Pulmonary hypertension in hepatitis B virus carriers.

Two autopsy cases of pulmonary hypertension (PH) associated with liver cirrhosis are presented. Both patients were hepatitis B (HB) virus carriers and suffered from type B cirrhosis during the clinical course. The first patient was a 52-year-old male with type B cirrhosis. He died of hepatic encephalopathy but did not have any specific symptoms for PH except abnormal laboratory findings. Chest roentgenograms displayed prominence of the central pulmonary artery. Cardiac catheterization indicated marked increment of pulmonary arterial pressure. Autopsy revealed dilatation and sclerosis of the main pulmonary artery and right ventricular hypertrophy. Microscopically, the pulmonary arteries showed intimal fibrosis, medial hypertrophy, and plexiform lesions throughout the lungs. The second patient, a 15-year-old boy, had PH with juvenile liver cirrhosis which had existed for 8 years prior to the onset of PH. He complained of severe dyspnea and dizziness before death. Electrocardiogram indicated right ventricular hypertrophy. Autopsy disclosed cardiomegaly, type B cirrhosis and sclerotic pulmonary arteries. Grade VI pulmonary plexogenic arteriopathy including plexiform lesions and necrotizing arteritis was observed. HBsAg was detected in both the hepatocytes and the pulmonary arterial walls. We discuss the possible relationship between persistent HB viral infection and PH with liver cirrhosis.

Effect of intestinal factors on extraction of insulin in perfused rat liver.

To study the direct effect of intestinal factors on hepatic extraction of insulin, an investigation was made into the extraction of insulin from isolated rat liver perfused with portal venous effluent (PVE) obtained from the isolated perfused rat intestine. Rat intestine was perfused with Krebs-Ringer bicarbonate medium for 45 min, and the PVE was collected from glucose-, lipid-, or NaCl-treated and untreated control intestines. The PVE, after adjustment of its glucose (180 mg/dl) and insulin (200 microU/ml) concentrations, was used as the perfusing medium for the liver of a different rat. The liver was perfused without recirculation with the PVE not containing insulin for 15 min and then perfused with the PVE containing insulin for the next 30 min. Insulin removal from liver perfused with PVE from lipid- or NaCl-treated intestine (52.6 +/- 5.4 or 46.6 +/- 4.1%) was similar to that from comparable controls (49.7 +/- 2.8 or 48.2 +/- 2.9%), respectively. However, that from glucose-treated intestine (39.7 +/- 6.2%) was significantly (P less than 0.01) lower than that from control intestine (51.1 +/- 2.5%). These results indicate that an intestinal factor secreted after glucose ingestion significantly reduces hepatic extraction of insulin and that at least a part of the incretin phenomenon is due to a decreased hepatic extraction of insulin after oral glucose administration.

Abnormal response of adrenocorticotropin to corticotropin releasing factor in spontaneously hypertensive rats.

The pituitary-adrenocortical response to ovine corticotropin-releasing factor (CRF) was investigated in spontaneously hypertensive rats (SHR) and normotensive Wistar Kyoto rats (WKY) anesthetized with pentobarbital. After intravenous administration of various doses of CRF (0.1, 1.0 and 10 micrograms), the plasma levels of ACTH were significantly increased in both groups. ACTH increments after various doses of CRF were dose-dependent in WKY. ACTH increments after injections of 0.1, 1.0, and 10 micrograms of CRF in SHR were 123.2 +/- 36.5 (SE), 123.0 +/- 52.2 and 60.3 +/- 48.5 pg/ml at 5 min, and 386.3 +/- 73.8, 243.4 +/- 86.6 and 220.0 +/- 31.4 pg/ml at 15 min, respectively. The ACTH increments in SHR at 15 min after administration of 0.1 microgram of CRF were higher (p less than 0.02) than those in WKY. However, ACTH increments in SHR after administration of 10 micrograms of CRF were lower than those in WKY at 5 min (p less than 0.05) and 15 min. These data suggest that the plasma ACTH responses to various doses of CRF represent a dose-unrelated plateau response in SHR. The plasma levels of corticosterone after administration of various doses of CRF did not change significantly and there were no significant differences between the two strains. The results of these experiments suggest that SHR have an abnormal response of the pituitary-adrenocortical axis to CRF, and the abnormal response may be attributable to desensitization of the pituitary to CRF.

Suppression of plasma aldosterone by chronic ACTH administration is offset by converting enzyme inhibitor.

In order to elucidate the mechanism of suppression of plasma aldosterone by chronic ACTH administration, especially the role of the renin-angiotensin system and dopamine, we administered ACTH with or without MK422, a converting enzyme inhibitor, to reduce the endogenous angiotensin II in rats, and measured the plasma renin activity, plasma corticoid concentrations and urinary dopamine excretion. The plasma aldosterone concentration (PAC) was decreased after chronic ACTH administration. However, in the ACTH + MK422 administered group, aldosterone suppression was not observed. It appeared therefore that the aldosterone suppressing mechanism was independent of the weakened renin-angiotensin system following chronic ACTH administration, since PAC was not decreased in the ACTH + MK422 administered group when angiotensin II might be completely eliminated. The urinary excretion of dopamine was significantly increased in the chronic ACTH + MK422 administered group as well as in the chronic ACTH administered group. This suggested that the inhibitory effect of dopamine on aldosterone did not contribute significantly to the suppression of plasma aldosterone. The present results suggest therefore that the mechanism of suppression of plasma aldosterone following chronic ACTH administration was not dependent on the renin-angiotensin system and dopamine.

