[The effect of the interval from dinner until retiring on the concentration of lithogenic substances during sleep].

The relationship between the interval from dinner until retiring and the concentration of various substances secreted into the urine during sleep was studied in 69 healthy men who had eaten the same meal. The specific gravity and the concentrations of creatinine, uric acid and magnesium in the morning spot urine were highest in subjects who had retired within 2 hours after dinner. The concentrations of calcium and oxalate in the morning spot urine were highest in those who had retired between 2 and 4 hours. The specific gravity and urinary concentrations of creatinine, uric acid and oxalate were markedly higher in those with the interval less than 4 hours than in those 4 hours or longer. The concentrations of calcium, magnesium and citrate were not significantly different between the two groups. From these results, we recommend that the interval from dinner until retiring be prolonged and fluid be drunk before going to bed to decrease the concentration of lithogenic substances during sleep.

[Dietary habits of Japanese renal stone formers and clinical effects of prophylactic dietary treatment].

The daily consumption of various nutrients and the daily habits of 241 male stone formers were investigated. The patients (especially uric acid stone formers and hypercalciuric (greater than or equal to 300 mg/day) calcium stone formers) ingested much more total protein and animal protein than healthy Japanese. However, the amount of ingested calcium by the patients (470 mg) was less than that of healthy subjects (553 mg) and did not reach the level of the daily nutritive requirement (600 mg). The amount of ingested nutrients during the evening meal by the patients was about 50% of the daily amount with over 60% of the daily animal protein being ingested at dinner. This tendency was high in subjects who were in their 20's to 40's. The interval from dinner until retiring in the patients was not different from that of healthy males, but it was under 4 hours in over 50% of those in their 30's and 40's in both groups. From these results, we thought that a rapid change in nutritional status and dietary style in the short period after World War II had strongly affected the incidence of renal stone disease in Japan. Therefore, we have established the following general guidelines: 1) increased fluid intake, especially after dinner, 2) correct and avoid unbalanced diet (the diet should include all kinds of food, with vegetables being eaten at every meal and avoidance of an excessive intake of meat), 3) eat three meals a day and avoid an excessive intake at dinner, and 4) extend the interval from dinner until retiring. By following these individual dietary guidelines, the 5-year stone recurrence rate and the stone episode rate decreased remarkably in the period of not only outpatient visits but also in the period when the outpatient visits were discontinued. From these results, we conclude that individual dietary management should be the primary measure for the prophylaxis of renal stone disease in Japan.

[A case of atresia hymen with urinary retention].

A case of atresia hymen with urinary retention was reported. The patient was a 13-year-old female, who visited our clinic complaining of urinary retention and constipation. Examination of the external genitalia revealed atresia hymen . Intravenous pyelography showed hydronephrosis was not present. The symptoms disappeared completely after a hymenal incision was made. This case indicates necessity for caution concerning urological symptoms in females.

[Multiple myeloma in a patient with primary hyperparathyroidism].

A 50-year-old man presented with knee pain and general fatigue and was found to have severe hypercalcemia and renal failure. Hyperparathyroidism was suspected by hypercalcemia and a nodular lesion of the thyroid gland with CT-scan of the patient's neck. Exploration of the neck disclosed two slightly enlarged parathyroid glands. After surgery, the patient's serum calcium levels remained normal for two weeks, but after that his serum calcium levels rose again and renal failure continued. So needle biopsy of the kidney was enforced, and myeloma of the kidney was suspected. Multiple myeloma was diagnosed by bone marrow puncture.

Relationship between forced expiratory flow and tracheal sounds. Possible effect of vortices on flow.

We measured both tracheal sounds and forced expiratory flows simultaneously in 20 normal subjects and 4 patients with stenosis of the main bronchus. Spectral analysis of the tracheal sounds revealed development of discrete line spectra, the frequencies of which ranged between 488 and 942 Hz. There was a positive correlation between these frequencies and the peak flows (r = 0.601). We speculate that the origin of these line spectra are vortices rather than the vibrations of the wall. It is postulated that the pressure drops caused by vortices make the airways more susceptible to collapse and may lead to flow limitation.

[A case of urinary retention due to a female paraurethral leiomyoma].

A case of urinary retention due to a female paraurethral leiomyoma is reported. The patient was a 67-year-old female who visited our clinic complaining of urinary retention. Urinary sediment and infravenous pyelography revealed no abnormal findings. An oval-shaped mass was palpated at the anterior vaginal wall. The tumor was 7 X 6 cm in size, elastic hard in consistency, irregular surfaced and well movable. Computed tomography revealed a calcified tumor in the urethrovaginal septum extending over all the urethral length from the bladder neck to the urethral meatus. Cystourethroscopic examination revealed a bladder neck elevation. Transvaginal needle biopsy was performed and the pathological diagnosis was benign leiomyoma. Transvaginal removal of the tumor was performed under the diagnosis of female paraurethral leiomyoma. The tumor was easily dissected from the vaginal wall but was adhered rather firmly to the urethra. The origin of the tumor could not be identified. Postoperatively, urination was improved. Female paraurethral leiomyoma is rare but must be considered as a cause of urinary retention.

