Diagnosis and screening of small hepatocellular carcinomas. Comparison of radionuclide imaging, ultrasound, computed tomography, hepatic angiography, and alpha 1-fetoprotein assay.

Twenty-nine small (less than 5 cm) hepatocellular carcinomas in 18 patients were examined by radionuclide imaging (RN), ultrasound (US), computed tomography (CT), hepatic angiography, and serum alpha 1-fetoprotein (AFP) assay. Sensitivity was 39% with RN, 50% with US, 56% with CT, and 94% with angiography, including infusion hepatic angiography (IHA). Lesions larger than 3 cm could be detected by all of these methods; those between 2 and 3 cm were generally shown by US and CT but not RN. IHA was essential for diagnosis of lesions less than 2 cm, which were otherwise difficult or impossible to detect except with angiography. As a screening method, AFP was best, followed by US and CT. The authors recommend using AFP and US to minimize expense and radiation exposure. In questionable cases, IHA should be performed.

Osteomatous changes and tooth abnormalities found in the jaw of patients with adenomatosis coli.

Oral findings on the panoramic tomograms of fifty-two patients with adenomatosis coli (AC) are presented. Osteomatous lesions, embedded teeth, and supernumerary teeth were found more frequently in AC patients than in a normal population. The osteomatous lesions were found in more than 80 percent of the AC patients, and the average number of such lesions per patient was 4.7. In a normal population, 15 percent had the osteomatous lesions, and the average number per patient was 1.1. The panoramic tomograms of twelve AC patients who had more than six osteomatous lesions showed very characteristic images. The possibility of early detection of AC through oral radiographic findings is suggested.

The diagnostic accuracy of computed tomography in obstructive biliary disease: a comparative evaluation with direct cholangiography.

Computed tomography (CT) and direct cholangiography were performed in 75 patients for the evaluation of obstructive biliary disease. The two modalities correlated well in cases of moderately or severly dilated intrahepatic bile ducts that showed tortuosity and/or engorgement, but mild dilatation with stretching of theperipheral intrahepatic bile ducts that was seen with direct cholangiography was detected with CT in only one of nine patients. Consequently, the CT diagnosis was correct in 41 of 51 patients (80.4%) with obstructive biliary disease; however, CT was correct in only 15 of the 25 patients (60%) with nontumorous obstructive biliary disease. It is concluded that, in cases of clinically suspected nonneoplastic obstructive disease, percutaneous transhepatic cholangiography or endoscopic retograde cholangiopancreatography should be performed even when dilated ducts are not demonstrated by CT.

Reduction of ferricytochrome c by human red cells.

Human red cells reduced extracellular ferricytochrome c to ferrocytochrome c under various conditions, suggesting that ferricytochrome c reducing systems are present at the outer surface of the red cell membrane.

Mechanism of methaemoglobin reduction by human erythrocytes.

The time course of methaemoglobin reduction in human erythrocytes treated with nitrite was studied at pH 7.4, 37 degrees C, in the presence or absence of Methylene Blue, and the changes in methaemoglobin, intermediate haemoglobins and oxyhaemoglobin during the reaction were analysed by isoelectric-focusing on Ampholine/polyacrylamide-gel plates. In both cases, with or without the dye, the intermediate haemoglobins were found to be present at (alpha 3+beta 2+)2 and (alpha 2+beta 3+)2 valency hybrids from their characteristic position on electrophoresis, but amounts changed consecutively with time. The amount of (alpha 3+beta 2+)2 was always greater than that of the (alpha 2+beta 3+)2 valency hybrid. This result is explained by the differences in redox potentials between alpha- and beta-chains in methaemoglobin tetramer. It was concluded that methaemoglobin was reduced in human erythrocytes through these two different pats: methaemoglobin leads to k+3 (alpha 2+beta 3+)2 leads to k+3 oxyhaemoglobin. The reaction rate constants k'"1 (= k+1+k+3) and k'+2(=k+2+k+4) were estimated from the changes in each component methaemoglobin, intermediate haemoglobins [(alpha 3+beta 2+)2+(alpha 2+beta 3+)2] and oxyhaemoglobin.

Effects of prostaglandin F2alpha on egg transport and in vivo egg recovery from the vaginas of rabbits.

The effects of prostaglandin F2alpha (PGF2alpha) on egg transport and vaginal egg recovery were examined in rabbits. Little disturbance of egg transport was induced by an injection of 2 or 5 mg/kg of PGF2alpha 12 hours after mating. A remarkable disturbance of egg transport was seen when 5 mg/kg were injected 22 hours after mating. The administration of PGF2alpha24 hours after mating led to maximal vaginal egg recovery: 48.4% and 54.8% of eggs were recovered by 51 hours after treatment, and implantation rates were 3.2% and 6.5% after treatment with 2 and 5 mg/kg, respectively. The earliest eggs were recovered vaginally 3 hours after administration of PGF2alpha at 24, 36, 48, or 96 hours after mating. The recovery of slightly bloody washing fluids at the first and second vaginal washings in animals treated 24 or 36 hours after mating appears to indicate that severe uterine contractions were induced by PGF2alpha.

Measurement of integral absorbed dose by chemical dosimeter in panoramic tomography.

As an aqueous chemical dosimeter for measuring ionizing radiation, the chemical 4,4' (5-chloro-2-thenilidene) bis [N,N-dimethylaniline], a derivative of the leuco triarylmethane compounds was used. This chemical dosimeter is an aqueous solution composed of 10(-4) M leuco compounds, 10(-4) M ferrous ammonium sulfate, 10(-4) M sodium chloride and 7 X 10(-3) M of hydrochloric acid. This solution is colourless but it becomes blue-green or bright blue after irradiation. The optical density of this solution at the main absorption peak of 635 millimicron increases linearly with the increasing x-ray dose of from 50R to 2,000 R and no dose-rate dependence is found from 13.5 R/min to 270 R/min of 60Co gamma-ray, 896 gram rads was the measured value of the integral absorbed dose per exposure in panoramic tomography (Orthopantomograph type OP-2).

Enzyme histochemical study on hepatoma--the relation between enzyme activity and histological type.

The activities of gamma-glutamyl transpeptidase (gamma-GTP), alkaline phosphatase (A-p), and leucine aminopeptidase (LAP) were examined in 18 cases of hepatomas. The activity of gamma-GTP was most remarkable in the hepatoma consisting of small to medium-sized tumor cells showing the least atypism. The enzyme activity found in the type composed of large tumor cells resembled that of normal liver and was considered to be the most mature form of the neoplasm. This enzyme was not found in the immature type composed of small typical tumor cells. A-P activity was seen in only a few cases of hepatoma; conspicuous in one case showing immature features and sporadically in one case with florid histological pattern. The activity of this enzyme could not be confirmed in the type demonstrating marked gamma-GTP activity. LAP activity was noted in the majority of cases, especially marked in the medium-sized tumor cells, but there was hardly any connection between this enzyme and histological type. In general, the cases demonstrating positive gamma-GTP activity tended to show LAP activity. Although the activity of gamma-GTP and that of A-p usually showed an inverse relation, all three enzymes demonstrated almost equal activity in the type showing a florid histological pattern.
