Simultaneous development of cells with large granular lymphocyte (LGL) morphology and natural killer (NK) cell lytic activity after bone marrow (BM) transplantation in mice.

In the present study we investigated the development of natural killer (NK) cell lytic activity, and its correlation with the appearance of cells with large granular lymphocyte (LGL) morphology after bone marrow transplantation (BMT). NK activity was first found 7 days after bone marrow (BM) reconstitution, simultaneously with the appearance of the first LGLs. The number of LGLs, as well as the lytic activity, increased until Day 16 after BM reconstitution, after which they started to decrease, reaching the normal values of controls in 30 days. These early appearing LGLs differed somewhat from mature-type LGLs; they were larger, blast-like cells (found in the lower density fractions of Percoll gradient) and had a basophilic cytoplasma, in contrast to a pale cytoplasm in mature LGLs, but they expressed the asialo GM 1 (AGM 1) antigen like normal NK cells. Those NK cells that appeared first also tended to be lytically more effective than their mature counterparts. Taken together, these data suggest that the correlation between LGL morphology and NK lytic activity also holds true during the development of NK cells from their non-lytic precursors in the bone marrow.

Simultaneous development of antibody-dependent cellular cytotoxicity (ADCC) and natural killer (NK) activity in irradiated mice reconstituted with bone marrow cells.

Spleen cells from irradiated, bone marrow-reconstituted mice were tested for their ability to mediate antibody-dependent cellular cytotoxicity against P815 target (ADCC-P815), ADCC against sheep red blood cells (ADCC-SRBC), and natural killer (NK) activity judged as YAC-1 lysis at different times after bone marrow reconstitution. Donor-derived ADCC-P815 effectors were found to appear in the spleens 10-12 days after bone marrow reconstitution simultaneously with the appearance of donor-derived NK cells. NK cells recently derived from bone marrow are known to express the Thy-1 antigen; the phenotype of the "early" ADCC-P815 effectors was found to be the same as that of NK cells, i.e., Thy-1+, asialo-GM1+. These data suggest that ADCC-P815 effector cells belong to the NK cell population. ADCC-SRBC, in contrast to ADCC-P815 and NK activity, was already high on Day 7 after bone marrow reconstitution. However, it was mediated partly by recipient-derived effectors. ADCC-SRBC effectors were characterized to be different from ADCC-P815 effectors.

Membrane-associated interleukin 1 is required for the activation of T cells in the anti-CD3 antibody-induced T cell response.

To establish the role of the membrane-associated form of IL 1 in T cell activation, we tested the accessory function of monocytes (Mo) that were unable to secrete or release soluble IL 1. The data obtained show that strictly purified resting T cells start to proliferate in response to anti-CD3 antibody (OKT3) stimulation in the presence of paraformaldehyde (PFA)-fixed Mo or 3-day cultured Mo (macrophages). The findings that i) PFA-fixed Mo did not produce or release IL 1, ii) the accessory function of PFA-fixed Mo could be inhibited with pretreatment with anti-IL 1 antibody, iii) PFA-fixed Mo had a comitogenic effect in the murine thymocyte assay, and iv) there was a temporal difference between the capacities to function in the comitogenic assay and to produce soluble IL 1, suggest that human Mo can express membrane-associated IL 1 and that it is functionally relevant.

Dendritic cells in human peripheral blood: effective enrichment from the nonadherent cells.

Dendritic cells (DC) from human peripheral blood were enriched using a method including adherence on plastic, depletion of phagocytic cells, flotation on density gradient column and panning on antibody-coated surfaces. The course of cell separation was evaluated by characterizing the morphological, antigenic and functional features of the different cell fractions. Using the population depleted of strongly adherent cells as the source, we were able to achieve a cell fraction containing 40-65% of DC (the main contaminants being monocytes and natural killer cells). Functionally these cells were highly stimulatory in autologous mixed leukocyte reaction. On the other hand, cells which primarily adhered well and were detached after overnight culture contained less than 5% of DC (the main contaminants being monocytes) after the same purification protocol. The calculated yields of DC were 1-2 X 10(6) and less than 0.5 X 10(6) from the nonadherent and adherent populations, respectively. Thus we concluded that the adhesiveness of DC from human blood is so weak that they can more efficiently and in a more reproducible way be enriched from the primarily nonadherent cell fraction.

Factors predicting the result of surgery for lumbar intervertebral disc herniation.

The prospective study was undertaken to find out preoperative or perioperative factors predicting the results of the first surgery for lumbar disc herniation. The results were evaluated 1 and 6 months postoperatively using, in addition to the patient evaluation, indices for pain, activities of daily living (ADL), and working capacity. The best result was achieved if the patient was operated on before 2 months' duration of disabling sciatica. The operative finding graded as protrusion predicted poor result. The social and psychological factors influenced the outcome more than the findings in the preoperative physical examination or the grade of operative finding. It is recommended to add indices for pain and ADL and a psychological test to the preoperative examinations of patients with lumbar disc herniation.