[Clinical studies on enoxacin in urinary tract infection].

Enoxacin was administered orally to 49 out-patients with urinary tract infections (UTI) at the doses of 600 mg/day and clinical effects were evaluated. Out of 13 patients with simple acute UTI evaluated by the UTI criteria, the results were excellent in 8 cases and good in 5 cases. Overall effectiveness rate was 100%. Out of 10 patients with simple acute UTI evaluated on over 5-day administration, the results were excellent in 9 cases and good in 1 case. Overall effectiveness rate was 100%. Out of 14 patients with simple acute UTI evaluated by our own criteria because their bacteriological response is unknown, the results were excellent in 7 cases and good in 7 cases. Overall effectiveness rate was 100%. Out of 12 patients with chronic complicated UTI evaluated by the UTI criteria, the results were excellent in 6 cases, good in 3 cases and poor in 3 cases. Overall effectiveness rate was 75.0%. Poor effective cases were mainly found in ones with mixed infection. Sensitivity test using discs did not always correspond to the bacteriological effect in cases with chronic complicated UTI. An adverse reaction was noted in 3 patients (6.1%). Two cases had gastrointestinal symptoms and 1 case had skin eruption. All symptoms were readily improved after discontinuing administration. From the above results, Enoxacin was considered to be a useful agent for urinary tract infection.

[Treatment in male infertile clinic of Kaizuka Municipal Hospital].

The results of treatment of 68 idiopathic male infertile cases are reported. The follow up period was 3 years and 8 months from the time the Department of Urology, Kaizuka Municipal Hospital had opened. The main treatments were human mammary gonadotropin-human chorionic gonadotropin (HMG-HCG) therapy and high ligation of left testicular vein in the cases accompanied with varicocele. Adjuvant therapy was administration of vitamin B12, herb medicine and antibiotic agents for prostatovesiculitis. Some cases were administered clomiphene citrate. The results in 43 cases at over 10 weeks after treatment was followed. Mean sperm count and mean sperm activity rate in 11 cases accompanied with no varicocele and whose sperm count was 2 approximately 45 x 10(6)/ml were raised from 11.0 +/- 8.0 SD x 10(6)/ml and 19.1 +/- 11.4 SD% to 22.7 +/- 16.8 SD x 10(6)/ml and 26.9 +/- 18.5 SD%, respectively after 20 weeks of HMG-HCG therapy. Efficacy was 7 out of 11 (63.6%) in sperm count and 6 out of 11 (54.5%) in sperm activity rate. Pregnancy was obtained in 3 cases. Effect of HMG-HCG therapy was not observed in 8 cases accompanied with varicocele and whose sperm count was 2 approximately 45 x 10(6)/ml. High ligation of left testicular vein was effective in all of the 4 cases accompanied with varicocele and whose sperm count was 2 approximately 45 x 10(6)/ml.(ABSTRACT TRUNCATED AT 250 WORDS)

[An experience of OK-432 intradermal administration in superficial bladder tumors].

A streptococcal preparation, OK432 was evaluated as the adjuvant immunotherapeutic agent for superficial bladder tumors. Twenty-nine cases were treated with complete transurethral resection and then randomized prospectively into two groups. One group had adjuvant therapy of OK432 intradermal administration and another group had no adjuvant therapy. OK432 administration significantly reduced the recurrence rates as compared to the control group. Although further studies were required for its full significance, in a short-term study, OK432 intradermal administration produced a beneficial effect in reducing the recurrence rates.

Parathyroid hormone and electrolytes during long-term treatment with allopurinol and thiazide.

We have investigated the composition of plasma and urine in patients with recurrent calcium calculi treated with allopurinol alone, thiazide alone, and both agents together for more than 18 months, and compared the results with those of patients receiving no treatment. In patients treated with allopurinol, both plasma and urinary uric acid were decreased, and plasma parathyroid hormone (PTH) and urinary calcium were increased between 1 and 2 years after treatment. In patients treated with thiazide, plasma calcium was decreased 3 months after treatment but then increased gradually, although the pre-treatment level was not reached. Urinary calcium was decreased 3 months after treatment but returned to pre-treatment levels at 6 and 12 months. Plasma PTH was gradually increased. In patients treated with both allopurinol and thiazide, plasma and urinary uric acid, plasma potassium and urinary calcium were decreased during treatment. Plasma PTH increased gradually during treatment. In patients receiving no treatment, the composition of plasma and urine did not alter during follow-up. The reasons for the changes in urinary calcium and plasma PTH have been discussed.
