Transcutaneous intrafold injection for unilateral vocal fold paralysis: functional results.

This paper presents the technique of transcutaneous intrafold silicone injection and the functional results of 44 procedures in 42 patients. The injection is given under local anesthesia with the patient in a supine position. The needle is inserted through the cricothyroid space. The location of the needle as well as the effect of injection is monitored by means of a fiberscope connected to a television camera and screen. The preoperative and postoperative vocal function was multidimensionally evaluated with the use of a test battery consisting of the maximum phonation time (MPT), mean airflow rate (MFRc), fundamental frequency (F0) range and sound pressure level (SPL) range of phonation, pitch perturbation quotient (PPQ), amplitude perturbation quotient (APQ), and normalized noise energy (NNEa). In the majority of the patients, MPT, F0 range, and SPL range increased and MFRc, PPQ, APQ, and NNEa decreased postoperatively. In the overall evaluation based on the test values of these parameters, the result was excellent for 16 procedures, good for 16, fair for 7, and poor for 5. The most frequent cause of failure was an insufficient amount injected.

Sulcus vocalis: functional aspects.

The vocal function of 126 patients with sulcus vocalis was evaluated with the use of a test battery of multidimensional evaluation items. Of the 126 patients, 31 had a unilateral sulcus and 95, bilateral lesions. The results were as follows. 1) The majority of the patients had a mild degree of hoarseness with a breathy quality. 2) An incomplete glottic closure, a small vibratory amplitude, and a small mucosal wave were frequently observed in the stroboscopic examination. 3) The maximum phonation time, fundamental frequency range, and sound pressure level range of phonation were decreased, whereas the airflow during phonation was increased. 4) The pitch perturbation quotient, amplitude perturbation quotient, and normalized noise energy were increased. 5) Abnormal test results were more frequent and more marked for bilateral lesions than for unilateral lesions.

Striatal glutamic acid and gamma-aminobutyric acid in transient cerebral ischemia in spontaneously hypertensive rats.

A massive striatal dopamine release (241-fold increase) was observed in a previous study during acute cerebral ischemia in rats. In this study, extracellular levels of glutamic acid (GLU), gamma-aminobutyric acid (GABA) and lactic acid were simultaneously determined using in vivo brain dialysis in the striatum of spontaneously hypertensive rats during cerebral ischemia and after recirculation. Extracellular GABA levels increased to 932 +/- 75% (mean +/- SEM) of the resting level and GLU increased to 390 +/- 63% during 20 min ischemia. Although ischemia-induced release of GLU and GABA was demonstrated in this study, the degree of increase was smaller than that of dopamine. These findings may be relevant to the pathophysiology of cerebral ischemia in the striatum.

[Micro powder retention for retaining acrylic resin veneers. 1. Application of pressure sensitive adhesive].

Powder retentions are widely used in clinical for retaining acrylic resin facing of veneered crown. These retentions show relatively high bond strength and easy manipulation compared with the traditional retention devices of bar, hole, bead, loop and etc. However, the particle size of powders commercially available are above 250 microns and reduced the thickness of facing resin. Thereupon, smaller diameter of powder were trially applied, however, microparticles were submerged under adhesive and undercut effective for retaining resin was lost. In this research, a new powder retention system adopted pressure sensitive adhesive was studied. The trial adhesive includes volatile solvent like as the traditional adhesives. However, the solvent vapors rapidly and the adhesive applied onto the wax pattern showed pressure sensitivity. Six different sizes of spherical powder were used and the smallest and largest particle size were 90-110 microns and 230-250 microns respectively. New powder retention system applied sensitive adhesive exhibited excellent bond strength to photo polimerized composite facing resin and denture base resin. The powder of 110-130 microns diameter showed shear bond strength values of above 200 kgf/cm2 after 30,000 thermocycles.

[Relationships between the degree of lesion and that of vocal dysfunction in vocal fold polyp].

Relationships between the degree of lesion and that of vocal dysfunction were investigated in 122 cases of vocal fold polyp. A normalized size of polyps and glottic gap were correlated to the results of phonatory ability tests, stroboscopic investigations, acoustic analyses, and perceptual evaluations. There was no significant correlation between the size and the gap. The size of polyp was negatively correlated to fundamental frequency, whereas it was positively correlated to roughness of voice, asymmetry of vocal fold vibration, irregularity of vocal fold vibration, pitch perturbation quotient, amplitude perturbation quotient, and normalized noise energy. The glottic gap showed negative correlations to maximum phonation time and sound pressure level, and positive correlations to mean airflow rate and fundamental frequency.

Water-soluble pradimicin derivatives, synthesis and antifungal evaluation of N,N-dimethyl pradimicins.

Three N,N-dimethyl pradimicins were synthesized by reductive alkylation of pradimicins A, E and FA-2 and evaluated for antifungal activity, water solubility and acute toxicity in mice. They showed in vitro antifungal activity superior to pradimicin A. N,N-Dimethylpradimicins E and FA-2 showed great improvement in water solubility and animal tolerance. N,N-Dimethylpradimicin FA-2 was effective in 3 experimental in vivo fungal infection models.

