[Surgical closure of persistent ductus arteriosus in small premature infants in an incubator].

In premature neonates a patent ductus arteriosus causes often other complications thus the immediate closure of the patent ductus arteriosus should be preferred or recommended. Performing the surgical closure of the ductus within the incubator can avoid complications during transportation from the premature neonates intensive care unit to the operating room. 25 very small premature neonates or those with another burden in addition were operated upon in the incubator on the premature infants intensive care unit of our hospital during the period from January 1987 to April 1989. No complications occurred during the operation, especially not caused by the strange circumstances for the operating team. Wound infections because of possible bacterial contamination of the incubator or the room were not observed during the postoperative period.

[Urea utilization of growing lambs. 2. Growth experiments].

In two growth experiments with 30 lambs per group the influence of urea (1-2% of the ration) and straw (20-30% of the ration) on the performance was studied under conditions of intensive fattening. The content of native crude protein in the rations varied between 10.1 and 18.4% of the DM. In experiment 1 the lambs of the two groups which received mixed feed for fattening lambs without urea on average consumed with 1.02 and 1.11 kg DM resp., 696 and 757 EFUcattle resp. and 173 and 189 g crude protein resp. per animal and day. The groups with wheat or barley and urea consumed 1.06 and 0.96 kg DM resp., 714 and 627 EFUcattle resp. and 209 and 155 g crude protein resp. Their weight gain of 247 g/animal and day was 24% and that of 230 g resp. 26% higher than that in the control groups without urea. The lowest dry matter intake (0.91 and 0.82 kg resp.) was shown by the animals of the groups which received wheat or barley without urea. In the second experiment the control group achieved an average daily weight gain of 327 g at an expenditure of 2.24 kEFUcattle/kg weight gain. Despite a partly higher dry matter intake, the daily EFUcattle intake was between 12.2 and 24.5% lower in the straw groups and energy expenditure increased between 12.0 and 27.8%. Urea supplements of between 1 and 2% improved dry matter, EFUcattle and crude protein intake and thus weight gain.

[Influence of emulsifiers and fat mixtures on the digestibility and sedimentation of fatty acids in calves].

In a digestibility experiment with 4 X 4 calves the animals received 100 g mixture for calves, 50 g dried green fodder and either 656 g dried whole milk and 164 g dried skim milk (VM) or 656 g dried whole milk and 164 g added fat. The added fat consisted of a mixture of tallow and lard in a 1:1 ratio plus 10% emulgator ES 20 (FE) or 8% ES 20 and 2% soybean lecithin (FL) or 25% lard, 25% tallow, 40% rape/sunflower oil with 10% ES 20 (FO). The apparent digestibility of the fat amounted to 91% in group VM and in groups FE/FL/FO to 66/70/67% resp. The composition of fatty acids and the use of an unsuitable charge of dried skim milk are considered to be the causes of the low apparent digestibility. The apparent digestibility of the fatty acids decreased with their growing chain length. The higher digestibility of the unsaturated fatty acids is largely caused by changes due to bacterial activity in these fatty acids in the intestines and by the influence of metabolically changed faecal fat. The combination of the synthetic emulgator with lecithin did not improve fat digestion but diminished the total fat content in the blood.

[Influence of energy concentration and source on the utilization of feed protein and NPN in lambs. 2. 15N balance and utilization of pure protein and NPN].

In an experiment 12 lambs of the merino species were divided into 4 groups. The variants HE received 740 or 718 EFU cattle/kg DM and the variants NE 689 or 671 EFU cattle/kg DM. The different energy concentrations resulted from differentiated quotas of dried sugar beet chips and wheat starch supplements. Within the variants, sub-variants with (HESZ, NESZ) or without (HES, NES) sugar supplements were formed. Due to varied DM intake, the average energy intake in all groups was 42 EFU cattle/kg LW0.75. N balance experiments using 15N-labelled urea were carried out, and 15N accumulation of N excretion was projected to a steady state. The partial utilization of pure protein and NPN in the ration was ascertained with the help of a 3-pool compartment model of N utilization in ruminants. In the non-amino acid N pool HE utilized 84% of NPN and NE 77% for the synthesis of amino and nucleic acids. The efficiency of protein synthesis in the amino acid N-pool were in HESZ 64%, HES 70%, NESZ 70% and NES 73%, resp. The total utilization of NPN is the sum of the partial utilization in the two pools, whereas the total utilization of pure protein is calculated from the true digestibility and the efficiency of the utilization in the AA-N pool. The total utilization of NPN for the synthesis of protein for the protein pool amounted to 40/35/41/33% and that of pure protein to 54/51/49/50%. Energy intake being identical, energy concentration did not have an influence on the utilization of pure protein and NPN, whereas NPN utilization was better in rations containing sugar. The pure protein in the ration was by 19 ... 52% better utilized than NPN.

Transcription signals of mitochondrial and nuclear genes for mitochondrial proteins in dicot plants.

