[Mental disturbances in children and adolescents in Germany. Results of a representative study:age,gender and rater effects].

METHOD: A study on behavioral and emotional problems and competence in children and adolescents in Germany (PAK-KID study) is described. It is the first nationwide representative survey of this kind of children and adolescents aged 4 to 18 years in Germany. For children aged 4 to 10 years the parents completed the German version of Achenbach's Child Behavior Checklist (CBCL 4-18) developed by the Arbeitsgruppe Deutsche Child Behavior Checklist. Children and adolescents aged 11 years and older filled out the German version of the Youth Self-Report that is part of Achenbach's CBCL in addition to the parents completing the German version of the CBCL. A total of 2856 parent questionnaires and 1798 self-report questionnaires completed by children and adolescents were analyzed. RESULTS: The sample was representative with respect to the main sociodemographic variables. On all problem scales children and adolescents aged 11 to 18 years reported significantly more problems than their parents did. The frequency of internalizing problems (social withdrawal, somatic complaints, anxiety/depression) and delinquent behavior of children and adolescents reported by parents increased with the children's age, whereas aggressive behavior and attention problems decreased with age. Girls reported significantly more problems than boys on all internalizing scales of the Youth Self-Report. The effect was not totally replicated in the parent reports. In the parent reports, boys had more attention problems and more aggressive and delinquent behavior than girls.

[Granulomatous peritonitis after laparoscopic surgery of an ovarian dermoid cyst. Diagnosis, management, prevention, a case report].

We report a case of granulomatous peritonitis which occurred following laparoscopic exeresis of a dermoid cyst of the ovary performed together with medically induced abortion. The peritoneal cavity was contaminated when the cyst ruptured during the procedure emptying sebum and hairs in the peritoneum. Abondant washing with aspiration of all the visible particles was done with a 5 mm canula. The diagnosis of granulomatous peritonitis was suggested with the development of persistant fever, degradation of the patient's general health, moderate abdominal pain and a nodular image at echography located on the operated ovary. The nodule was found to be an inflammatory granuloma on a foreign body. Two second-look laparotomies, one transrectal and the other via the xypho-pubian route, were required 1 month after the initial operation for complete cure. The diagnosis was confirmed on the pathology report.

[Maternal mortality in Nice. Results of a reproductive age mortality survey using death registries in the Nice University Hospital, 1986-1993].

OBJECTIVE: To define the rate of maternal death on the basis of maternal deaths recorded from January 1986 to May 1993. To determine whether the patients had received "standard" quality health care. METHOD: A Reproductive Age Mortality Survey was performed retrospectively on causes of death in women from 14 to 45 years of age; death registries and clinical records at the Nice University Hospital were used. RESULTS: There were 23 cases of pregnancy-associated death. Maternal mortality was 32.9 per 100000 live births. According to the INSERM survey, the "real" national rate would be 18.3 per 100000. This is a classical underestimation. The overmortality in Nice is partially explained by the AIDS epidemic which was the major cause of female deaths between 14 and 45 years (96/322). Nevertheless, there was an unsatisfactorily large number of cases in which standard obstetrical practises were not applied. This was often due to a combination of factors including poor obstetrical surveillance, insufficient attention by physicians, poor organisation of obstetrical care in the emergency room and referrals. Cesarean section was implicated in 7 out of 12 maternal deaths during the third trimester of pregnancy (58%). The cause of certain maternal deaths remained unexplained; amniotic embolus was always suggested. Unfortunately, an autopsy was performed in only 36% of the cases and its quality was insufficient to determine certain diagnosis.

[A difficult diagnosis: pelvic-abdominal actinomycosis abscess].

A laparotomy for hysterectomy was performed in a 46-year-old nulliparous woman who had worn an intra-uterine sterilization device for 8 years. The patient was in poor general health and had had abdominal pain and repeated episodes of fever over the preceding 3 months. An abscess of the abdominal muscle was found peroperatively. No perforations of the small or large intestine were observed. The pathology examination yielded the diagnosis of Actinomycosis. Actinomyces israeli is the most frequency observed Actinomycae in humans. It is a saprophyte organism of the intestinal flora, not usually found in the vagina and observed in 3.5% of cervical swabs of patients wearing an intra-uterine device (Gupta bodies). The frequency increases with the duration of insertion and the pathogenicity appears when the immune responses are deficient and/or when a cofactor is involved. Treatment relies on surgical exeresis and prolonged antibiotics (penicillin G or macrolides). Hyperbar treatment is sometimes used.

[Drug addiction and pregnancy].

In drug-addicted women pregnancy is always at risk and difficult to supervise owing to the social and psycological profiles of these women. At the moment, we are dealing in France mainly with pregnancies that occur in heroin addicts and frequently have the following complications: lack of follow-up, prematurity, short child stature, infections (notably HIV infection) and weaning syndrome in the newborn. Due to improvements in the management of these patients, the neonatal mortality has been considerably reduced and has become exceptional. In the USA the number of pregnancies occurring in cocaine addicts has increased, and complications similar to those of heroin addicts have been observed, particularly with crack. It is not impossible that we might soon be confronted with this type of drug addiction.

[Coping in the family context: active and avoidance strategies in adolescents from divorced families].

Studies on children's response to divorce have mainly emphasized negative psychological sequalae such as poor social adjustment and emotional disorders. In many studies children's coping strategies were important moderator variables. In a meta-analysis, Suls and Fletcher (1985) found better short-term outcomes related to avoidant coping. On the long run, however, active strategies were better off. To date, little is known about family climate influences on adolescents' coping. In the current study, 128 children (62 girls, 66 boys) from German middle class families were observed 6 months, 1, 2, and 7 years after the divorce of their parents. At the time of divorce their mean age was 14,8 years (SD = 2,5). The models presented in this article are based on 39 adolescents (17 girls, 22 boys) for whom complete data were available. The study relates adolescents' coping strategies to measures of family functioning, as assessed through Family Environment Scale (Moos 1974). Coping strategies were assessed by Ways of Coping (Lazarus 1980), Impact of Event Scale (Horowitz 1979), and Stressverarbeitungs-Fragebogen (Janke 1985). Results, analyzed with LISREL models, indicate that supportive family climate, operationalized by openness and control, are good cumulative predictors for active coping. In a second model constraining family climates predicts avoidant coping in adolescents. Clinical implications are discussed.
