Prediction of Fenton oxidation positions in polycyclic aromatic hydrocarbons by Frontier electron density.

Five recalcitrant polycyclic aromatic hydrocarbons (PAHs) in ethanol were subjected to Fenton oxidation, and following GC-MS identification of respective oxidation products, their oxidation positions were compared to those predicted by Frontier electron density. Quinone forms of oxidation products were identified in each PAH. With the exception of fluorene, oxidation positions of quinone forms of products of acenaphthylene, anthracene, benz(a)anthracene, and benzo(a)pyrene corresponded with predicted positions in which Frontier electron density was high. From these results, it appears that determining the Frontier electron density of a PAH is a promising method for predicting the Fenton oxidation position.

Photocatalytic degradation of gaseous perchloroethylene: products and pathway.

Batch photocatalytic degradation of 1000-ppm gaseous perchloroethylene (PCE) was conducted with UV irradiation such that nearly 100% was decomposed within 10 min. The main intermediate and final product were identified as trichloroacetylchloride (TCAC) and hydrogen chloride (HCl), respectively, and minor ones as dichloroacetic acid (DCAC), monochloroacetic acid (MCAC), carbon tetrachloride, chloroform, and phosgene. More than 90% of Cl- equivalent, i.e., the sum of the chlorine number in PCE, intermediates, and HCl, was compensated for during the time of PCE degradation; a result indicating that no other major chlorinated intermediates are present during the time of PCE degradation. In a similar experiment, 500 ppm of gaseous TCAC degraded into HCl within 3 h without producing DCAC or MCAC, where like PCE, more than 90% of Cl- equivalent, i.e., the sum of the chlorine number in TCAC and HCl, was compensated for during time of TCAC degradation. Accordingly, gaseous PCE is concluded to predominantly follow a degradation pathway of PCE --> TCAC --> HCl.

[ACTH-independent bilateral macronodular adrenocortical hyperplasia (AIMAH): a case report].

We report a case of adrenocorticotropic hormone (ACTH)-independent macronodular adrenocortical hyperplasia (AIMAH). A 54-year-old Japanese man was admitted to our hospital for further examination of obesity and hypertension. Endocrinological studies showed that plasma cortisol was high (22.5 micrograms/dl) without diurnal rhythm, and plasma ACTH was low. Two or 8 mg of dexamethasone did not suppress the plasma cortisol levels. Abdominal computed tomography revealed nodular hyperplasia of bilateral adrenal glands. Adrenal scintigraphy showed the positive uptake of 131I-adosterol to bilateral adrenal glands. Brain magnetic resonance imaging revealed no abnormalities. He was diagnosed as having Cushing's syndrome with bilateral adrenal hyperplasia, and bilateral adrenalectomy was performed. Left and right adrenal glands were 52 g and 35 g, respectively, and were occupied by yellow nodular lesions. Histologically, hyperplastic lesions were composed of clear cells. Finally he was diagnosed with AIMAH.

[Successful preoperative interferon-alpha therapy of renal cell carcinoma with tumor thrombus extending into the inferior vena cava--case report].

We report a case of renal cell carcinoma in whom preoperative interferon-alpha therapy was successful in reducing the primary lesion and the tumor thrombus extending into the inferior vena cava. A 67-year-old man was referred to our hospital because of his right renal mass. We made a diagnosis of a right renal tumor with tumor thrombus by imaging examinations. Because his performance status was 3, a radical operation was considered risky. So we started interferon-alpha therapy. Four months after the start of interferon-alpha therapy, the primary lesion and the tumor thrombus reduced in their size, and the clinical response was evaluated as partial response by the response criteria for urological cancer treatment. Because of improving the performance status and reducing the tumor size, radical nephrectomy and tumor thrombectomy could be performed. Pathological examinations revealed that viable renal cell carcinomas were found only in some small parts of the primary lesion, but not in the tumor thrombus. Postoperatively, no recurrence and metastasis of renal cell carcinoma of the patient have been detected in these two and a half years.

[The effect of hyaluronic acid on keratocyte proliferation].

