but not to sarcomeric proteins. In proliferating cells, FKBP12 expression correlates with cellular mitosis, but not with DNA synthesis. In earlier embryos (< day 8), suppressing the activity of FKBP by FK506 administration is lethal, and induces cardiomegaly at later stages. In cultured cardiomyocytes, FK506 reduces the level of contractile proteins and inhibits cell proliferation. These results show that FKBP12 is enriched in cell types involved in dynamic Ca handling, and is likely an important molecule for cardiac development. FKBP12 most likely functions by affecting cellular Ca handling, since its effects are modified by modulators of Ca handling by sarcoplasmic reticulum.

[Effectiveness of a health education class to increase fish intake evaluated by serum fatty acid compositions].

For prevention of cardiovascular disease, we recommended increased fish intake for 50 middle-aged women and men of 40-83 years old at a three-month community-based health education program in an inland farming community. The effectiveness of the education program was evaluated by examining changes in serum lipids and serum fatty acid compositions. For comparison, we selected 48 men and women of 38-82 years old did not participated in the program. Before education, there was no significant difference in the dietary frequency of fish, meat, egg and dishes prepared with oil, mean values of serum lipids or serum fatty acid compositions except for saturated fatty acid, gamma-linolenic acid, dihomo-gamma-linolenic acid and alpha-linolenic acid. After three months, the frequency of fish intake of one or more times per day increased from 33% to 43% in the education group while the frequency of fish intake did not change in the control group. In the education group, serum n3 polyunsaturated fatty acid composition increased from 8.8% to 10.6%. This fatty acid increase was primarily observed in eicosapentaenoic acid and docosahexaenoic acid. The n3/n6 ratio increased from 0.21 to 0.31. In the control group, no change was observed in n3 polyunsaturated fatty acids. This study suggests that the community-based dietary education increased fish intake and serum n3 polyunsaturated fatty acids in women and men of 40 years old or older.

A statistical study of fistula auris congenita in Japan.

A statistical study of the occurrence, in Japan, of fistula auris congenita was conducted in 15,114 subjects (8,018 elementary school and 7,096 junior high school students) in Kanagawa, Shizuoka, and Wakayama Prefectures. The incidence of fistula auris congenita was 2.6% (396 of 15,114 students). Many cases were unilateral and exhibited a single manifestation. The fistula sites were mainly in the preauricular region. Only 2.3% of the 396 fistula cases had a history of radical operation.

Visual-vestibular interaction--an evoked potential study in normal human subjects.

Positron emission tomography (PET) was used to demonstrate temporary down-regulation of visual cortex activities associated with eye movements. The aim of the present study was to test this paradigm with visual evoked potentials (VEPs) in the visual cortex of 7 healthy human subjects during caloric nystagmus. Caloric stimulation was performed by irrigating the right or left external ear canals with 100 ml of warm water at 44 degrees C for 50 s with a head position of 60 degrees tilted backward in order to stimulate the horizontal semicircular canal. The subjects kept their eyes closed during and after caloric stimulation. For recording the VEPs, the flashlight was used for activating the visual cortex with the subject's eyes closed. In the 7 subjects, no significant difference was detected between the VEPs induced with and without caloric stimulation.

Passive smoking and plasma fibrinogen concentrations.

To examine the effect of passive smoking on plasma fibrinogen, a coronary risk factor, a cross-sectional study was conducted between 1990 and 1993 for 1,780 Japanese women aged 45-74 years who resided in Kyowa town, Ibaraki-ken, Japan. Fibrinogen concentrations controlling for age, body mass index, ethanol intake, serum total cholesterol, diabetes mellitus, and menopausal status were 8.6 (95% confidence interval 1.6-15.6) mg/dl higher in women exposed passively to smoking outside the home (n = 435) and 11.2 (95% confidence interval 3.0-19.3) mg/dl higher in women exposed both in and outside the home (n = 272) than in women unexposed in either location (n = 524). These effects of passive smoking were about 40-60% of that of current active smoking. An effect of passive smoking at home only was small and not statistically significant. The association between fibrinogen and passive smoking was primarily observed in women aged 45-59 years but not in those aged 60-74 years. Passive smoking may raise the risk of coronary heart disease partly by increasing plasma fibrinogen concentrations.

Polymorphism of the apolipoprotein B gene and blood lipid concentrations in Japanese and Caucasian population samples.

