[The usefulness of percutaneous transhepatic cholecyst puncture (PTCCP) with ultrasoundimage control for acute cholecystitis].

Percutaneous transhepatic cholecyst puncture (PTCCP) is a unique treatment for acute cholecystitis by using of 21 gauge PTC needle with ultrasoundimage control. The procedure is as follows. A percutaneous transhepatic puncture of the gallbladder is made with ultrasonic guidance. After suction removal of the contents of the gallbladder, saline with antibiotics is injected. Finally PTC needle is removed after removal of the injected saline. We carried out PTCCP in 35 patients with acute cholecystitis, and compared its clinical effect with those of PTCCD (46 cases) or conservative treatment of antibiotics administration (38 cases). The treatment of PTCCP removed the clinical symptoms of acute cholecystitis rapidly compared with the conservative treatment and it made the hospitalization shorter compared with the treatment of PTCCD. PTCCP was also performed without any severe complications for patients with underlying diseases. The curative rate of PTCCP was the same as those of other former treatments. It was concluded that PTCCP is a safe, convenient, and useful treatment of acute cholecystitis.

[Bone marrow transplantation from an unrelated donor in patient with congenital heart diseases].

A 9-year-old with chronic myelogenous leukemia (CML) who received bone marrow transplantation from an unrelated donor (UBMT), is reported. He also suffered from congenital heart disease (CHD) consisting of corrected transposition of the great arteries and dextrocardia. The cardiac output was within normal limits. The conditioning regimen included busulfan, melphalan, ALG and TLI. Cyclophosphamide was not used because of it cardiotoxicity. The HLA-phenotype of the donor was identical with that of the patient. DNA typing showed was haplodentical DRB1. The patient is alive 18 months after the UBMT. This case showed that UBMT was possible in a CML patient with CHD, without congestive heart failure.

[The clinical significance of glitter-cells in the urine during urinary tract infection].

To clarify the relationship between glitter-cells and urine osmotic pressure, neutrophils were isolated from venous blood of 4 normal volunteers and incubated in urine specimens with different osmotic pressures. Glitter-cells were detected in the urine with 100-400 mOsm/l. The rate of glitter-cells in neutrophils incubated for 4 hours were significantly less than those incubated for 0 minute (p < 0.05). We clinically investigated the relationship between the presence of glitter-cells in fresh urine and the site of urinary tract infection (UTI) or organisms isolated from urine. Fresh urine samples were obtained from 95 patients (55 male and 40 female). Urine samples were examined for bacteriuria by the quantitative culture method, and significant bacteriuria was defined as more than 10(5)/ml of bacilli for midstream urine or as more than 10(4)/ml for urine collected by catheterization. Leukocytes and glitter-cells in the unspun and unstained urine were counted on a disposable slide using a counting chamber. Patients with upper UTI showed a significantly higher incidence of glitter-cells than patients with lower UTI (p < 0.05). Patients with glitter-cells had a significantly higher incidence of polymicrobial infections than those without glitter-cell (p < 0.05). These findings suggest that patients with glitter-cells in the urine show upper UTI (pyelonephritis) and polymicrobial infections.

[Detection of numerical chromosomal aberrations in hematopoietic malignancy by in situ hybridization on bone marrow aspirate paraffin sections].

We evaluated the usefulness of in situ hybridization (ISH) with chromosome specific DNA probe on paraffin sections of bone marrow aspirates. Twenty cases of hematopoietic malignancy and eight control cases of non-hematopoietic malignancy were examined with centromere-specific probes for chromosomes 8 and 17. In the eight control cases, the mean rates of cells with more than three hybridization signals were 1.13 (2SD = 1.90) for chromosome 8, and 0.88 (2SD = 2.25) for chromosome 17. The mean rates plus 2SD were 3.03 for chromosome 8, and 3.19 for chromosome 17. Therefore, we defined cases of more than 4.0% of cells showing more than three hybridization signals per nuclei as having a numerical abnormality (trisomy). We compared these results with conventional cytogenetic results by karyotype analysis. In twenty hematopoietic malignancy cases, three cases demonstrated trisomy 8 by ISH. Two cases also demonstrated this abnormality by karyotype analysis, but one case showed no abnormality by karyotype analysis. While trisomy 17 detected in one case that did not demonstrate numerical abnormality, only structural abnormality by karyotype analysis. The rate of discrepancy between results of ISH analysis and those of karyotype analysis was only 5% (2/40) for both chromosomes. In five cases, re-examinations were performed within three months. In one case, we could not obtain adequate material for karyotype analysis. However, this case showed trisomy 8 by ISH. Structural chromosomal abnormalities such as translocation or deletion could not be detected by this ISH analysis with centromere-specific probes. However, this method has the advantage result that we can perform retrospective assessments, do not need to culture cell, and can compare with pathological findings. Thus, we conclude that ISH analysis with paraffin sections of bone marrow aspirates will provide more useful information by combining ISH analysis and karyotype analysis.

