Effect of a hematopoietic-promoting factor derived from porcine kidney on megakaryocyte colony formation by murine bone marrow cells.

We investigated the effect of a hematopoietic-promoting factor (HPF) purified from porcine kidney on murine megakaryocyte colony formation. Murine bone marrow cells were cultured in agar in Iscove's modified Dulbecco's medium supplemented with 10% fetal calf serum in the presence of interleukin-3 (IL-3) as a source of megakaryocyte colony-stimulating factor. HPF alone exerted only a slight effect on megakaryocyte colony formation, and the combination of HPF with IL-3 enhanced colony formation compared with IL-3 alone. At days 11 and 13, 1.9-fold and 1.7-fold enhancements were observed, respectively. When erythropoietin (Epo) was added to the combinations of IL-3 plus HPF or IL-3 plus HPF plus SCF megakaryocyte colonies were enhanced compared with the cultures without Epo. As the result of determination the ploidy distribution, more than 20% of the megakaryocytes in IL-3 plus HPF showed 32N and above ploidy, but the number of 8N and under megakaryocytes was the same as those stimulated with IL-3 alone. These results suggest that HPF stimulates both the proliferation of megakaryocyte colony-forming units and the maturation of megakaryocytes in the presence of IL-3.

The concept of "template" assisted electronic medical record.

A new design for an electronic medical record with flexible template generation was developed. The "template" in this paper refers to one of the tools that calls a user's attention to entering the data items for each problem or to ordering tests when they are due. The template shows the patient's previous data and guides physicians to record the consistent description. Two kinds of medical data dictionaries are prepared. Data representations of signs, symptoms, laboratory tests, and other examinations are defined in the check items dictionary. The problem dictionary contains the possible patient problems with relation to check items and other kinds of related subjects. This system provides Problem Oriented Medical Record (POMR) and the graphical presentation of various patient data. The system was designed to establish a constant and integrated medical record.

Development of a health administration system for inhabitants in local community without abnormal findings by health examination.

This study was to establish practical methods to apply to the community health promotion program the data obtained by surveys in a fishing village, under "The Health and Medical Services Law for the Aged." This data, together with guidelines on health counseling for inhabitants in the local community with less abnormal findings on health examination, could be used for developing a health promotion support system. In 1990, a questionnaire survey was carried out on lifestyle such as diet, exercise, and rest, of 1,497 inhabitants (557 males, 940 females) attending for a health examination under "The Health and Medical Services Law for the Aged," at Nishiizu district, Shizuoka Prefecture, Japan. The questionnaire inquired about the subjects' health behavior and consciousness, i.e., on their dietary intake and preferences, dietary habits, regular exercise, exercise habits, and rest, both in fact as well as in perception. In 1991, a survey was also carried out on the daily physical activity and exercise of 1,737 inhabitants (620 males, 1,117 females) attending for a health examination under the law in the same district. The items measured were the daily physical activity, the exercise intensity, and the frequency score derived from several exercises. The relationship to the exercise score was made on the basis of the sex, age, daily physical activity, and the findings of the health examination as well as the self-evaluation of health. In 1992, based on these results and also on the know-how about the software made for similar purposes, "The Health Promotion Support System" for the personal computer was developed in order to help public health nurses perform health counseling for the local community. By means of this system, the answers concerning the health examination findings, habits, health conditions, etc., enabled us to obtain information about the appropriate amount of exercise needed for health, and of the total exercise time needed in a week at the level of 40% maximum oxygen intake. In order to ascertain the quality and effect of the program, the system was applied practically by the public health services, both at Nishiizu district and at Maruyama, Chiba Prefecture, Japan. A questionnaire survey was also conducted in Chiba Prefecture, the subjects were seven public health nurses. The results of this survey showed that the system was successfully applied to the people who received less care because of not having abnormal findings on the health examination.

Computer assisted instruction on multimedia environment for patients.

Computer Assisted Instruction (CAI) is a valuable tool that not only encourages patients, but also provides medical information and the motivation for self management. We developed three modules within a multimedia environment for patients with diabetes or bronchial asthma; positive results were obtained. By using CAI, a reduction in the nurses' workload is achieved along with patients gaining profound and standardized knowledge. We also found some substantial characteristics in the patients' learning behavior and the value of "active learning" in CAI.

