Evaluation of brain-stem auditory evoked potentials using dynamic time warping.

Dynamic time warping is a procedure whereby portions of a temporal sequence of values are stretched or shrunk to make it similar to another sequence. This procedure can be used to align the brain-stem auditory evoked potentials recorded from different subjects prior to averaging. The resultant warp-average more closely resembles the wave form of a typical subject than the conventional average. Dynamic time warping can also be used to compare one brain-stem auditory evoked potential to another. This comparison can show the differences that result from changes in a stimulus parameter such as intensity or repetition rate. When a patient's wave form is compared to a normal template, warping can identify the peaks in the patient's wave form that correspond most closely to the peaks in the normal template. Compared to an experienced human interpreter, warping is very accurate in identifying the waves of normal brain-stem auditory evoked potentials (error rate between 0 and 4%) and reasonably accurate in identifying the peaks in abnormal wave forms (error rate between 3 and 18%).

In vivo imaging and evaluation of platelet accumulation vs. time at arterial injury site.

The feasibility of quantitatively imaging platelet deposition over time following angioplasty of the abdominal aorta in a rabbit model, with and without antiplatelet treatment, was investigated. Ten male 3-4 kg rabbits were balloon de-endothelialized and placed on 2% cholesterol diet for eight weeks. Group A was untreated. In Group B, donors and recipients were treated with aspirin (5 mgm/kg) prior to and after angioplasty. Platelets were labelled with indium-111. Labelled platelets were injected just prior to PTA and images of 100,000 counts were obtained immediately and at 30 minutes, 1 hour, and 24 hours. In Group A, increased activity of angioplasty site vs. nonangioplastied aorta was seen immediately. This focal increase became more marked in hypercholesterolemic animals over the 24-hour period. In Group B, both hypercholesterolemic and normocholesterolemic, no focal uptake could be documented on sequential scans. This method and model are promising for in vivo evaluation of platelet-vessel wall interactions, in the setting of angioplasty and antiplatelet therapy.

Vocational evaluation of traumatic brain injury patients using the functional assessment inventory.

Impaired work capacity is one of the most common residual impairments encountered after either a severe or moderate traumatic brain injury (TBI), yet no instrument is available for screening TBI victims regarding their readiness to participate in more comprehensive vocational planning and evaluation. In the study reported here, the Functional Assessment Inventory (FAI) was administered to 76 subjects who suffered a moderate or severe TBI and the results obtained were compared to the Rancho Los Amigos Hospital Levels of Cognitive Functioning, the Mini-Mental State and the Glasgow Outcome Scale for sensitivity in discerning vocational readiness. The results obtained indicated that the FAI composite score has the greatest discriminating power in screening the vocational readiness of this population, followed by the Glasgow Outcome Scale. In addition, the FAI cluster scores demonstrated reasonable discriminative ability, which may prove useful in directing ongoing remediation.