Is nestin a marker for chemosensory precursor cells?

The vomeronasal and olfactory systems are unique in that their chemosensory neurons undergo continuous neurogenesis after development. Immunoreactivity to nestin, a neuronal precursor marker protein, was investigated in the developing rat vomeronasal organ to determine its potential as a cell marker. From postnatal day 1 (P1) to P22, the distribution of nestin positive cells became progressively restricted to the area adjacent to the basement membrane. By P29, the vomeronasal organ reached structural maturity and only a few nestin positive cells were observed. Results suggest that nestin may be a useful marker for neuronal precursor cells in studies of neurogenesis and development of chemosensory systems.

Identification of N-acetylglucosamine-alpha-1-phosphate transferase activity in Dictyostelium discoideum: an enzyme that initiates phosphoglycosylation.

A lysosomal proteinase from Dictyostelium discoideum was previously shown to have GlcNAc alpha-1-P residues in phosphodiester linkage to serine. We have identified a GlcNAc-alpha-1-P transferase activity in membrane preparations using UDP[3H]GlcNAc and a peptide acceptor with three tandem Ser-Gly repeats. We established an assay, proved the structure of the product, determined the Kms for donor and acceptor and showed that the glycopeptide binds a GlcNAc-alpha-1-P specific rabbit antibody. These findings provide the tools to search for mutants lacking GlcNAc-alpha-1-P transferase activity as a probe for the function of this modification we call phosphoglycosylation.

Metabolic reduction of novel 3,4-dichloro-5-nitrofurans in Salmonella typhimurium.

To gain insight on biochemical mechanisms of mutagenesis and carcinogenesis by the experimental carcinogens, 5-nitrofurans, a new series of 3,4-dichloro-5-nitrofurans, comprised of 3,4-dichloro-5-nitro-2-acetylfuran (I), 3,4-dichloro-5-nitro-2-bromoacetylfuran (II), methyl 3,4-dichloro-5-nitro-2-furoate (III), were synthesized and tested for their activation to mutagenic forms in the standard plate assay using Salmonella typhimurium TA98, TA100, and TA100NR, a derivative of TA100 deficient in nitroreductase activity. The mutagenic responses in TA98 were 2- to 6-fold lower compared to TA100. Furthermore, I and II were less active in TA100NR, while compound III was about four times more mutagenic in TA100NR compared to the parent strain TA100. Incubation of III with NADPH and bacterial lysates showed that the extent of reduction was greater in TA100 compared to TA100NR. High-pressure liquid chromatography analysis of the ethyl acetate extract obtained from incubation of III with lysates of TA100 revealed the formation of four metabolites with retention times of about 4.0, 5.7, 10.0, and 14.3 minutes. The spectroscopic and chromatographic properties of the components with retention times of 10.0 and 14.3 minutes were identical to two derivatives obtained by chemical reduction of III, and thus represent nitroreduction products. These derivatives have been identified as cis- and trans-oxime isomers of methyl 3,4-dichloro-2-furoate, based on spectroscopic analyses. These oximes were not mutagenic for TA100. Furthermore, III was more mutagenic under anaerobic conditions, suggesting that secondary superoxide or nitroanion free radicals generated from nitroreduction are not responsible for the mutagenicity of III. In addition, the higher mutagenic response in TA100NR, and the lack of mutagenic activities of the amino and the oxime analogs of III suggest that the mutagenic activation of III might be due to the nitroso intermediate or involve mechanisms other than nitroreduction.

Plastic effect of soiled bedding on the structure of synapses in rat accessory olfactory bulb.