Implementation and evaluation of computerized patient management problems.

We developed and implemented computerized patient management problems (CPMP) for medical school education as a simulation of case management and evaluated its efficacy in the School of Medicine at Chiba University. PMP was first developed in the USA; it was used for examinations by the board of American College of Physicians and then introduced to Japan for examination purposes. This test is a practical clinical test for decision making in various medical situations. PMP is, however, a kind of case simulation from a doctor's point of view; it is also a sophisticated tool in training medical students. PMP had many disadvantages in the past, but by computerizing PMP, we solved many problems. We developed the CPMP system for bedside teaching and evaluated its efficiency in terms of education by giving the same test twice and analyzing a survey.

Automated nurse rostering system for administrative efficiency as a subsystem of a total hospital information system.

After trying a variety of nurse scheduling packages, the Chiba University Hospital chose to develop their own system and implemented it in April 1994. The system is the newest component of their HIS which has been functioning since 1991. The rostering system avails itself of the modern client server technology which has been installed over the past few years.

[Experience with the Groshong port in a home TPN patient--a case report].

We experienced a home TPN patient using Groshong port. A 45-year-old woman, recurrent pancreatic head cancer, was treated with home TPN. The Groshong port was inserted via external jugular vein and stayed at home for 8 months with home TPN. Lipid emulsion was administered twice a week. After administration of lipid emulsion, 20 ml of saline was injected and catheter lock was performed with only saline. Heparin was not used for catheter lock. After 8 months' home TPN, patient died. We could not find any precipitate in the catheter and reservoir. A unique feature of Groshong port is a pressure-sensitive two-way valve at the intravascular end. The valve of this port opens outward during fluid infusion or bolus injection. During blood withdrawal, the valve opens inward and then closed after the procedure. Because the valve is designed to remain closed between -7 and 80 mmHg, it will not open spontaneously under normal central venous pressure conditions. This valve eliminates the need for heparin flusing. Catheter obstruction during prolonged TPN is a serious complication. Compound of heparin, lipid emulsion and electrolytes, especially calcium is now becoming one of the most important causes of catheter obstruction and catheter removal. We used the Groshong port to prevent catheter obstruction. After 8 months' use of this port, no precipitate was found in this port. We conclude that the Groshong port is useful for home TPN catheter because it can prevent catheter obstruction due to heparin-lipid-electrolytes compound.

Sterol mediated regulation of SREBP-1a,1b,1c and SREBP-2 in cultured human cells.

Under conditions of cholesterol depletion and SREBP-1 accumulation, changes in the levels of sterol regulatory element binding protein(s) (SREBPs) and sterol regulated gene mRNA were studied in Hep G2 cells by RNase protection assay. Cholesterol depletion increased the expression of mRNAs for cholesterol biosynthetic enzymes and low density lipoprotein (LDL) receptor. mRNAs levels for SREBP-1c and SREBP-2 were also increased by the cholesterol depletion. In contrast, levels for SREBP-1a and 1b (1a/b) mRNA increased transiently and then decreased. To examine the effect of SREBP-1 accumulation, Hep G2 cells were incubated with a SREBP-1 degradation inhibitor, N-acetyl-leucyl-leucyl-norleucinal (ALLN). The ALLN treatment increased the LDL receptor mRNA significantly, and also increased mRNA levels for HMG-CoA reductase, SREBP-1a/b and SREBP-2. The mRNA level for squalene synthase was not changed, and for SREBP-1c was decreased by the treatment. In conclusion, the regulation of differential expression of SREBP mRNA may be involved in sterol mediated regulation of gene expression. Moreover, the regulation of the SREBP-1 level may be a critical step in the regulation of sterol mediated LDL receptor expression.

Detection of the putative E2 protein of hepatitis C virus in human liver.

