Histologic investigation of the macula flava of the human vocal fold.

An investigation was carried out to determine the histologic structure of the maculae flavae of human adult vocal folds. Excised human adult larynges served as the materials for this study. Light microscopic, scanning, and transmission electron microscopic observations were made. The results are summarized as follows. 1) The maculae flavae are located at the anterior and posterior ends of the bilateral vocal folds. Vocal fold ligaments run between the anterior and posterior maculae flavae. 2) The maculae flavae are elliptical in shape and about 1.5 x 1.5 x 1 mm in size. 3) The maculae flavae are composed of fibroblasts, elastic fibers, collagenous fibers, and ground substance. 4) Fibroblasts in the maculae flavae synthesize, and occasionally engulf, the elastic and collagenous fibers. 5) The maculae flavae seem to control the synthesis of fibrous components in the vocal ligament.

Histologic investigation of the macula flava of the human newborn vocal fold.

This investigation was carried out to determine the histologic structure of the maculae flavae of human newborn vocal folds. Light microscopic and transmission electron microscopic observations were made with excised human newborn larynges. The results are summarized as follows. 1) Immature maculae flavae were present at the anterior and posterior ends of the bilateral vocal folds. 2) They were round in shape and about 1 x 1 x 1 mm in size. 3) The maculae flavae were composed of fibroblasts, elastic and collagenous fibers, and ground substance. 4) The fibroblast density was great. 5) In newborn maculae flavae, fibroblasts actively started to form elastic and collagenous fibers. 6) The macula flava of the newborn appears to be an important structure in the growth and development of the vocal ligament.

Age-related changes of the macula flava of the human vocal fold.

This investigation was carried out to determine the histologic structure and age-related changes of the macula flava of human aged vocal folds. Excised human adult senescent larynges served as the material for this study. Light microscopic and transmission electron microscopic observations were made. The results are summarized as follows. 1) The anterior and posterior maculae flavae were elliptical in shape and about 1.5 x 1.5 x 1 mm in size. 2) The macula flava was composed of fibroblasts, elastic fibers, collagenous fibers, and ground substance. 3) The number of fibroblasts decreased. 4) Components in the cytoplasm, such as Golgi apparatus and rough endoplasmic reticulum, were fewer than in younger adults. 5) Some fibroblasts and some components in the cytoplasm degenerated. 6) The accumulation of glycogen granules and lipid droplets were seen in the cytoplasm. 7) The number of collagenous and elastic fibers synthesized by the fibroblasts decreased. 8) These findings were evident to various degrees and suggested the reduction of fibroblast activation, abnormal metabolism, and degeneration. A decrease in the number and activation of fibroblasts in maculae flavae indicates decreased synthesis of fibrous components in the vocal fold mucosa. This in turn influences the viscoelasticity and stiffness of vibrating tissue and contributes partially to aging of the voice.

Analysis of mutations in the human HPRT gene induced by accelerated heavy-ion irradiation.

Multiplex PCR analysis of HPRT(-) mutations in human embryo (HE) cells induced by 230 keV/microns carbon-ion irradiation showed no large deletion around the exon regions of the locus gene in contrast to the irradiations at different LETs. To identify these mutations, the sequence alterations in a cDNA of hprt gene were determined for 18 mutant clones in this study. Missing of exon 6 was the most frequent mutational event (10 clones), and missing of both exons 6 and 8 was next most frequent event (6 clones), then base substitutions (2 clones). These characteristics were not seen in a similar analysis of spontaneous mutations, which showed base substitution (5 clones), frameshift (2 clones), missing of both exons 2 and 3 (2 clones), and a single unidentified clone. Direct sequencing and restriction enzyme digestion of the genomic DNA of the mutants which showed missing of exons 6 and 8 in the cDNA, supports the possibility that they were induced by aberrant mRNA splicing.

Angiotropic B-cell lymphoma presenting with progressive dementia: an autopsy case and review of the literature in Japan.

We report an autopsy case of angiotropic lymphoma and review the literature in Japan. A 68-year-old female presented with transient right facial nerve palsy, right hemiplegia and progressive dementia. Laboratory studies showed raised erythrocyte sedimentation rate, C-reactive protein and serum lactate dehydrogenase. Computed tomography of the brain did not show any abnormal density areas but on T2-weighted magnetic resonance imaging high signal areas were recognized at the basal ganglia. She was diagnosed as having dementia caused by cerebrovascular insufficiency. She soon developed a febrile state and died of respiratory failure within three months of presentation. Postmortem examination showed widespread angiotropic B-cell lymphoma.