Asialo GM1+ NK cells have opposite roles in the activation of alloreactive cytotoxic T lymphocyte (CTL) response in vitro and in vivo.

The immuno-regulatory role of AsialoGM1+ murine NK cells in the induction of allogeneic cytotoxic T cell responses was studied in vivo and in vitro. Depletion of ASGM1+ cells from the (CBAxC57BL/6)F1 cells used for the foot-pad immunization of CBA mice greatly reduced the formation of allospecific CTLs. However, highly purified ASGM1+ cells were not efficient stimulators of alloCTLs in vivo by themselves. When the same genetic combination was used in alloCTL stimulation culture in vitro, ASGM1+ cells were seen to suppress the activation of the CTL response. The opposite roles of ASGM1+ cells in the alloCTL activation in vivo and in vitro were seen when both lymphoid cells (spleen cells or peripheral blood cells) and non-lymphoid cells (epidermal cells) were used as stimulators. The data presented in this paper suggest that during the alloCTL activation, ASGM1+ cells have an important enhancing role in vivo; but in vitro these cells suppress the antigen-presenting cells.

Leisure time physical activities and the results of surgery of lumbar disc herniation.

The aim of this study was to elucidate the consequences of leisure time physical activities on the one-year results of surgery of lumbar intervertebral disc herniation. Pre- and postoperative leisure time physical activities and severity of occupation handicap were analyzed from 212 patients. Only to a certain degree the findings support the claim that a high preoperative level of physical activities during leisure time is linked with good postoperative results.

Induction of TNP-specific cytotoxic T lymphocyte memory in vivo in the absence of T helper cell activity.

The question of whether TH cells are required for the priming of CTL precursors (CTLp) in vivo was studied by using Txbm mice (Thymectomized, irradiated, and stem cell-reconstituted mice). In these mice, TNP-specific CTL could be induced in vitro with TNP-coupled spleen cells only if the cultures were supplemented with an IL 2-containing supernatant (ConAsup). In contrast to normal mice, TNP-specific Lyt-2-TH cells could not be induced by skin painting with trinitrochlorobenzene (TNCB) (as tested by the ability to help CTL formation from thymocyte or normal spleen precursors). These data confirm previous findings that Txbm mice possess CTLp but that their TH compartment is deficient. TNCB skin painting had, however, a clear priming effect on the CTLp population: spleen cells from TNCB-painted mice could give rise to specific CTL with a lower amount of ConAsup than spleen cells from unprimed mice. In addition to this, priming changed the CTLp so that stimulation with lightly coupled cells (0.1 mM trinitrobenzene sulfonic acid [TNBS] instead of 10 mM TNBS) became effective. These changes took place without a significant increase in the frequency of TNP-specific CTL precursors. The data obtained are consistent with the concept that at least with some antigens, CTLp proliferation (clonal expansion), which is probably caused by activated TH cells, is not required for the induction of immunologic memory in vivo.

Characterization of natural killer cells and their precursors in the murine bone marrow.

We have fractionated murine bone marrow cells according to their density on bovine serum albumin (BSA) gradient and studied (a) the NK activity against YAC-1 targets, (b) the proportion of asialo GM1+ lymphocytes, (c) and the presence of large granular lymphocytes (LGL) in the different fractions (A, B, C, D). The NK activity was found mainly in the C fraction, but the proportion of asialo GM1+ cells was the same in every fraction. No LGLs were found in the bone marrow. Cells from the various fractions were also transplanted into irradiated recipients. Seven days later the highest NK activity was found in the spleens of mice injected with cells from the A + B fractions indicating that the immediate precursors for NK cells reside in the low density fractions of the BSA gradient. Mice transplanted with C or D fractions needed longer time to develop normal NK levels. The treatment of bone marrow cells before transplantation with anti-asialo GM1+ complement did not inhibit the development of NK activity, so it can be concluded that the precursor for NK is asialo GM1-.

Rehabilitation after surgery for lumbar disc herniation: results of a randomized clinical trial.

The aim of this prospective study was to examine the one-year postoperative results in patients operated on for lumbar disc herniation randomized in two groups: one with comprehensive rehabilitation and the other taken care of by normal care facilities. A total of 212 patients without any previous spinal operations comprised the final study group. The physiatrist, the surgeon, the social worker, and the psychologist performed the handicap evaluation according to the occupation handicap scales of the WHO. The handicap was evaluated for two phases: before the onset of acute sciatica leading to operation and one year after operation. No significant differences in handicap distribution between the intervention and normal care groups were seen. The postoperative handicap correlated highly significantly with preoperative handicap for both groups. More than half (57%) of all the patients returned to work within two months of the operation. The amount of sick leaves did not differ significantly between the intervention and normal care groups. A total of 15 persons (7%) retired during the postoperative year.