To examine whether a racial difference in apolipoprotein B (Apo B) gene polymorphism between Japanese and American Caucasians corresponds with the lower blood cholesterol concentrations in Japanese than in Americans, we examined the EcoRI polymorphism of the Apo B gene for 271 nonsmoking men and women aged 47-69 years in two population-based samples: rural Japanese living in Akita and Caucasians living in Minneapolis-St. Paul, Minnesota, USA. Mean values of serum cholesterol concentrations were significantly lower in Japanese than in Caucasians for both men and women (difference = 25-26 mg/dl). An allele-specific polymerase chain reaction was conducted to examine the Eco RI cutting site at the 12669 cDNA position of the Apo B gene. The allele R2 (absence of the cutting site) has been associated with lower cholesterol concentrations in two previous studies. The frequency of the R2 allele was 6% for Japanese and 17% for Caucasians (P < 0.001), and this race difference in allele frequency was identical for men and women. After controlling for age, body mass index, alcohol intake, and for women, menopausal status and hormone replacement therapy, the adjusted mean (SE) cholesterol level among Japanese was 204 (3) mg/dl for genotype R1R1 and 185 (7) mg/dl for genotype R1R2 or R2R2 combined (P = 0.01). The respective mean values among Caucasians were 224(5) mg/dl and 232(7) mg/dl (P = 0.36). The polymorphism had a similar effect on total cholesterol concentrations for both men and women. The observed lower prevalence of the R2 allele in Japanese than in Caucasians indicates that this variation in the Apo B gene does not explain the racial difference in blood cholesterol concentrations.

Effect of indomethacin suppositories on rectal polyposis in patients with familial adenomatous polyposis.

BACKGROUND: Oral sulindac is known to reduce polyps in patients with familial adenomatous polyposis (FAP). The authors speculated that rectal administration of indomethacin would be effective therapy for adenomas in the rectal remnant of FAP. METHODS: Eight patients with FAP who had been treated by total colectomy with ileorectal anastomosis were administered an indomethacin suppository (50 mg) once or twice daily during a period of 4 or 8 weeks. The number of polyps at the same site within the rectum was counted under proctoscopy prior to, at the end of, and after the treatment. In four patients, proliferative activity of the rectal mucosa was assessed by immunohistochemical staining for MIB-1. RESULTS: In six of the eight patients who initially had ten or more polyps, the number of polyps decreased to fewer than five, whereas such a decrease could not be observed in the remaining two patients. In the six patients, the number of polyps increased after indomethacin was discontinued. The proliferative activity of the rectal mucosa was higher at the end of treatment than it was prior to indomethacin administration. CONCLUSIONS: Indomethacin suppositories may be effective in the management of rectal adenomatosis in patients with FAP.

Substitution at codon 269 (glutamine --> proline) of the leptin receptor (OB-R) cDNA is the only mutation found in the Zucker fatty (fa/fa) rat.

We recently cloned one of spliced variant forms of rat leptin receptor (OB-R), which contains a short intracellular domain, and found obese-phenotype-linked nucleotide alteration in the extracellular domain of the cDNA from the Zucker (fa/fa) rat, which results in a glutamine269 to proline269 amino acid substitution. Reported herein are the cloning and sequencing of another spliced variant forms of rat OB-R cDNA with a long intracellular domain. Both forms of OB-R cDNA share the same extracellular domain. In the Zucker (fa/fa) rat, no changes in either the gene structure nor in the nucleotide sequence of the long intracellular domain were observed. However, the expression level of OB-R mRNA in the brain of Zucker (fa/fa) rat was higher than for lean littermates. These facts suggest that the substitution at codon 269 of the OB-R cDNA represents the crucial mutation which results in the obese phenotype of Zucker (fa/fa) rat.

Phenotype-linked amino acid alteration in leptin receptor cDNA from Zucker fatty (fa/fa) rat.

The mouse obese (ob) gene product (leptin), expressed specifically in adipose cells, regulates energy balance in mice. Both mouse diabetes (db) and rat fatty (fa) gene products are thought to play major roles in leptin signaling pathways in the hypothalamic area. Mutations of these genes in murines result in marked obesity and type II diabetes as part of a syndrome that resembles morbid obesity in humans. Reported herein are the cloning and sequencing of one of spliced variant forms of rat leptin receptor (OB-R) cDNA with a short intracellular domain. In the Zucker (fa/fa) rat, no changes in either the gene structure or the expression levels were observed. However phenotype-linked nucleotide alteration exists in the cDNA from Zucker (fa/fa) rat, which results in an amino acid substitution.