Our previous study demonstrated that during healing of penetrating corneal incision, hyaluronic acid (HA) appears at the wound area with massive infiltration of activated keratocytes, suggesting a possible role of HA in keratocyte proliferation. In the present study, we investigated the effect of HA on keratocyte proliferation. A penetrating incision was made in the center of rabbit corneas. Wounded corneas were treated with HA eye drops or physiological saline every 2 hours for 24 hours. Then the corneas were excised, labeled with 3H-thymidine for 4 hours, and subjected to autoradiography. Rabbit keratocytes were cultured in chamber slides in TC199 medium alone or in TC199 containing HA for 24 hours, labeled with 3H-thymidine for 4 hours, and subjected to autoradiography. In the corneas treated with HA, the number of keratocytes incorporating 3H-thymidine was significantly higher than in the control corneas. The rate of 3H-thymidine uptake in keratocytes cultured with any concentration of HA was higher than that of the control. Both in vivo and in vitro studies demonstrated stimulatory effect of HA on keratocyte proliferation. It is possible that HA might promote corneal stromal wound healing by stimulating keratocyte proliferation.

Cellular fibronectin on intraocular lenses explanted from patients.

We investigated the origin of fibronectin (FN) on five posterior and four anterior chamber explanted intraocular lenses (IOLs) using immunohistochemical methods. Cellular deposits (assumed to be macrophages) and fibrous or membrane-like proteinaceous deposits on the IOLs showed immunoreactivity to an antibody against cellular FN. These proteinaceous deposits were believed to be products of the cells that adhered to the IOLs.

Home elemental enteral hyperalimentation (HEEH) for the maintenance of remission in patients with Crohn's disease.

Since specific treatment has not yet been decided on for Crohn's disease, the immediate target is the induction of remission and its maintenance. We examined the effects of an elemental diet (ED) in Crohn's disease with special reference to the maintenance of remission. Eighty-four patients received total enteral nutrition with the ED (35 to 40 kcal/kg ideal body weight/day) and/or conventional drug treatment for induction of remission. Sixty-one patients in remission were then followed-up with prolonged ED therapy (home elemental enteral hyperalimentation, HEEH) and/or drugs. During the follow-up periods the course of patients receiving HEEH was better than those of patients without HEEH, namely the cumulative continuous remission rates after one, 2 and 4 years were, 94%, 63% and 63% in the group receiving HEEH, 75%, 66% and 66% in the group receiving HEEH and drugs, 63%, 42% and 0% in the group receiving drugs, and 50%, 33% and 0% in the group receiving no maintenance therapy, respectively. In particular, when more than 30 kcal/kg ideal body weight/day of the ED was given, the maintenance of remission was successful in 95% of the patients. These results indicated that ED therapy was effective not only for the induction of remission but also for the maintenance of remission in Crohn's disease.

Alpha-fetoprotein-producing adenocarcinoma of the ureter.

We report a case of primary ureteral tumor producing alpha-fetoprotein (AFP). Computerized tomography, ultrasonography and endoscopy of the bile duct revealed no obvious tumor in the liver, gallbladder, bile duct, pancreas or ovary. Total nephroureterectomy was performed, and histopathological examination revealed adenocarcinoma of the ureter lined with transitional cell carcinoma in its base. The tumor was stained with immunohistological AFP stain, and the high serum AFP level normalized after resection of the tumor.

[Renal arteriovenous fistula detected 25 years after nephrolithotomy: a case report].

We report a case of intrarenal arteriovenous fistula (AVF) with macroscopic hematuria detected after nephrolithotomy performed 25 years previously. The present case, successfully treated by transcatheter embolization with steel coils, is the eighth case of postnephrolithotomy AVF reported in English and Japanese literatures. The necessity of renal arteriography in patients with postoperative massive hematuria is stressed.

[Retroperitoneal cavernous hemangioma: a case report].

