Do intronic mutations affecting splicing of WT1 exon 9 cause Frasier syndrome?

The WT1 gene, one of the genes responsible for Wilms tumour, is thought to play a crucial role in the development of the kidneys and gonads. This gene encodes four protein isoforms resulting from two alternative splicing sites, one of which involves inclusion or exclusion of lysine, threonine, and serine (KTS) between the third and fourth zinc finger domains. WT1 is virtually always mutationally inactivated in patients with Denys-Drash syndrome. We analysed WT1 in eight patients who had been diagnosed as having this syndrome, and identified five previously unknown mutations affecting splicing donor sites of intron 9. These mutations affect alternative splicing. The isoforms retaining KTS are not produced. The clinical features of the patients with these intronic mutations were consistent with those of Frasier syndrome, characterised by a more slowly progressive nephropathy than Denys-Drash syndrome, associated streak gonads, and no Wilms tumour development. Our results indicate that WT1 isoforms, including/excluding KTS, have different functions in tumorigenesis and organogenesis of the kidneys and gonads.

Integrative visuomotor behavior is associated with interregionally coherent oscillations in the human brain.

Coherent electrical brain activity has been demonstrated to be associated with perceptual events in mammals. It is unclear whether or not it is also a mechanism instrumental in the performance of sensorimotor tasks requiring the continuous processing of information between primarily executive and receptive brain areas. In particular it is unknown whether or not interregional coherent activity detectable in electroencephalographic (EEG) recordings on the scalp reflects interareal functional cooperativity in humans. We studied patterns of changes in EEG-coherence associated with a visuomotor force-tracking task in seven subjects. Interregional coherence of EEG signals recorded from scalp regions overlying the visual and the motor cortex increased in comparison to a resting condition when subjects tracked a visual target by producing an isometric force with their right index finger. Coherence between visual and motor cortex decreased when the subjects produced a similar motor output in the presence of a visual distractor and was unchanged in a purely visual and purely motor task. Increases and decreases of coherence were best differentiated in the low beta frequency range (13-21 Hz). This observation suggests a special functional significance of low frequency oscillations in information processing in large-scale networks. These findings substantiate the view that coherent brain activity underlies integrative sensorimotor behavior.

Investigation of the possible role of sialic acid in calcium oxalate urolithiasis.

OBJECTIVE: It is considered that sialic acid in urine comprises two different types of sialic acid, that is, the free sialic acid and the bound one which exists as a basic component of urinary macromolecules. The present studies were performed to clarify the role of sialic acid in urine and compare the excretion and concentrations of sialic acid in urine between calcium oxalate (CaOx) stone formers and healthy subjects. METHODS: First, we prepared the crystal surface binding substances from whole human urine. The crystal surface binding substances were digested with sialidase and both the inhibitory activity on CaOx crystal growth and concentrations of sialic acid before and after sialidase digestion were measured. The inhibitory activity of free sialic acid in ultrafiltered urine was also measured. The excretion and concentration of sialic acid in urine from patients with CaOx urolithiasis and healthy controls were determined and compared. RESULTS: The inhibitory activity of crystal surface binding substances on CaOx crystal growth did not show any significant difference before and after sialidase digestion. The free sialic acid had also no effect on CaOx crystal growth. There was no statistically significant difference between CaOx stone formers and healthy controls on the excretion and concentration of sialic acid in urine. The free:bound sialic acid ratio in urine was considered 1:0.97 and 1:1.33 for healthy controls and CaOx stone formers respectively (p = NS). CONCLUSIONS: It is suggested that neither the free sialic acid nor the bound one in urine has a significant inhibitory effect on CaOx crystal growth.

Laparoscopic adrenalectomy. Comparison of the transperitoneal and retroperitoneal approach.

OBJECTIVES: The efficacy of transperitoneal (TP) and retroperitoneal (RP) laparoscopic adrenalectomy was compared. METHODS: Twenty-nine patients (male: 13; female: 16) with benign adrenal tumor were evaluated in this study. Seventeen were treated with the TP, and 12 with the RP approach. RESULTS: Convalescence for the TP group took longer than that for the RP group and postoperative pain was stronger for the TP than the RP group, although the difference was not significant. In addition, the amount of blood loss was larger for the TP than the RP group. When the cases were divided according to site of disease, the operating time for the right side was 351 min for the TP group and 347 min for the RP group, while that for the left side was 528 min for the TP group and 267 min for the RP group. CONCLUSIONS: Therefore, we recently prefer the RP approach for left adrenalectomy, but consider either approach suitable for the right side. The reason for this preference is that shortening the operating time is most important, also in the case of laparoscopic adrenalectomy because of the resulting reduced blood loss and the patients' early convalescence.