[IgD-lambda type multiple myeloma associated with IgG-kappa type benign monoclonal gammopathy].

A 76-year-old man was admitted to Kisen hospital because of lumbago and chest pain. Laboratory examinations revealed a chronic renal failure with marked elevation of the serum BUN (48.8 mg/dl) and creatinine levels (8.2 mg/dl). The serum electrophoresis demonstrated a hypergammaglobulinemia with M peaks. An immunoelectrophoresis demonstrated monoclonal IgD-lambda and IgG-kappa proteins in the serum, and lambda-type Bence Jones protein in the urine (0.4 g/day). Bone marrow smears revealed an abnormal proliferation of atypical plasma cells (43%). A systemic X-ray examination of the skeletal system showed systemic osteoporosis without punched out lesion. The patient was diagnosed as having IgD-lambda type multiple myeloma and IgG-kappa type benign monoclonal gammopathy by quantifying concentration of two M proteins (1,160 mg/dl in IgD, 1,179 mg/dl in IgG, respectively). A combination chemotherapy with melphalan and prednisolone was administered monthly for multiple myeloma, and hemodialysis for the renal failure was performed 3 times a week. A marked improvement of his laboratory findings including a diminution of the serum IgD-lambda M-protein was obtained. On the other hand, IgG-kappa M-protein level was unchanged. Two M-protein levels showed a different behavior after the combination chemotherapy. Although the patient died of congestive heart failure, the partial remission of multiple myeloma has been maintained for 16 months with chemotherapy.

Effects of tonic neck reflex on optokinetic nystagmus in rabbits.

In this study, rabbits with black-pigmented eyes were suspended in air with none of their limbs touching the ground, and surrounded by a full-field optokinetic drum. The drum was rotated counterclockwise at 60 degrees/s. Horizontal eye movement was recorded by electronystagmograph with the animals in three postures: 1) initial posture, head and trunk aligned along the same body axis, 2) trunk rotated clockwise by 60 degrees with head stationary, and 3) trunk rotated counterclockwise by 60 degrees with head stationary. The recordings were repeated after cervical nerve roots were sectioned unilaterally. The speed of the slow phase of nystagmus was significantly faster in the second than in the initial posture, but significantly slower in the third than in the initial posture. These effects of trunk rotation on the speed of the slow phase were not seen when the side of cervical nerve section was extended. The results indicate that smooth following eye movement is influenced by the neck reflex. It is accelerated when the head turns in the same direction as the visual field, whereas it is inhibited when the head turns in the opposite direction. Proprioceptors of the extended neck appear to account for the effects observed.

Vocal function in patients with unilateral vocal fold paralysis before and after silicone injection.

A total of 240 patients with unilateral vocal fold paralysis (UVFP) underwent transcutaneous intrafold silicone injection (TCIFSI) for the purpose of vocal rehabilitation during the period from 1983 to 1993 in the Kurume University Hospital. Their vocal function was evaluated and the effects of TCIFSI upon the vocal function were investigated. The patients with UVFP presented with an increase in airflow during phonation; a decrease in maximum phonation time and fundamental frequency (F0) range and sound pressure level (SPL) range of phonation; and an increase in pitch perturbation quotient, amplitude perturbation quotient and normalized noise energy in the voice signal. TCIFSI resulted in a decrease in airflow, an increase in phonation time and F0 and SPL range of phonation, and a decrease in pitch and amplitude perturbation and noise in the voice signal. Patient's own subjective evaluation presented with an improvement of his/her voice after TCIFSI.

[Muscle strength and muscle blood flow of the quadriceps muscle].