A 55-year-old man was shot at the age of 50. At that time CT revealed a mass near the spleen. Thereafter, CT did not reveal any growth of the mass, but to examine the mass in detail he was hospitalized to our department. The mass was diagnosed as left adrenal cavernous hemangioma, since, on aortography, it was typical cavernous hemangioma and fed mainly from the left inferior phrenic artery. The mass was resected with the spleen, thoracic wall, and part of diaphragma. At the operation the left adrenal gland was identified to be intact. Histopathological diagnosis was retroperitoneal cavernous hemangioma. This is the 19th case of retroperitoneal cavernous hemangioma in the Japanese literature.

[Leiomyosarcoma of the spermatic cord: a case report].

A 63-year-old man presented with painless hard swelling of left scrotal contents. Left orchiectomy with high ligation of the spermatic cord was done with clinical diagnosis of testicular tumor. Histopathologically, this tumor was diagnosed as leiomyosarcoma, primarily, arising from the spermatic cord. After operation, prophylactic radiotherapy (60Co, 5,890 rad) was given locally. There has been neither local recurrence nor metastasis for 10 months after operation. This is the 18th case of leiomyosarcoma of the spermatic cord in Japan.

[A case of retroperitoneal Hodgkin's disease with dysuria].

A case of retroperitoneal Hodgkin's disease with dysuria is reported. A 56-year-old man visited our hospital with the complaints of dysuria and lower abdominal mass. On physical examination, an unmovable hard smooth mass of fist size was palpable in the lower abdomen and prostate was slightly swelling by rectal digital examination. Excretory urography demonstrated medial deviation of left lower ureter and bladder deformity. Retrograde urethrocystography showed deviation and compression of prostatic urethra. On CT, tumors were composed of several round masses, which surrounded the left common iliac artery on angiography. Surgical extirpation was carried out and histological examination revealed Hodgkin's disease. As postoperative treatment, chemotherapy with cyclophosphamide, adriamycin, vincristine and prednisolone was performed, and 30 months after the operation the patient was asymptomatic.

Spontaneous rupture of pyelocaliceal diverticulum.

One rare case of spontaneous rupture of pyelocaliceal diverticulum is reported in a 31-year-old Japanese man presented with a severe left flank pain. Intravenous pyelography and computerized tomography demonstrated extravasation of contrast medium from his diverticulum which contained numerous calculi. As a possible cause of spontaneous rupture of pyelocaliceal diverticulum in this case, we suggest that the paper-thin parenchyma covering the diverticulum was not strong enough to tolerate the minimum elevation of intrapelvic pressure due to the obstruction of delivered stones.

[Extramammary Paget's disease with a large mass in male genitalia: a case report].

A 57-year-old Japanese man presented with a mass 3 cm in diameter at the root of the penis. The patient had noticed the mass growing for 5 years, but had no pain or itching. Histological examination revealed it to be invasive Paget's disease, and the mass was resected along with skin of normal appearance within 3 cm around it. Regional lymph node metastasis was also revealed in the operation, and irradiation at the pelvic and inguinal region was done. Extramammary Paget's disease with a large mass is relatively rare, and invasive extramammary Paget's disease has a poor prognosis.

[A case of papillary cystadenoma of epididymis].

We present a case of papillary cystadenoma of the right epididymis in a 52-year-old man. He visited our hospital complaining of painless mass in his right scrotum. This painless hard mass was palpable at the head of his right epididymis and was 1 cm in diameter. Right epididymectomy was performed. A histological examination revealed epithelial proliferation of ectatic efferent ducts and microcysts filled with papillary processes, and a diagnosis of papillary cystadenoma of the right epididymis was made. In Japan, our present case is the 12th of papillary cystadenoma of epididymis. We chiefly reviewed the clinical aspects of this disease, especially the relationship between this disease and von Hippel-Lindau's disease.

[Congenital unilateral multicystic kidney in an adult].

Congenital unilateral multicystic kidney is relatively rare in adult (16% in adult, and 84% in children). Most of the adult cases are asymptomatic, and should be followed up conservatively, but nephrectomy was performed in many cases reported in Japan, because it was difficult to distinguish those cases from renal tuberculosis. Here we present one adult case, and discuss its clinical findings, diagnosis, treatment, etiology and embryology.