The effects of exposure to soiled bedding on synaptic morphology in the accessory olfactory bulb (AOB) were examined in adult male rats. Forty-day-old male rats were isolated. One group was exposed to bedding soiled by male and female rats (EC). Another group was exposed only to male-soiled bedding (SC). A third group was exposed to clean bedding (IC). After 2 months, the animals were sacrificed for electron microscopy. The size and the numerical density of synapses were measured in the glomerulus and the granule cell layer. In the glomerulus, the mean size of the synapses was significantly greater in the EC than in the IC group, whereas that in the SC group appeared to be intermediate between those in the EC and the IC groups but was not significantly different from those in the EC and the IC groups. There was no statistically significant difference in the density of synapses among the three groups. Synapses in the granule cell layer are classified into two types: 1) perforated synapses, which are characterized by discontinuities in their postsynaptic thickenings, and 2) nonperforated synapses. The mean size of perforated synapses was significantly greater in the EC than in the IC and SC groups; however, no statistically significant difference was observed in the size of nonperforated synapses among the three groups. Moreover, no statistically significant difference was observed in the numerical densities of either perforated or nonperforated synapses among the three groups. These results suggested that exposure to a more complex soiled bedding environment (i.e., to bedding soiled by both male and female rats) can induce greater structural changes of the synapses in the AOB of male adult rats.

Morphology of vomeronasal organ cultures from fetal rat.

The vomeronasal organs (VNOs) of rats were cultured from embryonic 15-day littermates on collagen-coated membrane in Dulbecco's modified Eagle's medium containing serum. The explants were observed sequentially and fixed at 4, 6, 8, 10 and 14 days in vitro (DIV). Organogenesis of VNOs and cell differentiation took place in vitro. Patterns of organogenesis of the VNO in vitro were different from those in vivo. Both sensory and supporting cells in the sensory epithelium had microvilli on their surface. Epithelial cells in aggregates of non-sensory epithelial cells had cilia and microvilli on their surface. Vomeronasal axons forming two to three large fascicles were seen originating from the VNO at 4, 6, and 8 DIV, and degenerated at 10 or 14 DIV. Glial cells (ensheathing cells) were observed in the fascicles. These morphological characteristics of VNO cells in vitro were similar to those observed in vivo.

Coculture of the vomeronasal organ and olfactory bulb of the fetal rat.

The vomeronasal organ and the olfactory bulb of the rat were cocultured from 15-day embryo siblings on collagen-coated membrane in Dulbecco's modified Eagle's medium containing fetal calf serum, horse serum, and antibiotics. At 4 days in vitro (DIV), vomeronasal axons forming two to three large fascicles were seen originating from the explants of the vomeronasal organ. Differential axonal growth was observed. Some fascicles made connections with the explants of the olfactory bulb. Twenty percent of the cocultures studied here showed the formation of connections. At 6-10 DIV many fascicles that did not connect with the olfactory bulb had degenerated, and large fascicles that were connected with the olfactory bulb survived for more than 10 DIV. The formation of connections between the vomeronasal organ and the olfactory bulb in coculture favors the survival of large nerve fascicles, but it could not be determined whether or not the presence of the olfactory bulb affects the initial orientation of the fibers and fascicles from the explants of the vomeronasal organ.

Differential binding patterns of three antibodies (VOBM1, VOBM2, and VOM2) in the rat vomeronasal organ and accessory olfactory bulb.

Immunohistochemical properties of monoclonal antibodies raised against the rat vomeronasal epithelium were examined in adult rats. Three monoclonal antibodies, VOBM1, VOBM2, and VOM2, reacted specifically to the luminal surface of the sensory epithelium of the vomeronasal organ. In addition, the reactivities of VOBM1 and VOBM2 were detected in the vomeronasal nerve layer and the glomerular layer of the accessory olfactory bulb. Electron-microscopic study revealed differential patterns of the immunoreactivity of the three antibodies to the microvilli of vomeronasal sensory epithelium. VOBM1 immunoreactivity was found on the microvilli of the supporting cells, whereas VOBM2 immunoreactivity was found on those of the sensory cells. VOM2 immunoreactivity was observed on the microvilli of both the sensory and supporting cells. These results suggest that the three antibodies recognize different antigens on the vomeronasal sensory epithelium. In particular, VOBM2 antibody appears to react to an antigen specific to the microvilli of the vomeronasal sensory cells.

Frantz's tumor (solid and cystic tumor of the pancreas) with liver metastasis: successful treatment and long-term follow-up.

A rare case of Frantz's tumor with liver metastasis is reported. An 11-year-old girl had local resection of a Frantz's tumor in the head of the pancreas. Ten years later, she had a recurrence in the pancreatic head, and two liver metastases were also disclosed at reoperation. Pancreaticoduodenectomy and enucleation of metastatic lesions in the liver were performed. Six years after the second operation, she is well and free of disease. A review of 174 cases of Frantz's tumor disclosed that metastatic disease in the pediatric age group is extremely rare and yet the prognosis is favorable. On the other hand, the outcome in older patients, especially those over 40 years old, is very poor.

Different responses of renal blood flow and sympathetic nerve activity to captopril and nicardipine in conscious renal hypertensive rabbits.

