[Evaluation of a radioimmunoassay of alpha 1-microglobulin (alpha 1-m) with simplified procedures].

A newly established double antibody radioimmunoassay (RIA) was fundamentally and clinically evaluated. Original procedures were partially modified as follows: Sample volume for serum and urine was changed to 25 microliters, and thus 200 mg/l of alpha 1-m standard was prepared using 50 microliters of original standard solution (100 mg/l). The results were satisfactory in sensitivity (0.3 mg/l obtained from -2SD method), intra-assay precision with its coefficient variation (CV) ranging from 3.0 to 7.4%, interassay precision with its CV ranging from 3.0 to 10.7%, and recovery with the mean value of 102.4% in serum and 108.2% in urine respectively. There were no changes about alpha 1-m value between diluted (2 times) and undiluted with high concentration samples. Normal levels of alpha 1-m were less than 25 mg/l in serum and less than 10 mg/l in urine. The present results indicate that the determination of alpha 1-m could be very simple and useful for the most sensitive screening test for the evaluation of renal function.

Incidence of cleft lip and/or palate in Aichi prefecture. Comparison of the incidence in Aichi prefecture (1982-1987) with those data of other districts in Japan.

To determine the incidence of cleft lip and/or palate (CL/P) in Aichi prefecture, 485,085 infants born between Jan. 1, 1982, and Dec. 31, 1987, were investigated. 385 infants (0.154%) were found to have these abnormalities; approximately 1.54/1000 live births. Of these infants the number of CL, CLP, and CP was 152 (34.0%), 212 (47.4%), and 83 (18.6%), respectively, (including 1981). These data were compared with those of other prefectures in Japan. The incidence of cleft lip and/or palate in Aichi prefecture was close to the mean for the whole country.

[Adrenal tumors discovered incidentally on ultrasonography: three case reports].

A series of three adrenal tumors is reported. They were discovered incidentally on ultrasonography for screening. One case was a benign pheochromocytoma and the others were non-functioning adrenocortical adenomas. They were hypoechoic on ultrasonography and were 4.5 x 3.5 x 4.0, 3.0 x 2.5 x 2.5 and 3.5 x 2.5 x 2.0 cm. Non-functioning adrenal tumors of a diameter smaller than 3.0 cm generally tended not to require surgery. Ultrasonography is useful for screening and evaluating patients with an adrenal mass without surgery as well as is computed tomographic scan.

[Clinical evaluation of aztreonam (AZT) in the treatment of complicated urinary tract infections].

Aztreonam (AZT, Azactam) was given to 176 patients with complicated urinary tract infections (UTI), of whom 110 cases satisfied with the "Criteria of UTI Committee for the Evaluation of Drug Efficacy in the UTI (3rd Ed.)", which was modified by adopting the midstream urine data in cases of women. Aztreonam was administered by drip infusion at a daily dose of 1 to 4 grams (mostly 2 grams) for at least five days long. The overall clinical efficacy rate of AZT in the 110 cases with the complicated UTI was 64%, estimated by the criteria cited above. The efficacy rate according to the infection type groupings was 66% for the 38 patients in the 1st group, 33% for the 3 in the 2nd group, 79% for the 19 in the 3rd group, 85% for the 21 in the 4th, 47% for the 19 in the 5th and 42% for the 12 in the 6th group. The disappearance rate of Gram negative bacilli was 87.1%. Especially 15 strains of Pseudomonas aeruginosa were eliminated out of 25 (60%) after AZT administration. Forty-six strains appeared after AZT treatment, 36 (76%) of which were Gram positive cocci. Subjective side effects and abnormal laboratory findings in a total 176 cases were: diarrhea in one, upper abdominal discomfort in one, increases in glutamic pyruvic transaminase in 8, increases in glutamic oxaloacetic transaminase in 5 and eosinophilia in 1. All of these disappeared after termination of the drug. AZT might therefore be a useful and safe antibiotic for the first choice use against complicated UTI.

[A report of two cases of absent pulmonary valve syndrome].

Two infants, aged 8 and 18 months, with absent pulmonary valve syndrome unaccompanied by severe pulmonary symptoms or roentgenographic evidence of emphysema underwent a corrective operation consisting of infundibular muscle resection, patch closure for ventricular septal defect (perimembranous outlet type), and right ventricular outflow tract reconstruction with a monocusp patch. The patients tolerated pulmonary regurgitation relatively well, although aneurysmal dilatation of the pulmonary arteries was present preoperatively. Since the postoperative course was uneventful, we think that, in milder cases of this syndrome, a pulmonary valve prosthesis, the insertion of which is advocated by some authors, is not essential, and that right ventricular outflow tract reconstruction with a monocusp patch may suffice in selected cases.

[Clinical application of extracorporeal microexplosive lithotripsy].

A total of 46 cases underwent 66 treatments with extracorporeal microexplosive lithotripsy (EML) for upper urinary tract calculi between March 10, 1987 and May 28, 1987. The efficacy of EML therapy was investigated in all cases over 3 months. The lithotripter of EML made by the Yachiyoda Co. Ltd (SZ-1) was adapted to a microexplosion (10 mg silver azide) as the source of energy for underwater shock wave generation. Fifteen cases (32.6%) had a history of previous open lithotomy of the same upper urinary tract as being treated by EML. Pre-treatments with ureteral catheters and ureteral stents were performed in 4 and 4 cases (8.7% and 8.7%), respectively. The microexplosions were conducted in syncronization with patient exhalation from 100 to 400 times during 1 session, depending upon the size of the stone. The patients felt pressure only on their back at the pulse of explosion and complained occasionally a dull pain, but this pain was mild and tolerable, and no patients required anesthesia. When stone disintegration was judged to be unsatisfactory, that is the stone remained unfragment or the size of the residual stone fragments was greater than 5 mm., an additional session was performed, usually 1 week after the previous session. On the X-ray film obtained three months after EML treatment, 26 cases (57%) were completely free from stone concerments, 17 cases (37%) had stone fragments of equal to or less than 5 mm and 3 cases (6%) had ones of greater than 5 mm. Treatment of EML alone was performed in 44 cases (96%), while 2 cases (4%) required transurethral stone manipulation following EML.(ABSTRACT TRUNCATED AT 250 WORDS)