Mitochondria contain several large multisubunit enzyme complexes that are composed of proteins encoded in the nuclear and mitochondrial genomes. Particularly for correct assembly of these enzyme complexes, expression of the respective mitochondrial and nuclear genes has to be coordinated to ensure correct stoichiometries of the protein subunits. Part of this control and the response to specific demands is exercised at the level of transcription. To determine the respective transcription signals we have analyzed the mitochondrial promoters in dicot plants and the promoter structure for nuclear-encoded genes of the respiratory chain complex I. We summarize the results of these investigations and extend the mitochondrial promoter survey to the mitochondrial genome in Arabidopsis thaliana.

Interleukin-1 production following T-cell-depleted and unmodified marrow grafts.

Interleukin-1 (IL-1) production by endotoxin-stimulated cultured monocytes from 31 participants in grafts of marrow depleted of mature cellular elements by treatment with soybean agglutinin and sheep red blood cells (SBA-E-) and 12 recipients of unfractionated bone marrow were studied and compared with normal controls. Patients were studied prior to marrow transplant (BMT) and at 1 month, 2 to 4 months, and 5 to 6 months post-transplant. Deficiencies in IL-1 production (<50 units) were detected in both transplant groups prior to and at 1 month post-BMT. From 2 to 4 months post-transplant, 67% of the recipients of unmodified marrow and 45% of the recipients of SBA-E- marrow grafts produced a normal level of IL-1. By 5 to 6 months post-transplant and thereafter, the proportions of patients exhibiting deficiencies in IL-1 production in each group were equally low. We also evaluated the impact of early deficiencies of IL-1 on engraftment, hematopoietic function, and immunological reconstitution. Deficiencies in IL-1 production persisting to 2 to 4 months post-BMT did not significantly affect the degree of chimerism or the time to recovery of neutrophil counts to 500/mu l in recipients of either unmodified or T-cell-depleted marrow. Platelet recovery during the first 50 days post-transplant was significantly slower in the IL-1-deficient group, but thereafter rebounded, so that by 4 months post-BMT patients with initial deficiencies in IL-1 production achieved levels comparable with those attained by patients with normal production of IL-1. When we looked at the lymphocyte response to phytohemagglutinin (PHA), there was no difference detected among patients with or without IL-1 deficiency receiving unmodified transplants. In contrast, recipients of T-cell-depleted grafts exhibiting a prolonged deficiency of IL-1 experienced a slower rate of recovery of PHA responses. Our results suggest that IL-1 may play an important role in the early expansion of megakaryocytic precursors following an allogeneic marrow transplant and may facilitate the functional development of allogeneic lymphoid progenitors following a T-cell-depleted marrow graft.

Cyfra 21-1: a serological help for detection of distant metastases in head and neck cancer.

Local and neck recurrences of squamous cell carcinoma of the head and neck (SCCHN) can mostly be detected early when the patient has regular follow-ups. Distant metastases though usually remain undiscovered until they produce clinical symptoms. Since Cyfra 21-1 correlates with the tumor size and stage in SCCHN, we looked for possible connections between Cyfra 21-1 increases and the development of distant metastases. The sera of 830 patients with SCCHN were tested for Cyfra 21-1. The levels were compared with the clinical run of the patients. When Cyfra 21-1 levels rose above the threshold of 3.3 ng/ml (71 out of 830) staging procedures were performed. Tumor growth was found in 50 out of 71 patients with elevated Cyfra levels (70.4%). Cyfra serum levels in those cases either represented development of distant metastases (27 out of 71), or local and neck recurrences. The results of this study show that Cyfra 21-1 is a suitable and helpful serological parameter for the follow-up of patients with SCCHN. In the event of an elevation of Cyfra 21-1 above the threshold during follow-up, we would recommend the performance of a thoracal CT-scan and abdominal omi ultrasound as staging procedures.

p53 analysis of laryngeal cancer in exon 4 to 9.

p53 gene mutations are a common genetic alteration in human cancer and codon 72/exon 4 polymorphism of the p53 gene has been implicated in cancer risk. Therefore in this study the p53 gene status of 32 shock-frozen tumor specimens from larynx carcinomas was analyzed by PCR and sequencing of exon 4 through 9. Four mutations (12.5%) in exon 5, 7, 8 and 9 were detected in the carcinoma specimen. Analysis of codon 72 revealed in eight cases a homozygosity for proline (CCC) and in 24 cases heterozygosity or homozygosity for arginine (CGC). The group with the proline/proline genotype had a median age 10.3 years lower than the remaining patients and included the only two non-smokers. Firstly, these results confirm the p53 mutational status of laryngeal cancer without any clinical correlation and secondly may suggest an oncogenic potential for the proline/proline genotype of codon 72 for laryngeal cancer as has already been assumed for lung cancer.

Arrhythmia during mitral commissurotomy.

The authors studied in detail arrhythmias recorded during 110 operations of mitral commissurotomy. In all cases supraventricular and ventricular arrhythmias developed and their type, as well as frequency, depended on the stage of operation. Life-endangering ventricular arrhythmias (ventricular flutter or fibrillation) were never observed and ventricular tachycardia occurring when the orifice was being dilated always regressed spontaneously when a Dubost dilator was removed. Rapid differential diagnosis of arrhythmias is facilitated greatly by additional ECG recording from oesophageal lead. Indications for pharmacological treatment are discussed. This treatment was used, however, in several cases only.