Pradimicins A, B and C: new antifungal antibiotics. II. In vitro and in vivo biological activities.

Pradimicins A, B and C specify novel antibiotics produced by Actinomadura hibisca No. P157-2 (ATCC 53557) possessing potent and broad antifungal activity in vivo. They showed moderate in vitro antifungal activity against a wide variety of fungi and yeasts including clinically important pathogens, and were highly effective in systemic infection with Candida albicans in mice after iv and im administrations. Pradimicin A showed in vivo therapeutic activity against C. albicans, Cryptococcus neoformans and Aspergillus fumigatus in both normal and immunocompromized mice. 5-Fluorocytosine- and azole-resistant C. albicans strains were susceptible to pradimicin A. This antibiotic also demonstrated therapeutic efficacy against lung candidiasis and aspergillosis, vaginal candidiasis and skin Trichophyton mentagrophytes infection in mice with iv or topical treatment. The LD50 values after a single iv or im administration were 120 mg/kg and more than 400 mg/kg, respectively. Against various cultured mammalian cells, pradimicin A was noncytotoxic at 100 or 500 micrograms/ml, and showed potent anti-influenza virus activity with an IC50 value of 6.8 micrograms/ml.

[A case of refractory anemia with excess of blasts (RAEB) having weakened expression of B blood group antigen].

A 52 year-old male was admitted to our hospital with the complaint of the hypochondrial pain after meal in February 1989. He was diagnosed to be RAEB and to have common bile duct stone. His red blood cells (RBC) could not be agglutinated with anti-A, anti-B, or anti-A, B. The agglutinability of the cells to Ulex europaeus (anti-H) was the same to the normal type B RBC, and his serum contained anti-I, anti-E, and anti-c. In his saliva, both B and H antigens were detected. The glycosyl B transferase in his serum showed similar activity to that of normal individual with type B RBC. When his RBC were treated with normal type B transferase, the cells obtained the reactivity to anti-B but they agglutinated much weaker than the type O cells treated with the enzyme. These results indicated that the low agglutinability of his RBC could not be due to the low transferase activity but the qualitative or quantitative changes of the precursor molecules of the type B substance of the cells.

[A study on preoperative administration of doxifluridine in carcinoma of the colon and rectum].

The usefulness of preoperative administration of Doxifluridine (5'-DFUR) for treating carcinoma of the colon and rectum was studied through a determination of 5-FU concentration in carcinoma, normal intestine, serum and lymph nodes concerned. 1,200 mg/day of 5'-DFUR was administrated orally to 16 patients with carcinoma of the colon and rectum over four days before surgery, and the specimens obtained by resecting the carcinoma were subjected to the mass spectrometry using a gas chromatography to determine 5-FU concentrations. The time needed for collection of the specimen from the final administration of 5'-DFUR was 3.0 to 20.5 hours (10.7 +/- 6.6 hrs). 5-FU concentrations were as follows: 75.8 +/- 61.2 ng/g in the carcinoma, 39.9 +/- 63.5 ng/g in the normal intestine and 26.8 +/- 52.4 ng/g in the lymph nodes concerned, which were significantly high as compared to serum level of 2.3 +/- 4.9 ng/ml (p less than 0.05). Further, a high level of 90.1 +/- 29.3 ng/g (n = 6) of the carcinoma was sustained even at 12 hours or more since final administration of 5'-DFUR. In view of histological patterns, the levels were 139.5 +/- 69.5 ng/g (n = 5) in well differentiated carcinoma and 46.9 +/- 27.4 ng/g (n = 11) in moderately differentiated carcinoma, the level being significantly high in the former (p less than 0.05). A histologic therapeutical effect was shown in one out of 16 cases, with the improvement that remained the score to grade 1. This was deemed due to the small dosage and a short duration of 5'-DFUR.

[Recent advances in rehabilitation of head and neck cancer patients].

This paper describes recent advances in surgical and behavioral rehabilitation for voice, speech and swallowing disorders caused by surgeries for head and neck cancer. Voice disorders result from total laryngectomy, partial laryngectomy and vocal fold laser surgery. Voice prostheses have been recently employed as one of the choices for laryngectomized patients. Surgical reconstruction and voice therapy are the modalities for voice disorders following partial laryngectomy. Dysphonia caused by laser surgery is treated by means of voice therapy. Speech disorders occur following extensive surgery for cancers of the oral cavity and oropharynx. Reconstructive surgery and speech therapy are the modalities to be employed. Swallowing problems are caused by supraglottic horizontal laryngectomy, partial laryngopharyngectomy, extensive surgery for cancers of the oral cavity and oropharynx, surgery for parotid gland and skin cancers associated with a section of the vagus nerve and surgery for esophageal cancers. Surgical treatment and swallow therapy are employed. Surgical treatment is divided into five groups: surgery for velopharyngeal incompetence, those for glottic incompetence, laryngeal suspension surgery, narrowing surgery of the pharynx and cricopharyngeal myotomy. Some cases are described for each treatment modality.