This study was conducted to measure muscle blood flow changes during increased muscle strength in order to determine what characteristic parameters of muscle strength were most closely correlated with increased muscle blood flow. The muscle blood flow and muscle strength in the quadriceps femoris were measured simultaneously during isokinetic extension of the knee joints in two groups--one with knee-joint-disease and the other of healthy volunteers. Muscle blood flow was measured by the heated thermocouple technique, while parameters of muscle strength were evaluated utilizing curves calculated by a Cybex II. The test results showed a positive correlation between parameters of muscle strength (peak torque, total work and average power) and peak blood flow. However, both the muscle strength and the muscle blood flow values were less in the group with knee joint disease than in the group of healthy volunteers. In both groups, the muscle blood flow was greatest when the knee was extended at an increased angular velocity. Although the muscle blood flow was greatest at this time, only the parameter of average power increased while the parameter of peak torque and total work decreased. As a result, the average power was correlated most closely with the increased muscle blood flow. In patients with a knee joint disease, the increase in the muscle blood flow was much higher than expected despite the fact that there was little increase in average power during knee movement at an increased angular velocity. It appeared that the pathological condition in these patients' quadriceps femoris required increased blood flow despite an inability of the muscle to perform increased muscle work.

[Usefulness of every-other day administration of proton pump inhibitors from the standpoint of continuous 48-hour pH measurements-- Comparison of cimetidine, omeprazole, and lansoprazole].

We compared the usefulness of proton pump inhibitors (PPIs) every other day, with H2-blocker, in terms of inhibitory effect on gastric acid secretion. The dosages were as follows; cimetidine (CIM), 800mg and 400mg every day; omeprazole (OPZ) 20mg, and lansoprazole (LPZ), 30mg every other day. We continuously measured intragastric pH and calculated the time that pH was maintained at pH4 or more (pH4 holding time). The pH4 holding time as a part of total measurement time was significantly longer in the PPI every other day groups than in the H2-blocker groups, but there were no significant differences between the groups during the night. In PPI every other day groups, pH4 holding time of daytime period was suggested to be longer than on H2-blocker groups. In conclusion, their efficacy was greater than in the CIM group, and they appear to have a broad range of applications to the treatment of peptic ulcer.

[A case of carpal tunnel syndrome caused by a ganglion cyst--diagnosis and follow-up study with magnetic resonance imaging].

We report a case of carpal tunnel syndrome (CTS) caused by a ganglion cyst which was diagnosed by magnetic resonance (MR) imaging of the left wrist. MR images depicted a 10-mm ganglion cyst which pressed the median nerve toward the ulnar side. The ganglion cyst was punctured under direct endoscopic visualization in the carpal canal, subsequently the clinical and the electrophysiologic findings of CTS improved. The median nerve was delineated in its proper position. MR imaging of the wrist, a noninvasive means to observe median nerve, is useful for the diagnosis and the follow-up study of CTS.

[Hematological neoplasm--how to treat low-grade lymphomas].

Low-grade lymphomas are rare in Japan, comprising about 10% of non-Hodgkin's lymphomas. They are indolent in natural history with median survival of 7-10 years. Despite a good initial response to radiotherapy or chemotherapy, most patients (Pts), being incurable, ultimately die of their disease. Pts with clinical stage (CS) I/II disease are generally treated with locoreginal radiotherapy. Long-term survival of 60% to 70% and disease-free survival (DFS) of 50% to 55% have been reported. The more precise the staging procedures, the better the results. Chemotherapy used as a primary therapy or as an adjuvant to radiotherapy may improve DFS of pts with CS II with many sites of lymph node involvement or bulky disease. For CS III/IV disease there is no accepted standard therapy. There is no survival advantage for any of the different first-line treatment methods ranging from initial deferral of treatment albeit in highly selected pts, an alkylating agent +/- steroid hormone, to combination chemotherapy (C-MOPP, CHOP, ProMace-MOPP etc) +/- irradiation. The international index for prognostication of aggressive lymphoma has also been shown to be applicable to stratification of low-grade lymphomas. In future studies effectiveness of treatment strategies should be investigated with long-term DFS and improved quality of life being set as primary endpoints.

Donor buffy coat infusions for a patient with myelodysplastic syndrome who relapsed following allogeneic bone marrow transplantation.

A 49-year-old man with myelodysplastic syndrome (MDS, RAEB in T) who relapsed following allogeneic bone marrow transplantation (allo BMT) was treated with infusions of donor buffy coat leukocytes. He sustained hematologic and cytogenetic remission, but severe graft-versus-host disease (GVHD) developed for which cyclosporin A and prednisolone were required. This therapeutic approach appears to be valuable for relapsed MDS following allo BMT as well as for chronic myelogenous leukemia (CML).

[Detection of Helicobacter pylori by culture and the 13C-urea breath test using an automated breath 13C analyzer].

