Vasculitis with eosinophilia and digital gangrene in a patient with acquired immunodeficiency syndrome.

Fewer than 40 cases of vasculitis have been described in the setting of human immunodeficiency virus infection. We describe a patient with the acquired immunodeficiency syndrome (AIDS), a heavy smoker, who developed a syndrome of constitutional symptoms, eosinophilia and digital gangrene. Vasculitis of the digital arteries was documented by angiography. He responded to high dose corticosteroid therapy with arrest of the ischemic process. After steroids were discontinued, he suffered a relapse of the vasculitis documented by skin biopsy. In patients with AIDS with this serious, potentially steroid responsive condition, steroid therapy should be considered in spite of the preexisting immunodeficiency state.

Study on the pathogenicity of some Nocardia spp isolated from tap water of Ismailia City, Egypt.

All actinomycetes isolated from canal water, Ismailia water plant and tap water were identified into 5 genera. All Nocardia isolates were identified into 3 species. Two of which, Nocardia transvalensis and Nocardia otitidus-caviarus were recorded in tap water. These species are pathogenic for human kind and animals. The pathogenicity of these isolates was tested on Swiss Albino mice. Symptomatic changes and death of mice inoculated with Nocardia suspension were recorded. The main symptoms were erythema, enduration and inflammation together with abcesses and an increase in the size of the spleen (spleenomegally). It is concluded from these experiments that: i) All Nocardia species isolated from tap water are pathogenic, at least for mice, ii) these isolates could present health hazards to human kind and needs an urgent investigation.

Estimating the cost of the most prevalent diseases of the retired beneficiaries of health insurance organizations in Alexandria.

A retrospective study was carried out in Alexandria Health Insurance Organization Polyclinics and pharmacies to determine the prevalence of diseases among the retired beneficiaries and their utilization of health insurance services, and to estimate the annual expenses by disease with comparison of the retired contributions and expenses. Arthritis (49.3%) and respiratory tract diseases (43.4%) were the for most prevalent disease groups. Ischaemic heart diseases ranked the first in term of total expenses. The outpatient medicaments constituted the largest burden of expenses and the expenditures were six to ten folds that of the retired contributions.

Cytopathology in cancer of larynx.

The results of brush cytological smears were compared with the microscopical examination of punch biopsies obtained from 60 patients with suspected cancer of the larynx. The bronchofibroscope 'Olympus BF B3' was used to obtain the brush specimens. The results of cytology and pathology specimens were found identical in 93.3% of cases. Comparison between our results and previous similar work is presented.

Patterns of sensory loss following fractional posterior fossa Vth nerve section for trigeminal neuralgia.

Twenty-five patients with trigeminal neuralgia have been treated by fractional posterior fossa Vth nerve section. The procedure is shown to be effective in relieving pain, preserving corneal sensation and the motor root. Nineteen patients showed little loss of light touch sensation, and in 15 of these there was dissociation of sensory loss to pinprick and light touch. The results support the concept that there is some anatomical separation of fibres subserving light touch and pain in the posterior root of the nerve.

Prevalence and intensity of Schistosoma haematobium infection in six villages of upper Egypt.

A cross-sectional survey for Schistosoma haematobium infection in 5,998 individuals, representing 31,803 persons in six rural villages of the Qena governorate, Upper Egypt, was completed. There were 2,223 persons (37.1%) excreting S. haematobium eggs identified by the nuclepore membrane filtration technique of a single urine specimen. Prevalence of infection ranged from 23.9% to 64.0% among the six villages. Quantitatively the overall geometric mean egg-count was 48.1 eggs/10 ml urine. A survey of proximal canals was conducted and 9 of 4,312 Bulinus snails were infected with schistosomes (0.21%). The stools of 2 of 507 adolescent males were positive for S. mansoni. No Biomphalaria snails were found. When these findings are compared to past studies, S haematobium prevalence appears to have substantially increased.

An epidemiologic approach to occupational health problems in Egypt.

Industrialization started in Egypt on a large scale only 20 a ago. This process resulted in a shift of a large proportion of the agricultural workers to industrial work. The health insurance for industrial workers is enforced by law for certain industrial groups. Although preplacement and periodic medical examinations should be provided for the insured groups, they are not yet properly carried out and are deficient both qualitatively and quantitatively. For a reliable view of the occupational health situation an epidemiologic approach is a must. As a start, a computer-assisted pilot plan has been developed using data of one of the periodic medical examinations for a group of miners in Egypt.

An epidemiological study of Schistosoma haematobium and S. mansoni infection in thirty-five rural Egyptian villages.

Probability samples of individuals from 35 village communities in the rural Egyptian Nile Valley were examined for the presence of schistosome ova by sedimentation of urine and by the MIFC technique for stools. In all there were 12,933 persons selected of which 11,337 provided specimens for examination from a total population of 66,768 persons divided among three governorates: 37% in North Central Delta, 31% in Upper-Middle Egypt and 32% in the Upper Egypt. After controlling the differences attributed to sampling methods, 30% of the villagers from Kafr El Sheikh, were found positive for S. haematobium and 20% for S. mansoni. The prevalence of schistosomiasis in the Kafr El Sheikh villages was 42%. South of the Nile Delta in the Beni Suef villages, 27% were found positive for S. haematobium and less than 1% positive for S. mansoni. In Aswan, prevalence was associated with the type and location of the village. Prevalence was low (4%) in villages located on high barren ground, but elevated (25%) in a village built within cropped and irrigated land. S. mansoni cases were also in the Aswan villagers; however, local acquisition of this infection was not substantiated. The specific age-sex distributions for both schistosomes species were characteristic with a notable difference in the male-female infection ratio that increased from north to south. The source of domestic water supply and prevalence of infection were consistently associated. These results were compared to past findings in order to provide a frame of reference to aid in the development of future surveillance and investigations.

Dinitrochlorobenzene sensitization in children with bronchial asthma.

Twenty-one patients with bronchial asthma were tested by dinitrochlorobenzene. Eleven did not react (52%). Five of the patients had been previously BCG vaccinated but did not react to PPD. The results indicated that some impairment of cell-mediated immunity in asthmatics exists. The correlation between this finding and pathogenesis of asthma is discussed.