The question was asked whether a predicted envelope protein, considered to be processed from the polyprotein precursor encoded by the putative E2/NS1 region of the hepatitis C virus (HCV) genome, may be observed in HCV-infected humans. Two polyclonal antibodies against recombinant E2/NS1 proteins were prepared and their reactivity tested against liver extracts from HCV-infected patients by immunoblotting analysis. A band corresponding to a size of 44 kDa was detected in liver extracts from patients who were positive for the HCV-specific antibody anti-C100-3 but not in liver extracts from patients who did not have anti-C100-3 antibody. Additionally, no band was detected using preimmune sera or antisera which had been preabsorbed with recombinant E2/NS1 proteins. Deglycosylation studies demonstrated that the 44 kDa protein was a glycosylated form of a 38 kDa protein which corresponds to the predicted molecular weight of the putative E2/NS1 protein. These results suggest that the 44 kDa protein is a product of the E2/NS1 region. Frequent observation of the 44 kDa band in cases of chronic active hepatitis C suggests a correlation between the expression of this protein and the progression of hepatitis.

Detection of Mycoplasma fermentans DNA from lymph nodes of acquired immunodeficiency syndrome patients.

Biopsy samples from seven patients with acquired immunodeficiency syndrome were screened for Mycoplasma fermentans, M. pneumoniae and M. genitalium infection by the polymerase chain reaction. M. fermentans DNA was detected in four patients. Various tissues were evaluated and the mycoplasma were mainly detected from lymph nodes. Moreover, mycoplasma genus-specific DNA was isolated from peripheral blood mononuclear cells of asymptomatic human immunodeficiency virus(HIV)-infected individuals (two of 31 HIV-infected individuals). These data suggest that mycoplasma infection in AIDS patients is not uncommon.

Right atrial to left atrial shunt through foramen ovale during pneumoperitoneum for laparoscopic cholecystectomy.

A 62-year old female patient in whom paradoxical right atrial to left atrial shunt flow through the foramen ovale was detected by transesophageal echocardiography during pneumoperitoneum for laparoscopic cholecystectomy was reported. This patient had a history of mitral valve replacement with residual tricuspid valve regurgitation. It was considered that the onset of paradoxical interatrial shunting through the foramen ovale might be caused not only by mechanical ventilation with positive end-expiratory pressure but also by pneumoperitoneum, especially in patients with heart disease, even if the intra-abdominal pressure was maintained at 12 mmHg.

Concentration of live retrovirus with a regenerated cellulose hollow fiber, BMM.

A concentrated live retrovirus is required for in vitro experiments. A cuprammonium-regenerated cellulose hollow fiber, termed BMM, originally developed for biohazardous viral removal, was used to concentrate two different retroviruses, an ecotropic murine leukemia virus (MuLV) and human immunodeficiency virus (HIV). The BMM was useful for concentrating live virus suspension 10- to 30-fold from 500-1000 ml of culture supernatant. The ecotropic MuLV concentrated by BMM was demonstrated to be viable and biologically intact by XC plaque-forming assay and reverse transcriptase assay. The concentrated MuLV reached a much higher titer in the spleen in mice than the original one. The virus concentration assessed by p24 antigen for HIV was clearly higher than that of the original culture supernatant of HIV-infected cell lines. Since BMM hollow fibers trapped viruses by the sieving mechanism but not by adsorption, the viral particles were recovered by washing and the total live virus recovery rate was high, about 50%. Furthermore 60 min sufficed to handle 1000 ml of supernatant in the case of a filtration area of 0.03 m2. These results show that the BMM provides us with a rapid, safe and efficient method for concentrating live retroviruses.

Analysis of chromosome-sized DNA and genome typing of isolated strains of Taylorella equigenitalis.

Analysis of chromosome-sized DNA and genome typing of Taylorella equigenitalis NCTC11184, Kentucky 188, and five strains of T. equigenitalis isolated in Japan were carried out. The three restriction enzymes used, ApaI, NaeI and NotI, cleaved the genomic DNAs of five Japanese strains of T. equigenitalis into relatively limited numbers of restriction fragments, which were well resolved on crossed-field gel electrophoresis (CFGE). The respective profiles after CFGE of the restriction fragments from all five strains were essentially identical to each other after digestion by ApaI, NaeI or NotI. Hence it appears that these strains have a common genome type with respect to these three restriction enzymes. It was also shown that the respective profiles from these strains were essentially different from those of T. equigenitalis NCTC11184 and those of Kentucky 188 after digestion with ApaI, NaeI or NotI.
