Co-existing tachycardia and bradycardia dependent incomplete and complete right bundle branch block: evidence for diffuse conduction system involvement.

A case of co-existing tachycardia and bradycardia dependent incomplete and complete right bundle branch block (BBB) is reported. The degree of BBB was related to the duration of the preceding cycle length. In addition, more diffuse conduction system disease was suggested by evidence of sino-atrial and His-Purkinje conduction abnormalities.

Immunoelectron microscopic localization of thyroglobulin in the human thyroid gland.

The microidentification of the organelles containing thyroglobulin (TG) in the follicular cells of human thyroid glands were studied by the immunoelectron microscopic method. Fresh human thyroid glands were used for the purification of TG. TG was purified by differential salt fractionation and DEAE-cellulose chromatography. Anti-human TG rabbit IgG antibody was obtained by immunization of this purified TG. F(ab')2 fragments of anti-TG rabbit IgG antibody were prepared by pepsin digestion. The specificity of the antibody was tested on an Ouchterlony immunodiffusion plate. Conjugation of the purified F(ab')2 fragments of anti-TG rabbit IgG antibody to horse radish peroxidase was performed in order to use the direct peroxidase labelled antibody method. Under immunohistochemical light microscopy, the luminal colloid and the follicular cells appeared heavily stained. Under immunoelectron microscopy, positive reactions were observed in the rough endoplasmic reticulum, the Golgi apparatus, and the apical cell border. To date, no clear direct evidence of the presence of TG in the rough endoplasmic reticulum and the Golgi apparatus of human thyroid cells has ever been reported in immunoelectron microscopic studies. This study indicates that the use of F(ab')2 fragments of anti-TG rabbit IgG antibody in immunoelectron microscopic study is useful for the identification of TG in organelles of human thyroid follicular cells.

[Epidemiology of lung cancer in Nagasaki--especially in relation to radiation exposure].

We investigated the present incidence of lung cancer and its distribution by histologic type in Nagasaki City using Nagasaki Tumor Registry data from the years 1973-1977. The risk of radiogenic lung cancer did not increase significantly in either sex. The difference in relative distribution by histologic type between the exposure and non-exposure groups was not significant. Further analyses should be carried out on the risk present for individuals who were young at the time the bomb was dropped and the effect of exposure distance on the incidence and histologic types of lung cancer in A-bomb survivors.

[Conservative treatment of traumatic intracerebral hematoma].

In the previous report, it was insisted that traumatic intracerebral hematoma should be regarded as a variety of cerebral confusion and that conservative treatment would be therapy of choice in these situations. Adversely, unjudicious operation would sometimes result in more expansion of hematoma by untamponade effect of decompressive craniectomy. In the present investigation, it was attempted to provide therapeutic policy in the management of traumatic intracerebral hematoma. Consecutive thirty two cases of traumatic intracerebral hematomas were selected for this study. Those cases with other hematoma such as epidural or subdural hematoma were excluded. These 32 cases were divided into two groups, operative (15 cases) and non-operative (17 cases). Whether to be operated or not was arbitrarily decided by the surgeons who were faced to the patients. Disturbance of consciousness of the patients were divided into three categories, namely severe (III-100 to 200 level), moderate (II-10 to 30 level), and mild (I-1 to 3 level). They were 8 cases, 5 cases, 1 case in operative cases and 6 cases, 10 cases, 1 case in non-operative cases respectively. Mortality rates were 13.3% (2 cases) in operative cases and none in non-operative ones. Concerning the patients of severely disturbed consciousness, there were no difference in their final outcomes between the two groups. On the other hand, 7 out of 10 cases of moderately disturbed consciousness recovered completely without operation, whereas all operative cases of the same category were, more or less, handicapped.

QRS normalization of bundle branch block by ventricular fusion.

A 61-year-old woman who suffered from acute inferior myocardial infarction with right bundle branch block developed complete atrio-ventricular block. QRS normalization of right bundle branch block as a result of ventricular fusion of the conducted sinus beat with right bundle branch block and idioventricular impulse originating in the ipsilateral side of the block is described.

[A case of arachnoid cyst associated with chronic subdural hematoma].

An 11-year-old boy was admitted to our clinic with complaint of headache. On admission he was in drowsy state and neurological examination revealed minimal motor weakness of the right hand. Plain skull roentgenogram demonstrated a thinning of the left temporal bone and elevation of the left sphenoidal wing. CT showed marked displacement of the midline structures from left to right. The left lateral ventricle was collapsed. A well circumscribed, homogenous, high density area was delineated in the left middle fossa. Contrast enhancement was negative. A-P view of left CAG showed a crescent shaped subdural avascular area in the parietal region. The anterior cerebral artery was roundly shifted from left to right. Chronic subdural hematoma due to bleeding of arachnoid cyst was most likely. Subdural collection was evacuated by trephination. The postoperative course was uneventful. Postoperative CT 3 week after operation delineated the arachnoid cyst more clearly as round low density area. Follow-up CT 6 months later showed the arachnoid cyst markedly reduced in size.

Cineangiographic evaluation of diastolic properties of the left ventricle in man.

Diastolic properties of the left ventricle were evaluated cineangiographically in 110 patients with various heart diseases (19 normal cases, 13 mitral stenosis, 19 left ventricular volume overload, 18 coronary artery diseases without myocardial infarction, 27 myocardial infarction, 6 hypertrophic cardiomyopathy, 5 congestive cardiomyopathy and 3 chronic constrictive pericarditis). Early diastolic ventricular filling was assessed by diastolic maximal circumferential relaxation velocity (maxVcf), diastolic maximal dV/dt, volume change during early 1/3 of ventricular filling (V1/3), rapid filling time and isovolumic relaxation time; myocardial stiffness constant K was calculated at end-diastole. Increase in myocardial stiffness constant was not necessarily associated with abnormal indices measured during the early diastolic period but well correlate with isovolumic relaxation time. During the isovolumic relaxation period, the affected left ventricular segments where myocardial infarction or myocardial hypertrophy existed, showed reduced and delayed relaxation resulting in a total increase in the isovolumic relaxation period. Calcium antagonist was effective in improving the delayed relaxation in patients with left ventricular hypertrophy, suggesting a role of calcium on delayed myocardial relaxation in the hypertrophied heart.

Congenital corrected transposition of the great arteries: case report of three adults.

Three cases in adults of corrected transposition of the great arteries (CTGA) with concomitant fairly rare cardiac anomalies are described; a 54-year-old man with left sided atrio-ventricular valvular (tricuspid) regurgitation, a 31-year-old woman with atrial septal defect and a 21-year-old man with partial dysplasia of pulmonary valve with mild pulmonary stenosis. Clinical findings and diagnostic methods including two-dimensional echocardiography and computed tomography are discussed briefly.