Heparin-selenocystamine conjugate with selenol groups.

Heparin-selenocystamine conjugate, which was intended to mimic the heparin-selenoprotein P complex, was prepared. The conjugate had glutathione peroxidase-like activity and activity was observed toward hydrogen peroxide, tert-butyl hydroperoxide, and cumene hydroperoxide. The ultraviolet spectrum of an aqueous solution of the conjugate was stable and had a similar shape to that observed transiently when selenocystamine was reduced by sodium cyanoborohydride; this suggests that the diselenide bond of selenocystamine introduced into heparin was cleaved during conjugate preparation and the selenol group is preserved. The conjugate reacted to the same degree as cysteine with 5,5'-dithiobis (2-nitrobenzoic acid) (DTNB) releasing thionitro-benzoic acid, which indicated that the selenium in the conjugate is present as selenol. However, the reaction rate of the conjugate was slower than cysteine which may be due to partially restricted access of DTNB to the selenol group in the conjugate. This conjugate had 1,1-diphenyl-2-picryl-hydrazyl(DPPH) radical scavenging activity as well as superoxide anion scavenging activity. These results indicate that the conjugate serves as a useful model compound with a stable selenol group having a range of biological activities, and suggest a possible antioxidant defensive role for the complex of endogenous heparin-like substance and selenoprotein P.

Doxorubicin cardiotoxicity: diastolic cardiac myocyte dysfunction as a result of impaired calcium handling in isolated cardiac myocytes.

We examined intracellular calcium transients of isolated single cardiac myocytes from rats with doxorubicin (DOX)-induced cardiomyopathy with simultaneous measurement of cell motion. DOX was administered i.p. to Sprague-Dawley rats at 2.5 mg/kg once a week for 10 weeks. Field-stimulated calcium transients and simultaneous cell motion in single myocytes were measured in the presence or absence of isoproterenol using fura-2/AM. Histopathologic examination revealed slight changes. The time courses of both calcium transients and cell motion were significantly prolonged by DOX. There was a slight but not significant reduction in parameters of contractility in both calcium transients and cell motion. The beta-adrenoceptor responsiveness of both calcium transients and cell motion was not significantly impaired compared with the controls. Our data indicated that, despite the slight histologic changes in the heart in DOX-induced cardiomyopathy, impaired sequestration of intracellular free calcium ions in individual myocytes may be one factor leading to diastolic dysfunction. Monitoring of diastolic function is important to detect early cardiotoxicity caused by DOX.

Familial hemiplegic migraine with irreversible brain damage.

Familial hemiplegic migraine (FHM) is an autosomal dominant syndrome characterized by recurrent episodes of varying degrees of hemiparesis associated with migraine. The aura including hemiparesis may be prolonged and in severe attacks may often be associated with confusion or coma. We describe a case of FHM whose aura was atypically prolonged and resulted in irreversible brain deficit which on magnetic resonance imaging (MRI) was suggestive of cortical hyperperfusion. A subsequent MRI showed left brain atrophy.

[Studies of prostate-specific antigen (PSA) immunoassays--differences in characteristic of immunoreactivity and reference materials among the kits].

PURPOSE: We conducted this study to examine differences in characteristics of immunoreactivity for free PSA and alpha(1)-antichymotrypsin complex PSA (ACT-PSA) as well as in compositions and concentrations of PSA reference materials among commercially available PSA kits. METHODS: Fractionated serum samples using a Sephacryl S-200 column were measured by Tandem-R, Delfia-PSA, Ab bead PSA, ACS-PSA, Markit-M and gamma-seminoprotein (gamma-Sm) kits. The calibrators of Tandem-R, Delfia-PSA, Ab bead PSA and Markit-M were fractionated by the same method and measured by Tandem-R. The calibrators of Delfia-PSA, Ab bead PSA and Markit-M and control serums of ACS-PSA were measured by Tandem-R. RESULTS: Although the characteristic of immunoreactivity of Tandem-R, Delfia-PSA, and Ab bead PSA were found to be similar, they were not shown identical. ACS-PSA was proved to recognize free PSA greater than above three PSA kits, while Markit-M could scarcely detect free PSA. gamma-Sm recognized only free PSA. The calibrators of Tandem-R, Delfia-PSA, Ab bead PSA and Markit-M were proved to be only free PSA. The linear correlation was obtained between Tandem-R and Delfia-PSA or Ab bead PSA or Markit-M. The ratio of Delfia-PSA to Tandem-R, Ab bead PSA to Tandem-R and Markit-M to Tandem-R was 0.66, 0.93 and 2.2, respectively. With regard to relation of ACS-PSA and Tandem-R, two ratios of 0.22 and 0.25 were obtained between the two kits according to the different concentrations of control sera. CONCLUSION: The present studies suggest that the difference in PSA values among the commercial PSA kits results from (1) different characteristics of immunoreactivity for ACT-PSA and free PSA among PSA kits, (2) compositions of PSA calibrators among the kits, and (3) different concentrations of PSA calibrators among the kits.