[Studies on an antibody-coated tube method of aldosterone].

We studied on the basic performance of a non-extraction coated tube RIA kit, DPC Aldosterone kit (Nippon DPC Corporation). 1. The average binding CVs of the calibrators (25 to 1,200 pg/ml) were stable in a range from 1.4 to 1.9%. 2. The minimum sensitivity was 17 pg/ml from 2SD method of 0 calibrator. 3. The intra-assay reproducibility test showed an average CV as 9.0%, and the inter-assay test showed as 8.8%. 4. The recovery ratios of a recovery test were 85.4 to 121.6%. 5. The correlation test with another maker's kit showed a correlation curve as y = 0.970 x + 19.68 and a correlation coefficiency as r = 0.970. 6. The normal range, the correlation between fasting recumbency and daytime sitting, and the correlation between walking and resting, showed similar results as former reports.

High levels of soluble interleukin 2 receptors released from human retrovirus-infected cells.

Soluble interleukin 2 receptors (IL 2R) from human retrovirus-infected cells were analyzed. All the T cell lines integrated with human T cell lymphotropic virus (HTLV)-I and -II, or transfected with HTLV-I gag-pX gene, were found to release high levels of IL 2R constitutively. In contrast, this was not found in T cell lines in which HTLV-I was integrated but not expressed, human immunodeficiency virus (HIV)-infected T cell lines, or T cell, B cell, granulocyte and macrophage cell lines which were HTLV-I negative. These results raise the possibility that the pX-gene product might stimulate the generation of soluble IL 2R. In the sera from patients with adult T cell leukemia, large amounts of IL 2R were released, in contrast to sera from healthy carriers of HTLV-I. The molecular weight of IL 2R was determined to be about 50 KD by size-exclusion HPLC.

[Missile therapy using monoclonal antibody drug conjugates in colorectal carcinoma].

For targeting chemotherapy of colorectal carcinoma, mitomycin C (MMC) and neocarzinostatin (NCS) were covalently bound to monoclonal antibody A7 which is highly specific to human colon cancer. The in vitro cytotoxic effects of the conjugates A7-MMC and A7-NCS on SW1116 were 77 times and 4 times stronger than those of the free MMC and free NCS, respectively. An in vivo study in nude mice bearing human colon carcinoma revealed that monoclonal antibody A7 alone had no effect, and that A7-MMC and A7-NCS had greater inhibitory effects than the free MMC and NCS, respectively. Thirty-five patients with carcinoma of the colon and rectum including 6 with postoperative liver metastasis, one with postoperative lung metastasis and one with postoperative peritoneal metastasis, were given the A7-NCS conjugate consisting of between 15 and 90 mg of antibody and between 1,000 and 6,000 units of NCS. Immunoperoxidase study of resected specimens revealed selective localization of NCS in the cancer cells. The conjugate had no serious adverse effects. Five of the six patients with postoperative liver metastasis responded favorably to the conjugate, showing a decrease in tumor size on CT scan or relief of pain. The conjugate was of no benefit to patients with multiple lung metastasis or peritoneal metastasis. The effect on other patients with surgically resected carcinoma remains to be determined by a follow-up study.

Differential diagnosis of unusual skin diseases in infants.

There are a number of relatively unique clinical skin diseases that may develop on the buttocks of infants. These include iatrogenic skin diseases caused by external medicine; skin tumors caused by methylrosaniline chloride (Pyoktanin); atrophy of the skin due to steroidal liniments; nevus and tumor diseases, such as nevoxanthoendothelioma and Letterer-Siwe disease. Finally, there is chronic granulomatous disease. These unusual cases are reviewed and their differential diagnoses discussed.

[Tegafur and 5-FU concentrations in cancerous and normal tissues in nude mice after oral administration of UFT].

Tegafur and 5-fluorouracil (5-FU) concentrations in blood, tumor, kidney and liver tissues of nude mice implanted with human urogenital carcinoma were determined after oral administration of UFT. The results were as follows: The concentration of tegafur in serum and other tissues rose quickly reading to the peak levels by 15 minutes after administration of UFT, then decreasing gradually. The concentration of 5-FU in serum, liver and kidney showed similar changes, but concentration of tumor tissues were maintained high until 2 hours after administration. These results suggest that coadministration of uracil with tegafur increase the antitumor activity of tegafur.

Genetic recombination between closely linked makers of bacteriophage T4. IV. Mutations which interfere with mismatch repair.

T4 phage mutations MCO1 and MCO3 reduced recombination between multiple closely linked markers preferentially and were located between gene 24 and gene 25. The MCO1 and the MCO3 mutants complemented each other. The results of UV-cross reactivation experiments indicated that the MCO1 and the MCO3 mutants could rescue the markets of UV-damaged phage, but could not segregate them from flanking mutations. Therefore, it is concluded that MCO1 mutation and MCO3 mutation interfere with mismatch repair.

[A case of breast carcinoma, possibly the result of malignant transformation of an intraductal papilloma in a 26-year period].

A 74-year old female presented a giant tumor and serosanguineous nipple discharge from the left breast. The tumor was first recognized 26 years ago, and untreated. The resected breast contained several cysts and some small white nodules. Some of the cysts contained intracystic tumors. Histologically, the tumor consisted of a mixture of papillotubular carcinoma and intraductal papilloma. This case seems to represent a malignant change in intraductal papilloma from its long clinical course and by the microscopic findings. Review of this case seems to warrant clinical attention to the possibility that benign intraductal papilloma may transform itself into malignancy after many years.