The multifidus muscle in patients with lumbar disc herniation. A histochemical and morphometric analysis of intraoperative biopsies.

Structural changes in the multifidus muscle were analyzed in 41 patients operated on for herniated intervertebral disc. Twelve cadavers served as controls. The two main findings follow: Both in the patients and in the controls the Type 2 muscle fibers were markedly and selectively smaller than the Type 1 fibers, which were of normal size for striated muscles, and the internal structure of Type 1 fibers showed so-called core-targetoid and/or moth-eaten change. Group atrophy or fiber-type grouping (indicators of denervation and reinnervation) were observed only in a few patients. The selective small size of the Type 2 fibers may indicate atrophy due to relative inactivity of the multifidus muscle both in the patients and in the controls, ie, it does not need to be related to the herniated disc. Definite proof for denervation of the multifidus muscle was not observed, but neither the possibility be excluded. The cause of the core-targetoid and/or moth-eaten changes cannot yet be determined with certainty, because these changes are not specific for any single entity but may be due, for example, to denervation, ischemia, or altered use of the muscles because of pain. In any case, because the changes were significantly more common in the patients than in the controls, they signal for a pathologic condition, the character of which remains to be elucidated.

Genetic variation in the in vitro veto activity of bone-marrow cells.

Cells exerting a veto type of suppressive activity, that is, suppressing the cytotoxic T lymphocyte formation against the class I major histocompatibility complex antigen on their surface, can be found in different lymphoid tissues, for example, in the bone marrow. In this study we show that there is variation in the vitro veto-cell activity between bone marrow cells derived from different mouse strains: bone marrow cells derived from mice carrying the non-H-2 genes of B10/B6 mice had clearly the strongest veto activity.

Surgical treatment of patellar tendon pain in athletes.

A series of surgically treated patellar tendon lesions among athletes is presented. The material was collected during 5 years from three sports injury clinics and from two hospitals. During this period the authors treated about 150 cases of jumper's knee, of which 34 cases were treated by operation. The athletes were mostly volley ball players, jumpers or runners. The operation revealed a necrotic focus of the patellar tendon in 21 cases, the retinaculum was thick and adherent in 16 patients and an exostosis of the patellar insertion was seen in two cases. The necrotic areas were excised, the thick and adherent retinaculum was divided and the exostoses were excised and drilled. Surgical treatment of chronic patellar tendon pains may give good results in selected cases.

Handicap 1 year following surgery for lumbar disc herniation: special reference to social and vocational factors.

The aim of this cross-sectional study was to clarify social and vocational factors defining occupation handicap in 212 patients operated 1 year earlier for lumbar disc herniation. It seemed that many social and vocational factors had a significant correlation with low back pain problems. Although it is not always relevant to offer social and vocational factors as the cause of back complaint, these may be regarded as associated factors where the link could exist through coping with life situation.

Diflunisal in the treatment of low back pain in a multicentre study.

An open trial with diflunisal (500 mg twice daily) in 766 outpatients consulting their doctor for low back pain, was carried out as part of the project "Back Pain 1981" in Finland. Of the patients (mean age 41 years), 460 had acute lumbago, 144 sciatica and 162 had chronic low back pain as a clinical diagnosis. In each diagnostic group there was a control group of patients receiving no drug therapy. Of the patients 65 percent were men and 35 percent women and the groups did not differ from this distribution in any diagnosis. The efficacy of the treatment was evaluated using four criteria: change of pain at rest and during exercise, patient's evaluation of the efficacy of the treatment, and the need for any supportive treatment. Side-effects of the drug therapy were registered. In all diagnostic groups the relief of pain both at rest and during exercise was greater in patients receiving diflunisal than in the controls. The greatest difference was found in lumbago. The patient's evaluation did not differ between the groups. The drug therapy diminished the need for supportive physical therapy. The frequency of side-effects was 8.6%. They led to the discontinuation of medication in 14 cases (3%). No severe side-effects were found. Diflunisal therapy for the relief of low back pain is therefore considered to be indicated in outpatient care.

Development of natural killer cells: comparison of the maturation rates in different lymphoid compartments.

By means of semisyngeneic bone marrow transplantation, the appearance of donor-type natural killer (NK) cells in different organs (spleen, blood, lungs) of the recipients was studied. Seven days after the transplantation the first donor-type NK cells appeared in all these organs, and adult NK levels were reached simultaneously within a couple of days. The first NK cells to appear in every organ divided rapidly (sensitive to hydroxyurea) and contained a high proportion of Thy-1+ cells. These data suggest that NK cell precursors mature locally and simultaneously in different organs.