[Studies of type I collagen related substances as bone metastatic markers of prostate carcinoma with special regard to serum carboxyterminal propeptide of type. I. Procollagen (PICP), cross-linked carboxyterminal telopeptide of type I collagen (ICTP) and Urinary deoxypyridinoline levels].

BACKGROUND: To investigate if serum levels of carboxyterminal propeptide of type I procollagen (PICP), cross-linked carboxyterminal telopeptide of type I collagen (ICTP) and urinary levels of deoxypyridinoline (D-Pyr) are useful markers of bone metastasis in patients with prostate carcinoma, we measured these markers in patients with untreated benign prostatic hyperplasia (BPH) and untreated prostate carcinoma (PCA). METHODS: Serum PICP, ICTP and urinary D-Pyr levels were determined in 53 patients; 16 patients with BPH, 15 patients with PCA without bone metastasis (stage A, B, C and D1) and 22 patients with PCA with bone metastasis (stage D2). At the same time correlations among these markers and serum total alkaline phosphatase (ALP) activity were studied. RESULTS: Serum PICP, ICTP and urinary D-Pyr levels in the PCA patients with bone metastasis were significantly higher than those of BPH. The serum levels of PICP in patients with PCA with bone metastasis group were significantly higher than those of without bone metastasis group. The serum levels of ICTP in patients with PCA without bone metastasis group were significantly higher than those of BPH group, while no significant difference was observed between PCA group with and without bone metastasis. In the PCA patients with bone metastasis, serum PICP and serum total alkaline phosphatase (ALP) activity were significantly correlated (r = 0.80). In these patients, serum ICTP and urinary D-Pyr levels were also significantly correlated (r = 0.70). CONCLUSION: These results suggest that serum PIPC, ICTP and urinary D-Pyr are the useful markers to quantitate bone metastasis in the patients with PCA. Moreover, the determination of serum ICTP levels may be significant for detecting occult bone metastasis in the patients with PCA.

[Effect of chorei-to and gorei-san on calcium oxalate crystallization in human urine].

The inhibitory effect of two kinds of Kampou medicine, Chorei-to and Gorei-san, on calcium oxalate crystallization in human urine, was examined. Chorei-to or Gorei-san was administered to 15 healthy male volunteers for three consecutive days under normal dietary conditions and 24-hour urine samples were collected before and after the administration. Urinary parameters including calcium, phosphorus, uric acid, oxalate and citrate were measured on each sample obtained. The inhibitory effect on calcium oxalate crystallization was examined using part of the collected 24-hour urine. There was no significant difference in urinary parameters before and after Chorei-to administration. Gorei-san administration significantly increased magnesium excretion in urine. A significant inhibitory effect on calcium oxalate crystallization in human urine was recognized when Gorei-san was administered, whereas Chorei-to had no inhibitory effect on calcium oxalate crystallization in urine.

[Idiopathic hypersomnia--review of literatures and clinical experiences on 16 Japanese cases].

The concept of idiopathic hypersomnia (IHS) was reviewed. The minimal criteria for IHS in the International Classification of Sleep Disorders (ICSD) are not clear enough to designate a clinical entity. We propose our criteria for IHS, which consist of 6 items. 1. Recurrent daytime sleep episodes with duration of longer than one hour that occur almost everyday for at least 6 months. 2. Absence of refreshed feeling after the daytime sleep episodes. 3. Length of nocturnal sleep is either normal or prolonged. 4. Total sleep time per day exceeds 10 hours. 5. Absence of cataplexy. 6. Does not meet criteria for other sleep disorders that account for the excessive daytime sleep episodes. Our clinical experiences with 16 Japanese IHS patients were described. Decrease in HLA Cw3 was found.