To elucidate the roles of endogenous angiotensin II (AII) and renal sympathetic nerve activity (RSNA) in modulation of renal blood flow (RBF), we recorded RBF and RSNA in conscious two-kidney, one-clip (2K1C) hypertensive rabbits with blood pressure (BP) reduced to a similar extent by captopril (5 mg/kg) and nicardipine (4.3 micrograms/kg/min). We measured plasma concentrations of AII, arginine vasopressin (AVP), and norepinephrine (NE). Despite comparable depressor effects, changes in RBF showed different profiles with the two drugs in renal hypertensive rabbits. After captopril injection, RBF was consistently increased to 143 +/- 7%. In contrast, with nicardipine infusion, RBF was initially increased to 114 +/- 5% and then significantly decreased to 86 +/- 4%. The increase in RSNA was greater with captopril than with nicardipine. Plasma concentration of AII was decreased with captopril but significantly increased with nicardipine. In sham-clipped normotensive rabbits in which plasma AII was not increased, RBF was not reduced with nicardipine. Thus, vasoconstrictor actions of RSNA and increased AII may have overcome the vasodilatory effect of nicardipine in conscious renal hypertensive rabbits. Because the increase in RSNA was smaller with nicardipine, we speculate that the vasoconstriction induced by AII, as well as background BP level, played a substantial role in determining RBF.

A case of drug eruption caused by the crude drug Boi (Sinomenium stem/Sinomeni caulis et Rhizoma).

We report a case of drug eruption caused by the crude drug Boi. A 41-year-old female patient had been diagnosed with chronic rheumatoid arthritis in the department of internal medicine. After ingestion of a decoction of the crude drug Boi for the alleviation of arthralgia, a slight fever developed, which was followed by systemic edematous erythema with itching. HPLC showed that the main components of the crude drug Boi are sinomenine and magnoflorine. The results of patch tests were negative for all oral drugs that the patient had been taking. Oral ingestion tests showed that the patient showed positive reactions to the as-is Boi boiling-water decoction and 1/10-volume sinomenine. Based on this, the drug eruption was judged to be caused by sinomenine. It is considered the first time that the causative component of a drug eruption was confirmed by oral ingestion tests with components of a crude drug of Kampo medicine (Sino-Japanese traditional medicine).

Baroreceptor function is restored by antihypertensive therapy through lowering of blood pressure in adult SHR.

1. We investigated the effects of antihypertensive treatment (8 weeks) with four different agents (trichlormethiazide, atenolol, nicardipine and enalapril) on baroreceptor function in 28 week old spontaneously hypertensive rats (SHR) to measure aortic depressor nerve (ADN) activity. 2. Threshold pressure (Pth) of ADN activity was elevated and the gain sensitivity of the pressure-activity curve, as determined by the maximum gain (Gmax) of a logistic function curve, was depressed in untreated SHR compared to those in untreated Wistar-Kyoto (WKY) rats. 3. Treatment with the four agents similarly reduced blood pressure in SHR. Each of the four agents induced a decrease in Pth and an increase in Gmax to a similar extent in SHR. 4. These findings suggest that antihypertensive therapy in chronic hypertension augments baroreceptor function through the lowering of blood pressure but not through specific pharmacological actions.

Comprehensive treatment of advanced neuroblastoma involving autologous bone marrow transplant.

Encouraging results are reported with high-dose chemotherapy and total body irradiation followed by autologous bone marrow transplantation in the treatment of advanced neuroblastoma. However, relapse remains a significant problem. We used high-dose chemotherapy, surgery, intraoperative radiation and an autologous bone marrow transplant treated in vitro to remove tumor cells followed by 13-cis-retinoic acid to treat 36 children with advanced neuroblastoma. This comprehensive treatment appears to improve the survival rate of patients with advanced neuroblastoma, including those with N-myc amplification and bony involvement. The disease-free survival rate was 66% (95% confidence interval, 49-84%) at 3 years. All patients who received 13-cis-retinoic acid developed cheilitis, but no bone marrow depression occurred in these patients. Five patients developed hemolytic uremic syndrome (HUS) post-transplant. This may have been related to the procedure used for total body irradiation. Patients who had their kidneys shielded during this procedure did not develop this syndrome. Patients who received local irradiation at the primary site showed no evidence of relapse in this region, indicating that such therapy may help to prevent a relapse. These data suggest a high rate of 3 year disease-free survival with this treatment strategy. The nonrandomized nature of the study and use of multiple modalities precludes analysis of the specific contribution of each.