[Alternating non-cross resistant multi-drug chemotherapy against malignant lymphoma (CAMBO-VIP)--consideration of the dose intensity].

Dose intensity defined as the amount of drug used per unit time, expressed as mg/m2/week, was reported to be a significant determinant of antitumor activity of single or combined drugs in cancer chemotherapy. We formulated a 12 week high dose intensity chemotherapy (CAMBO-VIP) for the treatment of advanced non Hodgkin's lymphoma with aggressive histology. The treatment consists of weekly alternate administration of myelosuppressive and non-myelosuppressive agents. Doxorubicin was administered every other week in combination with either cyclophosphamide, etoposide or ifosfamide. On the weeks in between, non-myelosuppressive vincristine was given with either methotrexate with leucovorin rescue or bleomycin. Prednisolone was given for the first and the last 4 weeks. Dose reduction and treatment delay were kept minimal for the purpose not lowering dose intensity. As of February 1990, 32 patients (pts), median age 52, entered the study, 29 of whom completed the treatment. There were 3 incomplete cases, in which 2 were due to interruption of the treatment at 5 and 6 weeks, respectively and 1 due to a half dose given because of the old age. CR was obtained in 29 pts (90.6%). Relapse occurred in 3 (10.4%) with the median follow-up of 12 months. Two year disease-free survival (DFS) was estimated to be 76.0% for all the patients and 83.9% for CR patients. Toxicity of CAMBO-VIP was moderate with no chemotherapeutic death. Myelosuppression was severe but of short duration, requiring virtually no dose reduction. Treatment delay was 3 days, median, and maximally 28 days. The average actual dose intensity calculated from given amount of drugs and treatment duration was as high as 90% of the protocol dose intensity. Dose intensity of CAMBO-VIP protocol is highest, equaling to that of MACOP-B, among representative series of reported lymphoma protocols. A highly significant correlation was observed between 9 drug relative dose intensity and DFS of the patients treated with each protocol. Significance of dose intensity as an independent prognostic factor, however, should be determined, by a prospective carefully designed stratified randomized studies.

[A case of systemic lupus erythematosus showing abnormal lipoproteins due to accompanied drug induced hepatitis].

We describe here a 28-years-male with AIHA and SLE who had lipid and lipoprotein abnormalities during cholestasis induced by PGE1 administration. High free cholesterol level, 792 mg/dl was found in his serum, and markedly elevated, phospholipid level 1,614 mg/dl. But, LCAT activity was within normal range in this case. An agarose gel electrophoresis of lipoproteins showed abnormal bands which were located in slow alpha 2, pre beta and slow beta, and between beta and origin point. Moreover, it was detected formation of Lp-X from serum of the patient. Serum levels of apoprotein B, C-II, C-III, and E were higher, while apoprotein A-I, A-II were very lower than reference value. From these results, it was suspected that the patient might occur transient abnormal lipid metabolism according to the drug induced hepatic injury.

[Intramural duodenal hematoma: a case report].

A 55-year-old male patient with acute onset of abdominal pain was examined. He had no apparent history of abdominal trauma or bleeding tendency. MRI showed three layered architecture, so-called "ring sign" distinctive of hematoma. MRI seemed to be very useful in non-invasively diagnosing intramural duodenal hematoma.

[The study of endotoxin levels in portal venous system in liver cirrhosis].

In the patients with liver cirrhosis, endotoxin levels were measured in portal venous system by the toxicolor method and the mechanism of endotoxemia in the peripheral vein was studied. On endotoxin levels in portal venous system, the superior mesenteric vein had the highest level. Endotoxemia was observed most frequently in the superior mesenteric vein (78%), and 50% in the truncal portal vein and 55% in the peripheral vein. The hepatic "endotoxin extraction ratio" which is considered to reflect the Kupffer cell function of clearing endotoxin and the severity of cirrhosis were not related each other. On the other hand, in view of the extrahepatic portal shunt, systemic endotoxemia appeared more frequently in the patients with the other collaterals than the esophageal varices. Therefore, it was concluded that systemic endotoxemia in liver cirrhosis was related with the type of extrahepatic portal collaterals.

[A case of medial pneumothorax in the aged].

A 81-year-old man was admitted because of left recurrent pneumothorax with pleural adhesion. A drainage tube was inserted through the left lateral thoracic wall. The lung seemed to be fully expanded in the AP chest X-ray film but his dyspnea was not relieved. The CT examination showed severe medial pneumothorax. He was operated on and recovered uneventfully. It is important to note medial pneumothorax in patients in the supine position and/or with pleural adhesion.

Ultrasonically guided biopsy of nonpalpable lesions of the breast by the spot method.

A new biopsy method by injecting dye under ultrasonic guidance is most efficient and accurate for nonpalpable lesions revealed by ultrasonography. It can also minimize the amount of tissue that is resected. The conventional method by needle-wire localization has to be used in instances of calcification which cannot be defined by ultrasonography.