[Arterial infusion chemotherapy with SMANCS-Lipiodol for multiple hepatocellular carcinoma].

Fifty-five patients with hepatocellular carcinoma were treated with oily anticancer agent SMANCS dissolved in Lipiodol (SMANCS-LPD). The local response rate after the first arterial infusion in all patients was 39%, against 63% in 27 patients with Lipiodol accumulation occupying more than two thirds of tumor areas. The infusion therapy with SMANCS-LPD is adapted for a vascular-rich hepatocellular carcinoma. An infusion of 4 mg of SMANCS was ineffective for patients with tumors distributing in bilateral lobes of liver. Thus, an increase of infusion dosage or repeated infusions were recommended for such cases.

7-Dehydrocholesterol down-regulates cholesterol biosynthesis in cultured Smith-Lemli-Opitz syndrome skin fibroblasts.

The Smith-Lemli-Opitz syndrome (SLOS) is a common birth defect-mental retardation syndrome caused by a defect in the enzyme that reduces 7-dehydrocholesterol to cholesterol. Because of this block, patients' plasma cholesterol levels are generally low while 7-dehydrocholesterol concentrations are markedly elevated. In addition, plasma total sterols are abnormally low and correlate negatively with the percent of 7-dehydrocholesterol (r = -0.65, P < 0.0001) suggesting that 7-dehydrocholesterol might inhibit the activity of HMG-CoA reductase. Cultured skin fibroblasts from SLOS patients grown in fetal bovine serum or for 1 day in delipidated medium contain little 7-dehydrocholesterol (3 +/- 1% of total sterols) and HMG-CoA reductase activities are indistinguishable from that measured in control cells. However, raising the 7-dehydrocholesterol concentration to 20 +/- 3% of total sterols, equal to the mean proportion in plasma of SLOS patients, by either growing cells for 1 week in delipidated medium or adding 20 microg/ml 7-dehydrocholesterol directly to the cells reduced HMG-CoA reductase activities from 74 +/- 7 to 9 +/- 2 pmol/min per mg protein, or from 92 +/- 22 to 16 +/- 4 pmol/min per mg protein, respectively (P < 0.01). In contrast, adding 20 microg/ml cholesterol evoked a 2- to 4-fold lesser suppression of activity (39 +/- 8 pmol/min per mg protein, P < 0.05, vs. 7-dehydrocholesterol). HMG-CoA synthase and LDL binding were inhibited equally by 7-dehydrocholesterol and cholesterol. Ketaconazole prevented the down-regulation of HMG-CoA reductase by 7-dehydrocholesterol, suggesting that an hydroxylated derivative of 7-dehydrocholesterol may be especially important in suppressing cholesterol synthesis. These results demonstrate that 7-dehydrocholesterol, perhaps as an hydroxylated derivative(s), is a very effective feedback inhibitor of HMG-CoA reductase.

[Breath-hold 3D MR urography with 2 ml Gd-DTPA injection].

Breath-hold gadolinium-enhanced 3D MR urography was performed in five healthy volunteers. Enhanced 3D fast gradient-echo with a spectral IR pulse sequence was used for depicting MR urography, which was obtained 5-10 minutes after the injection of 2 ml of gadopentate dimeglumine (0.013-0.02 mmol/kg). The urinary tract was depicted as a high-signal intensity area, and detectability of the non-dilated urinary tracts was superior to that of heavy T2-weighted images. At the same time, comparison between the urinary tract and vascular structure could be made using breath-hold contrast-enhanced 3D MR angiography with an additional Gd-DTPA injection.

[Prognostic value of myocardial 123I-BMIPP imaging in patients with congestive heart failure without coronary artery disease].