Up to now, the diagnosis of H. pylori infection has been made by the breath test using 13C-urea. In this study, 13C-urea breath samples were tested in 34 patients (peptic ulcer scar 17, chronic gastritis 17 cases) with an automated breath 13C analyzer (ABCA. Europa Scientific, Crewe, UK) and compared with the results of endoscopical diagnosis for H. pylori infection. Endoscopic and 13C-urea breath test (13C-UBT) were performed before eradicative medication. We described a modified protocol for the growth grade of H. pylori colonies in microbiology (H. pylori score), and for the delta 13C area under curve (AUC; permil*hr) obtained from each sample of expired breath. There was a significant correlation between delta 13C-AUC and the delta 13C level of each sample, but the correlation coefficient obtained at 10min (R2 = 0.582) was lower than that obtained at the other four time points (20min; 0.891, 30min; 0.949, 40min; 0.946, 50min; 0.946, 60min; 0.820). The delta 13C-AUC well correlated with H. pylori score (p < 0.01), none of 26 H. pylori positive patients detected by culture was 13C-UBT negative (delta 13C-AUC < 8.2 permil*hr in mean + 2SD of H. pylori negative group). In conclusion, 13C-UBT using ABCA has high sensitivity and specificity, and it provides a non-invasive method for the detection of H. pylori urease activity.

Touch-inducible genes for calmodulin and a calmodulin-related protein are located in tandem on a chromosome of Arabidopsis thaliana.

Genes for calmodulin and calmodulin-related proteins in Arabidopsis are up-regulated by a variety of physical stimuli, which include rain, wind and touch [Braam and Davis (1990) Cell 60: 357]. We have isolated five genes for calmodulin (AtCAL1, 2, 3, 5, 6) and one gene for a calmodulin-related protein (AtCAL4) from an Arabidopsis genomic library. Touch stimulus of Arabidopsis plants induces the accumulation of mRNA transcribed from AtCAL4 and AtCAL5, but not from the other isolated genes. The two touch-inducible genes are arrayed in tandem with a short intergenic region of 700 bp but they show different organ-specific patterns of expression.

[Long-term selective beta 1-blockade therapy for a patient with anthracycline-induced cardiomyopathy].

A 44 year-old woman with acute myeloid leukemia (AML, FAB, M4E) developed heart failure during treatment with anthracyclines for AML. She had not experienced heart disease and her left ventricular ejection fraction (LVEF) was 59% at the end of a successful remission induction therapy. Because her LVEF decreased to 33% after early consolidation therapy, the chemotherapy for AML was discontinued. The cumulative dose of daunorubicin, aclarubicin and mitoxantrone was 486 mg/m2, 135 mg/m2 and 55 mg/m2, respectively. In October 1990, four months after the end of the chemotherapy, heart failure (class III, NYHA) developed and did not improve by treatment consisting of dobutamin, digoxin and diuretics. Anthracycline cardiomyopathy was histologically confirmed by endomyocardial biopsy. Then we administered selective beta 1-antagonist, metoprolol (Seloken), with an initial dose of 5 mg/day which was doubled 3 times every 4 or 8 weeks to 40 mg/day, according to the treatment schedule of dilated cardiomyopathy. She recuperated satisfactorily (Class I, NYHA), and was discharged on February '91. Her LVEF gradually improved and it has been maintained at above 50% on an outpatient basis. The patient has been in complete hematological remission during this period. It seems that low dose selective beta 1-antagonist therapy has a potential to improve myocardial function in some patients with anthracycline cardiomyopathy.

Temperature-dependent Ca2+ mobilization induced by hypoxia-hypoglycemia in the monkey hippocampal slices.

Mobilization of [Ca2+]i in the monkey hippocampal slices during transient hypoxia-hypoglycemia and KCl-induced depolarization was analyzed by microfluorometric imaging and anti-PIP2 immunohistochemistry. Hypoxia-hypoglycemia provoked the largest [Ca2+]i mobilization of CA-1 temperature-dependently whereas [Ca2+]i mobilization by KCl-induced depolarization occurred independent of the temperature in CA-2. Immunohistochemical analysis of the hippocampus after hypoxia-hypoglycemia showed an increased PIP2 staining preferentially in the perikarya of CA-1 neurons. These data suggest that release of Ca2+ from intracellular stores caused by PIP2 breakdown may induce elevated [Ca2+]i.