We performed 123I-BMIPP (BMIPP) imaging in order to clarify the characteristics of fatty acid metabolism and its effect on prognosis in patients with congestive heart failure (CHF) without coronary artery disease. We studied 15 normal subjects and 42 patients with CHF (idiopathic dilated cardiomyopathy; 24, hypertensive heart disease; 12, valvular heart disease; 6). We obtained cardiac imaging with BMIPP and 201Tl at rest on separate days before discharge. Using whole body imaging, the ratio of cardiac uptake of the isotope to total injected dose was calculated (%Uptake). We calculated the cardiac uptake ratio of BMIPP (%Uptake of BMIPP divided by %Uptake of 201Tl), percent washout of BMIPP from the heart over 3 hours and unhomogeneity of myocardial distribution of BMIPP (coefficient of variation of BMIPP, CV) as scintigraphic parameters. Cardiac events were defined as cardiac death or deterioration of CHF which requiring readmission. Uptake ratio (CHF: 0.91 +/- 0.17, normal; 1.10 +/- 0.09, p = 0.0000) and CV (CHF; 20 +/- 5, normal; 16 +/- 5, p = 0.0385) were different significantly between CHF and normal subjects. During the follow-up period of 27.2 +/- 14.4 months (4.5-53.5 months) cardiac events developed in 15 patients (cardiac death; 8, deterioration of CHF; 7). In univariate analysis, %uptake of 201Tl, uptake ratio, % FS, serum norepinephrine level and serum renin activity were different significantly between event and event-free groups. Cox proportional hazard analysis showed that the uptake ratio (p = 0.0000) and age (p = 0.018) were independent predictors of cardiac events. In patients with uptake ratio less than 0.88, relative risks of cardiac events was 23.7 times greater than in patients with uptake ratio more than 0.89. These data suggested that in patients with CHF fatty acid metabolism was deteriorated and cardiac uptake of BMIPP was a strong and independent predictor of long-term prognosis in patients with heart failure.

[Retrovesical leiomyoma: a case report].

A 41-year-old woman was admitted to our hospital with the chief complaint of difficulty of urination during the previous 2 years. Physical examinations revealed a smooth round mass of about 6 cm in diameter at the vaginal anterior wall. DIP and MRI showed a retrovesical tumor which was composed of benign leiomyoma, according to the transvaginal needle biopsy report. The retrovesical tumor was excised by the transvaginal approach. The tumor, 7 x 6 x 4 cm in size and 109 g in weight, was histologically diagnosed as leiomyoma. This is the 21st case of retrovesical leiomyoma reported in the literature in Japan.

[Indocyanine green video angiographic findings of frosted branch angitis in a child].

A 10-year-old boy presented with typical fundus findings of frosted branch angitis in both eyes. Fluorescein angiography showed late-phase hyperfluorescence in some active areas of vascular lesions. Indo-cyanine green (ICG) angiography showed similar but less extensive hyperfluorescence along the frosted vessels and in the disc. ICG angiography also showed filling delay in the choriocapillaris. These findings suggest that frosted branch angitis is a manifestation of inflammation in the retina and the choroid. Laboratory studies showed increase in the toxoplasma titer at 1:10,240 and in the serum levels of alpha 1 and alpha 2 globulin. Frosted branch angitis in this case seemed to be the consequence of local allergic reaction, or immune complex deposition, presumably due to toxoplasma infections.

Fetal Smith-Lemli-Opitz syndrome can be detected accurately and reliably by measuring amniotic fluid dehydrocholesterols.

The Smith-Lemli-Opitz syndrome, characterized by limb, face and organ abnormalities, and mental retardation, is caused by an inherited block in the step of cholesterol biosynthesis in which the delta 7 double bond of 7-dehydrocholesterol is reduced. It is diagnosed by the presence of markedly elevated levels of 7-dehydrocholesterol and 8-dehydrocholesterol in plasma and tissue. We measured amniotic fluid sterols in 15 pregnancies in 13 women who had previously carried an affected fetus. Cholesterol, 7-dehydrocholesterol and 8-dehydrocholesterol concentrations averaged 18 +/- 3, 9.8 +/- 2.9 and 5.0 +/- 1.7 micrograms/ml, respectively, in seven pregnancies with an affected fetus or child. In contrast, these levels were 19 +/- 3, 0.05 +/- 0.01 and < 0.005 micrograms/ml, respectively, in eight increased-risk pregnancies with normal outcomes and 16 +/- 2, 0.07 +/- 0.01 and < 0.005 micrograms/ml in normal controls. 7-dehydrocholesterol concentrations, 2.2-26 and 0.05-0.10 micrograms/ml in pregnancies with an affected and unaffected fetus, respectively, did not overlap. Thus, abnormally elevated amniotic fluid dehydrocholesterol concentrations are an accurate predictor of fetal Smith-Lemli-Opitz syndrome. A false-positive or a false-negative result is highly unlikely.
